381 E-Tracking technique in assessment of carotid elasticity of the patients withhead and neck tumor after radiotherapy

Objective: To explore the clinical value of E-Tracking technique in assessment of carotid elasticity of the patients with head and neck tumor after radiotherapy. Methods: Seventy-five cases of head and neck tumor patients after radiotherapy(radiation group) and 65 cases of healthy volunteers(control group) were enrolled in this study and the parameters of the carotid artery of two groups were compared. For radiation group, the carotid artery parameters of 39 patients were compared before and after radiotherapy. E-Tracking parameters including carotid artery stiffness parameters(β), pressure-strain elastic modulus(Eρ), compliance(AC), pulse wave velocity(PWVβ) and carotid artery intima-media thickness(IMT) were measured and recorded by E-Tracking technique. And the injury factors of carotid artery after radiotherapy were analyzed too. Results: Compared with control group, β, Eρ, PWVβ, IMT in radiation group increased, AC decreased, the differences had the statistical significance(all P<0.05). After radiotherapy, β, Eρ, PWVβ increased, AC decreased(P<0.05) compared with before radiotherapy in the radiation group, but IMT had no statistical significance(P>0.05). Logistic regression analysis showed that age(>60 years), time interval of radiotherapy(>5 years) were the risk factors of carotid artery injury in patients with head and neck tumor. Conclusion: The carotid elasticity after radiotherapy in patients with head and neck tumor decreased compared with healthy subjects. E-Tracking parameters can reflect early the abnormal changes of carotid elasticity. Age, time interval of radiotherapy are the risk factors of carotid artery injury after radiotherapy for head and neck tumor patients.

385 The application research of enhanced 3D-FIESTA-c sequence MRI in displaying the preganglionic nerve of the brachial plexus

Objective: To investigate the feasibility and values of enhanced MRI using 3D-FIESTA-c sequence in displaying the preganglionic nerve of the brachial plexus. Methods: Twenty volunteers and 10 patients suffering from brachial plexus preganglionic injury underwent MRI using 3D-FIESTA, unenhanced and enhanced 3D-FIESTA-c sequences, respectively. The displaying number of the nerve bundle of anterior and posterior rootlets of brachial plexus and contrast noise ratio(CNR) and artifacts of images at the three sequences were evaluated. The signs of the normal brachial plexus preganglionic part and its lesions at contrast enhanced MRI using 3D-FIESTA-c sequence were observed. Results: 3D-FIESTA, unenhanced and enhanced 3D-FIESTA-c in number of the nerve bundle of anterior and posterior rootlets of the preganglionic nerve of the brachial plexus(C5~C8, T1) showed statistically significant. Unenhanced and enhanced 3D-FIESTA-c in number of the nerve bundle of anterior and posterior rootlets of the preganglionic nerve of the brachial plexus(C5~C8, T1) were more than 3D-FIESTA-c. And unenhanced and enhanced 3D-FIESTA-c in number of the nerve bundle of anterior and posterior rootlets of the preganglionic nerve of the brachial plexus(C5~C8, T1) had no statistical significance. The CNR of three sequences above were 39.44±6.65, 39.91±6.52, 25.92±6.95, respectively. The CNR of 3D-FIESTA, unenhanced and enhanced 3D-FIESTA-c in the preganglionic nerve of the brachial plexus showed statistically significant. The CNR of 3D-FIESTA, unenhanced 3D-FIESTA-c were more than enhanced 3D-FIESTA-c. And the CNR of 3D-FIESTA and unenhanced 3D-FIESTA-c had no statistical significance. Enhanced sequence 3D-FIESTA-c increased the muscle signal strength in the tissues of background, unfavorable to observe the normal brachial plexus preganglionic part and its lesions. Conclusion: Although unenhanced and enhanced 3D-FIESTA-c in number of the nerve bundle of anterior and posterior rootlets of the preganglionic nerve of the brachial plexus have no statistical significance. Due to their increased muscle signal strength in the tissues of background, the author thinks that it is good to perform 3D-FIESTA-c scanning before the enhancement.

