Application of ICA-R algorithm in resting state fMRI

Objective: In order to facilitate the functional magnetic resonance imaging(fMRI) data processing using classical independent component analysis(ICA) method, ICA with reference(ICA-R) algorithm is proposed to reduce the computational load and to increase calculating efficiency. Methods: The signal amplitude information of the default network is introduced to ICA-R algorithm as a reference signal to investigate the orientation of activation in the resting state fMRI data. Results: Experiments with synthetic signals and real fMRI data demonstrate the efficacy and accuracy of the proposed algorithm. Conclusion: ICA-R algorithm could overcome the weakness of inefficiency of classical ICA method.

A voxel-based MRI morphometric study of Parkinson’s disease

Objective: To investigate the abnormal region of gray matter and its’ distribution in Parkinson’s disease(PD) patients using voxel-based morphometry(VBM) method. Method: Twenty-five cases of clinically defined PD patients and 23 cases of healthy controls received MR scan on 3.0T MR system. The T1WI data were collected. Using VBM method to compare the images of brain gray matter between two groups. The results were shown by statistical parametric mapping. Result: PD patients showed extensively decreased gray matter volume in widespread brain regions involving bilateral insular, parahippocampal gyrus, amygdale and right frontal, temporal, occipital, parietal lobes and left medial frontal gyrus. Conclusion: The VBM could thoroughly and accurately reflect the abnormal area of brain gray matter quantitatively which were common in PD patients, and it also provided the necessary neural imaging evidence for the pathological mechanism of PD patients with multiple system involvement.

Value of ultrasound in the diagnosis of medullary thyroid carcinoma

Objective: To analyze the ultrasonographic(US) features of thyroid medulary carcinoma(MTC), and to discuss the diagnostic value of US. Methods: Between January 2000 and December 2012, 51 malignant lesions proved to be MTC by histopathology in 39 patients were included in the study. Results: An anteroposterior/transverse diameter ratio less than one was seen in most of the nodules(80%). Irregular shape was seen in 44 nodules(86%). Clear border was seen in 32 nodules(63%). Forty-eight nodules were hypoechoic(94%). Calcification was seen in 33 nodules(62%), most of which was microcalcification(52%, 17/33). Degeneration was relatively infrequent. Pattern Ⅳ color flow image was seen in 28 nodules(55%). Pattern Ⅱor Ⅳ in real time elastography was seen in 6 nodules(50%, 6/12). Fourteen patients had cervical metastatic nodules(36%, 14/39). Conclusion: Some US features of MTCs were similar with papillary thyroid carcinoma or follicular tumors.

Detection of iodine and air distribution in abnormal pulmonary perfusion using spectral CT imaging

Objective: To explore the feasibility of using spectral CT to evaluate the distribution of iodine and air in pulmonary perfusion disorders with different pathological changes. Materials and Methods: Seventy patients suspected of pulmonary embolism underwent CT scanning with GE Discovery CT 750HD scanner. 70 keV CTPA images, iodine-based material decomposition images and Min IP images were acquired. Location, type and number of pulmonary emboli were observed on CTPA. Location and type of abnormal perfusion were observed on iodine-based material decomposition images, and iodine content was calculated. Aeration of abnormally perfused lung parenchyma was analyzed based on Min IP images. Results: In 33 normal persons, CTPA showed no pulmonary embolism, with even iodine distribution. Iodine content in anterior, middle and posterior regions of lung parenchyma showed gradient distribution from ventral to dorsal. There was no obvious difference between left and right lungs(P>0.05) for iodine, and air distributed homogeneously on Min IP. In 18 patients with pulmonary embolism, a total of 110 emboli were found, including 38 occlusive clots and 72 non-occlusive clots. Thirty-five cases with non-occlusive clots showed reduced perfusion(iodine content 0.62 mg/mL), and the air content presented no obvious increment. Perfusion exhibited noticeable reduction in all cases with occlusive clots(iodine content 0.13 mg/mL), and the air content increased on Min IP. Five patients with pulmonary ground-glass opacity showed areas of hyper-perfusion(iodine content 2.56 mg/mL) and reduced aeration on the Min IP. Six patients with pulmonary hypertension showed diffuse perfusion defect of iodine(iodine content 0.48 mg/mL) but normal CTPA. On Min IP, areas of increased aeration were seen, corresponding to the areas of reduced perfusion. In 4 cases of diffuse emphysema, the iodine content was 0.20 mg/mL. Areas with markedly reduced density on the Min IP matched those with reduced perfusion on the iodine maps. In 4 cases of interstitial fibrosis, reduced perfusion(iodine content -0.07 mg/mL) of the pulmonary parenchyma was found, together with reduced aeration on Min IP. Conclusions: Spectral CT imaging is able to quantitatively measure the content of iodine and the distribution of gas in lungs with different pathological changes, which can be used as a visualization of the pulmonary blood flow/ventilation changes.

