Location of periventricular nodular heterotopia and its relationship with associated brain abnormalities on MRI

Objective: The purpose of this study was to determine whether specific associated brain abnormalities may be related to the location of periventricular nodular heterotopia(PNH) assessed by MR imaging. Methods: MR images of 40 patients previously diagnosed as PNH were retrospectively analyzed. Relationship between the location, numbers and sides of PNH and associated brain abnormalities were tested by using χ2 test. Results: Of the 40 PNH cases, anterior PNH(aPNH) was found in 12 patients(30%), posterior PNH(pPNH) in 13(32.5%), and diffuse PNH(dPNH) in 15(37.5%), cortical malformations were present in 18 patients. Associated cortical malformations were significantly more frequent in pPNH(92.3%) compared with aPNH(8.3%) and dPNH(33.3%), there were significant differences among the three groups(P<0.001). Abnormalities of the corpus callosum present in 9 patients, 5 cases in dPNH, 3 cases in pPNH and 1 case in aPNH, there were significant difference among the three groups(P<0.05). Reduced volume of cerebral white matter was noticed in 17 patients. A significant difference in the incidence of reduced white matter volume was found between aPNH(16.7%) and pPNH(53.8%, P<0.05) and between aPNH and dPNH(53.3%, P<0.05). Conclusion: PNH occurs in different locations that tend to be associated with different types of malformations. Among these location, pPNH is particularly distinct because of its significant association with malformations of the cerebral cortex and anomalies of corpus callosum.

Diagnostic value of 18F-FDG PET/CT in recurrent and metastatic breast cancer

Objective: To investigate the clinical value of 18F-FDG PET/CT in the diagnosis of recurrence and metastasis in post-operative breast cancer. Methods: From January 2008 to December 2012, whole body 18F-FDG PET/CT imaging with attenuation correction was performed in 58 female patients with suspected recurrent or metastasis of breast cancer, age ranged from 32yrs old~79yrs old with an average of 52.7 years old. The primary breast lesions were all confirmed by surgery and pathology. Among these cases, 2 were ductal carcinoma in situ, 1 case was breast invasive lobular carcinoma, 1 was medullary carcinoma, 1 was phyllodes tumor, the rest were invasive ductal carcinoma. Results: Among the 58 cases, 7 cases were recurrence, 34 cases were of metastasis. The range of SUVmax of the local recurrent lesions was 3.64~18.70, with an average of 8.04. The sensitivity, specificity, accuracy, positive predictive value and negative predictive value of 18F-FDG PET/CT in the diagnosis of breast cancer recurrence and metastasis were 97.50%, 88.89%, 94.83%, 95.12% and 94.12%, respectively. Conclusion: 18F-FDG PET/CT imaging can accurately diagnose breast cancer recurrence and metastasis, can provide important information for clinical establishment of a right treatment plan, and is valuable in monitoring of breast cancer recurrence and metastasis.

Application of speckle tracking imaging in evaluating left ventricular rotation and torsion inpatients with acute myocardial infarction

Objective: To evaluate the value of speckle tracking imaging(STI) in detecting left ventricular(LV) rotation and torsion after percutaneous coronary intervention(PCI) in acute myocardial infarction patients with non ST-segment elevation(NSTEAMI) and with ST-segment elevation(STEAMI) NSTEAMI and STEAMI using STI. Methods: Nineteen patients with NSTEAMI(group A) and 31 patients with STEAMI(group B), who had acute myocardial infarction(AMI) for the first time and have been treated by PCI, were enrolled in this study. High frame rate two-dimensional images were collected from LV short views at the basal level and the apical level, and the parameters of rotation and torsion were calculated using STI in those patients at one week and three months after PCI. So were the 27 normal controls(group C). Results: The apex was counterclockwise movement and rotation value was positive, and the base was clockwise movement and rotation value was negative in group A, B and C. Compared with group C, the values of apical rotation, basal rotation and LV twist were decreased in group A and STEAMI group at one week after PCI(P<0.05), especially in group B. Those parameters were increased in group A and B at three months after PCI, and the apical rotation in part of segments was significantly improved in group A compared with one week after PCI(P<0.05). Conclusion: The rotation of apex and base and LV twist were severely damaged in patients with different transmural extent of myocardial necrosis. There are significant improvements of those parameters in group A than that in group B after PCI for a long time.

