The whole brain CT perfusion imaging findings of symptomatic severe stenosis or occlusion of intracranial internal carotid artery

Objective: To study the hemodynamic patterns demonstrated in whole brain CT perfusion images(CTPI) in patients with symptomatic severe stenosis or occlusion of intracranial internal carotid artery(ICA). Materials and Methods: The CTPI data of 23 patients with symptomatic severe stenosis(>70%) or occlusion of ICA were analyzed retrospectively. The 4 dimensional CT angiography(4D-CTA) and the parameter maps of CTPI including cerebral blood volume(CBV), cerebral blood flow(CBF), mean transit time(MTT), time to peak(TTP) and delay maps were generated in the off-line workstation by using specific software. The stenosis degree of ICA, clinical symptoms, location of infarction and features of CTPI and the values of parameter maps were recorded. Paired t-test was used to compare the values of ipsilateral with contralateral normal areas in CTP maps. Results: Fifteen patients had occlusion and 8 patients had severe stenosis of ICA. Thirteen cases had infarction in middle cerebral artery(MCA) or anterior cerebral artery(ACA) areas, 6 cases had watershed infarction, 2 cases had lacunar infarction and 2 cases had no infarction. The patterns of CTPI abnormalities included the following: ①Normal or slightly increased CBV/CBF and increased MTT/TTP/Delay time(4 csaes). ②Normal CBV, slightly decreased in CBF and decreased in MTT/TTP/Delay time(3 cases). ③Decreased obviously in CBV/CBF, decreased in regional MTT associated with prolong MTT/TTP/Delay time(16 cases). CBV, CBF, TTP and Delay values of ipsilateral side brain areas had significant difference when compared with those of contralateral brain areas(P<0.05). Conclusions: 320-row CT is a reliable technique in showing intracranial ICA stenosis and perfusion abnormalities.

The value of shear wave elastography in differential diagnosis of breast lesions

Objective: To obtain the elasticity value of benign and malignant breast lesions with supersonic shear wave elastrography(SWE), in order to assess the value of quantitative elastography with SWE in differential diagnosis of benign and malignant breast lesions. Methods: SWE was performed in 67 breast lesions of 51 female patients with pathologically confirmed results, to obtain the elastic modulus(mean, maximum, minimum, standard deviation (SD)) of breast lesions. The value of SWE and mammography BI-RADS classification in differential diagnosis of breast lesions was compared. Results: The mean, maximum and SD of elasticity value for benign lesions were (25.21±16.45) kPa, (39.18±31.20) kPa and (6.19±7.66) kPa respectively; whereas those for malignant ones were (59.05±36.19) kPa, (108.72±45.17) kPa and (20.01±12.27) kPa, respectively(P<0.05). The area under the curve(AUC) of the mean and maximum elasticity value was 0.814 and 0.908. Taking 40.2 kPa and 58.79 kPa as the threshold of the mean and maximum elasticity value, the diagnostic accuracy of maximum is higher than that of the BI-RADS classification. Conclusion: The mean, maximum and SD of elasticity value can provide diagnostic evidence for benign and malignant breast lesions. Maximum has a higher value than the BI-RADS classification in the differential diagnosis of breast lesions.

The value of 2D speckle tracking imaging in assessing right ventricular function of atrial septal defectaccompanied with pulmonary hypertension after transcather closure

Objective: To evaluate right ventricular function of patients with atrial septal defect(ASD) and pulmonary hypertension by 2D speckle tracking imaging(STI). Methods: Peak systolic strain(£), peak systolic strain rate(SRs), early diastolic strain rate(SRe) and late diastolic strain rate(SRa) were measured in basal and mid segment of right ventricular free wall in 30 patients of ASD with pulmonary hypertension before surgery and a week after surgery by STI from the apical 4-chamber view. The chamber size was measured by two-dimensional ultrasound in 31 healthy subjects as controls. Results: It was showed that the £, SRs, SRe and SRa in basal and mid segment of right ventricular free wall in the ASD group were significantly lower than those in the control group. In the ASD group, a week after transcatheter closure, RA, RV and PASP decreased, while £, SRs, SRe and SRa increased in basal and mid segment of the right ventricular free wall. Conclusions: The right ventricular function of the ASD patients with pulmonary hypertension degraded in different degrees. The right ventricular function is improved significantly after transcatheter closure. Right ventricular function can be accurately and objectively evaluated by STI.

Value of low-dose adaptive cardio sequence scan for dual-source CT coronary angiography

Objective: To evaluate the value of low-dose adaptive cardio sequence scan of dual-source CT angiography(DSCTA). Methods: Fifty-one patients with heart rate(HR)≥65 bpm underwent DSCTA with adaptive cardio sequence. They were divided into two groups according to the average HR of patients: For group A with 65 bpm≤HR<75 bpm, 70% of R-R interval was used. For group B with HR≥75 bpm, 40% of R-R interval was chosen. Patients with a body mass index(BMI)≥25 kg/m2 were examined with a tube voltage of 120 kV, whereas BMI<25 kg/m2 were examined with 100 kV. Both image quality and radiation dose of the two groups were analyzed. Results: A total of 299 coronary artery segments were evaluated in group A and 345 segments in group B. The third and forth grade of the coronary segments for two groups were 91.3% and 92.2%; Right coronary artery(RCA) had the largest percentage of coronary segments in the first and second grade for two groups, and the next was left circumflex artery(LCX); The range of heart rate was 3~44 bpm. The mean score of image quality of group A and group B was (3.53±0.73) and (3.59±0.63) scores, while the total mean effective dose was (2.85±0.93) mSv and (3.10±0.86) mSv. Conclusion: Low-dose adaptive cardio sequence scan for DSCTA can substantially reduce radiation exposure, and obtain good diagnostic image quality in patients with HR≥65 bpm.

