229 Effect of intravenous Gd-DTPA on diffusion-weighted imaging incentral nervous system: a preliminary clinical study

Objective: To evaluate the effect of intravenous Gd-DTPA on diffusion weighted imaging(DWI) and apparent diffusion coefficient(ADC) value. Materials and Methods: Forty-one patients were prospectively recruited with mean age (43.3±17.3) years. Transverse single shot echo planar imaging DWI was performed immediately before, and about 3.4 min after the injection of 0.1 mmol/kg Gd-DTPA on GE Signa HDxt 3.0T MR scanner. Signal-to-noise ratio(SNR) and contrast-to-noise ratio(CNR) of the lesion, and the SNR of normal brain tissue were calculated on b=0 s/mm2 and b=1 000 s/mm2 DWI images respectively. ADC values were measured on ADC maps. A paired t-test was used to determine the significance of differences between the values before and after administration of contrast medium. P<0.05 was considered statistically significant. Results: Images before and after contrast administration were not significantly different regarding SNR and CNR on DWI of either normal brain or 44 intraxial lesions. SNR and CNR were slightly decreased on b=0 s/mm2 DWI of 11 extraaxial lesions(P=0.026 and 0.017). ADC values decreased the most after contrast medium injection on DWI for extraaxial lesions(about 5.9%, P=0.002), and no significant statistical difference for ADC decrease shown in normal brain or intraaxial lesions. Conclusion: In routine clinical practice, Gd-DTPA has slight effects on low b-value DWI images and ADC values for extraaxial lesions. Therefore, it is better to perform DWI before contrast medium injection for those lesions with rich blood supply, especially extraaxial lesions.

234 A susceptibility weighted imaging quantitative study of brain iron deposition in idiopathic Parkinson’s disease

Objective: To compare the difference of T2* value in several cerebral regions between idiopathic Parkinson’s disease(PD) patients and control subjects and to quantitatively analyze the variations of brain iron. Methods: Twenty-four patients with idiopathic PD and 24 healthy controls, ranging in same ages and gender, had routine sequences and SWI sequences by 3.0T MR. T2* value was measured in both sides of substantia nigra pars compacta(SNc), substantia nigra pars reticulata(SNr), red nucleus(RN), globus pallidus(GP), putamen(PUT), head of caudate nucleus(CN), thalamus(THA) and frontal white matter(FWM), and statistically analyzed. Results: ①The T2* values was reduced in homolateral side SNc of symptoms of subjects with idiopathic PD compared with the contralateral side(P<0.05). There were no differences in other regions(P>0.05). ②The T2* values was reduced in contralateral side SNc and SNr of symptomatic subjects with idiopathic PD compared with the homolateral side(P<0.05). There were no significant difference in other regions(P>0.05). ③There was a significant reduction in the T2* values in contralateral side SNc and RN of symptomatic subjects with idiopathic PD compared with the contralateral side(P<0.05). There were no differences in other regions(P>0.05). ④No significant difference was demonstrated in T2* values in all regions in homolateral side of symptomatic subjects with idiopathic PD compared with the contralateral side(P>0.05). There was no significant difference in the T2* values between the two sides in the idiopathic PD group, the two sides in the subjects group(P>0.05). Conclusion: There exists abnormal brain iron deposition before subclinical period in PD. The sites of pathologic iron deposition include SNc, SNr and RN, but not include GP, PUT, CN, THA and FWM.

239 Application of cone beam computed tomography in the assessment of the crown-root angulation of impacted maxillary tooth

Objective: To evaluate the clinical application of cone beam computed tomography(CBCT) in diagnosing the crown-root angulation of impacted maxillary tooth. Methods: Thirteen patients with maxillary impacted tooth, whose crown-root angle could not be judged by panoramic radiographs and occlusal films, were scanned by CBCT and then the images were taken three-dimensional reconstruction to locate the impacted tooth, detect the presence of crown-root angulation, measure the crown-root angle and ratio. Then the results of measurement were analyzed and discussed. Results: Among the 13 maxillary impacted tooth cases, 10 were diagnosed as crown-root angulation, of whom the angles of 6 cases were between 120°~150°, those in 4 cases were between 150°~180°, 90°~120°, 60°~90° respectively; besides, the crown-root ratio was between 1∶1.5~1∶1 in 7 cases, 1∶1~1∶0.5 in 5 cases and 1∶2~1∶1.5 in 1 case. Conclusion: CBCT can definitely diagnose the crown-root angle of impacted maxillary tooth, which provides important clinical significance for making treatment plans.

