229 MRI diagnosis of infantile neuroaxonal dystrophy

Objective: To investigate the MRI characteristics of infantile neuroaxonal dystrophy(INAD) by retrospective summary of INAD patients in our hospital, and then deepen the understanding of imaging of this disease to help the early diagnosis. Methods: MR imagings of 13 patients with clinical diagnosis of INAD were analyzed retrospectively, and the MRI features of INAD were summarized. Results: ①Cerebellar atrophy was found in all cases, 8 cases were mild and 5 cases were severe. ②2, 2 and 1 cases were accompanied by cerebrum atropy, brainstem atrophy and corpus callosum atrophy respectively. ③T2-FLAIR high signal was showed in cerebellar cortex in 2 cases. ④T2WI high signal was manifested in supratentoria cerebrum white matter in 5 cases. ⑤T2WI, T2-FLAIR and DWI low signal was showed in bilateral globus pallidus in one case. Conclusion: The MRI imaging of INAD has certain characteristics, which plays an important role in the early diagnosis.

232 Imaging diagnosis of non-Hodgkin’s lymphoma in nasal cavity

Objective: To investigate the imaging features of non-Hodgkin’s lymphoma(NHL) in nasal cavity. Methods: Thirteen cases of primary NHL confirmed by pathology located in nasal cavity were retrospectively analyzed. Results: Of the 13 cases of NHL, 12 cases were NK/T cell NHL and 1 was giant B cell NHL. All cases were located in one nasal cavity unilaterally, either located in the anterior portion or fulfilling the whole nasal cavity. Three cases showed swollen or invasion of the adjacent facial soft tissue, 1 case was accompanied with any invasion of the orbit, and 10 cases were accompanied with sinus inflammation. No bone structure destruction was found in all cases. Conclusions: There are some specific imaging features of the primary NHL in nasal cavities. The conventional CT and MRI examination can supply some diagnostic indications, but the definitive pathological typing still relies on pathology and immunohistochemical methods.

234 Relationship between parameters of biexponential diffusion weighted imaging and nasopharyngeal carcinoma clinical stage

Objective: To explore the correlations between biexponential model(IVIM) of diffusion-weighted imaging parameters and nasopharyngeal carcinoma(NPC) clinical stage, and to evaluate its value in assessing invasion and metastasis of NPC. Methods: Forty-nine newly diagnosed NPC patients received MRI examination and clinical staging was decided based on Chinese 2008 staging system. The correlations between IVIM parameters(ADCstand, ADCslow, ADCfast, Ffast) and clinical stage were analyzed. Results: T stage showed negative correlation with ADCslow(r=-0.356, P=0.012) and positive correlation with ADCfast(r=0.388, P=0.006). ADCstand showed a positive correlation with overall stage(r=0.410, P=0.003), N stage(r=0.313, P=0.029) and M stage(r=0.349, P=0.014). ADCstand showed significant differences in different overall stages, N stages and M stages(P<0.05), and ADCslow showed significant differences in the different overall stages, T stages and N stages(P<0.05). Conclusion: IVIM parameters of primary NPC lesions have significant correlations with clinical stages. IVIM parameters may act as imaging biomarkers for non-invasive assessing of the tumour microstructure, disease level and aggressiveness of NPC.

238 Morphological abnormalities of the internal carotid artery: comparative study on imaging diagnostic methods and relationship with ischemic cerebrovascular disease

Objective: To study the valve of CTA and color Doppler flow imaging(CDFI) in the diagnosis of the internal carotid artery(ICA) morphological abnormalities, and to explore the possible association with ischemic cerebrovascular disease. Methods: A total of 126 patients with ischemic cerebrovascular disease were enrolled in the study, and carotid abnormalities(CA) were classified by CTA and CDFI respectively. The Kappa-test was performed to check the results of the two examination methods with digital subtraction angiography(DSA) as the standard, and the accuracy of diagnosis and classification of CA was evaluated. One hundred and twenty-six cases were divided into three groups(cerebral infarction group, TIA group and asymptomatic group) according to patients’ condition. Relationship between ICA abnormal morphology with three groups were analyzed by chi-square test individually. Results: Results of CTA and CDFI in 38 cases(76 carotid arteries) were compared with DSA diagnosis, respectively, with Kappa=0.95(P<0.001) and Kappa=0.88(P<0.001), showing good consistency. And CTA in the diagnosis of ICA morphological abnormalities was slightly better than CDFI. Results of CTA and CDFI in 126 cases(252 carotid arteries) were compared by Kappa test, with Kappa=0.93(P<0.001), representing high consistency of the two methods. There was significant difference in prevalence of patients with ischemic cerebrovascular disease between three groups(χ2=16.22, P=0.001). The prevalence of distorted ICA of cerebral infarction group and TIA group was higher then asymptomatic group, but there was no significant difference between cerebral infarction group and TIA group. Conclusion: CTA has good agreement with CDFI in assessing ICA morphologic abnormalities. The existence of distorted ICA may be a risk factor for ischemic cerebrovascular disease.

