The clinical value of color Doppler ultrasound in detecting the changes ofthyroid parameters at different gestational period

Objective: To investigate the clinical value of color Doppler ultrasound in observing the changes of thyroid ultrasound parameters at different gestational period. Methods: Fifty-three non-pregnant women were involved as the control group, in whom the thyroid function is identified normal by the ATA guidelines. Moreover 30 women in the early gestational period, 60 women in the middle gestational period, and 55 women in the late gestational period were enrolled as the study group. The thyroid size, thyroid artery diameter(D), resistance index(RI), end diastolic blood flow velocity(PDV), and systolic peak blood flow velocity(PSV) were measured with color Doppler ultrasound, statistical analysis were also performed. Results: Thyroid artery peak systolic flow velocities in different gestational period were compared with those of the control group, there were statistical difference, which increased more obviously in the early gestational period. The thyroid artery PSV was statistically lower in the late gestational period than that of the early period. The PSV and TSH levels were negatively correlated with those of different gestational period, and positively correlated with FT4, FT3 levels. Conclusion: There are no significant changes in thyroid size, thyroid artery D, and RI in different gestational period compared with those of the control group. In different gestational group, PSV is higher than that of the control group. In the late period, the PSV is lower than that in the early period. In different gestational period the increase of PSV correlated iwth FT4 and FT3.

The analysis of correlation between ultrasonic image and thyroid functional parameters in Hashimoto’s thyroiditis

Objective: To investigate the correlation between ultrasonic image and thyroid functional parameters in Hashimoto’s thyroiditis(HT). Methods: The 2-Dimentional and color Doppler ultrasonographic features of 68 patients clinically diagnosed as HT were analyzed, and the results were compared with thyroid functional parameters(includes FT3, FT4, TSH). Results: All cases were divided into three groups according to the thyroid functional results: normal group, hyperthyroidism group and hypothyroidism group; ultrasonografic features and blood flow grades were different in each group with statistical significance(P<0.01): focal hypoechoic patterns appeared in normal group(7/8, 87.5%), diffuse hyperechoic patterns appeared in hypothyroidism group(4/5, 80%); In CDFI, grade 0 appeared in normal group and hypothyroidism group (21/22, 95.45%), grade Ⅰ~Ⅱ appeared in normal group and hyperthyroidism group(37/42, 88.1%), grade Ⅲ only appeared in hyperthyroidism group(4/4, 100%). The correlation between blood flow grades and thyroid function was statistically significant only in diffuse hypoechoic pattern(P<0.01). Conclusions: The ultrasound image of HT was related to thyroid functional parameters, and could partly reflect thyroid function in helping clinical diagnosis and treatment.

The role of BI-RADS classification in ultrasonic diagnosis of breast cancer

Objective: To study the role of BI-RADS classification(Breast imaging reporting and data system) in ultrasonic diagnosis of breast cancer. Methods: A total of 71 patients with breast diseases who would undergo surgery were selected for study. All the patients underwent bilateral breast examination with high-frequency color Doppler ultrasonography. According to BI-RADS classification, 61 patients(78 breast lesions) with BI-RADS grade 3~5 were recruited. The diagnostic characteristics of high-frequency color Doppler ultrasonography about breast lesions with different BI-RADS were compared with the histopathological diagnosis. Then the efficacy of BI-RADS classification for the identification of breast cancer was assessed. Results: In all the 61 patients(78 breast lesions), confirmed by histopathology, 24 were BI-RADS grade 3, in which 20(83.33%) were benign, 4(16.67%) were malignant. Twenty-nine breast lesions were BI-RADS grade 4, and histopathology confirmed 3(10.34%) were benign, 26(89.66%) were malignant. Twenty-five lesions were BI-RADS grade 5, and all the lesions were malignant confirmed with histopathology(100%). Sensitivity and specificity were 92.73% and 86.96% respectively in the diagnosis of breast cancer by BI-RADS classification. Statistical difference was found in BI-RADS grade 3, 4, 5 in margin, lesion boundary, acoustic feature,the changes of surrounding tissue,calcification in mass, vascularity in lesion blood flow and resistance index(RI). Conclusion: The sensitivity and specificity of the BI-RADS classification were high in diagnosing breast cancer. It has important clinical significance for high-frequency color Doppler ultrasound in the diagnosis of breast cancer and treatment planning.