Meta-analysis of low-dose CT for urolithiasis

Objective: To summarize the accuracy of low-dose CT(LDCT) for urolithiasis with Meta-analysis. Materials and Methods: Published studies were identified by Pub/Medline and Ovid databases and Chinese databases(1990—2011) in search of LDCT studies for urolithiasis. A standard extractive database was obtained. Inclusion and exclusion criteria were used to extract relevant data. QUADAS was analyzed. Using software Excel 2003 and Metadisc 1.4, the summary sensitivity and specificity, summary positive likelihood ratio and summary negative likelihood ratio were caculated. Heterogeneity and biase were also evaluated. Result: Of 264 identified studies, 9 involving studies were considered appropriate for pooled analysis. There were 1298 cases all together. The pooled sensitivity, specificity, positive likelihood ratio, negative likelihood ratio were 0.959(95% CI: 0.944~0.972), 0.957(95% CI: 0.934~0.973), 17.853(95% CI: 11.735~27.161) and 0.045(95% CI: 0.025~0.078), respectively. Conclusion: LDCT is of high specificity and sensitivity for urolithiasis. But there were biases in the studies and appropriate studies are needed.

The application of spatio-temporal image correlation technique in diagnosis of congenital heart disease

Objective: To study the clinical value of spatio-temporal image correlation(STIC) technique in congenital heart disease(CHD). Methods: STIC was used to acquire 3D datasets in 22 fetuses, which were diagnosed as CHD with 2D echocardiography. Then the data was analyzed offline. Results: All image quality of the 22 cases was satisfied. After processing the datasets, we acquired not only standard planes, but also some extra planes, which could not be shown in common 2D echocardiography. Conclusion: STIC provides more information for the diagnosis of CHD, and it is a complement for 2D echocardiography.

Diagnosic value of transabdominal sonography and three-dimensional transvaginal ultrasound in screening for fetal malformations during 11~14 weeks

Objective: To evaluate the value and the accuracy of transabdominal sonography(TAS) and three-dimensional transvaginal ultrasound(3D-TVS) examination in the diagnosis of fetal malformations during first trimester and second trimester. Methods: We summarized and analyzed retrospectively the ultrasonic image features of 26 cases, wihch were diagnosed as fetal structural abnormalities by TAS and 3D-TVS at 11~14 weeks gestation, then confirmed at second trimester or by postmortem after induction. Results: Twenty-four cases were followed up, and 2 cases lost contact. Only one with conjoined twins was missed at 11 weeks of gestation. Other fetal structural abnormalities were consistent with the TAS and 3D-TVS findings. Conclusions: TAS and 3D-TVS examination have important value in the early detection of fetal structural abnormalities. Standardization of the examination is the key to reduce misdiagnosis.

Subintimal angioplasty treatment for arteriosclerosis obliterans of lower extremity

Objective: To explore the method and effectiveness of subintimal angioplasty(SIA) treatment for arteriosclerosis obliterans disease of lower extremity. Methods: Between Jun 2007 and Jun 2010, 55 patients(60 limbs) with arteriosclerosis obliterans disease of lower extremity in our hospital were treated by interventional therapy. SIA was used for recanalization of the occlusive arteries. SIA treatment techniques, clinical effect, perioperative complications and follow-up results were analyzed. Results: The technical success rate of SIA was 91.7%(55/60), with the clinical symptoms disappeared or improved significantly. Ankle-brachial index(ABI) increased from 0.40±0.11 to 0.82±0.13 with significant difference(P<0.000 1, t=18.29). The perioperative rate was 0%. Five patients(8.33%) had perioperative complications, with 3 cases of hematoma and 2 cases of artery perforation. Four cases of restenosis were treated by PTA during midterm follow-up. The primary patency rates were 81.6% (31/38) and 73.9%(17/23), and the secondary patency rates were 89.5%(34/38) and 78.3%(18/23) at 12 and 24 months, respectively. The limb salvage rate was 100%(23/23) at 24 months. Conclusion: SIA in total occlusive disease of lower extremity is safe and effective. The patency and limb salvage rates can be achieved.

The value of proximal femoral geometric structure and bone mineral density in evaluating the risk of hip osteoporotic fracture

Objective: To evaluate the influence of hip osteoporotic fracture on geometric structure of the proximal femur, to study the influence of bone mineral density and its value in predicting hip osteoporotic fracture. Material and Methods: Two hundred and thirty-three cases examined by dual energy X-ray absorptiometry(DXA) were collected. There were 46 cases in the group of fresh hip osteoporotic fracture and 187 cases in the control group. Bone mineral density(BMD) and geometric structure of proximal femur(PFG) were measured for the uninjured hip of fracture group and the left hip of control group with Osteocore 3 DXA and senior orthopaedic special analysis tool, including femoral neck axis length(FNAL), femoral neck angle(FNA), medial offset(MO) and intertrochanter head(IH). Results: The FNAL, IH and MO in the hip fracture group were longer(P<0.05) than those in the control group, but the FNA was smaller(P<0.05). There were significantly difference between the two groups. Conclusions: The geometric structure difference of FNAL, FNA, MO, IH may add load to proximal femoral, which can increase the risk of hip fracture. Screening the osteoporotic patients with anomalous PFG and taking preventive measures and treatment are important to reduce the risk of hip fracture.
