1 The conventional MRI and diffusion-weighted imaging in cerebral venous sinus thrombosis

Objective: To investigate value of conventional MRI, diffusion-weighted imaging(DWI) in diagnosing cerebral venous sinus thrombosis(CVST), to evaluate diffusion signal intensity in occluded venous sinuses and predict the chances of vessel recanalization at the corresponding site. Methods: The conventional MRI, MR venography and DWI findings were analyzed retrospectively in 32 patients with CVST. The patients were divided into 2 groups according to the signal of the thrombosed sinus on the first DWI, that is a group with hyperintense signal and isointense/hypointense signal. Complete recanalization between groups were compared by χ2 test. Results: Thirty-two patients with CVST were observed on MRI combined with MR venography. One venous sinus was involved in 5 patients, 2 in 8 patients, 3 in 13 patients, 4 in 4 patients, and 5 in 2 patients. Among the 86 sites of CVST, thrombosis demonstrated hyperintense signal on T1WI in 61.6% of sites, on T2WI in 47.7%, on T2 FLAIR in 46.5%, on DWI in 32.6%. Twenty-five thrombosed sinuses demonstrated hyperintense signal on the first DWI, complete recanalization was found in 7 sinuses, partial in 10, and occluded in 8 at follow-up examinations. Fifty-one thrombosed sinuses demonstrated isointense or hypointense signal on the first DWI, complete recanalization was found in 36 sinuses, partial in 9, and occluded in 6 by follow-up examinations. Chances of complete recanalization was lower in the presence than in the absence of hyperintense signal in thrombosed sinuses on the first DWI, there was statistically significant difference(χ2=12.46, P<0.05). Conclusion: DWI examination is of no additional diagnostic value when conventional MRI are already obtained for patients with CVST, The hyperintense signal in thrombosed sinus on DWI suggests low rate of vessel recanalization at follow-up examination.

5 Imaging analysis of enlarged vestibular aqueduct complicated with inner ear malformations

Objective: To investigate relationship between enlarged vestibular aqueduct(EVA) and inner ear malformations such as cochlear dysplasia, vestibulocochlear dysplasia and modiolus dysplasia. Methods: A retrospective analysis of 60 ears from 30 patients with EVA in the Third Affiliated Hospital of Zhengzhou University, between 2010 and 2012. MSCT and MRI were performed to assess the EVA inner ear abnormalities in patients with the other malformations. Result: The 60 ears were all suffering from EVA, bilateral EVA 26 cases, unilateral EVA 4 cases, 12 females and 18 males. The average age patients was 1.8 years(six month~4.2 years old). In 45(80.4%) ears with EVA also had one or more other inner ear abnormalities. Thirty ears(53.6%) associated with cochlear dysplasia, 9(16.1%) had vestibular dysplasia. Conclusion: According to small FOV, thin layer MSCT and magnetic resonance imaging of the inner ear, EVA is usually found to be comlicated inner ear malformations, particularly dysplasia of the cochlea.

9 Evaluation of CT in postoperative imaging of pediatric cochlear implants

Objective: Postoperative imaging of cochlear implants needs to provide detailed information on localization, insertion depth and visualization of the electrode array. We evaluated these information with 256-slice CT and several processing techniques. Materials and Methods: The clinical histories and CT studies of 15 children with cochlear implants were retrospectively analyzed. Images in this thesis were acquired from 256-slice Brillance iCT(Philips Healthcare) scanner with helical mode. Then we reconstructed two image sequences of the interesting temporal with a small FOV(9 cm) using a high resolution reconstruction algorithm and low resolution reconstruction algorithm respectively. Multi-planar reformations perpendicular and parallel to the modiolar axis were made with contiguous 0.2 mm sections. The volume rendering was made with the image sequence of low resolution reconstruction algorithm. On oblique coronal and oblique saggital reformation images, we evaluate the information of cochlear implants on the precise localization of the implant and its individual contacts as well as the visibility of electrode array. Additionally the number of the electrode array was counted and compared with the surgical recording. On a multi-planar reformation perpendicular to the modiolus axial image, we evaluate the visibility of electrode array. The image was evaluated by 2 head and neck radiologists using a 2-point scale. Results: The interobserver agreement for the visibility of electrodes was very high, t=2.092, P=0.055>0.05. Axial CT and multi-planar reformation images can provide the inserted electrode number and had no significant difference between surgical recording, t=1.468, P=0.164>0.05. Conclusion: Postoperative CT of cochlear implants should provide information on the precise localization of the implant and its individual electrode. And on the multi-planar reformation perpendicular and parallel to the modiolus axis, the number of the implanted electrodes can be evaluated. The volume rendering technique can give a outlook of totality of the implants.