243 The expression of CD31, CD34 in nodular goiter and thyroid adenoma and the correlation with contrast-enhanced ultrasound

Objective: To study the expression of CD31 and CD34 in nodular goiter and thyroid adenoma, and to provide the pathologic bases for differentiating nodular goiter from thyroid adenoma with contrast-enhanced ultrasound. Methods: CEUS process of 30 cases of nodular goiter and 30 cases of thyroid adenoma was retrospectively analyzed, and the time-intensity curve was analyzed by TomTec software. Immunohistochemical staining was performed to evaluate the MVD in surgical specimens. Quantitative parameters and MVD were assessed by the Spearman’s Rank correlation analysis. Results: After enhancement, 23 cases of nodular goiter showed isoechoic, 4 cases of nodular goiter showed hyperechoic, 3 cases of nodular goiter showed hypotonic, 29 cases of thyroid adenoma showed hyperechoic, and 1 case of thyroid adenoma showed isoechoic. There was significant difference in mTT and IMAX. RT and TTP were not statistically significant. Thyroid adenoma had rich microvessels. RT, TTP and mTT showed negative correlations while IMAX showed positive correlation with MVD. Conclusions: CEUS enhancement patterns are different between nodular goiter and thyroid adenoma, and it helps the diagnosis and differential diagnosis.

247 The clinical effect of ultrasound-guided fine-needle aspiration of benign thyroid cysts or partially cystic thyroid nodules

Objective: To observe clinical efficacy of ultrasound-guided fine-needle aspiration(US-FNA) of benign thyroid cysts or partially cystic thyroid nodules. Methods: Clinical data of 98 patients were divided into different groups according to cystic component and ultrasound features. The clinical efficacy was compared and valued according to the change of cyst size and the postoperative adverse reaction rate. Results: The total effective rate of simple cyst group(94.7%) after US-FNA was higher than that of the complexity cyst group(83.3%), and the difference was statistically significant(P=0.001). The clinical efficacy of the dark bloody(90.0%), yellow jell(80.0%) and yellow sticky group(87.5%) after US-FNA treatment was better than that of yellow/colorless group(50%), and the differences were statistically significant(P=0.001, P=0.001, P=0.023). In the comparison of the recurrence rate among groups according to the degree of aspiration, the recurrence rate of the complete group(7.0%) was obviously lower than that of the other two groups(66.7%, 47.0%), and the difference was statistically significant(P=0.001, P=0.001). In the comparison of the postoperative adverse reactions, the incidence of adverse reactions of simple cyst group(10%) was much lower than that of the complexity group(20%) and the difference was statistically significant(P=0.001). Conclusion: The US-FNA is a relatively safe and feasible treatment for thyroid cyst or partly cystic thyroid nodules, especially for the simple cyst and that with aspirates close to gelatin. The postoperative complications are milder, and most can relieve by itself. Aspiration should be done as thoroughly as possible to reduce the recurrence rate effectively.

251 Imaging diagnosis and surgical treatment of four cases of congenital aortic-left ventricular tunnel

Objective: To evaluate the imaging diagnosis and surgical treatment of aortic-left ventricular tunnel. Methods: Four cases(2 males and 2 females) of congenital aortic-left ventricular tunnel(ALVT) were analyzed retrospectively, age ranged from 2 to 5 years. All patients were diagnosed by multi-spiral CTA(MSCTA) and echocardiography. Results: MSCTA and echocardiography showed the tunnel that bypassed the aortic valve and terminated in the left ventricle chamber. There were 1 case of ALVT with type Ⅲ and 3 cases with type Ⅱ. Two cases had a tunnel of 5~6 mm to the outflow tract of left ventricle, moderate aortic regurgitation and mildly dilated ascending aorta, and located at the edge of the aortic root between the left and right sinus approximating to the aortic valve(type Ⅱ). One case had a tunnel of 5~6 mm to the chamber of left ventricle and aneurism-like enlargement of the Valsalva sinus between the left sinus and ante-latus wall of left ventricle(type Ⅱ). One case had a tunnel of 4~5 mm, associated with moderate to severe aortic regurgitation, located at the edge of the aortic root between the right and noncoronary sinus approximating to the aortic valve, with an aneurysm located at the perimembranous ventricular septum sticking out to the outflow tract of right ventricular(type Ⅲ). One case was discharged without surgery. Three cases were operated with better functional results. The operation showed an abnormal tunnel that bypassed the aortic valve and terminated in the left ventricle chamber and showed the dilated ascending aorta with an aneurism-like enlargement of the Valsalva sinus, and the aneurysm sticking out to the outflow of right ventricle could also be seen at the perimembranous ventricular septum. The aortic regurgitation was detected in 2 cases. The aortic-left ventricular tunnel repair and the aortic valvuloplasty were performed. Conclusion: Aortic-left ventricular tunnel is a rare congenital abnormal communication between the aorta and the left ventricle. Since they are often accompanied with moderate to severe aortic regurgitation resulting in myocardial remodeling and congestive heart failure, aortic-left ventricular tunnel should be diagnosed and treated early.

