77 RT-3DE in quantitative assessment of left ventricular function after aspiration thrombectomy and percutaneous coronary intervention for patients with acute myocardial infarction

Objective: To investigate the change of global and regional left ventricular systolic function after aspiration thrombectomy in percutaneous coronary intervention(PCI) or single PCI for patients with acute myocardial infarction by real-time three-dimensional echocardiography(RT-3DE). Methods: Fifty-five subjects with acute myocardial infarction were studied by RT-3DE. And they were divided into two groups with aspiration thrombectomy or without PCI. Then respectively compared the global and regional diastolic volumes(EDV, rEDV), the global and systolic volumes(ESV, rESV), the global and ejections fractions(EF, rEF) between the two groups after 1 week and 6 months of PCI, and observed the thrombolysis in myocardial infarction trial(TIMI) and ST segment resolution of electrocardiogram(STR). Results: Compared with single PCI, TIMI grade 3 and STR≥50% was better in patients with aspiration thrombectomy. And the EDV, rEDV, ESV, rESV by RT-3DE of patients with single PCI at the six months were larger than that with aspiration thrombectomy in PCI(P<0.05), while EF, rEF was lower(P<0.05). And the △EDV, △EF of all the patients at the six months had positive correlation with STR. Conclusion: Aspiration thrombectomy in PCI with AMI were more conductive to recover of the left ventricular function by RT-3DE. And △EDV, △EF by RT-3DE and STR can predict clinical prognosis more accurately for AMI with PCI.

81 Evaluation of the carotid elasticity in patients with coronary heart disease by two-dimensional strain imaging

Objective: To evaluate the carotid elasticity in patients with coronary heart disease(CHD) by two-dimensional strain imaging. Methods: Two hundred and six hospitaized patients with suspected CHD who were to undergo coronary angiography were enrolled in the research. According to the coronary angiography, the patients were divided into one diseased branch group inculding sixty-three cases, multiple diseased branch group including one hundred and three cases and non-CHD group including thirty cases. All patients were examined by M mode echocardiogram and two-dimensional strain imaging respectively, the measurement results were recorded. All clinical datas were statistically analyzed and processed. Results: ① The systolic peak global CS and early systolic global CSr in multiple diseased branch group were lower than those in one diseased branch group and those in non-CHD group(P<0.01), while the late systolic global CSr and β2 in multiple diseased branch group were greater than those in one diseased branch group and those in non-CHD group(P<0.01), the systolic peak global CS and early systolic global CSr in one diseased branch group were lower than those in non-CHD group(P<0.01), while the late systolic global CSr and β2 in one diseased branch group were greater than those in non-CHD group(P<0.01). ②The ROC curve analysis indicated that the under-curve areas of the systolic peak global CS, the early systolic global CSr, the late systolic global CSr and β2 were 86.9%, 82.7%, 80.3% and 91.4% respectively. All of them showed high diagnostic value to CHD(P<0.001). ③The discriminant function “Y=0.817β2+0.500IMT-0.185plaque(1 or 0)” was obtained by discriminant ananlysis and its sensitivity and specificity of diagnosis of CHD were 78.4% and 93.3% respectively, the accuracy rate was 80.6%. Conclusion: The elasticity of blood vessel wall in CHD patients changed earlier than morphological changes and related with the degree of stenosis of coronary artery. CS and CSr which were carotid elasticity indexes may become two of the noninvasive indexes in evaluating CHD.

86 Reproducibility of the quantitative measurements of the left atrial appendage using 256-slice spiral CT

Objective: To assess the reproducibility of the quantitative measurement of left atrial appendage(LAA) using 256-slice spiral CT. Methods: In the population of patients referred to our hospital for 256 MSCT coronary angiography, 50 individuals were randomly selected. The images from 75% of the cardiac cycle were reconstructed and then sent to the workstation for measuements of the longitudinal and transverse diameters of LAA orifice, the area of LAA orifice, LAA volume, et al. Inter-observer and intra-observer reproducibility were evaluated. Results: There was no significant difference between the results from two different observers or between the results at different time points for the same observer(P>0.05). Conclusion: Philips 256 MSCT coronary angiography shows high reproducibility and agreement in measuring the LAA parameters, which can provide important reference for clinical ablative treatments and the elimination of the LAA.