13 The value of DWI in differential diagnosing benign and malignant nasopharyngeal lesions

Objective: Study on the DWI imaging features of nasopharyngeal mucosa to differentiate nasopharyngeal benign or malignant lesions. Methods: 112 cases with DWI of nasopharyngeal mucosa were divided into 6 groups according to the imaging features(symmetry, signal intensity of bilateral fractures, thickness of mucosa), symmetric and asymmetric group, high signal and low signal group, mucosal thickening group and non-thickening group. Grouping and qualitative results were compared and analyzed. To study the value of the features of the 3 groups to judge the nature of nasopharyngeal lesions. Results: There were significant differences between symmetric group and asymmetric group, high signal group and low signal group, thickening group and non-thickening group. The sensitivity and specificity of symmetry were 79.5%, 65.8%, signal intensity were 59%, 95.9%, the thickness of mucosa were 74.4%, 94.5%, and their Az values were 0.726, 0.774, 0.844. Conclusion: Symmetry, signal intensity and thickness of mucosa can be used as diagnostic criteria of nasopharyngeal benign or malignant lesions on the DWI image, and diagnostic value of mucosal thickness is the largest.

17 Diagnostic value of ultrasound-guided fine-needle aspiration cytology of thyroid nodules before operation

Objective: This study aimed to assess diagnostic value of ultrasound-guided fine-needle aspiration cytology(US-G FNAC) of thyroid nodules before operation. Methods: Data of 360 US-G FNAC of thyroid nodules in our hospital from 2009 to 2012 were retrospectively collected. Classification and interpretation of US-G FNAC using the Bethesda system standard. 197 nodules were operated on, the results of US-G FNAC were compared with final histology, the area under ROC curve(AUC) was used to assess diagnostic accuracy of US-G FNAC of thyroid nodules, and compared all the risks prediction of malignant nodules(quoted in the Bethesda system) and the actual risks of malignancy(final histology results). Results: ① Among 360 nodules, 197 were operated on, US-G FNAC results were reported as: 72 benign(BEN) (36.55%), 7 atypia of unknown significance(AUS)(3.55%), 65 suspicious for follicular neoplasm(SFN)(32.99%), 7 suspicious for malignancy(SFM)(3.55%), 33 papillary carcinoma(16.75%), 2 medullary carcinoma(1.02%), 2 other(1.02%) and 9 insufficient(INS)(4.57%). Histology results were reported as: 124 BEN(62.94%), 21 Follicular adenoma(10.65%), 2 follicular carcinoma(1.02%), 46 papillary carcinoma(23.35%), 2 medullary carcinoma(1.02%), 2 other(1.02%). US-G FNAC results were compared with final histology, AUC of US-G FNAC=0.903, diagnostic accuracy was high, with statistically significant difference. ②Risk prediction of malignant nodules(quoted the Bethesda system) was same as the actual risk of malignancy(final histology results). Conclusions: Before operation, diagnostic accuracy of US-G FNAC was high. The Bethesda system appropriately stratified lesions for risk of malignancy.

21 To explore the influence of depth and size of breast lesion on the quality of elastography images

Objective: To explore the influence of breast lesion depth, lesion size, adipose thickness, and breast thickness on the quality of compression elastography images obtained from women in northeast China. Methods: Ultrasonic elastography was conducted on 326 breast lesions in 272 women. The quality of the images was evaluated according to the criteria for satisfactory and unsatisfactory elastograms. Univariate and multivariate logistic regression analyses were conducted to determine the relationship between measured factors and the quality of the elastography images. Results: Among the 326 breast lesions, satisfactory images were obtained in 300(92%); 26(8%) were unsatisfactory. Univariate analysis revealed that the averages of the lesion depth, adispose thickness anterior to the lesion, and breast thickness at the lesion location in the unsatisfactory group were significantly higher than those of the satisfactory group(P<0.05). There were 62 lesions with their deep edge located 2.1 cm to 2.5 cm below the skin and 66 lesions with their superficial edge located 1.6 cm to 2.3 cm below the skin; furthermore, the lesions in the unsatisfactory group were larger than those of the satisfactory group(P<0.05). Logistic regression analysis revealed that the distance from the superficial edge of the lesions to the skin(OR: 8.999, P=0.012) and breast thickness at the lesion location(OR: 38.669; P=0.000) were the independent risk factors. Conclusion: Compression elastography is suitable for evaluating breast lesions in women from northeast China. Images were less likely to be satisfactory if they were deep or located in areas of greater breast thickness. Satisfactory images were more likely to be obtained if the deeper lesions were larger. Furthermore, breast thickness at the lesion location was a more important factor than lesion depth for obtaining a high quality elastography image.

25 Ultrasound and mammography in the diagnosis of sclerosing adenosis of breast andcorrelation with pathological findings

Objective: To assess the value of ultrasound, and mammography in diagnosing sclerosing adenosis of the breast. Methods: The record of 25 cases who had ultrasonogram and mammogram in our institute from August 2007 to November 2012 were reviewed retrospectively. Altogether 29 breasts were proved to be sclerosing adenosis by histopathology. The ultrasonographic and mammographic findings were analyzed. Results: The sonographic findings were: mass in 27, normal in one. The most common sonographic findings of the masses were ≤2 cm, irregular in shape, without blood flow, no posterior shadow. Misdiagnosed in 10 cases which were irregular in shape, angled edge, associated with posterior acoustic shadow. The mammographic findings were: mass in 10, the most common mammographic findings of the masses were oval or lobulated, homogeneous in density, well-circumscribed or partially obscured edge, microcalcifications in one(clustered, small round microcalcifications). Normal mammograms in five. Conclusion: Combination of ultrasound and mammography, the diagnostic accuracy of sclerosing adenosis can be improved.

