The CT appearance of some parts of fissura fossa canal in aural region

Objective: To explore normal CT appearances of fissura fossa canal in the aural region. Materials and Methods: Thirty-three normal ears were investigated by axial and sagittal views on multi-slice CT. Results: Both fissura petrotympanica and fissura tympanosquamosa were divided into external and internal parts. The external part of fissura petrotympanica communicated with the internal part and showed a curved linear form. The external part of fissura typanosquamosa approximated a triangular shape in the axial view and corniform shape in the sagittal view. The fissura petrosquamosa of adults showed thread like appearance. All of the three fissuras, the internal fissura tympanosquamosa, fissura petrotympanica and fissura petrosquamosa, showed a transverse Y shape structure. The posterior canal of chorda tmpani in eminence chorda tympani showed short thread form. Fossa of incus included short crus of incus and posterior ligament of incus, the depth was about 1.5 mm, showed a wedge shape in axial view. Stapedius duct connected with facial canal through stapedial nerve duct, it was divided into mastoid segment and eminentia pyramidalis segment, the former approximated a round shape in axial view, and the latter showed an arc-like girdle shape. Conclusion: Sixty-four row spiral CT can disclose fissure fossa canal in the aural region satisfactorily.

Clinical study on endovascular treatment for thrombosis of intracranial venous sinus

Objective: To observe the effect and safety of combined intravenous and intra-arterial thrombolysis therapy for intracranial venous sinus thrombosis(CVST). Twenty-one patients with CVST who accepted endovascular therapy were analysed retrospectively. Methods: Twenty-one patients diagnosed CVST with CTA, MRA and/or DSA. The time of onset of illness, the treatment history and the curative effect before the therapy were recorded for each patient. Then each of them were treated with multiple modality of endovascular thrombolysis including intra-arterial thrombolysis, intravenous thrombolysis, mechanical thrombus maceration, lied up micro-catheter thrombolysis. The relationship between the operative time and the curative effect, complication and treatment were analysed. Results: CVST reopened in 14 cases, partly reopened in 6 cases and no effect in 1 case. Balloon ruptured in 1 case, in one patient the cerebral superficial vein ruptured without get the surgical repaired at that time, and one week later ruptured again. The effective rate was 95.2%, the rate of complications of the therapy was 9.5%. Follow-up for 1~2 years, no patient recurred except one case. Conclusions: The endovascular therapy is safe and effective in treating patients with CVST, but the course of disease must be within 8 weeks. The shorter the course, the result will be better. Most complications are due to manipulating technique.

Qualitative and quantitative analysis and the relevance between contrast enhanced sonography ofbreast cancer and the expression of p53

Objective: To analyze relationship between contrast-enhanced ultrasound qualitative and quantitative features and expression of p53 in breast cancer. Methods: The contrast-enhanced ultrasound appearances of 37 breast carcinomas confirmed by pathology were analyzed. Immunohistochemical method was used to test the expression of p53 in all the breast carcinomas, to analyze the relationship between it and the contrast-enhanced ultrasound appearances. Results: ①The expression rate of p53 were 43.2%(16/37). ②The main enhanced pattern of breast carcinomas was fast and markedly enhanced, which was not related to the expression of p53(P>0.05). ③The rate of perfusion defect or perforative vessels increased in breast carcinomas with p53 positive expression(P<0.05). ④The qualitative and quantitative analysis from the intensity-time curve of contrast-enhanced ultrasound in breast cancer showed, the peak intensity and the area under the curve were bigger in breast carcinomas with p53 positive expression compared to the breast carcinomas with p53 negative expression(P<0.05). Conclusion: Qualitative and quantitative features of contrast-enhanced ultrasound were related to the expression of p53 in breast cancer in some extent. Quantitative analysis from intensity-time curve can give more accurate information about the blood supply of breast carcinoma, which will be helpful for selecting correct therapy method.

