305 Correlative analysis between imaging manifestations and pathological features ofoligodendrogliomas in different grades

Objective: To explore the imaging features of different grades of oligodendrogliomas and pathological results for guidance of treatment. Methods: Imaging findings of 57 oligodenodrogliomas of different grades confirmed by pathology were retrospectively analyzed. Results: Thirty-five patients were grade Ⅱ oligodendrogliomas(WHO Ⅱ grade), with a median age of 34 years. Most of the tumors occurred in the cortical and medullary junction of parietal and frontal lobes, with ill defined boundary. 29 cases were slightly low density and 6 cases were mixed density on CT. On T1WI, 31 cases showed low signal and 4 showed equal signal. On T2WI, 35 cases showed high signal. Enhanced CT scan showed mild enhancement in 32 cases and heterogeneous enhancement in 3 cases. Thirty-two cases had calcification; 7 cases had cystic degeneration; 8 cases had bleeding and 5 cases had peritumoral edema. Anaplastic oligodendrogliomas(WHO Ⅲ grade) were found in 22 cases with a median age of 32 years. Most of the tumors occurred in cortex and medulla junction of frontal, parietal and temporal lobes, with clear boundary. On CT images, 10 cases was slightly low-density, 12 cases were mixed density. On T1WI, 19 cases were low signal and 3 cases were iso-intense. 19 patients showed high signal on T2WI. On enhanced CT scan, 2 cases showed mild enhancement and 20 cases showed significant enhancement. 3 cases had calcification; 12 cases had cystic necrosis; nine cases had hemorrhage and 22 cases had peritumoral edema. Conclusions: Cystic necrosis, hemorrhage, edema and enhancement were more common in anaplastic oligodendrogliomas(WHO Ⅲ grade) than in glioblastoma. These imaging features contribute to tumor classification before surgery, and treatment planning for the tumor.

309 Assessment of left atrial function in patients with rheumatic mitral stenosis and sinus rhythm before andafter percutaneous balloon mitral valvuloplasty by real time three-dimensional echocardiography

Objective: To evaluate left atrial(LA) function in patients with rheumatic mitral stenosis(MS) and sinus rhythm before and after percutaneous balloon mitral valvuloplasty(PBMV) by real time three-dimensional echocardiography(RT-3DE). Methods: Thirty healthy volunteers and thirty patients with MS and sinus rhythm who underwent PBMV were examined by RT-3DE. LA voulme indices including: maximum, minimum, pre-atrial contraction LA volume index(LAVmaxI, LAVminI, LAVpreAI), LA function indices including total atrial, passive atrial, active atrial stroke volume index(TASVI, PASVI, AASVI) and total, passive, active atrial ejection fraction(LAEF, LAEFpassive, LAEFactive) were measured and calculated. Relationships between the changes of LA volume and function indices and the changes of mitral valve area(MVA) and mean mitral gradient(MMG) were analyzed. Results: ①LA volume indices were significantly greater in patients with MS than the controls, and the indices were decreased after PBMV(P<0.05). PASVI, TASVI were significantly greater and AASVI were significantly lower in patients with MS than the controls(P<0.05). PASVI were increased and AASVI were decreased. TASVI was not significantly different from healthy volunteers after PBMV(P<0.05). LAEFpassive, LAEFactive, LAEF were significantly lower in patients with MS than the controls, and the indices were increased after PBMV(P<0.05). ②There were significant correlations between the changes of LA volume and function indices with the changes of MVA and MMG(P<0.05). Conclusion: RT-3DE can evaluate LA function in patients with MS and sinus rhythm before and after PBMV.

312 Left ventricular endocardial and epicardial circumferential dysfunction bytwo-dimensional speckle tracking imaging in thalassemia major children

Objective: To analyze the early circumferential dysfunction of left ventricular(LV) endocardium and epicardium in thalassemia major children by using two-dimensional speckle tracking imaging(2D-STI). Methods: Routine echocardiography was performed in 60 thalassemia major children and 30 normal children. Patients were divided into 2 groups: 30 cases of thalassemia A, with serum ferritin(SF)≥2 500 μg/L, 30 cases of thalassemia B, with serum ferritin(SF)<2 500 μg/L. Two-dimensional images were recorded. Then the circumferential peak systolic strains(PSS) of endocardium and epicardium were measured at basal, mid and apical levels of LV for all the subjects. Results: ①Compared with normal group and group thalassemia B, endocardial circumferential PSS of group thalassemia A decreased significantly(P<0.05) in all the three level views of LV(P<0.05). ② Compared with normal group, epicardial circumferential PSS of group thalassemia A decreased significantly only in apical level view(P<0.05). ③ Compared with normal group, endocardial circumferential PSS of group thalassemia B decreased significantly only in apical level view(P<0.05). Conclusions: 2D-STI circumferential strain can detect the early circumferential dysfunction of LV in thalassemia major children, which is difficult to be detected by routine echocardiography. The LV circumferential dysfunction of group A with high level SF is more serious than that of group B with low level SF.

