Fractional amplitude analysis of low frequency fluctuation in generalized tonic-clonic seizure:a resting state functional MRI study

Objective: To explore the whole brain activity features during the interictal time of generalized tonic-clonic seizure(GTCS) using resting state MRI. Methods: Twenty-one GTCS individuals and 21 healthy control subjects, well matched in gender, age, education and handedness, were enrolled as the GTCS group and control group, respectively. A Siemens 3.0T MR scanner was used to acquire all the subjects’ resting state data with GRE-EPI sequence, DPARSF software was used to process resting functional MRI data. fALFF of the two groups were calculated and compared. The correlation between fALFF and NHS3 scores of GTCS was observed. Results: In comparison with the control group, the GTCS group showed reduced fALFF in cingulate gyrus, medial frontal gyrus, right opercula insulae, left supramarginal gyrus(t=-4.63~-3.88, P<0.01). fALFF was increased in the cerebellum posterior lobe/cerebellar tonsil, right fusiform gyrus, inferior temporal gyrus(t=5.21~6.04, P<0.01). In addition, fALFF in the left parietal lobe, inferior parietal lobule, right fusiform gyrus, inferior temporal gyrus positively correlated with the NHS3 scores of the patients(P<0.01), while in the medial frontal gyrus, superior frontal gyrus, left insular lobe superiortemporal gyrus correlated negatively with the NHS3 scores of the patients(P<0.01). Conclusion: GTCS patients had abnormal activity at the cingulate gyrus, medial frontal gyrus, inferior temporal gyrus, cerebellum posterior lobe, cerebellar tonsil et al, during the interictal time; and these abnormal activities might be related with clinical manifestation and pathophysiology, which may help for explanation of the clinical manifestations and pathophysiological mechanisms in GTCS.

The value of the radioreceptor imaging in the diagnosis of thyroid associated ophthalmopathy

Objective: To investigate the value of 99mTc-OCT SPECT imaging in the diagnosis of thyroid associated ophthalmopathy(TAO). Methods: Nineteen outpatients(38 eyes) in Nuclear Medicine Department whose main complaint is discomfort around the eye area(tearing, photophobia, eye pain, changes in vision, eyelid contracture and exophthalmos) underwent orbital 99mTc-OCT SPECT. Five healthy volunteers(10 eyes) recruited(healthy control group) underwent the same examination. Taking the EUGOGO clinical diagnostic criteria, all outpatients’ eyes were divided into TAO group(30) and non-TAO group(8). According to its clinical activity score(CAS), the TAO group was divided into active group(24) and inactive group(6); and at the same time, the TAO group was divided into mild group(11) and moderate-severe group(19) by the disease severity. Then the ratios of the highest eye radioactive count(T) to the lowest occipital scalp radioactive count(B) in all eyes were calculated, as the eye 99mTc-OCT intake. Receiver operating characteristic(ROC) analysis was employed to define the threshold value of T/B ratio. Results: There is a significant difference in T/B ratio between non-TAO group and healthy control group(t=2.76, P=0.014), and a highly significant difference between TAO group and healthy control group(t=5.86, P<0.001). The significant difference in T/B ratios were still found between TAO group and non-TAO(t=2.78, P=0.009), while a highly significant difference also found between active group and inactive group(t=2.59, P=0.015). The area under the curve of ROC for TAO group and non-TAO group was 0.821, and 0.854 for active group and inactive group; the threshold value of T/B ratio was 1.637 for identifying TAO, and its sensitivity, specificity and accuracy were 83.3%, 87.5% and 84.2%; the threshold value of T/B ratio for distinguishing between active group and inactive group was 1.674, and its sensitivity, specificity and accuracy was 87.5%, 83.3% and 86.7%, respectively. Conclusion: 99mTc-OCT SPECT imaging combined with semi-quantitative analysis can give more accurate diagnosis in identifying TAO and distinguishing active TAO, and it is helpful to guide the individualized treatment plan for TAO.