391 CT-guided percutaneous biopsy for lung nodules(≤30 mm): investigatethe factors influencing accuracy rate and puncture security

Objective: To analyze the factors influencing the accuracy rate of CT-guided percutaneous biopsy and the security for lung nodules(≤30 mm): to facilitate the clinical application. Methods: The study has been allowed by the IRB in Shengjing hospital. Retrospectively analyze the clinical and pathological data of 155 patients who had undergone CT-guided percutaneous biopsy and with lung nodules(≤30 mm). The factors which influence the success rate such as gender, age, smoking history, whether the lesion and pleural adhesion, lesion size, puncture depth, times of percutaneous biopsy and the incidence of major complications such as hemorrhage, pneumothorax, will be analyzed. Two senior radiology physicians redetermine whether patients have pneumothorax, bleeding and other complications. If there is disagreement, a third radiologist will make a final diagnosis. Results: Among 155 cases, the accuracy rate of percutaneous biopsy was 90.32%(140/155). For the largest lesion diameter ≤10 mm, >10~20 mm and >20~30 mm, the accuracy rate was 85.71%(6/7), 85.00%(68/80) and 97.06%(66/68), separately, differences reach statistic significance(χ2=6.293, P<0.05). The accuracy rate of lesions with or without lesion and pleural adhesion was 100%(31/31) and 87.90%(109/124), differences reach statistic significance(χ2=4.152, P<0.05). The incidence of pneumothorax and hemorrhage during 4~6 times puncture is much more than that during 3 times or even low, and the difference was statistically significant. The smaller the diameter, the greater the depth of the puncture, the more the times of puncture, the more prone to bleeding, and the difference was statistically significant. Age, gender, smoking history, times of percutaneous biopsy and puncture depth had no impact on the accuracy rate. One hundred and fourteen cases of pulmonary adenocarcinoma, 10 cases of squamous cell carcinoma, 4 cases of pulmonary chronic inflammation, 4 cases of metastatic lung cancer, 3 cases of pulmonary tuberculosis, 1 case of alveolar cell carcinoma insitu, 1 case of pulmonary coccidioidomycosis, 1 case of adenosquamous carcinoma of the lung, 1 case of small cell lung cancer, 1 case of malignant mesothelioma, 15 cases of lung tissue and necrotic tissue. Conclusion: The nodule size and whether with pleural adhesions have a greater impact on lung nodule(≤30 mm) accuracy of CT-guided puncture. With the increase in the times of puncture patients are more likely to have major complications, but all the above two points have no significant correlation between nodule size, nodules whether with pleural adhesions, age, gender, smoking history, depth of the puncture. CT guided percutaneous lung biopsy in diameter less than 30 mm nodules has high accuracy rate, simple operation, safe and practical advantages, which can be used as a major diagnosis method of less than 30 mm lung nodules. Mastering the puncture skills of pulmonary nodules can effectively improve the accuracy rate, and worth promoting in clinical application.

395 Application and diagnostic value of echocardiography in classification of endocardial cushion defect

Objective: To observe the diagnostic significance and application value of echocardiography in detecting endocardial cushion defect(ECD). Methods: We performed a retrospective review of 172 patients with ECD diagnosed by echocardiography in our hospital, and their echocardiographic characters and clinic datas were studied. Results: Among all 172 cases of ECD, 89 cases(51.7%) were with complete type: type A occurred in 58 cases, type B in 7 cases and type C in 24 cases. Five cases(2.9%) were with intermediate type, 8 cases(4.7%) were with transitional type and 70 cases(40.7%) were with partial type. Valve hypoplasia and valve cleft(106 cases, 61.6%) were the most common accompanied endocardial malformations in patients with ECD, the second was secundum atrial septal defect(44 cases, 25.5%). While the complicated endocardial malformations were seen more often in complete type. Twenty-three cases were confirmed by surgery. Conclusion: Echocardiography can make accurate diagnosis about the classification of endocardial cushion defect and the associated abnormalities, and provide comprehensive reference information for making operation strategies.