Feasibility of low concentration contrast medium in coronary CT angiography

Objective: To explore the feasibility of using low concentration contrast medium(270 mgI/mL) with the low tube voltage in coronary artery CTA. Materials and Methods: Ninty patients with coronary CTA examination were divided into group A and group B randomly. Group A(n=60) used contrast medium of 270 mgI/mL with sinogram-affirmed iterative reconstruction(SAFIRE). Group A was further divided into group A1(n=25) and A2(n=35) according to body mass index(BMI). Group A1(BMI<24 kg/m2) 80 kV was used for scanning, group A2(BMI≥24 kg/m2) 100 kV. The concentration of contrast medium and the tube voltage of group B(n=30) were 370 mgI/mL and 120 kV. All groups were scanned in prospective ECG-trigger gated. Radiation dose was recorded and image quality was analysed subjectively and objectively. Results: The effective dose(ED) of group A1, A2 and B were (2.00±0.56) mSv, (2.35±0.74) mSv and (3.98±1.78) mSv respectively, between group A1 and B, group A2 and B there were statistically significant(t=-5.34 and -4.95, P<0.05). In image quality, there were no statistical differences between each groups in the excellent rate(χ2=1.10 and 2.04, P>0.05). The SD, SNR and CNR of images also showed no statistical differences by objectively evaluated. Conclusion: Using low concentration contrast medium(270 mgI/mL) with low tube voltage in coronary artery CTA is feasible, especially in overweight(BMI≥24 kg/m2) population. At the same time, the radiation dose and concentration of iodine injection are reduced significantly.

The value of real-time elastography in evaluating the effect of radiofrequency ablativetreatment in liver lesions: an experimental study

Objective: To investigate the elasticity change of ablative lesions resulting from radiofrequency ablation(RFA) in vitro liver and in vivo liver by real-time elastography(RTE), evaluate RTE in depicting circumscription of ablative lesions. Materials and Methods: Five in vitro porcine livers and twenty rabbits livers were chosen for this study. Twenty ablative lesions were obtained using RFA under ultrasound guidance in each experiment. After RFA, RTE was performed. The maximum vertical diameter and transverse diameter of ablative lesions were taken on RTE image and compared with the measurements obtained from pathologic examination. Results: Homogeneous blue and distinct circumscription from liver tissue was showed on RTE image of all ablative lesions performed RFA in experiments. Pearson correlation analysis demonstrated that the area of ablative lesions measured on RTE image performed after RFA correlated well with the measurement on macropathology specimen(r=0.91, P<0.05) in vitro liver experment. And Spearman correlation analysis demonstrated that the area of ablative lesions measured on RTE image performed after RFA correlated well with the measurement on macropathology correlated(r=0.49, P<0.05) in vivo liver experment. Conclusions: RTE may be reliable for depicting the circumscription of the RFA lesions. RTE may be the effective method for evaluating RFA lesions.

Quantitative analysis of ultrasonic elastography in the diagnosis of chronic allograft nephropathy:a clinical application study

Objective: To investigate the value of ultrasonic elastography(UE) 5 scores method, strain ratio determination and quantitative analysis in the diagnosis of chronic allograft nephropathy(CAN) after transplantation. Methods: Forty-six patients with renal transplantation were included in the study, including 26 patients with stable renal function(SF group) and 20 patients with chronic allograft nephropathy and renal insufficiency(CAN group). UE 5 scores method, strain rate ratio determination and quantitative analysis were used to analyze differences in and between groups. Result: UE sonography in the CAN group showed almost all green with spotty blue in renal column and pyramid, while the cortex was most or totally blue. UE sonography in the SF group showed green in pyramid and cortex evenly and red in perirenal fat and arcuate arteries. UE 5 scores method showed that all were below 2 scores in SF group, but all were above 2 scores in CAN group, of whom eighty percent(16/20) were equal to or above 3 scores(including 3 scores). There were significant differences in B/A, mean and AREA% between SF and CAN group. Conclusion: UE quantitative analysis techniques can be used for the diagnosis of CAN. It should be a new, non-invasive and convenient imaging method for monitoring renal function.