Application value of combined dynamic contrast-enhanced MR imaging and diffusion-weighted MR imaging inmagnetic resonance mammography BI-RADS category 3-5 breast mass lesions

Objective: To evaluate the diagnostic value of conventional MR mammography(MRM) combined with dynamic contrast-enhanced(DCE) MR imaging, and diffusion-weighted MR imaging(DWI) in the diagnosis and differential diagnosis of BI-RADS category 3-5 breast lesions. Methods: In 118 cases diagnosed as BI-RADS category 3-5 mass lesions, tumor morphology, hemodynamics of the masses and apparent diffusion coefficient(ADC) value were analysed retrospectively. Sensitivity, specificity, positive predictive value(PPV), negative predictive value(NPV) and accuracy were calculated. Results: In DCE-MRI, irregular and lobular shape, irregular and speculated margins, rim enhancement, early contrast-enhanced rate more than 100% and washout time-intensity curve(TIC) were the indicators of malignancy. The optimal threshold of 1.25×10-3mm2/s from receiver operating characteristic(ROC) curve, and have better diagnosis values(>0.900). The combined application provide sensitivity, specificity, PPV, NPV and accuracy were 90.4%, 91.4%, 96.2%, 80% and 90.7%. Conclusion: Conventional MRI combined with DCE-MRI and DWI were helpful to improve the diagnostic specificity and accuracy of BI-RADS category 3-5 breast mass lesions.

Diagnostic value of automated breast volume scanner images on benign and malignant breast masses

Objective: To assess the value of automated breast volume scanner(ABVS) for the diagnosis of benign and malignant breast masses. Methods: ABVS images and molybdenum X-ray imaging information of 93 patients, 120 breast masses were analyzed retrospectively. All the masses had pathological findings after operations. Results: Compared total lesion detection rate, ABVS relevance ratio was 95%(114/120). Molybdenum X-ray relevance ratio was 80%(96/120), it had statistical significance(P<0.05). Compared relevance rate of malignant breast tumors, ABVS relevance ratio was 98.3%(58/59), molybdenum X-ray relevance ratio was 83.1%(49/59), it had statistical significance(P<0.05). Compared relevance rate of benign masses, ABVS relevance ratio was 91.8%(56/61), molybdenum X-ray relevance ratio was 77.0%(47/61), it had statistical significance(P<0.05). There were 35 malignant lesions less than 2 centimeter. ABVS examined 32 lesions, relevance ratio was 91.4%. Molybdenum X-ray examination found 28 lesions, relevance ratio was 80%, it had statistical significance(P<0.05). Occurrence rate of masses in the coronal section of ABVS presented with “crater” sign was 89.7%(52/58), whereas it was 3.6%(2/56) in molybdenum X-ray. The detection rate of malignant lesions of less than 2 centimeter presented with “crater” sign was 91.4%(32/35), whereas it was 5.3% in benign lesions. “Crater” sign are specific indicators for differentiation between malignant and benign breast tumors. Conclusion: ABVS system provides the omni-directional imaging breast masses of information. Compared with molybdenum X-ray, ABVS imaging system provides much more useful information on lesions with a higher detection rate and accuracy. The “crater” sign can serve as indicators of differential characteristics between malignant and benign masses.

The application value of 320-slice CT in follow-up for patients after coronary artery bypass grafts

Objective: To evaluate the value of 320-slice CT in the follow-up after coronary artery bypass grafts(CABG). Method: One hundred and forty-three patients after CABG were studied by 320-slice computed tomography coronary angiography(CTA), images were retrospectively reconstructed under volume rendering technology(VR), maximum intensity projection(MIP), multi-planar reconstruction(MPR) and curved planar reformation(CPR). The image quality and stenosis of grafts, native vessel were evaluated; and compared with the CAG data in 14 cases of coronary angiography at the same period. Result: All the vessels including grafts and native vessel showed satisfactorily. The graft failure was found in 14.2%(53/372). One year after surgery, the patency rates of arterial and venous grafts were 94.3%, 86.3%, the rate of artery grafts lesions showed a progressive increase. There were statistically significant differences of graft patency rate between the chest pain group and non-chest pain group. Compared with 14 patients underwent CAG, the sensitivity, specificity, positive and negative predictive values of 320-slice CT were 92.3%, 100%, 100%, 92.5%. The diagnostic sensitivity, specificity, positive and negative predictive values of native vascular disease were 94.3%, 96.5%, 92.6%, 97.4%. Conclusion: The 320-slice CT in the evaluation of the lesions after CABG grafts has a high accuracy rate, it can be used as the first noninvasive method in follow-up after CABG.