90 Pathologic cardiac fatty infiltration in MSCT

Objective: To assess the MSCT manifestations of the pathologic cardiac fatty infiltration. Methods: The morphological characteristics and distribution pattern of cardiac fatty infiltration on MSCT in 85 patients were analyzed retrospectively. Results: Of the 85 cases of pathologic fatty infiltration which included healed myocardial infarction(69 cases), arrhythmogenic right ventricular cardiomyopathy(4 cases), lipomatous hypertrophy of the interatrial septum(7 cases), viral myocarditis(5 cases). Conclusions: The morphological characteristics and distribution pattern of cardiac fat can help to differentiate pathologic fatty infiltration.

92 Prognostic factors for portal vein tumor thrombosis in patients with hepatocellular carcinoma receiving transcatheter arterial chemoembolization therapy

Objective: To evaluate the clinical prognosis and survival outcomes of hepatocellular carcinoma(HCC) patients with portal vein tumor thrombosis(PVTT). And to compare survival rate between groups of synchronous and asynchronous PVTT. Methods: A prognostic model was developed for 57 HCC patients with PVTT treated with transcatheter arterial chemoembolization(TACE) from 2007 January to 2013 March. Local tumor response was evaluated by modified response evaluation criteria in solid tumor. Clinical characteristics, prognosis, and patient survival were analyzed by the life table method, the Kaplan-Meier method and Cox’s proportional hazards model. Results: According to mRECIST, complete response, partial response, stable disease and progressive disease were found in 1 patient(1.5%), 14 patients(24.5%), 20 patients(35.5%) and 22 patients(38.5%), respectively. The 6-, 12-, 18- and 24-month overall survivals for the HCC patients with PVTT after TACE were 50.8%, 29.8%, 24.6% and 10.5%, respectively. The median survival was 6.8 months. In the multivariate analysis, pre-treatment prognostic factors for overall survival were AST, CHE, APS, complete portal vein occlusion and local tumor response. Conclusion: There are many factors affecting the survival outcomes of HCC patients with PVTT, and there is no difference in survival analysis study between synchronous and asynchronous portal vein tumor thrombosis.

98 The value of 3.0T MRI in detecting new micro-hepatocellular carcinoma during follow-up examination

Objective: To study the value of 3.0T MRI in finding new micro-hepatocellular carcinoma(MHCC) lesions during follow-up examination. Methods: Seventeen patients who were confirmed by pathology to be hepatocellular carcinoma and were follow-up by MRI for long term after treatment. All of these cases had conventional MRI, DWI and dynamic contrast enhance(DCE) MRI examination on a 3.0T MR system. Among them there were 23 newly found MHCC, these lesions were confirmed by clinical and short term follow-up. The MRI features of these lesions were studied retrospectively. Results: The 23 MHCC<1.0 cm showed slightly high signal on T2WI, 23 and 19 lesions showed high signal on small and large b value DWI respectively, 21 lesions showed rich blood supply on DCE among these lesions, 16 lesions showed the characteristic “fast in and fast out” feature, 5 lesions showed “fast in and slow out” feature, lack of blood supply in 2 lesions which showed no enhancement on different phases of DCE, 10 nodules(≥0.7 cm) showed pseudocapsule enhancement on equilibrium phase. Conclusion: Combination of different sequences using 3.0T MR system has important clinical application value in detecting newly appeared MHCC.

103 Application of coefficient variance of apparent diffusion coefficient value in differentiating pancreatic carcinoma from focal pancreatitis

Objective: To evaluate the usefulness of coefficient variance(CV) of apparent diffusion coefficient(ADC) value in differentiating pancreatic carcinoma from focal pancreatitis. Methods: Diffusion weighted MR data were obtained from 28 patients of pancreatic carcinoma and 7 patients of focal pancreatitis. The mean value(x) and standard deviation(s) of ADC value in every lesion were measured. The CV of the lesion was defined as CV=s/x. The x values and CVs were compared between groups using Student’s t test. Result: The mean value of pancreatic carcinoma was 1.46±0.29, and the mean value of focal pancreatitis was 1.58±0.28. There were no obvious differences between the mean value of pancreatic carcinoma and focal pancreatitis(t=1.02, P=0.32). CV of pancreatic carcinoma was 0.16±0.06, and CV of focal pancreatitis was 0.10±0.04. CV of pancreatic carcinoma was significantly higher than that of focal pancreatitis(t=2.44, P=0.02). Conclusion: The CVs of ADC values in lesions were more useful than that of mean value to distinguish pancreatic carcinoma and focal pancreatitis.