255 Evaluation on left ventricular systolic function using three-dimensional speckle-tracking echocardiography in patients with left ventricular noncompaction

Objective: To evaluate the left systolic function of noncompacted and compacted left ventricular segments in patients with left ventricular noncompaction(LVNC) using three-dimensional speckle-tracking echocardiography(3D-STE). Methods: Thirty-one patients with LVNC(LVNC group) and thirty-one healthy volunteers(Control group) were enrolled in the study. Three-dimensional full-volume image storage was acquired. Parameters of two-dimensional echocardiography and left ventricular global area strain(GAS/AS), longitudinal strain(GLS/LS), radial strain(GRS/RS) and circumferential strain(GCS/CS) between groups and within groups were measured respectively. The correlations of GAS with the above parameters were analyzed. Results: ①Compared with control group, LVEDD, EDV, ESV and SPI of the patients with LVNC increased significantly(P<0.01), while IVSd, LVPWD and EF reduced significantly(P<0.01). ②Compared with control group, GAS, GLS, GRS and GCS reduced significantly(P<0.01). ③AS, LS, RS and CS at the level of the papillary muscle and the apical of noncompacted myocardium in patients with LVNC reduced compared with those of compacted segments at the same level(P<0.05). ④It showed a positive correlation between GAS and EF, and negative correlations between GAS and EDV as well as ESV and SPI. Conclusion: Left ventricular systolic function reduces in patients with LVNC with the most obvious reduction in the noncompacted segmental systolic function. 3D-STE can accurately assess left ventricular function in LVNC patients, which is a noninvasive and sensitive tool to detect regional systolic dysfunction of noncompacted myocardium.

260 DCE-MRI in differential diagnosis of liver lesions

Objective: To evaluate the value of quantitative parameters of DCE-MRI in the diagnosis of liver lesions. Methods: Fifty-five patients of liver tumors underwent MRI examination. MR scanner was used to obtain the quantitative MR pharmacokinetic parameters: Ktrans(volume transfer constant), Kep(exchange rate constant) and Ve(extravascular extracellular volume fraction). The mean Ktrans, Kep and Ve of liver cancer, liver hemangioma and liver metastasis were calculated and compared with each other using LSD method. Receiver operating characteristic curve(ROC) analysis was used for comparing the difference between liver cancer and liver hemangioma. Results: There were significant differences in Ktrans and Ve(P<0.05) between liver cancer and liver hemangioma, liver hemangioma and liver metastasis, liver cancer and liver metastasis, and also in Kep(P<0.05) between liver hemangioma and liver cancer, liver cancer and liver metastasis. There was no significant difference between liver hemangioma and liver metastasis in Kep(P>0.05). The sensitivity of Ktrans, Ve, and Kep were 88.2%, 82.4% and 94.1% respectively, and the specificity of Ktrans, Ve and Kep were 81.8%, 59.1% and 81.8% respectively for the differential diagnosis of liver cancer and liver hemangioma. The sensitivity of Ktrans, Ve and Kep were 68.2%, 68.2% and 50.0% respectively, and the specificity of Ktrans, Ve and Kep were 81.2%, 87.5% and 81.2% respectively for the differential diagnosis of liver hemangioma and liver metastasis. The sensitivity of Ktrans, Ve and Kep were 100.0%, 82.4% and 100.0% respectively, and the specificity of Ktrans, Ve, and Kep were 93.7%, 93.7% and 93.7% respectively for the differential diagnosis of liver hemangioma and liver metastasis, with the maximum Youden’s index as cut-off. Conclusion: The differential diagnosis of liver lesions by Ktrans, Kep derived from DCE-MRI is valuable.