Comparative study of CTA between isolated peripheral arterial dissection and aortic dissection

Objective: To investigate the different imaging features between isolated peripheral arterial dissection and aortic dissection. Materials and Methods: CTA images were retrospectively analyzed for isolated peripheral arterial dissection in 22 cases and aortic dissection in 25 cases, and then imaging features were compared. Results: The true lumen, false lumen and involved length of arterial dissection could be clearly distinguished from CTA images of aortic dissection and isolated peripheral arterial dissection. Most patients of the two groups could be found varying degrees of atherosclerosis. Between the two groups, there were statistically significant difference in the ratio of false-true lumen, the involved length of artery and the incidence of atherosclerosis, P<0.05. Conclusion: Isolated peripheral artery dissection often occurred on the basis of atherosclerosis, and the affected artery was limited, and the true lumen is often greater than the false lumen. Its condition was relatively stable, and its imaging features existed obvious differences from aortic dissection.

Quantitative analysis of early liver fibrosis in rats by 13N-NH3·H2O PET/CT

Objective: To investigate the value of 13N-NH3·H2O PET/CT in quantitative diagnosis of early liver fibrosis. Methods: Using 13N-NH3·H2O as imaging agent and acetazolamide(ACZ) as inhibitor, 5 rats of control group and 12 rats of liver fibrosis(7 of stage S1 and 5 of stage S2+S3+S4) undertook dynamic PET/CT scan. With data reconstruction and imaging analysis, we calculated the uptake rate of 13N-NH3·H2O by livers in the two groups at time points of 20 s, 90 s, 5 min, 10 min, 15 min, 20 min and 25 min. Blood perfusion, interstitial diffusion and 13N-NH3·H2O uptake were compared between the two groups at different time points. Results: The SUV of S0 at time points of 10 min and 15 min, S1 at 20 s, S2+S3+S4 at 10 min, 15 min, 20 min and 25 min showed statistical difference before and after using the inhibitor(ACZ). The SUV of each group at any time point had no obvious difference. Conclusion: 13N-NH3·H2O PET/CT combined with ACZ can diagnose early liver fibrosis and staging of liver fibrosis.

The initial clinical study of acoustic radiation force impulse imaging in quantitative evaluatingthe degree of primary biliary cirrhosis

Objective: To investigate the clinical diagnostic value of the acoustic radiation force impulse (ARFI) technique in the quantitative evaluation of primary biliary cirrhosis(PBC). Methods: Fifty eight patients with PBC and 30 normal healthy controls were examined using ARFI technique. ARFI was compared by correlation with liver pathological grades in PBC. Referring to the histological stage on liver biopsy, the ARFI value was assessed by using receiver operating characteristic(ROC) curve analysis. Results: The parameter of ARFI was significantly higher in PBC group((2.05±0.70) m/s) than that in control group((1.02±0.18) m/s, P<0.01). The liver stiffness at ARFI sono-elastography were (1.64±0.58)m/s for the patients with group Ⅰ, (1.83±0.53) m/s for those with group Ⅱ, (2.37±0.65) m/s for those with group Ⅲ, and (2.68±0.67) m/s for those with group Ⅳ, respectively. This results showed that the ARFI values were significantly different(P<0.01) among the different stages of PBC. A positive correlation was found between the stiffness of the liver and the pathological grade of PBC(r=0.57, P<0.01). The ARFI cut-off that best distinguished group of cirrhosis from patients with PBC was 2.05 m/s(accuracy: AUC=0.81). Conclusion: ARFI technique not only can examine the stiffness of the liver noninvasively, but also has potentially valuable for quantitative evaluating the degree of PBC.

Correlation research between pancreatic fat accumulation and abdominal fat through 64 rows of VCT

Objective: To measure and analyze the amount of the fatty tissue in pancreas and abdominal fat content through 64 multislice VCT. Try to find out the relationship between abdominal obesity and pancreatic fat accumulation. Materials and Methods: The measurements of the relevant parameters in 81 healthy adults through 64 multislice VCT were analyzed retrospectively. The 81 healthy adults were divided into 3 groups that is normal, overweight and obesity groups, according to the body mass index(BMI). Firstly, the density of the pancreas and pancreatic volume were analyzed. Then the correlation between the density of pancreas and other related parameters were analyzed. Results: The density of pancreas in the normal group was obviously higher than those of the overweight and obesity groups, the difference was statistically significant(P<0.01). The difference of the pancreatic density between the overweight and obesity group also had statistical significance(P<0.05). The average density of the pancreas in the 81 healthy adults was (42.41±42.41) HU which was significantly negative correlated with age, abdominal fat content(P<0.001), but the correlation of the density of pancreas and the subcutaneous fat volume had no statistical significance(P>0.05). Conclusion: With the increase of abdominal fat, the pancreas volume change is not obvious, but the reduce of density of the pancreas is significant. Thus it is obvious that the degree of pancreas fatty infiltration increase closely with abdominal obesity.