29 Evaluation of left ventricular systolic function in patients with systemic lupus erythematosus usingtwo-dimensional speckle tracking technique

Objective: To assess the left ventricular systolic function using the longitudinal peak strain(S) and strain rate(Sr) with two-dimensional(2D) speckle tracking technique and evaluate its clinical value in patients with systemic lupus erythematosus(SLE). Methods: Thirty SLE patients and 32 control subjects underwent the examination of echocardiography. 2D images were recorded in apical four chamber, two chamber and apical long axis view from the left ventricle. The global peak S and Sr and the peak S and Sr of segments were obtained in the above views using 2D speckle tracking technique. Results: The Tei index was increased in SLE patients(P<0.001). The global peak S and Sr and the peak S and Sr of segments were decreased in SLE patients(P<0.05). Conclusions: The global and regional left ventricular longitudinal systolic function is weakened in patients with SLE. 2D Speckle tracking technique is an accurate way for assessing the left ventricular systolic function.

32 The application of split bolus CT enhancement in renal clear cell carcinoma

Objective: To explore the clinical value of split bolus multidetector computed tomographic(MDCT) enhanced examination in clear cell renal carcinomas(ccRCC). Methods: 120 patients suspected of kidney tumor by ultrasound or physical examination were divided into two groups randomly. The experimental group contained 60 cases who were given split bolus two phase CT enhanced scanning, the control group also contained 60 cases who had routine three phase enhanced scanning. The results of surgical operation were correlated with pathology. Ninety cases were ccRCC confirmed by pathology. Among these 90 cases, 44 cases were in the experimental group, 46 cases were in the control group. The degree of contrast of the tumor and the normal kidney in each phase were compared and also the radiation dosage were calculated. Results: ①Compared the mean CT value of the ccRCC of the experimental group in the cortico-medullary phase(CMP), nephrographic-excretory phase(N-EP) and the CMP, N-EP of the control group had no significant difference(P=0.064, P=0.105); Compared the mean CT value of N-EP in ccRCC of the experimental group and the NP of that in the control group had significant difference(P=0.000). ②The mean CT value in the CMP of the normal kidney in the experimental group compared with that of the control group had no significant difference(P=0.105). The CT value in the N-EP of the normal kidney in the experimental group compared with the mean CT values of nephrographic phase(NP), and excretory phase(EP) of the normal kidney in the control group had significant differences(P=0.000, P=0.001). ③In the experimental group, the average effective radiation dosage was 15.25 mSv. There was 30% less than the radiation effective dose of the control group. Conclusion: The split bolus MDCT enhancement examination can accurately diagnosed ccRCC, and at the same time, the effective dosage decreased obviously. There is important clinical application value of the method.

37 The value of clinical application of three-dimensional ultrasound in the diagnosis of fetal renal duplication

Objective: To investigate the clinical application value of three-dimensional ultrasound in the diagnosis of fetal renal duplication. Methods: Retrospective analysis 14 fetuses with fetal renal duplication from 2011 July to 2012 July, and analyze the three-dimensional image of deformity kidney, so as to explore the application value of three-dimensional ultrasound for fetal renal duplication. Results: All 14 cases of fetal renal duplication can obtain 3D image, the three-dimensional image contains more information than a single 2D image contains. The diagnosis of all fetuses were confirmed by autopsy, fetal MRI examination and postnatal renal ultrasound. Conclusions: Three-dimensional ultrasound could directly display the anatomy of fetal renal duplication deformity compared with two-dimensional ultrasound, and has important value in the diagnosis of fetal renal duplication.

40 The initial experience of using the Viabahn for SFA in-stent restenosis

Objective: To report the initial experience of using Viabahn endoprothesis to treat the restenosis in superficial femoral artery. Materials and Methods: Ten cases of superficial femoral artery(SFA) in-stent restenosis were admitted due to lower limb ischemia in April 2013. All of the 10 patients were suffering from Fortaine Ⅱb or Ⅲ stage ischemia, with mean age of 68 years old. Preoperative CTA demonstrated obvious SFA in-stent restenosis, in which there were 9 total occlusion of the nitinol stents, the ABI: 0~0.48. We accomplished re-intervention with Viabahn(5~7 mm in diameter) for all 10 cases. Evaluation of treatment at immediate angiography, postoperative ABI, and 1 week after intervention for Fortaine ischemia classification. Results: Fifteen Vibahn endoprothesis were successfully implanted in all of the 10 cases. There are no residual stenosis in the immediate angiography with a significant improvement of ABI to 0.76~0.96(t=-11.589,P=0.000). The Fortaine ischemia classification were improved from Ⅱb toⅠand from Ⅲ to Ⅱa or Ⅰstage. Conclusion: The Viabahn endoprothesis might be a safe and effective approach for the treatment of SFA in-stent restenosis.