400 Visual improvement for 3 mm or less coronary artery stent imaging by using monoenergetic imaging of dual-energy CT

Objective: To evaluate the improvement for 3 mm or less coronary artery stent visualization by using monoenergetic imaging of dual-energy CT. Methods: We selected seven 3 mm or 2.5 mm coronary artery stents of different materials to make in-vitro coronary artery model, and then underwent perspective coronary artery scan using dual-energy CT, then reconstruct the two different data(100 and 140 kV) using dual-energy scanning sequence with monoenergetic technique, monoenergetic software was used to post-process with the flowing 9 photon energies: 60~140 keV(interval 10 keV) and average weighted 120 kV images with different data at 100 and 140 simulation of coronary vascular model. Double-blind and independent qualitative evaluation and quantitative assessment including artificial lumen narrowing(ALN), luminal attenuation difference and image noise in the groups were performed, and analysis the differences between the groups. Results: The interobserver agreement for the image subjective quality scores was substantial(Kappa=0.833), the image quality of the 80 keV group was the highest total score, for 31 points, in addition to the difference with 90 keV group and 120 kV group did not reach statistical significance(P>0.05), there was statistically significant difference with the other groups(P<0.05). The ALN value of the 80 keV group was the lowest (36.1%±3.7%), which was much lower than the other groups, and the difference with other groups was statistically significant(P<0.05). But the value of the stent luminal attenuation difference between the ten groups all did not reach statistical significance(P=0.151). The noise value of the 80 keV group was the lowest((12.51±1.27) HU), and the difference with other groups was statistically significant(P<0.05). Conclusion: To visually evaluate 3 mm or less coronary artery stents, the best image quality is to using the 80 keV energy level, more conform to the requirements of clinical work, recommended to use.

405 The pilot research of assessing the efficacy of telbivudin anti-fibrosis treatment in chronic hepatitis B by real-time shear wave elastography

Objective: To assess the effect of telbivudin anti-fibrosis treatment in chronic hepatitis B by using the liver Young’s modulus of patients with chronic hepatitis B tested by real-time shear wave elastography. Methods: Eighty chronic hepatitis B patients with liver fibration(liver biopsies were performed before treatment) were enrolled and divided into 3 groups, group A(S0, S1) which contains 22 patients, group B(S2, S3) which contains 40 patients, and group C(S4) which contains 18 patients. All the patients were under the treatment of telbivudin, and respectively tested the liver Young’s modulus before treatment and 24 weeks after treatment by using real-time shear wave elastography, and the serum test including hyaluronic acid(HA), laminin(LN), type Ⅳ collagen(CL-Ⅳ), serum type Ⅲ procollagen(PⅢPN-P) were measured in all patients. Result: All serum index(HA, LN, CL-Ⅳ and PⅢPN-P) in three groups decrease 24 weeks after treatment vs before, the difference is statistically significant(P<0.05), and Young’s modulus of liver tested by real-time shear wave elastography also decrease 24 weeks after treatment vs before, the difference is statistically significant(P<0.05), Young’s modulus of liver in three groups were group A<group B<group C 24 weeks after treatment and before. Conclusion: Real-time shear wave elastography testing liver flexible modulus could be used in assessing the effect of telbivudin treatment in chronic hepatitis B.