242 Application of 18F-FDG PET/CT in thyroid disease and correlative with histopathological results

Objective: To explore the value of 18F-FDG PET/CT in the diagnosis of thyroid disease. Methods: Twenty-eight patients(13 male and 15 female, aged ranged from 25 to 84 years old) with pathological diagnosis of thyroid disease were retrospectively analyzed on 18F-FDG PET/CT. Surgical operation(26 cases) and fine-needle aspiration(2 cases) were performed on these patients, of whom 10 patients had more than one lesion in thyroid gland. Therefore, 8 malignant thyroid lesions and 31 benign thyroid lesions were included in the study. Results: Focal uptake and diffuse uptake were respectively shown in 7 thyroid cancer and 1 thyroid lymphoma. Of 31 benign thyroid lesions, both PET and CT were negative in 5, both PET and CT were positive in 9, PET was negative and CT was positive in 17. Nine benign thyroid lesions with increased FDG uptake comprised nodular goiter, follicular adenoma, Hashimoto’s thyroiditis, adenomatoid hyperplasia, chronic thyroiditis and subacute thyroiditis. The SUVmax was not correlated with maximum length and average density in thyroid cancer. There was no statistically significant difference on SUVmax between 8 malignant thyroid lesions and 9 benign thyroid lesions with high FDG uptake. The calcification did not show statistically significant difference between 8 malignant thyroid lesions and 31 benign thyroid lesions, but the sand-like calcification showed so. Conclusions: When PET/CT is used to evaluate the thyroid disease, only with SUV value can not discriminate malignant lesions from benign lesions and the sand-like calcification seen on CT may aid to identify thyroid cancer. The lesions with no FDG uptake and diffuse FDG uptake in thyroid gland are benign to a great extent, but the lesion with focal uptake in it is likely malignant. The former two are suggestive of follow-up with ultrasound, the latter is suggestive of fine-needle aspiration by ultrasound guidance.

247 MRI in the diagnosis of complications due to breast augmentation with polyacrylamide hydrogel

Objective: To evaluate the clinical value of MRI in the diagnosis of the complications due to breast augmentation with polyacrylamide hydrogel(PAMHG). Methods: The MRI and clinical data of 40 patients who had breast augmentation with polyacrylamide hydrogel injection were retrospectively reviewed. Results: PAMHG showed iso-intensity compared with breast gland on T1/TSE/SPIR and hyperintensity on T2/TSE/SPIR. There are low signal septa within PAMHG in 75.0%(30/40) breast. There were 80.0%(32/40) PAMHG without capsule, 20.0%(8/40) with smooth and regular capsule. Subcutaneous lump of PAMHG was 22.5%(9/40), lump of that in the breast gland was 37.5%(15/40). Diffuse PAMHG along spatium intermusculare of pectoralis major was 30.0%(12/40), that along spatium intermusculare of intercostal muscle in the chest was 20.0%(8/40). One intraductal papilloma and one fibroadenoma in breast were observed in two patients. Breast infection were observed in three patients. All distributions of PAMHG and appearances of its complications on MRI were consistent with the results of the clinical. Conclusions: Axial T1/TSE/SPIR, T2/TSE/SPIR and sagittal T2/TSE/SPIR can be the optimized protocol of MRI examination for augmentation mammaplasty by injected polyacrylamide hydrogel. MRI is useful for preoperative assessment of the distribution of PAMHG and follow up after operation, and its complications.