MRI evaluation of medial meniscus subluxation and knee injury

Objective: To evaluate the relationship between medial meniscus subluxation and knee injury by MRI. Methods: Knee MRI of 77 patients with medial meniscus subluxation was analyzed retrospectively. Results: ①Ligament injury: 26 cases had anterior cruciate ligament rupture(4 complete rupture and 22 partial rupture), with a consistency of 88.8% with the arthroscopic diagnosis; 26 cases had posterior cruciate ligament rupture(3 complete rupture and 23 partial rupture), with a consistency of 95.0% with the arthroscopic diagnosis; 17 cases had medial collateral ligament injury(1 grade Ⅲ, 13 grade Ⅱ and 3 grade Ⅰ injury); 13 cases had lateral collateral ligament injury(8 grade Ⅱ and 5 grade Ⅰ injury), with a consistency of 90.0% with the arthroscopic diagnosis. ②Meniscus injury: 73 cases had medial meniscus injury(50 degree Ⅲ, 13 degree Ⅱ and 10 degree Ⅰ injury); 30 cases had lateral meniscus injury(10 degree Ⅲ, 8 degree Ⅱ and 12 Ⅰ degree injury), with a consistency of 90.5% with the arthroscopic diagnosis; 46 cases had posterior root injury of the medial meniscus, with a consistency of 92.21% with the arthroscopic diagnosis. ③Cartilage damage: 68 cases had cartilage damage(1 grade 4, 21 grade 3, 46 grade 2 damage), with a consistency of 73% with the arthroscopic diagnosis. ④Bone hyperplasia: 40 cases had osteophyte formation. Conclusion: Medial meniscus subluxation is an important indirect sign of ligament injury, meniscus injury, cartilage injury and osteoarthritis. MRI diagnosis of medial meniscus subluxation is significant for the detection of knee injuries, and can effectively prevent knee osteoarthritis initiation and progress.

Molecular imaging of aquaporin by DWI-MRI with multiple b values: mechanism and method

Objective: To study the mechanism and method of molecular imaging of aquaporin by DWI-MRI with multi-b-values. Materials and Methods: On 3T MRI, multi-b-value(0~4 500 s/mm2) DWI was performed for water, contrast agent, whole blood, plasma and red blood cells using acetazolamide as an inhibitor of water channel protein on red cell membrane. Effects of the inhibitor on red blood cell morphology and signal variation of different tissues in multi-b-value imaging were observed. Results: Acetazolamide increased the diameter of red blood cell by more than 10%. B value in the range of 0~200 s/mm2 could distinguish different tissues. B value in the range of 300~1 500 s/mm2 couldn’t distinguish different tissues. When b value was greater than 1 700 s/mm2, it showed the information of water channel protein on red cell membrane. Conclusion: In DWI-MRI with multiple b values, low b value imaging has tissue specificity, but high b value can achieve aquaporin molecular imaging.

Investigation impact of dual model iterative reconstruction method(iDose4) onthe CT image quality and dose reduction—a phantom approach

Objective: To investigate the impact of iterative reconstruction method(iDose4) on the CT image quality and dose reduction by using phantom studies. Methods: A Catphan phantom was scanned by a 256-slice CT(Philips Brilliance iCT) with varied mAs from 20 to 200, the axial images were reconstructed by filtered back projection(FBP) and iDose4 with a level of 1 to 6. Image quality was assessed with following aspect: CT value accuracy and consistency, image noise and signal-noise-ratio(SNR), low density resolution. Meanwhile the volume CT dose index CTDIvol and Dose length product DLP were recorded for dose evaluation. Results: The axial image CT values has a high degree of consistency(P>0.05) for both iDose4 and FBP reconstruction method. The image noise can be reduced in different degree with different iDose4 level 1~6 reconstruction(9.46%~43.30%). When tube current reduced to 40%~50%(80~100 mAs), iDose4 level 6 got image without significant difference regarding to the image noise compared to 200 mAs FBP image(P>0.05); under the conditions of 50%~60% initial tube current(100~120 mAs), low density resolution could remain unchanged. Conclusion: iDose4 can significantly reduce image noise and improve image quality, under the condition of 40%~50% scanning dose can get satisfactory image quality. Thus iterative reconstruction has larger space to lower the scanning dose.