408 The correlation of pancreatic volume and pancreatic fat content in type 1 diabetes mellitus

Objective: To investigate the related factors of pancreatic volume(PV) and pancreatic fat content(PFC) in type 1 diabetes mellitus(T1DM) patients. Methods: We retrospectively reviewed 92 patients who were diagnosed as T1DM in our hospital. And the patients were divided into three groups according to the duration of diabetes(Group A<60 months, 51 patients; Group B 60~<120 months, 20 patients; Group C≥120 months, 21 patients). The difference and correlation among PV, pancreatic fat volume(PFV), PFC, total abdominal adiposity(TA), visceral adiposity(VA), superficial adiposity(SA), VA/SA, body mass index(BMI), duration of diabetes(months), triglyceride(TG), high density lipoprotein(HDL-C), total cholesterol(TC), low density lipoprotein(LDL-C), blood pressure(systolic and diastolic pressure), blood sugar(OGTT fasting and 2 hours after meal), insulin(fasting, 1, 2, 3 hours after meal), C-peptide(fasting, 1, 2, 3 hours after meal) and glycosylated hemoglobin A1c(HbA1c) were analyzed. Results: There was significant difference in age, TG, C-peptide(fasting, 1 hour after meal), PFV and PFC among group A~C(P<0.05); C-peptide(fasting) and PV between group A and group B; Age, C-peptide(fasting, 1 hour after meal), TG, PFV, PFC and VA between group A and group C; TG, PFV and PFC between group B and group C(P<0.05). PV was not correlated with age, C-peptide or duration of diabetes. PFV was positively correlated with VA and positively correlated with BMI. PFV was positively correlated with HDL-C. PFC was positively correlated with duration of diabetes. Conclusion: The change of PV and PFC in T1DM had its own characteristics. PV was not correlated with pancreatic external secretion function. PV decreased in patients with a diabetic duration of more than 5 years and the trend was less obvious in patients with duration of 5~10 years, which was in step with C-peptide. PFC increased gradually with duration of diabetes, which was extremely obvious in patients with duration of diabetes more than 10 years and was in step with TG. PFC was positively correlated with VA and HDL-C. The change of PFC was earlier than VA. C-peptide was correlated with age and BMI. We inferred that the increase of PFC may have some correlation with the function of pancreas cell, which is different from type 2 diabetes mellitus whose PFC is correlated with insulin resistance. Further study is needed to verify this conclusion.

413 Study on contrast-enhanced ultrasonography in quantitative evaluation of rabbit’s cortex hemodynamics after renal ischemia reperfusion injury

Objective: To explore the value of contrast-enhanced ultrasonography(CEUS) in quantitative evaluation of rabbit’s cortex hemodynamic changes after renal ischemia reperfusion injury(RIRI). Methods: All the 54 rabbits were randomly divided into control group and different time groups at 2 h, 16 h, 24 h, 3 d, 5 d after ischemia reperfusion. Using CEUS to observe the pattern of rabbits’ renal perfusion and perfusion peak time(TTP), curve rising slope rate(β), amplitude of peak intensity(A), and area under the curve(AUC). And researched the relativity between quantitative evaluation of rabbit’s cortex hemodynamics by CEUS and renal pathologic changes. Result: Compared with control group, the TTP and AUC extend, β decreased after RIRI(P<0.05). TTP and β had difference in RIRI groups, while the most significant changes happened at 3 d after reperfusion(P<0.01). Compared with control group, the A decreased at 16 h, 24 h, 3 d after RIRI(P<0.05), while the most significant changes happened at 3 d after reperfusion(P<0.01). Conclusion: CEUS combined with quantitative analysis can catch early the rabbit’s cortex hemodynamic changes after RIRI. Renal cortex microcirculation suffered the most severe damage at 3 d after RIRI. It would get better while passed through the crisis. CEUS can reflect these changes sensitively.

417 Assessment of the color M-mode echocardiography on right ventricular diastolic functionin fetuses with umbilical cord around neck

Objective: To investigate the diastolic function of right ventricle of fetuses with umbilical cord around neck, and to analyze the changes of propagation velocity(Vp) of the right ventricle, then to discuss the clinical value of color M-mode echocardiography in quantitatively evaluating ventricular diastolic function in fetuses. Methods: The experimental group consisted of fetuses with umbilical cord around neck in 22 to 38 weeks of gestational age, including 60 cases; The control group consisted of normal fetuses without umbilical cord around neck in 22 to 38 weeks of gestational age, including 60 cases. And analyze the changes of Vp of the right ventricle by color M-mode echocardiography. Results: The Vp of right ventricular showed significantly different between the experimental group and control group(P<0.05), Vp of the right ventricle in fetuses with umbilical cord around neck was significantly lower than the control group, indicating that right ventricular diastolic function reduced in fetuses with umbilical cord around neck. Vp of right ventricular has no significant difference in different gestational fetuses with umbilical cord around neck(P>0.05), indicating that right ventricular diastolic function has no significant changes with gestational age increasing in fetuses with umbilical cord around neck. Conclusion: Vp of the right ventricle in fetuses with umbilical cord around neck was significantly lower than the control group, indicating that right ventricular diastolic function reduced in fetuses with umbilical cord around neck. Vp of right ventricular has no significant difference in different gestational fetuses with umbilical cord around neck(P>0.05), indicating that right ventricular diastolic function has no significant changes with gestational age increasing in fetuses with umbilical cord around neck. Vp measured by color M-mode echocardiography can better reflect the changes of right ventricular diastolic function in fetuses, which was able to provide valuable reference about the ventricular diastolic function in fetuses with umbilical cord around neck.

