Abstract  Objective: To evaluate the 18F-FDG PET-CT in detecting primary foci in cases of suspected metastatic tumor. Methods: One hundred and sixty-four patients with pathologically proved metastasis of unknown primary tumor underwent 18F-FDG PET-CT scan. Statistical analysis was performed after the final follow up. Result: The total positive rate, specificity, sensitivity, accuracy, negative predictive value and positive predictive value were 51.2%(84/164), 97.3%(71/73), 92.3%(84/91), 94.5%(155/164), 91.0%(71/78), 97.7%(84/86) respectively. The positive rate of the primary foci for metastatic adenocarcinoma, metastatic squamous carcinoma and metastatic undifferentiated carcinoma were 54.0%(47/87), 56.8%(21/37) and 40.0%(12/30) respectively. According to the statistical analysis, there was no statistical difference among the positive rate of the three groups. Conclusion: 18F-FDG PET-CT might be of importance in the detection of primary site in unknown primary tumors
