1H-magnetic resonance spectroscopy analysis of peri-tumor region in brain glioma

Objective: To study the value of 1H magnetic resonance spectroscopy(1H-MRS) in the peri-tumor edema region of brain glioma. Materials and Methods: MRI and MRS data of 32 patients confirmed by clinical or histopathologic results were analyzed retrospectively. MRS analysis data was obtained in the medial and lateral zones of peri-tumor regions and normal brain regions. The concentration of Cho, NAA, Cr and the ratio of Cho/Cr, NAA/Cr, NAA/Cho in the lateral and medial edema regions of peri-tumor and normal brain regions were calculated. Results: NAA/Cho ratio of the normal brain regions, lateral and medial edema zones of peri-tumor was 1.67±0.64, 1.32±1.13, 0.84±0.62, respectively, there were significant difference between them(P<0.05). The Cho concentration of the normal brain regions, lateral and medial edema zones of peri-tumor was 13.28±3.49, 15.66±8.13 and 16.38±7.77, respectively, there was no significant difference between them(P>0.05). The NAA concentration of the normal brain regions, lateral and medial edema zones of peri-tumor was 20.67±5.52, 14.25±5.52 and 10.48±4.82, respectively, there was significant difference between them(P<0.05). The Cr concentration of the normal brain regions, lateral and medial edema zones of peri-tumor was 11.82±3.77, 10.10±3.49 and 10.51±5.18, respectively, there was no significant difference between them(P>0.05). Conclusion: Different metabolic changes of NAA and NAA/Cho in the peri-tumor regions can be detected by using 1H-MRS.

Comparative studies of the value of 18F-FDG PET and EEG in temporal epilepsy patients

Objective: To prospectively evaluate the efficacy of 18F-FDG PET/CT brain imaging and electroencephalogram(EEG) in preoperative localization of temporal lobe epileptic focus in 62 patients. Methods: Sixty-two patients who were clinically diagnosed as temporal lobe epilepsy and received surgical treatment were prospectively studied. All of those patients underwent interictal PET-CT brain imaging examination and long-range/video EEG examination prior to surgery. The researchers analyzed all the images and test results obtained and compared with postoperative pathology. The diagnostic accuracy between the two methods were compared and analyzed using statistical methods. Results: The rate of accuracy in locating epileptic focus was 67.7%(42/62) for PET/CT brain imaging and 48.4%(30/62) for long-range or video EEG, there was significant difference of accurate localization rate between the two methods(P<0.05). Conclusion: Interictal PET-CT brain imaging is better than long-distance/video EEG examination in preoperative localization of temporal lobe epileptic focus. PET-CT brain imaging is too expensive, so EEG can be used as the preferred method in the clinical application. PET imaging can significantly improve the rate of accuracy in locating epileptic focus when there are difficulties in the localization of temporal lobe epileptic focus with EEG.

Imaging features and pathologic study of peripheral primitive neuroectodermal tumors

Objective: To explore the pathology and X-ray, CT and MR images of peripheral primitive neuroectodermal tumors(PPNET) as well as correlate with pathologic results, to improve the diagnostic accuracy of peripheral primitive neuroectodermal tumors. Materials and Methods: X-ray, CT and MRI findings in 10 patients with PPNET were retrospectively analyzed, then correlated with pathology. Results: In the ten lesions 3 were located in the tibia, 1 in humerus, 2 in chest wall, 2 in pelvic bone, 1 in inguinal region, 1 in retroperitoneum. The X-ray findings of 4 cases were lytic bone destruction, lytic lesion with irregular sclerosis and expansion(1 case). The CT findings included irregular lytic lesion(4 cases), lytic lesion with rim-like sclerosis and expansion(1 case). Five tumors in soft tissue showed ill-defined, irregular and heterogenous cystic areas. The tumors showed moderate heterogeneous enhancement after injection of contrast agents. The 10 cases in bone and soft tissue MRI findings showed iso- or hyperintensity on T1WI, and heterogeneous hyperintensity on T2WI. The tumors showed heterogeneous enhancement and reticular enhancement after injection of contrast agents. Homer-Wright daisy-group was a specific pathologic feature of PPNETs. According to immunohistochemical results, all tumor cells showed positive for CD99, and showed various degrees of antigen in epithelium, including NSE, Syn, S-100 and Vimentin. Conclusion: If X-ray, CT and MRI can be used reasonably, the accuracy of diagnosis of PPNET can be improved, the definite diagnosis is depended on the pathological and immunohistochemical results.