Feasibility study of evaluation of liver function using Gd-EOB-DTPA-enhanced MR imaging

Objective: To investigate the relationship between the liver signal intensity in the hepatobiliary phase in gadolinium ethoxybenzyl diethylenetriamine pentaacetic acid(gadoxetic acid, Gd-EOB-DTPA)-enhanced magnetic resonance imaging and liver function. Methods: This study included 55 patients with cirrosis due to hepatitis B and 18 adult normal as control group, the subjects received laboratory tests of liver function to perform the Child-Pugh score and MR imaging before injection and at 5, 10, 15, 20 and 25 minutes after bolus administration of Gd-EOB-DTPA, the signal intensities of liver and spleen at each time point were measured. The differences of the absolute value of liver, DSI(the differences of liver signal intensity between the hepatobiliary phase and plain scan), liver-to-spleen contrast ratio were analysed. The relationships between the signal intensity and total bilirubin level, albumin level, and prothrombin time were also evaluated. Results: According to the Child-Pugh score, there were 35 patients in Child-Pugh A group, 18 in B group and 2 in C group. The signal intensity, DSI, and liver-to-spleen contrast ratio showed significant differences among the control group, Child-Pugh A group and Child-Pugh B group at each time point. There were no differences of the liver signal intensity at 15, 20 and 25 minutes in 55 patients(F=0.121, P=0.886). Liver signal intensity in hepatobiliary phase positively correlated with and serum albumin level and negtively correlated with the total bilirubin level, prothrombin time. Conclusion: The degree of liver enhancement in hepatobiliary phase may be used to estimated liver function and correlate with the serum albumin level, total bilirubin level, prothrombin time. The delay time of 15 minutes was suitable for liver functional assessment.

Diagnostic value of ultrasonic hepatic subcapsular flow in infants with biliary atresia

Objectives: To describe color Doppler ultrasonographic(US) findings in livers of neonates with biliary atresia(BA) and to compare them with US findings in livers of neonates with non-BA and control subjects. Methods: US and color Doppler US findings were retrospectively reviewed in 50 patients with neonatal cholestasis and 25 control subjects. Results: BA and non-BA were confirmed in 20 and 30 patients, respectively. In the diagnosis of BA, sensitivity, specificity, positive predictive value and negative predictive value of the triangular cord sign(TC sign) on US images were 60%, 96%, 92% and 79%, respectively. The sensitivity, specificity, positive predictive value and negative predictive value of hepatic subcapsular flow on color Doppler US images were 95%, 93%, 91% and 96%, respectively. Mean diameter of the hepatic artery in patients with BA((2.1±0.2) mm) was significantly larger than that in patients with non-BA((1.2±0.2) mm, P<0.001) and control subjects((1.2±0.1) mm, P<0.001). In patients with biliary atresia, about 90% of children with its γ-GGT>800 IU/L. Conclusion: The detection of BA can be supplemented by performing color Doppler US in addition to routine US, when evaluating hepatic subcapsular flow by using color Doppler US, we can potentially prevent delayed diagnosis of BA.

Sonographic findings of the neovascularization in the retained liver VX2 tumor ofrabbit after radiofrequency ablation: an experimental study

Objective: To observe the sonographic findings of the residual tumor in different time intervals after radiofrequency ablation(RFA) treatment to rabbit VX2 liver tumor, and to study the changes of vascular endothelial grow factor(VEGF) and the expression of microvascular density(MVD) in the residual tumor tissue. Method: The rabbit VX2 liver cancer model was established, and radius of the radiofrequency was controlled under ultrasound guidance, then the residual VX2 liver tumor in rabbit was established. The specimens before and after treatment at 0 h, 1 w, 2 w, 4 w were acquired for ultrasonic observation. It was to observe the expression of VEGF and MVD in the residual cancer tissue by the immune histochemical method. Results: ①Both the 1 w and 2 w residual tumor were smaller and the vascularization were lesser than those of the 4 w specimen. The signal of vascularization reappeared at 4 w specimen. ②Compared with the tumor before RFA, the expression of VEGF and MVD in the residual tumor tissues at 0 h, 1 w, 2 w decreased(P<0.05). However compared with the tumor before RFA and the residual tumor tissues at 0 h, 1 w, 2 w, the expression of VEGF and MVD in 4 w specimen increased(P<0.05). Conclusion: In the short term(within 2 w) RFA destroys tumor vessels thus controls and inhibits VEGF to obtain short term curative effect, but in the long term can induce neovascularization due to hypoxia and tumor recurrency occurred.