264 The significance of contrast-enhanced ultrasound in the differential diagnosis of atypical liver abscess

Objective: To investigate the enhancement mode and to evaluate the qualitative diagnosis of contrast-enhanced ultrasound of atypical liver abscess. Methods: The contrast-enhanced ultrasound features of 33 cases of atypical liver abscesses were compared with contrast-enhanced CT retrospectively. Results: The “Rosette sign”, “Honeycombing sign” and transient enhancement sign were found in atypical liver abscesses. The measurements of lesions could increase, diminish, or basically remain unchanged. The sensitivity, specificity and diagnosis accuracy of atypical liver abscesses were not statistically different between contrast-enhanced ultrasound and contrast-enhanced CT(P>0.05). Conclusion: Contrast-enhanced ultrasound has higher value in the diagnosis of atypical liver abscesses, and combination with enhanced CT can improve the diagnostic accuracy significantly.

267 Evaluation of the clinical value of 640-slice spiral CT angiography and 3.0T dynamic MR angiography in the diagnosis of spinal vascular malformations

Objective: To evaluate the clinical value of 640-slice spiral CT angiography(CTA) and 3.0T dynamic MR angiography(CE-MRA) in the diagnosis of spinal vascular malformations. Methods: Nineteen cases of clinically suspected spinal vascular disease patients, including 11 cases with CTA examinations, 10 patients with CE-MRA examination, 16 with DSA treatment and 3 with surgery were studied. Results: Four of the 11 cases with CTA examinations were SDAVF, 4 were PMAVF and 2 were SCAVF. Eight were consistent with the DSA diagnosis, and the DSA of one case was negative. Seven of the 10 cases with MRA examinations were SDAVF, in 2 of which CTA examinations did not display the artery and the fistula, but CE-MRA showed the artery and the fistula well. Two cases were PMAVF and 1 case was SCAVM, all consistent with the DSA diagnosis. Conclusion: 3.0T CE-MRA is better than CTA in displaying the spinal vascular malformations artery and fistula, and in diagnosing of spinal vascular malformations. CTA spinal angiography has a great value in the diagnosis of the spinal vascular malformations, and can be used as a screening examination prior to the DSA to guide the DSA. While 3.0T CE-MRA can display the spinal vascular malformations artery and fistula quickly, noninvasively and clearly. CTA and CE-MRA are of great value in diagnosing the spinal vascular malformations, in assisting determining the treatments and in the follow-up visits, etc.

271 Evaluation of posterior fossa in normal fetuses at the 11~13+6 gestational weeks

Objective: To establish the normal reference ranges of the posterior fossa structure in normal fetuses at the 11~13+6 gestational weeks. Methods: Two hundreds and thirty-eight normal fetuses at the gestational age 11 to 13+6 weeks were selected. Maternal and fetal factors were analyzed which would affect the posterior fossa’s appearance. The parameters of the posterior fossa were measured in mid-sagittal plane. We compared the parameters with the crown-rump length(CRL), used linear regression analysis to get the linear regression equation and drew scatter diagram. Thirty randomly selected cases were reviewed for intraobserver and interobserver reliability. Results: In the 238 normal fetuses, intracranial translucency was seen in 222(93%). The main factors influencing the image quality were obesity(n=13), fetal position(n=10), retroverted uterus(n=5) and cesarean scar(n=3). With increasing CRL, there was a significant increase in brain stem diameter, intracranial translucency diameter and brain stem-occipital bone diameter. The brain stem diameter-to-brain stem-occipital bone diameter ratio decreased with increasing CRL. Intraobserver intraclass correlation coefficients of BS, IT, BSOB were 0.621 3, 0.696 2 and 0.936 1. Intreobserver intraclass correlation coefficients of BS, IT, BSOB were 0.581 3, 0.812 5 and 0.960 1. Conclusions: To obtain clear fetal posterior fossa structures’ sonogram, we not only need to standardize the training of the ultrasound physician, but also need to avoid the influence of maternal and fetal adverse factors on image quality. The measurement of posterior fossa structures is reproducible, and it will be useful for the future of early pregnancy screening for fetal malformation.

275 Duplex Doppler ultrasonography on the blood flow of fetal hepatic vein and inferior vena cava

Objective: To study the Doppler flow spectrum of fetal hepatic vein and inferior vena cava and determine the gestational age-related reference range of their preload index. Methods: Doppler measurements were obtained for the 4 veins of 118 normal fetuses at the 26~40 week of gestation. A and S values were measured from the recorded flow velocity waveform of each vein and the A/S ratio was calculated and analyzed as the preload index. Results: There was a three-phase blood flow spectrum which could be observed in the hepatic vein and the inferior vena cava. The A/S ratios decreased gradually with the increase of the gestational age, which were less than 0.5. There was no significant difference between the four regression equations. Conclusions: The flow indexes of fetal hepatic vein and inferior vena cava are statistically similar, which may be utilized as an index for the estimation of fetal cardiac function.

300 Paraganglioma of tympanic body: report of one case