Research of stent fracture in revascularization for symptomatic ostial vertebral artery stenosis

Objective: To analyse the reasons of stent fracture in revascularization for symptomatic ostial vertebral artery stenosis. Methods: A retrospective study was performed to evaluate 230 patients with symptomatic ostial vertebral artery stenosis who underwent PTAS. Restenosis was found in 10 patients by DSA, with 14 stents fractured totall. As a result, 8 patients suffered correlated symptoms. Results: Metallic characteristics, smaller internal diameter and longer length of stent, circuity of ostial vertebral artery stenosis, as well as repeated overexpansion are potentially contributed to stent fracture. Conclusion: Selecting suitable stents, avoiding repeated overexpansion, controlling of hypertension and smoking cessation may decrease stent fracture and avoid vertebral artery restenosis after endovascular intervention.

Correlation between intima-media thickness of the carotid artery and left ventricular remodeling inessential hypertension patients

Objective: To investigate the relationship between carotid artery intima-media thickness(IMT) and left ventricular remodeling in essential hypertension(EH) patients. Methods: Carotid and cardiac ultrasonography were performed on 78 patients with EH from our hospital and 30 healthy aged persons were selected as normal control. The interventricular septum thickness(IVST), left ventricular posterior wall thickness(LVPWT), left ventricular end diastolic diameter(LVDd), intima-media thickness in common carotid artery(CCA-IMT) and its bifurcation(BIMT), plaque and hemodynamic parameters in carotid artery were measured respectively, according to corresponding formula to obtain left ventricular mass index(LVMI). According to left ventricular mass index(LVMI), EH patients were divided into left ventricular hypertrophy(LVH) group and non-left ventricular hypertrophy(NLVH) group. Statistical analysis was carried out. Results: The IMT and BIMT were higher in EH group than those in the control group, and LVH group is the highest among the three groups. The prevalence of carotid plaque in EH group was higher than that in control group. Compared with the control group, peak systolic velocity(PSV) of carotid artery was lower in EH group, whereas resistance index(RI) and pulsatility index(PI) were higher. There were positive correlation between LVMI and CCA-IMT, BIMT, RI, PI, and negative correlation between LVMI and PSV. Conclusion: Color Doppler ultrasound is a reliable method to detect the carotid remodeling and left ventricular remodeling in EH patients. The occurrence of carotid remodeling is earlier than that of the left ventricular myocardial remodeling, and there is a close correlation between them.

Compare visualization of the peripheral pulmonary arteries of different parameters withMSCT angiography of the pulmonary arteries

Objective: To assess visualization of the peripheral pulmonary arteries under different concentration of contrast medium, flow injected rate of iodine and different collimation with MSCT. Methods: One hundred patients who met the inclusion criteria were detected by thoracic enhanced CT scan from February to October 2011 in our hospital. These people were divided into 4 groups(Groups A, B, C, D) according to different concentration contrast medium, flow injected rate of iodine and 2 groups of different collimation(Group D 0.6 mm, Group E 1.5 mm). And statistical analysis of the CT values of pulmonary trunk, left and right pulmonary arteris and the fourth- order arteries. The visualization of the fifth- and sixth- order arteries were measured. Results: The differences of CT values of pulmonary trunk, left and right pulmonary arteries and the fourth- order arteries were statistical significance(P<0.05) and there was no statistically significant in the total number of the fourth- order arteries(P=0.19) in the five groups. There were statistical significance in the visualization of fifth- order arteries in lower lobe and sixth- order arteries in each lobe. There was no statistical significance in the visualization of fifth- order arteries(P=0.48) and the opposite conclusion in sixth- order arteries(P=0.00) in group D and group E. Conclusion: Using 0.6 mm collimation, 350 mgI/mL concentration contrast medium and 5 mL/s flow injected rate of iodine in MSCT angiography of the pulmonary arteries can significantly increase visualization of the peripheral pulmonary arteries.

The diagnostic value of echocardiography on mixed total anomalous pulmonary venous connection

Objective: To study the anatomy variants and diagnostic methods for the mixed total anomalous pulmonary venous connection(TAPVC), and the value of echocardiography as a diagnostic tool for this disease. Methods: To retrospectively analyze the main imaging data for those who were diagnosed as mixed TAPVC in our tertiary center in recent 5 years. Results: Among the 17 mixed TAPVC patients, 14 were supracardiac plus cardiac, 3 were supracardiac plus infracardiac. Pulmonary venous obstruction as observed in 5 of them, all by echocardiography, and only 1 by multi-detector CT scan. Echocardiography detected all pulmonary veins precisely in 7 patients, 3 pulmonary veins correctly but 1 incorrectly in 4 patients. Conclusion: Supracardiac plus cardiac are the most common type of mixed TAPVC. The combination of echocardiography and multi-detector CT scan are the best strategy for preoperative evaluation of these patients.