421 Differential diagnosis of pulmonary atresia, persistent truncus arteriosus and aortic atresia by prenatal echocardiography

Objective: To investigate the differential diagnosis value of pulmonary atresia, persistent truncus arteriosus and aortic atresia by prenatal echocardiography, to improve the diagnostic rate of three kinds of diseases. Methods: We analyzed retrospectively 402 fetal hearts of congenital heart disease in our department. Twelve fetuses were diagnosed as the three kinds of diseases. Results: In all 12 fetuses, 7 fetuses were proved as persistent truncus arteriosus(one case misdiagnosed as pulmonary atresia), 2 fetuses with aortic atresia, 3 fetuses with pulmonary atresia. All of them were accompanied with other heart malformation. Conclusion: Echocardiography is the most important method to diagnose fetus with artery malformation. Although there are lots of common points among these three ultrasonographic performance, each of them has its own characteristics, which could be used for differential diagnosis. The difficulty is the differential diagnosis on pulmonary artery complete atresia and persistent truncus arteriosus.

424 Normative research of measuring method on reconstructed CT images of mandible in molar region

Objective: To establish the normative measuring method on reconstructed CT images of mandible for guiding the dental implant surgical procedure more accurately, the mandibular specimens were measured on reconstructed CT images and compared with the actual data of the specimens. Methods: Spiral CT(Siemens Somatom Spirit) was performed on 30 cases of mandible specimens which were numbered for scan, the normal coronal and sagittal images were reconstructed for measurement of some set line in molar region. Then the adjusted coronal and sagittal images were reconstructed according to the morphological structure of each specimen, and the same set line was measured again. The two series data obtained on two series of reconstructed CT images were compared with the actual data of mandibular specimens. Results: The data get from the adjusted coronal and sagittal reconstructed images were more accurate than that get from the normal coronal and sagittal reconstructed images. Conclusions: The adjusted coronal and sagittal reconstructed images were more useful for measurement of the set line on preoperative CT images in molar region to guide the dental implant surgical procedure.

427 The predictive value of MRI signs and necrosis volume quantitative measurement for non-traumatic osteonecrosis of femoral head

Objective: To investigate the risk factors affecting the collapse of non-traumatic osteonecrosis of femoral head(ONFH). And assess the value of clinical application in predicting the collapse of ONFH. Methods: By the imaging diagnosis of early osteonecrosis(ARCOⅠ, Ⅱ) a total of 112 cases of patients(152 hips) were collected for follow-up observation. All patients underwent initial examination of hip with anteroposterior and frog-position X-ray and MRI scans at the time of diagnosis. To evaluate the collapse of the femoral head at each X-ray examination; observe the hip joint effusion, bone marrow edema, necrosis signal features, location of necrotic lesions and necrotic morphology at the first MRI images, and measure the volume of femoral head necrosis, calculate the percentage of necrosis volume. Nominal data were analyzed with the chi-square test and measurement data were analyzed with independent sample T-test. Logistic regression analysis was used to analyze the risk factors. Results: In 112 cases(152 hips), 62 hips were with femoral head collapse, 90 hips were not. In addition to the difference of sex(P=0.078), age(P=0.631), etiology(P=0.604) and necrotic signal comparison(P=0.071) between the collapse group and non-collapse group, other indicators were statistically significant. The total average of the necrosis volume percentage in the collapse and non-collapse groups were 40.1%±20.1%, 18.3%±19.8%, seperately. In addition to PIM quadrant comparison(P=0.143), the difference between collapse group and non-collapse group was statistically significant. ARCO stage, necrotic morphology, bone marrow edema, and necrosis of the total volume percentage, ASL quadrant necrosis volume percentage, PSL quadrant volume percentage of necrosis were statistically significant(P<0.05). Conclusion: MRI examination and quantitative measurement has important clinical value for evaluation and prediction of the collapse of femoral head, but various factors should be taken into account for predicting collapse of the femoral head, thus improving the accuracy of predictions.