The differential diagnostic value of 99Tcm-MIBI SPECT/CT fusion imaging for lung lesions

Objective: To investigate the differential diagnostic value of 99Tcm-MIBI SPECT/CT fusion imaging for lung lesions, and to establish a cost-effective imaging modal in the detection of malignant lung lesions. Methods: From December 2008 to May 2011, we had prospectively studied 45 consecutive patients with suspicious malignant lung lesions. Ten minutes and 2 hours after 99Tcm-MIBI injection, early and delayed SPECT/CT fusion imaging of the chest were performed respectively. The uptake ratios of malignant and benign lung lesions were compared by independent-samples T test. In addition, the diagnostic efficiency was analyzed with receiver operating characteristic(ROC) curve. Results: There are 30 patients with primary lung cancers and 15 patients with benign lung lesions. The sensitivity, specificity, accuracy, positive predictive value(PPV), and negative predictive value(NPV) of qualitative analysis were 96.7%, 73.3%, 88.9% 87.9% and 91.7% respectively. In patients with malignant lesions, the early mean T/N ratio value±standard deviation 2.80±2.21(95%CI: 1.97, 3.62), whereas in patients with benign lesions was 1.56±0.71(95%CI: 1.17, 1.95). In patients with malignant lesions, the delayed mean T/N ratio value±standard deviation was 2.93±1.86(95%CI: 2.24, 3.63), whereas in patients with benign lesions was 1.64±0.81(95%CI: 1.20, 2.09). The T/N ratio had significant difference between malignant lung lesion and benign lung lesion(t=-2.10, t=-2.56, respectively. P<0.05). Semiquantitative analysis showed that for an early T/N value≥1.50, the value of sensitivity and specificity was 96.7% and 73.3%, respectively(ROC curve), for a delayed T/N value≥1.459, the value of sensitivity and specificity was 100% and 66.7%, respectively(ROC curve). Conclusion: Our study shows that 99Tcm-MIBI SPECT/CT fusion imaging has highly clinical value in differentiating malignant from benign lung lesions, and is a practical method for medium-to-low income patients with lung lesions in small and medium-sized cities.

Study of the correlation between DSCT and echocardiography in assessment ofleft ventricular function parameters

Objective: To investigate the correlation and concordance between dual source CT(DSCT) and echocardiography in the assessment of left ventricular function parameters. Methods: Seventy-nine patients who underwent DSCT coronary angiography and echocardiography were studied in two groups. Group 1 consisted of thirty-nine consecutive patients, who were enrolled with clinic proved chronic myocardial infarction(MI) and group 2 consisted 40 consecutive patients with no prior history of acute MI as control. All of them underwent echocardiography within a week. The correlation and concordance between DSCT and echocardiography in evaluating left ventricular ejection fraction(LVEF) and left ventricular fractional shortening(LVFS) was assessed. Results: Results of DSCT and echocardiography for LVEF had a good correlation and concordance for the two groups(control group r=0.751, and MI group r=0.718, P<0.01). There was median correlation between LVFS by using DSCT and echocardiography assessment LVFS(control group r=0.543, MI group r=0.606, P<0.01). Conclusion: DSCT coronary angiography can be used in diagnosing coronary lesions and in quantitative assessment of left ventricular function parameters simultaneously.

Ultrasonic features of hepatic veins and collateral circulations in Budd-Chiari syndrome in Henan Province

Objective: To investigate ultrasonic imaging features of hepatic veins and collateral circulations in Budd-Chiari syndrome(BCS) in Henan Province. Methods: Sonograms of 567 patients with BCS were analyzed. Results: ①There were 517 branches of middle hepatic vein(MHV) had pathological changes, 507 branches of left hepatic vein(LHV) had pathological changes and 418 branches of right hepatic vein(RHV) had pathological changes. The main lesion type of LHV and MHV was membranous occlusion. The main lesion type of RHV was streak-like stenosis. ②There were 403 patients with dilated accessary hepatic veins(AHVs) in 567 patients with BCS. In them, there were 27 branches with pathological changes. There was a significant difference in AHVs with or without pathological changes between unobstructed inferior vena cava(IVC) and obstructed IVC(P=0.000, continuity correction). ③There were 8 types of collateral circulation. ④There were 395 patients with collateral circulation wiehin liver in 567 patients with BCS. There was a significant difference in collateral circulation with MHVs between caudate lobe veins and right posterior veins(P=0.000). ⑤There were 331 branches of LHV with collateral circulation, 319 branches of MHV with collateral circulation and 104 branches of RHV with collateral circulation. There was a significant difference in collateral circulation between LHV, MHV and RHV(P=0.000). There was no a significant difference in collateral circulation between LHV and MHV(P=0.234). There was a significant difference in collateral circulation between LHV and RHV(P=0.000). There was a significant difference in collateral circulation between MHV and RHV(P=0.000). Conclusions: ①The features of MHVs of BCS in Henan Province of China which had a high affection rate, more dilated AHVs and collateral circulations in liver. ②There are abundant types of collateral circulation in liver of patients with BCS. There are more collateral circulations between caudate lobe veins and MHVs than that between right posterior veins and MHVs. There are more collateral circulations of LHV and MHV than that of RHV. The main affection category of MHVs concerned with quantity of collateral circulation. ③Some AHVs have pathological changes. The affection of IVC is not the major factor for AHVs.