The biexponential analysis of diffusion weighted imaging with multi-b factors used forgrading of clean cell renal cell carcinoma preoperatively

Objective: To probe the feasibility of biexponential analysis of multi-b DWI used to the grading of the clear cell renal cell carcinoma(CCRCC) before the surgery. Material and Methods: The imaging and pathological data of 32 cases with CCRCC was assessed retrospectively. There were two groups confirmed by Fuhrman Grading, including high and low grade. Those parameters, including the F, Dfast and Dslow, were compared between two groups using nonparametric test(Mann-Whitney U), and the receiver operating characteristic(ROC) was used to test the predictive values in grading CCRCC preoperatively. There was a significant difference when the P<0.05. Results: Twenty cases of low grade(group 1) and 12 cases of high grade(group 2) CCRCCs were confirmed by pathology. Compared with group 1, the F decreased in group 2, but Dfast, Dslow in group 2 increased(U=18.0, 20.0 and 15.5, P<0.05). And the three parameters have the similar sensitivity and specificity for grading of CCRCC preoperatively(P>0.05). Conclusion: The biexponential analysis of multi-b DWI is suitable for the grading of CCRCC preoperatively.

Noninvasive evaluation of chronic kidney disease by virtual touch tissue quantification imaging

Objective: To evaluate virtual touch tissue quantification(VTQ) imaging in chronic kidney disease. Methods: VTQ, given shear wave velocity(SWV), of the renal parenchyma was measured in 107 patients with chronic kidney disease and 20 healthy volunteers. The mean SWV measurements were compared in different stage groups and control volunteers. Correlations between SWV measurements and clinical stage or serum creatinine were investigated. Results: The mean SWV in stage 2, stage 3, stage 4 and stage 5 patients with chronic kidney disease were significantly lower than that in stage 1 and volunteer groups(P<0.001); There were significant differences between stage 4, stage 5 vs stage 1, stage 2 and stage 3 patient groups(P<0.05); SWV did not differ significantly in stage 4 vs stage 5, stage 2 vs stage 3, and stage 1 vs control volunteers(P=0.996, 0.339, 0.139, respectively). The mean SWV of renal parenchyma were negatively correlated with clinical stage(Spearman r=-0.596, P<0.001) and serum creatinine(Pearson r=-0.458, P<0.001). Conclusions: VTQ imaging is a promising noninvasive technique for evaluation renal elasticity in chronic kidney disease patients.

Quantitative evaluation of proteoglycan in lumbar disc with diffusion weighted ADC value: an experimental study

Objective: To study the value of diffusion-weighted imaging ADC values for quantitative evaluation of proteoglycan in lumbar disc. Materials and Methods: The ADC value of lumbar disc in patients with lumbar disc herniation was measured with diffusion-weighted imaging before operation. The content of proteoglycan in the surgical removal nucleus pulposus was assessed. Degenerative disc were categorized with Minna tertti’s method. The difference of proteoglycan and average ADC value for various degenerative lumbar discs were assessed. The correlation of the content of proteoglycan with ADC in disc degeneration was analyzed. Results: Fifteen surgical removal specimen of lumbar disc were obtained in 13 patients with lumbar disc herniation. There are 7 grade Ⅱ and 8 grade Ⅲ degenerative lumbar disc. The correlation coefficient(r) between ADC value and proteoglycan in grade Ⅱ lumbar intervertebral disc was 0.826, P<0.05. The correlation coefficient r between ADC and proteoglycan in grade Ⅲ lumbar intervertebral disc was 0.210, P>0.05. Conclusions: ADC values can be used to evaluate the content of polysaccharides and collagen protein in lumbar disc indirectly.