316 Change of regional myocardial function after cardiac resynchronization therapy valued byspeckle tracking imaging

Objective: To find the dynamic regional myocardial function and its systolic synchronization in patients of chronic heart failure who were treated with cardiac resynchronization therapy(CRT), The longitudinal strain radial strain, circumferential strain and rotation were observed. Methods: Fifteen patients suffering from heart failure, who received CRT, were select. Speckle tracking imaging in addition to standard echocardiography were performed prior to CRT, one week and 3 months after CRT. We observed the longitudinal strain, radial strain, circumferential strain and rotation, as well as time to peak strain and peak rotation. The standard deviation of time to each peak systolic strain and rotation (Ts-sl-SD, Ts-Sr-SD, Ts-sc-SD, Ts-srot-SD)were calculated. Results: One week after CRT, the Ts-sl-SD shortened(P<0.05), but there were no significant difference in Ts-Sr-SD, Ts-sc-SD, and Ts-srot-SD(P<0.05). Three months after CRT, all of them were shortened(P<0.05). The longitudinal and circumferential strain increased one week after CRT compared with those prior to CRT(P<0.05). Three months after CRT, not only longitudinal and circumferential strain but radial strain and rotation increased(P<0.05). Conclusions: Regional myocardial function can be valued by speckle tracking imaging. There is dynamic change in regional myocardial function of different direction of cardiac contraction in patients who response to CRT.

319 Interventional therapy for extrahepatic collateral arterial supply of primary hepatic carcinoma

Objective: To study the character of extrahepatic collateral arterial supply of primary hepatic carcinoma and its interventional therapy. Methods: 343 patients with primary hepatic carcinoma who underwent transcatheter arterial chemoembolization for many times were retrospectively analyzed. The interventional therapy of transcatheter arterial chemoembolization was performed by selective catheterization or superselective catheterization in the patients with extrahepatic collateral arteries. Results: A total of 83 extrahepatic collateral arteries were found in 73 patients. The incidence was 20.7%(71/343). They originated from right inferior phrenic artery(41.0%), gastroduodenal and epiploic artery(18.1%), right adrenal artery or subcapsular artery(12.1%), cystic artery(8.4%), right intercostal artery(6.0%), superior mesenteric artery(4.8%), lumbar artery(3.6%), left gastric artery(3.6%), and right internal mammary artery(2.4%), respectively. The successful rate of super selective catheterization of extrahepatic collateral arteries was 92.8%(77/83). The transcatheter arterial chemoembolization or arterial embolization with lipiodol was performed in the arteries of successful catheterization. Conclusions: It is necessary to determine the existence of extrahepatic collateral arterial supply and perform the super selective transcatheter arterial embolization in primary hepatic carcinoma with repeated TACE, which is clinically significant to improve the efficacy of interventional therapy.

322 CT manifestations of liver mesenchymal hamartoma and correlation with pathologic findings

Objective: To improve the awareness of the CT appearances and clinical presentations of mesenchymal hamartoma of the liver(MHL). Methods: Clinical data, pre- and post-enhanced CT images of 5 cases of MHL were retrospectively reviewed from January 2005 to June 2011 in our hospital. Results: In these 5 cases, there were 3 boys and 2 girls. All of the lesions were solitary, with 4 in the right lobe and one in the left lobe. The biggest size of the mass was 16.6 cm×11.9 cm, while the smallest size was 5.1 cm×4.1 cm. Pre-enhanced CT scan showed the mass was a mixture of cysts cavities and solid septations. Post-enhanced CT scan showed an increased enhancement of the septa, while the cysts had no enhancement. Conclusion: Mesenchymal hamartoma is a kind of rare benign tumors which arise from liver in children. As its main imaging study method, CT scan has some specific features: Pre-enhanced CT scan shows a mixed mass which usually locates in right lobe, while post-enhanced CT scan shows an enhancement of the septa. Pathological examination is necessary for the final results.