A comparative study of three-dimensional real-time echocardiography and MRI inthe assessment of right ventricular cardiac function

Objective: To investigate the value of three-dimensional real-time echocardiography in the assessment of right ventricular global function, in comparison with MRI. Methods: Thirty healthy volunteers (16 male, 14 female, average age 49±3.2 years old) underwent three-dimensional echocardiography and breath-hold MRI. Using iE33 medical Philips system, the end-diastolic volume(EDV), end-systolic volume(ESV), stroke volume(SV) and ejection fraction(EF) were determined by TomTec 4D RV-analysis software. The results from echocardiography were compared with those from MRI using Pearson correlation test and Bland-Altman-plot. Results: Mean EDV((143.5±23.2) mL), ESV((68.4±19.2) mL), SV((78.1±11.7) mL) and EF((51.2±5.6)%) determined by echocardiography correlated well with those by MRI((147.5±26.8) mL(r=0.947), (63.2±15.9) mL(r=0.909), (78.8±15.0) mL(r=0.788), (55.2±7.7)%(r=0.627). A strong correlation between three-dimensional real-time echocardiography and MRI was also found in Bland-Altman-plot. Conclusion: Three-dimensional real-time echocardiography can assess right ventricular global function accurately. It is a valuable method for evaluating cardiac function.

Study of radiation dose and image quality of aorta at dual-source Flash Spiral CT

Objective: To evaluate the application of dual-source Flash Spiral CT in aortography, and compare it’s image quality and radiation dose with traditional scanning mode. Methods: 110 patients suspected or known aortic dissection were divided into two groups. Group A underwent traditional scan mode and Group B underwent Flash Spiral scan mode. Both the image quality and radiation dose of the two groups were compared. Results: There was no significant difference in age and body mass index of the two groups. The average image quality score was 2.1±0.7 in group A and 1.4±0.6 in group B, with significant difference(t=5.2, P<0.05). The mean radiation dose of group B was lower than that of group A((2.7±0.6) mSv, (3.9±0.9) mSv; t=2.1, P<0.05). Conclusion: Dual-source Flash Spiral CT scanning with low radiation dose and good image quality in demonstrating aortic dissection with high clinical application value.

Assessment of hepatic fibrosis stage by Gd-EOB-DTPA enhanced MR imaging: a Meta-analysis

Objective: To evaluate the accuracy and quality of Gd-EOB-DTPA-enhanced MR imaging for the diagnosis of staging hepatic fibrosis. Methods: We searched the Chinese Database, Cochrane Library, Pubmed, Embase, Web of science. The retrieval times were from 2004. 1 to 2012. 12. Diagnostic tests of Gd-EOB-DTPA-enhanced MR imaging for the diagnosis of hepatic fibrosis stage were included. Data were extracted, and the quality of included studies was evaluated according to the QUADAS(Quality assessment of diagnosic accuracy studies) items. The heterogeneity test and the SROC(Summary receiver operating characteristc) curve and Meta-analysis were performed by Meta-Disc and Revman. Result: A total of 264 relevant articles were retrieved and 6 studies were included in our review involving 564 participants. Meta-analysis showed that the heterogeneities among studies were high, the pooled sensitivity and specificity of ≥F1, ≥F2, ≥F3, F4 hepatic fibrosis stages were 0.92 and 0.81, 0.76 and 0.86, 0.85 and 0.95, 0.87 and 0.92, respectively. The AUCs of SROC were 0.847 8, 0.932 2, 0.983 7 and 0.932 6, respectively. Conclusion: Gd-EOB-DTPA-enhanced MR imaging has a high value for staging hepatic fibrosis ≥F2, ≥F3, F4.

Value of 3.0T MRI R2* values in diagnosing and staging of liver fibrosis: a preliminary clinical study

Objective: To study the value of MRI R2* values in diagnosing and staging liver fibrosis. Methods: Eight healthy volunteers and 36 pathologically proved and 4 clinically proved liver fibrosis patients were enrolled, they were scanned by 3.0T MR unit with BOLD-MR sequence, R2* maps were reconstructed and mean R2* values were calculated. Correlation between stages of fibrosis and R2* values with non-parameter Spearman test were analyzed, and the R2* values of stages were compared with Dunnett T3 test and Mann-Whitney U test, ROC curve was conducted to evaluate R2* values for prediction of fibrosis and its stage. Results: Mean R2* values of healthy volunteers and liver fibrotic patients were (47.63±5.98) Hz, (60.11±12.18) Hz respectively. Mean R2* values increased during the progression of the liver fibrosis progression, and were positively correlated with the stage of liver fibrosis(r=0.698, P<0.01). There were significant differences of R2* values among the stages of liver fibrosis(F=17.05, P<0.001), and group S0 versus S1, S2, S3, S4, and S1 versus S2, and S2 versus S3(P<0.05, all). Predictor for S1 or greater is better than predictor for S2 or greater, S3 or greater and S4 or greater, with the AUC 0.927, the sensitivity was 0.966 and the specificity was 0.737(R2* ≥49.32 Hz). Conclusion: BOLD-MRI R2* values can reflect the severity of fibrosis, are sensitive and specific in detecting liver fibrosis and distinguishing different stages of fibrosis, and have the potential value in further clinical diagnosis.

