MRI findings of fibrous meningioma

Objective: To study MRI findings of fibrous meningioma. Methods: MRI findings of 56 cases fibrous meningioma confirmed by surgery and pathology were analyzed retrospectively. All patient underwent conventional axial T1WI, T2WI, DWI and enhanced MRI scan. ADC map was obtained from DWI sequence by post-processing and ADC values of tumor and normal white matter in corresponding area were acquired. Conventional MRI, DWI findings and ADC values of fibrous meningioma were analyzed. Results: Compared with brain cortex, the tumors were isointense in 40 cases, hypointense in 14 cases and mixed signal intensity in 2 cases on conventional T1WI of MRI. On T2WI, the tumors were isointense in 35 cases, slightly high signal intensity in 12 cases, mixed signal intensity in 6 cases and hypointensity in 3 cases. Peritumormal edema was found in 16 cases. On enhanced T1WI, 34 cases showed homogeneous enhancement, 21 cases showed heterogeneous enhancement and 1 case showed ring-like enhancement. Dural tail sign was seen in 32 cases. On DWI, 31 cases showed isointensity, 12 cases showed slightly high signal intensity, 10 cases showed mixed signal intensity and 3 cases showed low signal intensity. On ADC map, 31 cases had isointensity, 15 cases had mixed signal intensity, 6 cases had slightly high signal intensity and 4 cases were low signal intensity. The difference of ADC values between tumor parenchyma and normal white matter was statistically significant(P<0.05). In the 16 patients with peritumoral edema, the difference of ADC values among normal white matter, tumor parenchyma and peritumoral edema was statistically significant(P<0.05). Conclusions: There are various of DWI findings for fibrous meningioma. DWI can work as a supplement means for diagnosis of fibrous meningioma based on conventional MRI.

B-mode and Doppler ultrasonography of carotid artery in obese children

Objective: To study the structure and blood flow of carotid artery in children with simple obesity by B-mode and Doppler ultrasound. Methods: The diameter(d) of common carotid artery(CCA), internal carotid artery(ICA), external carotid artery(ECA) and vertebral artery(VA), and intimal-medial thickness(IMT) of CCA were measured in 60 obese children and 30 normal children in the control group by B-dimensional ultrasound. Systolic peak velocity(Vs), diastolic velocity(Vd), mean flow velocity(Vm), RI and S/D were measured by Doppler ultrasound. Results: ①The IMT of CCA in obese group was significantly higher than those of the control group, but there was no difference between the fatty liver group and nonfatty liver group(P﹥0.05). There was a positive linear correlation between IMT and BMI. ②CCA-Vs, ICA-Vd and ECA-Vd in obesity group were significantly lower than those in the control group, and carotid resistance(ICA-RI, ECA-RI, ECA-S/D) was higher in obseity than that of the control group(P<0.05). Conclusion: Carotid structure and blood flow are changed in obese children. Ultrasound technology can evaluate the characteristics in obese children.

Comparative study of strain ratio in ultrasonic elestography and gray-scale ultrasonography for differential diagnosis of thyroid nodules

Objective: To compare the value of strain ratio in ultrasonic elestography and gray-scale ultrasonography in differentiating benign and malignant thyroid nodules. Methods: A total of 165 patients with 196 thyroid nodules underwent conventional ultrasonography and elastography. The strain ratio of nodules were calculated. Receiver operating characteristic (ROC) curves were used to assess the diagnostic value of strain ratio. The sensitivity, specificity and accuracy were compared with gray-scale ultrasonography. Results: The area under the ROC curve(Az) of the strain ratio was 0.826. The strain ratio of 2.51 was the best diagnostic cut-off point. The sensitivity, specificity and accuracy of strain ratio were 81.63%, 72.79% and 75.00%. The sensitivity, specificity and accuracy of gray-scale ultrasonography were 77.55%, 74.83% and 75.51%. The sensitivity, specificity and accuracy of combined diagnosis were 73.47%, 88.43% and 84.69%, respectively. The difference between strain ratio and gray-scale ultrasonography was not significant(χ2=1.45, P>0.05). The accuracy of combined diagnosis was higher than that of strain ratio(χ2=20.48, P<0.05) or gray-scale ultrasonography(χ2=14.70, P<0.05) alone. Conclusion: Strain ratio has moderate degree of diagnostic value in differentiating the benign and malignant thyroid nodules. Combination of strain ratio and gray scale ultrasonography can increase diagnostic accuracy significantly.