Application of MR True FISP sequence in evaluating invasion of portal vein by hepatic carcinoma

Objective: To explore the accuracy of magnetic resonance true fast imaging with steady precession(True FISP) in diagnosing invasion of portal vein by hepatic carcinoma and establish the valuable system of non-invasive imaging technology to assess portal vein invaded by hepatic carcinoma and the method of choice for surgical planning. Materials and Methods: Ninety cases with clinical diagnosis of liver carcinoma and highly suspicious of having portal vein invasion were collected, all cases were proved by surgical pathology or lesion biopsy. The features of MRI were collected and analysed, the features of True FISP were correlated with the images of portal venous phase enhancement, surgical results, and/or biopsy results. Results: Both of the image quality of True FISP sequence and portal venous phase enhancement images were good,and were 100% diagnostic. True FISP sequence in the diagnosis of portal venous invasion by hepatic carcinoma has good correlation with pathologic results. Conclusion: Standard MR True FISP sequence proved to be the best image modality for displaying portal vein and liver lesions. MR True FISP can be used to diagnose portal venous invasion due to hepatic carcinoma accurately, and can be used to establish a valuable system of non-invasive imaging examination for showing portal invasion by liver cancer, thereby useful in guiding surgical planning.

The influence of gallbladder fossa fluid accumulation on gallbladder wall thickness in liver cirrhosis onsonographic imaging correlation with multi-slice CT scanning

Objective: To discuss the difference of gallbladder wall thickness in liver cirrhosis with gallbladder fossa fluid accumulation on ultrasound scanning and CT multi-plane reconstruction images. Materials and Methods: Ultrasound scanning and multi-slice CT were performed in 39 patients who had gallbladder fossa fluid accumulation with liver cirrhosis, all cases were under fasting condition. Gallbladder wall thickness was measured both on ultrasonographic and multi-slice CT 3D reconstruction images. The scatter diagram was drawn and Pearson correlation analysis and paired sample t test comparison of gallbladder wall thickness were also carried out. Result: Compared with multi-plane CT reconstruction, ①Ultrasound missed the diagnosis of gallbladder fossa fluid accumulation in 31 cases, over estimation of gallbladder wall thickness in 16 cases, and under estimation of the thickness in 2 cases. ②The correlation coefficient of ultrasound detection of gallbladder wall thickness with CT detection was 0.648(P<0.000). ③Ultrasound detection of gallbladder wall thickness was(6.2±2.6)mm, CT detection of gallbladder wall thickness was (4.5±2.0)mm, there was statistical significance(t=4.465, P<0.000). The coefficient of variation was 0.42 and 0.44 respectively. Conclusion: Multi-slice CT scanning is more sensitive in the detection of limited gallbladder fossa fluid accumulation than ultrasound. It can accurately differentiate gallbladder fossa fluid accumulation with gallbladder wall thickening in liver cirrhosis.

The research on transrectal ultrasonography in diagnosing female pelvic inflammatory diseases

Objective: This study is to research the value of transrectal ultrasonography(TRS) in diagnosing female pelvic inflammatory diseases and to compare TRS technique with traditional transabdominal ultrasonography(TAS) technique. Methods: Sixty-four gynecological patients with clinical diagnosis of pelvic inflammatory diseases were selected. They received routine TAS exam followed by TRS exam with transvaginal intracavitary probe after urination and defecation. TRS technique was compared with TAS technique in diagnosing pelvic inflammatory diseases, in order to explore the differences of the two ultrasound techniques in visualization and diagnosis rates of abnormal sonogram. Results: The detail visualizing ability in near field(uterus/ovary-center) of TRS was better than TAS significantly(P<0.01). Compared with traditional TAS technique, TRS technique could increase visualization ratio of abnormal sonogram significantly such as abnormal echo of endometria, thickening of fallopian tube, fluid accumulation and pus in Douglas pouch(all P<0.05). The detecting ability of TRS was better than TAS for a small quantity of fluid in uterus and in Douglas pouch, fallopian tube thickening, but abnormalities in superior position and far field should be depleted. Conclusion: TRS was a supplement of ultrasonography technique in diagnosing gynecological diseases. It was better than TAS technique in diagnosing pelvic inflammatory diseases and could be used as auxiliary diagnostic method for female upper reproductive tract infections.