251 Real-time three-dimensional echocardiography in the assessment of pediatric perioperative right ventricular systolic function in tetralogy of Fallot and analysis of its influencing factors

Objective: To assess the perioperative right ventricular systolic function in pediatric patients with tetralogy of Fallot(TOF) who went through total correction surgery by real-time three dimensional echocardiography(RT3DE) and analyze the related factors with immediate postoperative right ventricular systolic function. Methods: To compare the right ventricular systolic functional parameters measured by RT3DE such as right ventricular end diastolic volume(RVEDV), end systolic volume(RVESV) and ejection fraction(RVEF) before and two or three days after surgical correction, and then using statistical analysis to explore the related clinical factors with post-RVEF. Results: Compared with the preoperative measurements, in two or three days after surgery, RVEDV and RVEF were significantly reduced, while RVESV was not changed significantly. Good correlation between the post-operative RVEF and four factors including adoption of trans-annular patch surgery, aortic cross-clamp time, residual shunting between two ventricles and residual obstruction of right ventricular outflow tract were found to be significant statistically. Adoption of the trans-annular patch surgery and aortic cross-clamp time were the more related factors with the post-operative RVEF. Conclusions: Right ventricular systolic function in children with TOF immediate after total correction surgery is reduced and mainly correlated with four operative factors. It is helpful to maintain the immediate post-operative right ventricular systolic function by shortening the aortic cross-clamp time, adopting the non-trans-annular patch surgery as well as completely correcting the anatomic deformities.

255 Computed tomography findings of pulmonary chondroma

Objective: To study the CT features of primary pulmonary chondroma for improving diagnostic accuracy. Methods: The clinical data of eight patients with primary pulmonary chondroma proved by pathology of surgical specimens or biopsy were analyzed retrospectively. All patients had CT plain scanning, including 6 cases of enhanced scanning. Results: All of the 8 lesions were solitary. Six lesions were in the right lung and 2 in the left lung. CT findings: All lesions were solitary nodule or mass with well defined boundary, without cavity and satellites; 3 lesions had slightly lobular borderl; 5 lesions showed inner or periphery punctate and patchy calcifications, 3 lesions without evident calcifications; in 6 cases of enhanced scanning, 4 lesions had no evident enhancement and 2 lesions demonstrated moderate heterogeneous enhancement. Conclusions: Primary pulmonary chondroma is a rare benign tumor of lung, its CT findings have some characteristics, and should be differentiated from tuberculoma, harmatoma and peripheral lung cancer.

259 The effectiveness of endovascular revascularization for atherosclerosis obliterans in lower extremity: a 2-year experience

Objective: To determine the efficacy, safety, and 2-year outcome, including patency and limb salvage, of endovascular revascularization in patients with lower extremity atherosclerosis obliterans. Methods: From July 2006 to June 2011, 274 limbs in 222 patients with lower extremity atherosclerosis obliterans were treated by endovascular revascularization. There were 162 males and 60 females, with the mean age of 67.5±11.2 years old. Ninty-two limbs in the aorta-iliac group, 101 limbs in the infrainguinal group, 31 limbs in the infra-popliteal group, and 50 limbs in multiple segment group. The most common presenting symptom was severe intermittent claudication(Rutherford 3, n=116), followed by rest pain(Rutherford 4, n=87), non-healing ulcer(Rutherford 5, n=37) and gangrene(Rutherford 6, n=34). Patency was evaluated by using ultrasound scanning, CTA or DSA if necessary, and follow-up status was evaluated with the Kaplan-Meier method. Results: Immediate procedural and clinical success were achieved in 269 and 265 limbs respectively. Eighty-six limbs underwent angioplasty, while the rest 183 limbs underwent angioplasty plus stent placement. There were four perioperative death, and two major amputations within the first 30 days. Mean follow-up was 20.8±4.7 months(range, 9~75 months). Two hundred and thirty-five limbs had effective follow-up. Primary patenty rate, secondary patenty and limb salvage rate at 2 years were 71.5%, 75.7%, and 96.2%, respectively. Conclusion: Endovascular revascularization is a feasible, safe, and effective procedure for the treatment of lower extremity atherosclerosis obliterans.