Value of CT in the differential diagnosis of papillary thyroid microcarcinoma and micronodular goiters

Objective: To evaluate the value of CT in the differential diagnosis of papillary thyroid microcarcinoma(PTMC) and micronodular goiters(MNG). Methods: CT findings of 313 micronodules of thyroid in 212 patients(diameter 0.5 to 1.0 cm) were retrospecctively analyzed. All of the patients had pathologic diagnosis, and 143 leisions in 106 patients were PTMC(15 leisions in 8 patients were combined with MNG), and 170 lesions in 106 patients were MNG. Results: There was statistical difference in pathogenetic number, the appearance of lesions, the margin of the lesions on contrast-enhanced CT scan, the discontinous edge of the thyroids and the fine calcification ratio between PTMC and MNG(P<0.05). However no statistical difference in mixed calcification ratio was found between the two groups(P>0.05). MNG with adenoma were found in 7 lesions, and 5 of them showed higher density than thyroid on post-contrast CT. Eighteen lesions of PTMC and 41 lesions of MNG were not shown on non-enhanced CT, among which 9 PTMC and 36 MNG were combined with thyroiditis. Conclusions: Single lesions with irregular shape, more indistinct margin of the lesion on post-contrast CT than on non-enhanced CT, discontinous edge of the thyroids, fine calcifications and multiple small lymph nodes around the lesion conduce to the diagnosis of PTMC. Multiple lesions with round or oval shape, more distinct margin of lesions on post-contrast CT than on non-enhanced CT, continous edge of the thyroids conduce to the diagnosis of MNG. High degree of enhancement on post-contrast CT conduces to the diagnosis of adnoma or MNG with adenoma. It should be concerned that thyroiditis may hide PTMC.

The application of high-frequency ultrasound in differential diagnosis of benign and malignant small breast lesions

Objective: To explore the value of high-frequency ultrasound in the differential diagnosis of benign and malignant small breast lesions. Methods: A total of 95 cases(112 small breast lesions) confirmed by pathology received ultrasonography examinations, and the shape, edge, echoes, microcalcifications blood flow distribution were evaluated. Results: The differences between benign and malignant small breast lesions(≤1 cm) in shape, edge, microcalcifications and RI were of statistical significance(P<0.05). The sensibility, specificity and accuracy for diagnosing small breast cancer were 88.46%, 95%, 91.96% respectively. Conclusions: High-frequency ultrasound is valuable in differentiating benign and malignant small breast lesions, and improve the rate of diagnosis in small breast cancer.

Assessment of left atrial volume and function by real-time three dimensional echocardiography and LA tracking in essential hypertension patients with different patterns of left ventricular geometric models

Objective: To evaluate the change of left atrial volume and function in essential hypertension(EH) patients with different patterns of left ventricular geometric models using RT-3DE and LA tracking(LAT). Methods: 133 patients with EH were divided into four groups according to Ganau: normal pattern group(group Ⅰ), concentric remodeling pattern group(group Ⅱ), concentric hypertrophy pattern group(group Ⅲ), eccentric hypertrophy pattern group(group Ⅳ). 33 control subjects were enrolled. RT-3DE was used to measure maximal volume, minimal volume, stroke volume, conduit volume and ejection fraction of the left atrium. According to the time-volume curve obtained by LAT, LAVmax’, LAVmin’, LA presystolic volume(LApre) were measured. And then LASV’, LAEF’ and LA passive emptying volume(LAVp) were calculated. The difference between the control group and EH group was compared. Results: Compared with the control group, LAVmax, LAVmin, LASV, LAEF, LAVmax’, LAVmin’, LASV’, LAEF’ in EH group increased, but LACV and LAVp decreased(P<0.05). Comparison within the EH groups: All parameters in the group Ⅲ and group Ⅳ markedly increased than other groups(P<0.01). LAEF was lower in group Ⅳ than group Ⅲ(P<0.05). There was no difference of LACV and LAVp between group Ⅱ and group Ⅰ(P>0.05). Conclusion: Left atrial reservoir function and booster pump function increase in normal pattern of left ventricular geometric models, while left atrial conduit function decreases. Left atrial booster pump function decreases in eccentric hypertrophy pattern. There is no significant difference of left atrial booster pump function between group Ⅰ and group Ⅱ. Combination of RT-3DE and LAT technology can evaluate the left atrial volume and function of hypertension accurately.

Evaluation of cardiac function by echocardiography in newborns and infants with different concentrations of sevoflurane

Objective: To explore the significance of echocardiography in evaluating the effects of inhalational induction with different concentrations of sevoflurane on cardiac function of newborns and infants. Methods: Totally 24 infants from zero to three months old undergoing cutaneous hemangioma operation were randomly divided into three groups according to the inhalation concentration of sevoflurane(6%, 7%, 8%). The values of EF, SV, CO, HR, Sa, Ea and Tei index were calculated before and after induction. Results: After induction, the values of EF, SV, HR, Sa, Ea and Tei index were significantly different from the baseline values in each group(P<0.05). The values of EF, SV, CO, HR, Sa and Ea were not significantly different among each group after induction(P>0.05), while the value of Tei index showed significant difference among the three groups(P<0.05). The value of Tei index of infants inducted with 6% sevoflurane was lower than those of the groups of 7% and 8%(P<0.05), while the latter two were similar(P>0.05). Conclusion: Cardiac function of newborns and infants is lower after inhalation induction with the three concentrations of sevoflurane. The influence of 6% sevoflurane is lower than that of 7% and 8%. Tei index can evaluate the effects of inhalational induction with sevoflurane on cardiac function of newborns and infants.