Diagnostic value of prenatal ultrasound for fetal holoprosencephaly

Objective: To investigate the clinical significance of antepartum ultrasound in holoprosencephaly(HPE) diagnosis. Methods: A total of 34 fetuses diagnosed as HPE from 2007 to 2013 in our hospital. Sonographic features and postpartum results were analysed retrospectively. Results: Among the 34 fetuses, 21 cases were diagnosed as alobar HPE, 5 cases as semilobar HPE, and 8 as lobar HPE. The characteristic ultrasound findings in alobar HPE were related to the single primitive ventricle, fused thalami, and absence of midline structures; semilobar HPE were related to the single primitive ventricle, partial fused thalami and partia1 absence of midline structures; and lobar HPE were related to the absence of septum pellucidum cavity, and fused anterior horn of lateral ventricle. There were 23 cases combined with other craniofacial malformations, 8 with cardiovascular malformations, 4 with kidney malformations, 1 with spina bifida, 6 with limbs abnormality, 3 with subcutaneous edema, 6 with single umbilical artery, 1 with umbilical cord cyst, 2 with absence of stomach, 5 with polyhydramnios and 2 with oligohydramnios. Ten karyotype analysis resulted in 3 Trisomy 13, 1 Triploid and 5 normal. Conclusions: Prenata1 ultrasound is a useful tool in diagnosing HPE with highly accuracy. It is necessary to undergo antepartum ultrasound for the pregnant women. The antepartum ultrasound is an effective method to screen HPE.

The application of transvaginal color Doppler ultrasound in the diagnosis and treatment of cesarean scar pregnancy

Objective: To explore the adoption valuation of transvaginal color Doppler ultrasonography flow imaging(TV-CDFI) in the diagnosis and treatment of cesarean scar pregnancy(CSP). Methods: The characteristics of TV-CDFI and the different methods of CSP treatment including color Doppler ultrasound-guided punctures(CDUGP) from March 2010 to June 2013 in 45 cases were analyzed. Results: Forty-five cases with CSP were divided into 3 types: gestational sac type(19 cases), mass type(23 cases), mixed type(3 cases). The patients received such treatments as drug intervention by CDUGP(16 cases), curettage of uterine cavity under ultrasound-guidance(3 cases), conservative drug treatment(6 cases), hysteroscopic or laparoscopic treatment and curettage of uterine cavity(12 cases), transuterine artery chemotherapy and embolization(7 cases), transabdominal CSP lesion removal(1 case). Thirty-nine cases were diagnosed correctly, diagnostic accuracy was 86.7%, 6 cases were misdiagnosed, including uterine pregnancy threatened abortion(2 cases), inevitable abortion(1 case), missed abortion(1 case), uterine pregnancy incomplete abortion(1 case), cervix pregnancy(1 case). Conclusions: TV-CDFI is a reliable method for early diagnosing CSP, drug intervention treatment by CDUGP is an effective therapeutic method.

CT and MR diagnosis of elastofibroma dorsi

Objective: To assess the value of MSCT and MR in the diagnosis of elastofibroma dorsi(EFD). Materials and Methods: Eight patients with elastofibroma dorsi confirmed by surgery and pathology enrolled into this study. All patients underwent plain chest CT, while 2 underwent contrast enhanced CT and 1 underwent plain chest MR. Clinical features and imaging(CT and MR) characteristics of EFD were analyzed. Results: Eight patients were all female. Five patients had ten lesions bilaterally, on right side 2 patients, on left side 1 patient. All lesions located in the deep subscapular muscle at the scapular angle area in the back. Totally 13 lesions were found on CT. Masses were flat mound-like or semi-circular in shape, mainly were muscular-like density, and fat densities were seen along the long axis of masses, presenting as stripes in all lesions. The masses had irregular edges, and with no clear boundary between adjacent ribs and skeletal muscle. All 4 lesions did not enhance. 1 mass had inhomogeneous signal intensity on MR with intermingled muscle and fat intensities. Conclusion: EFD has its characteristic imaging findings, a definite diagnosis can be made in most patients with CT and MR images.

A prospective study on the X-ray features and staging of femoroacetabular syndrome

Objective: To evaluate the characteristic X-ray features of femoroacetabular impingement(FAI) syndrome and also prospectively establish a diagnostic and staging criteria for FAI syndrome. Methods: Combined with X-ray plain film of the hip joint and scoring method, the X-ray features of the hip in 31 patients with FAI and 3 patients after hip surgery were analyzed for classification and staging. Results: ①X-ray plain film can clearly show the characteristic changes of the hip joint in FAI syndrome. By quantifying X-ray features, 11 items were set up for measurement, scoring and staging of FAI. Low scoring of FAI, the pathological changes were also early and mild. ②Among the 31 FAI patients, 9 patients belong to Cam type impingement, 7 patients were pincer type impingement, 15 patients belong to mixed type. Conclusion: The main X-ray features of FAI are abnormal appearances of the acetabulum, femoral-neck junction. Analysis of the morphological deformation and measurement of the 11 items are valuable for the staging of FAI syndrome.