327 The effect of different doses of hypotonic contrast agent on the dog tubular epithelial cell apoptosis andthe expression of Caspase-3

Objective: To study the effect of hypotonic ion-contrast agents dose on the apoptosis of canine renal tubular epithelial cell and the expression of Caspase-3, so that to provide the evidence that low dose of hypotonic contrast agents can help reduce the side effects. Materials and Methods: Different doses(1.5 mL/kg, 1.0 mL/kg) of Iopromiden(300 mgI/mL) were injected with high pressure(1.0 mL/s) for dogs in study groups. Control group received an equal dose injection of normal saline. After 48 hours, kidneys were taken out. TUNEL dyeing was done to detect tubular epithelial cell apoptosis, and S-P stain was to observe Caspase-3 expression. Western blot was used to semi-quantitatively observe the Caspase-3 expression in kidney medulla. Results: After injection of contrast agents, tubular epithelial cell apoptosis rate and Caspase-3 expression significantly increased than control group(P<0.01). And the high dose group increased more obviously than the low dose group(P<0.01). Conclusion: Regular doses of hypotonic iodine-contrast agents commonly used in clinics can also cause apoptosis of the dog kidney tubular epithelial cell, which is relative to the dosage. Low dose contrast agents can reduce side effects. Caspase-3 may be involved in the tubular epithelial cell apoptosis controlled by the contrast agents.

331 The value of MR diffusion-weighted imaging in the differential diagnosis of urinary bladder lesions

Objective: To investigate the value of MR diffusion-weighted imaging(DWI) and apparent diffusion coefficient(ADC) in the differential diagnosis of urinary bladder lesions. Methods: Fifty-three patients with urinary bladder cancer and 12 patients with benign urinary bladder lesions constituted the study group and 11 individuals with normal urinary bladder constituted the control group. DWI of all individuals were analyzed and ADC values of lesions and normal walls were measured. Results: All the 53 urinary bladder cancers displayed a restriction in diffusion. Two of 12 the benign lesions displayed a mild restriction in diffusion and the others displayed no restriciton. The sensitivity, specificity and accuracy of DWI in the diagnosis of malignant urinary bladder cancer was 100%, 83.3%, and 96.9% respectively. The mean ADC value of the urinary bladder cancer was (0.76±0.17)×10-3 mm2/s, which was significantly lower than that of the benign lesions(1.21±0.20)×10-3 mm2/s and the control group(1.55±0.15)×10-3 mm2/s(F=89.04, P<0.001). Conclusion: DWI and ADC have a high value in the differential diagnosis of benign and malignant urinary bladder lesions.

335 The value of MRI in diagnosing congenital uterine malformation

Objective: To estimate the value of MRI in the diagnosis of congenital uterine anomalies. Methods: MRI, ultrasonic and clinical data of 34 cases with diagnosis of congenital uterine anomalies were retrospectively analyzed, then we compared the MR and ultrasonic diagnosis with hysteroscopy and/or laparoscopy results. Results: The diagnostic accuracies of MRI and ultrasonic were 97.1% and 85.3%, using hysteroscopy and/or laparoscopy results as gold standard. Conclusion: MRI can show stereoscopic view of uterine cavity configuration and fundus contour, and is one of the best non-invasive methods in the diagnosis of congenital uterine anomalies.

338 3D tomographic ultrasound imaging of the levator hiatus in late pregnancy and correlations with delivery outcomes

Objective: To determine the correlations between dimensions of the levator hiatus in late pregnant nulliparous women and mode of delivery, including length of second stage. Methods: This was a prospective observation study of 67 nulliparous women examined between 36 and 40 weeks of pregnancy. A translabial ultrasound examination was performed, and three-dimensional volume comprising the levator hiatus was recorded at rest, during a pelvic floor muscle contraction and on Valsalva. The volumes were analyzed in a blinded fashion using TUI to determine linear dimensions and hiatal area. Information regarding delivery outcomes and courses of labour was obtained from the obstetric database. Statistical analysis was undertaken for correlations between hiatal dimensions and labour outcomes. Results: No consistent correlation was found between levator dimensions and delivery mode. However, a negative correlation was demonstrated between the area of the hiatus, particularly on pelvic floor contraction, and length of total second stage. Conclusions: Levator hiatal dimensions are associated with the length of the second stage of labour. Although the number was small, our findings suggest that the effect of pelvic floor structures on progress in labour is worth of further study.