CT findings of intra-abdominal lymphangioma in children

Objective: Intra-abdominal lymphangioma is very rare and comprises less than 5% of all lymphangiomas. The aim of this study is to discuss the CT findings of intra-abdominal lymphangioma in children. Materials and Methods: A database of 32 pediatric patients(17 males and 15 females, aged from 2 to 14 years old) with intra-abdominal lymphangioma proved by pathology in Shengjing Hospital from Feb. 2007 to Feb. 2012 was reviewed. CT scan had been performed in all cases, including 28 patients who also had enhanced CT scans. MR had been performed in 4 cases. The CT findings of all cases were reviewed to summarize the CT features of intra-abdominal lymphangioma. Results: In pathology, the tumors originated from mesentery(n=17), omentum(n=9), retroperitoneal space(n=4), and uncertain tissues(n=2) respectively. In CT images, the lesions located in the right side(n=16), the left side(n=7), the middle(n=4), and the whole abdomen(n=5) respectively. Five lesions which located in the whole abdomin all originated from omentum. The lesions were diagnosed as lymphangioma(n=18), cyst(n=4), mesenteric malignant mass(n=1), ovarian tumor(n=2), teratoma(n=2), pancreatic cystadenoma(n=1), duplication of intestine(n=1), and uncertain disease(n=2) based on CT findings, and omission occured in 1 case. Lymphangioma usually appeared in the CT images as a scrotiform tumor with wall and septa(32/32) which could be enhanced significantly after injection of contrast. Most of the wall and septa were thin(27/32) and the contents of the tumors were almost clean fluid(31/32), solid content(1/32), bleeding(2/32), and calcification(1/32) were rarely seen. This kind of tumor often grew through or around the adjacent tissue and organ, including fat(15/32), small bowels(11/32), and vessels(24/28) in our study. Conclusion: CT is very helpful in finding the lesion and making a definite diagnosis of intra-abdominal lymphangioma in children, which is one of the best choice of examination.

Detection of Gamna-Gandy bodies in the spleen with SWI: compared with T1WI, T2WI, T2*WI

Objective: To evaluate the role of abdominal susceptibility weighted imaging(SWI) in the detection of Gamna-Gandy bodies(GGBs) in the spleens of patients with portal hypertension. Materials and Methods: T1WI, T2WI, T2*WI and SWI were performed in 135 patients with portal hypertension. Two radiologists analyzed all the MRI data. The number of patients with GGB in the spleen were determined in four MRI sequences, and the numbers of GGBs were counted in GGB-positive patients. Results: Of the 135 patients with portal hypertension, 51 were GGB positive on SWI, and 17, 15, and 38 were positive on T1WI, T2WI and T2*WI, respectiely. Mean GGB numbers on T1WI, T2WI, T2*WI and SWI were 11.1±9.9, 5.4±5.5, 28.3±25.5, and 40.9±28.4, respectively. The GGB detection rate was significantly greater by SWI than by T1WI, T2WI and T2*WI(χ2=35.69, P<0.05). The number of GGBs detected was significantly greater by SWI than by T1WI, T2WI and T2*WI(P<0.05). Conclusion: SWI is the most sensitive method for detecting GGBs in spleen in patients with portal hypertension.

MRI evaluation of fetal sacrococcygeal teratoma

Objective: To analyze the MRI characteristics of fetal sacrococcygeal teratoma and to determine the value of prenatal MRI. Materials and Methods: From December 2009 to January 2012, 7 cases suspected of having fetal sacrococcygeal tumors by ultrasound, MRI was performed in all of the cases. Pregnant women aged 21~33 years, with the gestational age of 24 weeks to 37 weeks+5, the size, location, signal characteristics, and compressive effects of the masses were determined. Results: The sacrococcygeal teratoma appeared entirely cystic in six fetuses, mixed cystic and solid in 1, type Ⅰ in two patients, type Ⅱ in 2, and type Ⅲ in 1, type Ⅳ in 2, MRI findings of fetal sacrococcygeal teratomas were as follows: fetal sacrococcygeal region tumors with low T1 high T2 signal intensity, the sacrococcygeal teratomas with septa in two, intrapelvic and intraabdominal extent of the tumor in two. which was proved by follow-up or pathologic findings. Conclusion: Fetal sacrococcygeal teratomas had characteristic MR imaging appearances.

Progress in the study of molecular mechanism in water diffusion weighted MRI

This paper briefly introduces the principles and influence factors of diffusion weighted MRI(DW MRI), and multi-b values in clinical application of DWI. The challenges exist in using traditional simple water diffusion to interpret the principle of clinical DW MR imaging were also discussed. The theory of aquaporin and its recent research status, as well as the relationship between apparent diffusion coefficient(ADC) under pathological and physiological condition and water channel protein(aquaproins, AQPs) expression were focused on. Until now, no result was acquired through using molecular imaging technique for direct access of the relationship between ADC and AQP expression. Finally, the new prospect of the water channel protein molecular imaging was introduced. From the aspect of molecular biology, molecular medicine and molecular imaging to renew knowledge and upgrade the point of view in recognizing water diffusion MR imaging and its potential clinical application is urgent and important.