Study of fetal lung volume using virtual organ computer aided analytical technique

Objective: To evaluate the correlation between fetal lung volumes and gestational age, and estimated fetal weight, and to evaluate the potential diagnostic value of ultrasound fetal lung to body weight ratio(UFLB) in fetal pulmonary hypoplasia(PH). Methods: The fetal lung volumes were assessed in 315 normal fetuses and 28 fetuses with high risk of PH using the technique of virtual organ computer-aided analysis(VOCAL). The relationships between the lung volumes and gestational age, and fetal weight were assessed by correlation and regression analysis. The value of UFLB in the diagnosis of fetal PH was acquired by compared with the postnatal findings. Results: Fetal lung volumes were highly correlated with gestational age(r=0.93,P<0.05), and with fetal weight(r=0.97,P<0.05). The sensitivity, specificity, positive and negative predictive values of UFLB in diagnosing fetal PH was 89.47%, 85.71%, 94.44% and 75.00%, respectively, and the accuracy was 88.46%. Conclusion: In normal fetuses, fetal lung volumes increase with gestational age and fetal weight. UFLB may be useful in the prenatal diagnosis of fetal PH.

The value of single photon emission computed tomography/spiral computed tomography fusion imaging forthe differential diagnosis of benign and malignant skull lesions

Objective: To evaluate the value of single photon emission computed tomography(SPECT)/spiral computed tomography(CT) fusion imaging in the differential diagnosis of benign and malignant skull lesions. Methods: Nineteen cancer patients with 21 skull lesions on whole-body bone scan(WBS) were recruited and underwent SPECT/CT fused imaging of the head. The images were evaluated by two independent reviewers. The patients underwent MRI and/or CT examinations within two weeks after SPECT/CT. An average follow-up interval after SPECT/CT was 9 months to correlate with additional radiologic imaging studies and clinical informations. Results: Ten lesions were classified as metastatic lesions and 11 lesions were classified as benign based on the other radiological examinations performed at the same period and follow-up. The SPECT/CT correctly identified 10 out of 10 metastatic lesions and 8 out of 11 benign lesions, 85.71% of lesions were correctly classified. One lesion was classified incorrectly as metastatic lesion with SPECT/CT while it was proven benign by other imaging modalities and follow-up. Two lesions remained indeterminate even after the SPECT/CT examination and were confirmed benign by other imaging modalities and follow-up. The sensitivity, specificity, positive and negative predictive values of SPECT/CT images in the diagnosis of malignant skull lesions were 100.00%, 88.89%, 90.91% and 100.00%, respectively. Conclusions: SPECT/spiral CT can help identify benign versus metastatic skull lesions with high sensitivity and specificity.

Preoperative assessment of medial sural artery perforator flap by color Doppler flow imaging

Objective: To evaluate the application of color Doppler flow imaging(CDFI) in the medial sural artery perforator(MSAP) flap. Methods: CDFI and acoustic Doppler ultrasound were compared in evaluating the number, location, diameter of perforator and main vascular traveling by single-blind trials, the data were compared with intraoperative anatomical finding. Results: In this study eight flaps were transplanted, 24 perforating arteries were involved. Totally 22 MSAPs were found by preoperative CDFI, and 17 by acoustic Doppler ultrasound, 2 and 6 perforators were misdiagnosed respectively, 2 and 7 were missed respectively. There were significant statistical differences between these two methods(P<0.05). The measurement of diameter by CDFI have no significant statistical difference with the finding in operation(P>0.05), and 16 vascular bifurcation detected by CDFI was accordance with that found in intraoperative anatomy. Flap survived well with no significant postoperative complications in 8 cases. Conclusion: CDFI is very useful for preoperative planning of medial sural artery perforator flap and increase the reliability of the perforator flap.