106 MRI in the differential diagnosis of ovarian cystic and solid mass

Objective: To analyse the MRI manifestations of ovarian cystic and solid mass. Methods: Forty-four cases with pathological confirmed ovarian cystic lesions were enrolled in this study. For all of the patients, routine MRI was performed. Image findings of the masses were analyzed using discriminant function. Results: Significant differences between benign tumors and malignant tumors were found in regular or irregular wall, with or without mural nodule, sharp or unclear edge, with or without swelling pelvic nodule and with or without ascites(P<0.05). Discriminant function analysis using these regular or irregular wall, with or without mural nodule, and markers of edge was used to identify discriminatory variables using a stepwise procedure. The function classified overall 93.18% of the cases correctly. Conclusion: MRI may be used to differentiate between benign ovarian tumors and malignant tumors. Using discriminant functions could further improve the accuracy of the diagnosis.

109 Flow mediated dilation of brachial artery and popliteal artery in diabetic kidney disease using high frequency ultrasound: a comparative study

Objective: To study the difference of vascular endothelial function by evaluating flow mediated dilation(FMD) of brachial artery and popliteal artery in diabetic kidney disease(DKD) patients with high frequency ultrasound. Methods: Vascular diameter and velocity in baseline and after reactive hyperaemia of brachial artery and popliteal artery were detected with high frequency ultrasound in 20 healthy volunteers, type 2 diabetic mellitus(T2DM) and patients with DKD-V pre-hemodialysis, FMD were calculated and compared between the 3 groups. Results: Difference of brachial artery FMD and popliteal artery FMD between each two groups were significant(P<0.05). Difference of FMD between brachial artery and popliteal artery in each group were significant(P<0.05). The difference between each two groups of brachial artery and popliteal artery FMD were significant(P<0.05) and increased as disease progress. Conclusion: When T2DM involved kidney, endothelial dysfunction of popliteal artery progress faster than brachial artery.

113 The value of 3D dynamic contrast enhanced LAVA in evaluating the progression of rheumatoid arthritis of the wrist

Objective: To investigate the value of 3.0T MRI dynamic contrast enhanced LAVA in the evaluation of the progress of rheumatoid arthritis(RA) disease of the wrist. Methods: Thirty-five patients with RA were enrolled into the study, 20 cases were in the active stage, 15 cases were in the remission stage. Conventional MR and dynamic enhanced scanning of the wrist were performed for these patients, the enhanced images were processed. Six ROIs were put in the enhanced synovial membrane of the wrist joints of the patients, the dynamic enhancement time signal curve was acquired, the synovial enhanced ratio of each ROI were obtained; the mean value was calculated, at the same time, measured the thickness of the synovial membrane in each ROI and calculated the mean value. Compared the enhancement ratio the difference of the synovial thickness at the active and remission stage respectively. The correlation of the enhancement ratio, synovial thickness, erythrocyte sedimentation rate(ESR), and C reactive protein(CRP) were statistically studied respectively in all the patients. Results: The time signal curve in the active stage was of rapid rise platform type, in the remission stage, the time signal curve was of slowly rise type. The mean synovial enhancement ration in the active stage was 2.27±0.56, compared to the mean enhancement ratio in the remission stage which was 1.04±0.42, it had statistical significance, P<0.01. The mean synovial thickness in the active stage was ((5.9±0.4) mm), compared to that of the remission stage which was ((2.9±0.6) mm) had statistical significance, P<0.01. The patients’ synovial enhancement ratio positively correlated with ESR and CRP respectively(correlation coefficient R value was 0.778 and 0.816 respectively, P=0.02 and 0.01). Synovial thickness and ESR and CRP had certain correlation(correlation coefficient R value was 0.506 and 0.402 respectively, P=0.05). Conclusion: The synovial dynamic enhanced MRI time signal curve type, synovial dynamic enhancement ratio and synovial thickness have important value in evaluating the progress of RA.