A retrospective analysis of complications in interventional therapy for 524 patients with Budd-Chiari syndrome

Objective: To discuss the prevention and treatment of complications in interventional therapy for Budd-Chiari syndrome(BCS). Methods: Clinical data of 524 patients with BCS treated via interventional therapy were retrospectively analyzed. Among the patients, 74 cases suffered from intraoperative and postoperative complications(74/524, 14.1%). Types of complications, together with their causes and treatment methods were studied. Results: Among the 74 patients who suffered from complications, death occurred in three and successful treatment was obtained in all the other cases, with a death rate of 0.57%. The intraoperative and postoperative complications included those occurring in the puncture sites(n=6), inferior vena cava thrombosis after early diagnostic angiography(n=3), bleeding(n=6), disseminated intravascular coagulation(n=2), mistakenly puncturing into pericardium(n=16), pericardial tamponade(n=4, death in one), acute restenosis during the treatment via PTA(n=5), postoperative restenosis(n=17), cases related to stents(n=19). The incidence rate related to stents was 13.4%, which accounted for 25.7% in all the cases(19/74). Conclusion: Interventional therapy is safe and effective for the treatment of BCS with the advantage of little trauma. However, many kinds of complications still might happen. All kinds of preventive measures should be taken to avoid or reduce the possible complications so as to improve the clinical success rate of interventional therapy.

An experimental study of contrast enhanced ultrasound in the evaluation of retroperitoneal arterial hemorrhage

Objective: To explore the value of contrast-enhanced ultrasonography(CEUS) in the diagnosis of retroperitoneal arterial hemorrhage. Methods: Five active bleeding sites were created in the external iliac artery of five rabbits. CEUS was used to observe the feature of active bleeding 2 min, 10 min and 40 min after the arterial injury. DSA was also used at 20 min and 30 min after the arterial injury, as a gold standard. Results: 2 min after the arterial injury, CEUS showed active bleeding sites with the feature of jet-like and high flow at the bleeding point. At 10 min and 40 min after the arterial injury, CEUS showed the feature of seeping-out and lower speed flow than before. The results were consistent with DSA. Conclusion: CEUS can display active bleeding of retroperitoneal arterial injury, and it will have good prospects for clinical application.

The diagnostic value of CT histogram analysis in characterization of lipid-poor adrenal cortical adenomas

Objective: To evaluate the diagnostic value of CT histogram analysis in the characterization of lipid-poor adrenal adenomas. Methods: A total of 192 adrenal masses in 184 patients were confirmed either by histopathology or by following up for at least 12 months, including 97 lipid-rich adenomas, 47 lipid-poor adenomas and 48 nonadenomas. Unenhanced CT attenuation was measured on every mass and the percentage of negative pixel was calculated by CT histogram analysis. Receiver operating characteristic curve analysis was performed to choose the optimal cut-off value to differentiate between lipid-poor adenomas and nonadenomas. Result: There was no statistical difference of unenhanced CT attenuation between lipid-poor adenomas (mean (24.90±9.58) HU) and nonadenomas (35.48±8.37) HU(P>0.05), however, lipid-poor adenomas (n=47,mean (19.49±10.31)%) showed significantly higher(t=11.55, P=0.000) negative value percentage than nonadenomas (n=48,mean (1.89±2.23)%). All 31 lipid-poor adenomas with CT attenuations between 10 HU to 30 HU had a negative pixel percentage over 10%. Only 8 of 15 lipid-poor adenomas with CT attenuations above 30 HU had a negative pixel percentage above 10%. At a cut-off value of 10%, the sensitivity and specificity of calculated negative value percentage to diagnose lipid-poor adenomas were 80.9% and 97.9% respectively. Conclusion: CT histogram analysis can help to improve diagnostic accuracy of CT densitometry in characterization of lipid-poor adenomas, especially lipid-poor adenomas with unenhanced CT attenuations between 10 HU to 30 HU.

Comparison of ratio method and score method in the diagnosis of prostate cancer with transrectal real-time tissue elastography

Objective: To compare score method and ratio method in the diagnosis of prostate cancer(PCa) with transrectal real-time tissue elastography(TRTE). Methods: Sixty-seven patients suspected of PCa were examined with two-dimensional and elastosonography. Elastography score and strain index(SI) of the nodules were calculated. The curves of receiver operating characteristic(ROC) were built to find the diagnostic point. The value of TRTE score method and the ratio method in the diagnosis of PCa were analyzed compared with pathological results. Results: TRTE ratio method showed a significant difference of SI between benign and malignant nodules(P<0.05). Compared with digital rectal examination(DRE) and transrectal ultrasonography(TRUS), TRTE ratio method had statistical significance in diagnostic sensitivity, specificity, and accuracy and the two methods above(P<0.05), but there was no significant difference between score method and TRUS(P>0.05). The difference of diagnostic sensitivity, specificity, accuracy was statistically significant between score method and ratio method(P<0.05). Conclusion: TRTE can increase the diagnostic rate of PCa, and the ratio method provides a more objective and valuable information for the diagnosis of PCa.
