77 The application of diffusion tensor imaging on 3.0T MR in amnestic mild cognitive impairment

Objective: To evaluate the fractional anisotrophy(FA) value of white matter in patients with amnestic mild cognitive impairment(aMCI) and normal controls(NC) with diffusion tensor imaging(DTI) on 3.0T MR. Methods: Twenty-eight NC and 28 aMCI who were diagnosed with Montreal cognitive assessment(MoCA) and minimental state examination(MMSE) scores underwent conventional MRI and DTI. FA values were measured in different brain regions with FiberTrak Package and then were compared between the two groups. Results: FA value decreased in both sides of parahippocampal gyrus, genu and splenium of corpus callusum in patients with aMCI. Conclusions: Changes in the microstructure of the white matter integrity can be observed in patients with aMCI, indicating that the DTI examination is helpful for early diagnosis of aMCI.

81 Relationship between quantitative measurement of fatty liver with in-phase and out-of-phase of MRI and CT attenuation value

Objective: To analyze the relationship between quantitative measurement of fatty liver with in-phase and out-of-phase of MRI and CT attenuation value, and to evaluate the application of the quantitative measurement with in-phase and out-of-phase of MRI in the diagnosis of fatty liver. Methods: Signal intensity of in-phase and out-of-phase of MRI and CT attenuation value were measured retrospectively in 86 patients clinically suspected fatty liver. Three CT attenuation value ratios and 4 signal indexes were calculated and the relationship between the CT attenuation value ratios and the signal indexes were analyzed by Pearson correlation coefficient. The sensitivity, specificity, accuracy of signal indexes for diagnosing fatty liver at different thresholds were calculated. Results: Significantly negative correlations were shown between 3 CT attenuation value ratios and 4 signal indexes(r=-0.823~-0.917), with the highest correlation between CT attenuation value ratio of liver to spleen and signal index 1(r=-0.917, P=0.000). Signal index 1 and signal index 4 had relatively high accuracies for diagnosing fatty liver with the threshold ≥0.1 for both, with the accuracy of 86.05% and 89.53% respectively, using CT attenuation value ratio of liver to spleen ≤1 as the standard. Conclusion: Quantitative measurement with in-phase and out-of-phase of MRI significantly correlate to CT attenuation value ratio of liver to spleen, and both can be used for quantitative evaluation of fatty liver. Signal index 1 or signal index 4 is optimal in diagnosing fatty liver with the threshold ≥0.1.

85 Intraoperative ultrasound in surgery of the pancreas

Objective: To discuss the utilization of intraoperative ultrasound(IOUS) in pancreatic surgery and evaluate the clinical value. Methods: Imaging of CT, trans-abdominal ultrasound and IOUS in 14 cases of pancreatic surgery in our hospital from February 2007 to January 2010 were analyzed retrospectively. Surgical results were compared with pathological findings. Indications were mainly unclear preoperative diagnosis or location, complicated anatomy or contradiction between intraoperative palpation with the image data. Results: Fourteen cases included 4 cases of insulinoma, 4 cases of solid pseudopapillary tumor, 3 cases of pancreatitis, 1 case of adenocarcinoma, 1 case of pancreatic stone after cyst resection, 1 case with normal intraoperative biopsy result which gave up surgery. The detection rate of preoperative CT examination and transabdominal ultrasound was 92.3%(12/13) and 83.3%(10/12) respectively; and diagnostic accuracy was 71.4%(10/14) and 53.8%(7/13), respectively. All the lesions were detected by IOUS, with a better detection rate and better diagnostic accuracy compared with CT, and there was a significant difference between transabdominal ultrasound and IOUS. Laparoscopic surgery was changed to open surgery in 2 cases because of vascular invasion detected by IOUS. Conclusion: IOUS not only can improve detection and diagnostic rates of disease, but also guide surgical plan selection.

88 The value of color Doppler ultrasound for the diagnosis of early diabetic nephropathy

Objective: To evaluate the value of color Doppler ultrasound in the diagnosis of early diabetic nephropathy. Methods: Seventy-two cases with type 2 diabetes were classified into three groups according to urinary albumin excretion rate (UAER): group Ⅰ(UAER<20 μg/min), group Ⅱ(UAER 20~<200 μg/min), group Ⅲ(UAER≥200 μg/min). Twenty normal persons were selected as controls(group Ⅳ). In each group, renal size and blood flow parameters of renal arteries were measured and analyzed by color Doppler ultrasound. Results: The more severe the renal dysfunction, the more obvious the decrease of blood flow. Vs and Vd of renal arterial flow decreased and RI increased with renal dysfunction. RI and Vd correlated well with the renal dysfunction from diabetic mellitus. The renal volume in group Ⅲ reduced and had significant difference with other groups. There was no significant difference among other groups. Conclusion: Color Doppler ultrasound is valuable in early diagnosis and evaluation of diabetic naphropathy by measuring renal size, blood flow distribution and renal flow parameters.

91 Clinical value of CT spinal angiography in diagnosing spinal arteriovenous fistula with 256-detector row spiral CT

Objective: To explore the clinical value of CT spinal angiography with 256-detector row spiral CT in diagnosing spinal arteriovenous fistula. Materials and Methods: Thirteen patients with initial MR and clinical manifestations suggestive of spinal vascular diseases underwent CT spinal angiography. Among them, 11 patients took DSA examination within 3 to 5 days after CT scan, and 8 patients underwent surgical treatment. Results: Seven cases of 13 patients were diagnosed as dural arteriovenous fistula(SDAVF) and 6 cases were spinal cord perimedullary arteriovenous fistula(SPMAVF), which were consistent with the diagnosis of DSA or surgery. All the 13 patients showed the abnormality of spinal cord vessels and the range of lesions clearly by CT spinal angiography. In the 7 cases of SDAVF, CTA displayed 5 feeding vessels and level of fistula opening in 6 cases, which were consistent with the DSA or surgery. One case was negative in DSA; In 6 cases with SPMAVF, CTA displayed 3 feeding vessels and level of fistula opening in 5 cases, which were consistent with the DSA or surgery. Conclusion: CT spinal angiography with 256-detector row spiral CT can show the extent of arteriovenous fistula, feeding artery and the level of fistula clearly. It is safe, non-invasive, rapid compared with DSA, and can be a necessary means for screening exam before DSA, serve as a DSA examination before surgery and postoperative follow-up, or even partially replace DSA.

96 MRI features of dermatofibrosarcoma protuberans

Objective: To discuss the MRI features of dermatofibrosarcoma protuberans(DFSP). Methods: MRI features in 6 pathologically confirmed DFSP were analyzed. The 6 patients included 1 female and 5 males, age ranged from 21 to 76 years, with a mean age of 56 years. MR imaging was performed in all the 6 patients, with gadolinium-enhanced scan in four patients. Results: All of the 6 lesions located in subcutaneous region. Three of them located in trunk and 3 in extremities. Four of the 6 tumors demonstrated as round or oval soft tissues with clearly defined margin. One of the 6 tumors appeared as a lobulated mass, and the other one presented as an irregular mass with relatively unclear margin. On T1WI, three tumors were isointense to skeletal muscle, and the other three showed slightly hyperintense to skeletal muscle. On fat-saturation T2WI(T2WI fs), all of the tumors were slightly heterogenous hyperintense signal, with some strip-like or patchy areas of hypointensity in the masses. After intravenous injection of Gd-DTPA, all the masses enhanced remarkably, 4 cases showed various weakly enhanced regions. Conclusion: DFSP had some specific MRI features. MRI plays an important role in the accurate preoperative assessment and detection of tumor recurrence.

100 Multi-slice CT perfusion of high voltage electrical injured limbs

Objective: To investigate the role and advantages of 256-slice CT perfusion in displaying blood vessel and muscle-microcirculation of high voltage electrical injured limbs. Methods: Perfusion scanning was performed for both-limbs and trunk by Phillips 256 slice intelligent CT(iCT). All the images were imported to EBW workstation. Images were post-processed, perfusion parameters were calculated and perfusion picture was reconstructed using Functional CT-general mode. Blood vessels were rebuilt using MPR, CPR, VR, MIP and AVA software. Results: All the 21 cases got clear CTA imaging. Stenosis, discontinuity and obstruction of blood vessels were seen in 7 cases. Fourteen cases had normal CTA. Muscle damage was seen in 21 cases. Conclusions: 256 slice iCT perfusion is a safe, convenient and simple examination method. We can obtain the information of blood vessel and muscle-microcirculation from it. It could provide much more and full-scale image data for clinical treatment.
