1 High-frequency ultrasonographic features of normal parathyroid glands

Objective: To evaluate the detecting rate, scanning techniques, locations and sonographic features of normal parathyroid gland by high frequency ultrasonography. Methods: High-frequency ultrasonography was used to detect parathyroid glands in 300 cases of healthy volunteers. The size, shape, position, number, internal echo and blood flow of normal parathyroid glands were carefully documented and analyzed. Results: Altogether 465 parathyroid glands were found in 300 cases of healthy volunteers. Among them, 4 glands were detected in 12 cases, 3 glands in 27 cases, 2 glands in 139 cases, 1 gland in 58 cases and no gland was detected in 64 cases, with the detecting rate of 78.7%(236/300). Normal parathyroid glands have three shapes: oval(73.5%), spindle(18.1%), irregular(8.4%), and there are three kinds of echoes: hyperechoic(78.3%), hypoechoic(16.5%) and isoechoic(5.2%). Color Doppler flow imaging(CDFI) showed no blood flow(81.9%), little blood flow(17.9%) and abundant blood flow(only one case). On longitudinal scanning section, parathyroid glands mostly locate near the posterior border of the lateral thyroid lobe and inferior pole; on horizontal scanning section, they mostly locate at the posterior border of lateral lobe of thyroid, which stand between the trachea and the common carotid artery. Conclusions: Normal parathyroid glands can be detected by high-frequency ultrasonography and they commonly appear as oval hyperechoic lesions with no or little blood flow.

5 The study of correlation between the severity of systolic compression of the mural coronary artery andlength and depth of myocardial bridge evaluated by 128-slice spiral CT

Objective: To discuss the correlation between the severity of systolic phase compression of the mural coronary artery(MCA) and length and depth of myocardial bridge(MB) evaluated by 128-slice SCT. Method: 580 cases examined by multi-slice spiral CT coronary artery angiography(MSCTCA) were analyzed to observe the presence, location and occurrence rate of MB-MCA. Taking LAD as the research object, the length and depth of MB, the degree of systolic phase compression of MCA were measured. All the research objects were divided into mild, moderate or severe stenostic groups according to degree of systolic phase compression of MCA, and then the difference in the length and depth of each MB-MCA was compared among groups. Results: 140 MB-MCAs were found in 580 patients, with an incidence rate of 24.14%. 104 lesions(74.3%) were in the middle part of LAD, 16 lesions(11.4%) in the distal part, 8 lesions(5.7%) in obtuse marginal branch, 7 lesions(5.0%) in the first diagonal branch, 3 lesions(2.1%) in intermediate branch, 2 lesions(1.5%) in posterior descending branch of right coronary artery. In LAD, the MB length ranged from 8 mm to 46 mm and depth ranged from 0.7 mm to 4.4 mm, with the average length of (21.80±5.98) mm and the average depth of (2.15±0.74) mm. The average compression degree was (38.5±19.6)%. MB was not different in length among different groups(P>0.05), but was different in depth(P<0.05). MCA systolic compression was positively correlated with MB depth(r=0.408, P<0.05), but not with the length of MB(r=0.076, P>0.05). Conclusion: 128-slice SCTCA can accurately display the anatomical relationship between the MCA and myocardial tissue and is a preferred method to diagnose MB-MCA. To evaluate the correlation of the degree of MCA systolic compression and MB length and depth can provide the basis for the clinic.

9 Research of carotid artery wave intensity in chronic renal insufficiency and its clinical significance

Objective: To study the changes and clinical significance of carotid artery wave intensity(WI) in patients with chronic renal insufficiency(CRI), and to assess the value of which to the early diagnosis of the changes in cardiovascular function. Materials and Methods: WI examination were carried out in 32 cases of chronic renal insufficiency and also in 36 cases of age matching volunteers without cardiovascular diseases. WI results were compared between the two groups, statistical analysis performed with SPSS 17.0. Results: The results of WI were positively related to age and blood pressure in chronic renal insufficiency group. The correlation coefficient was statistically significant(P<0.05). The thickness of tunica media and intima at the carotid sinus and 2 cm below the sinus in chronic renal insufficiency group were thicker than those of the normal volunteer group, there was significant difference between the two groups(P<0.01). The instantaneous accelerate speed wave intensity(W1), instantaneous reducing speed wave intensity(W2), negative wave area(NA), hardening parameter beta(β), strain elastic modulus(Ep), and pulse wave transmission velocity(PWVβ) were significantly higher than those of the normal group, there was statistical significance. Conclusions: WI is a non-invasive method of testing cardiovascular function changes in kidney failure before the appearances of organic changes of the vascular wall. It is worthy of application in clinical.

13 The efficacy of spectral CT colonography in detecting small phantom polyps

Objective: To evaluate the efficacy of CT colonography with spectral imaging in detecting small phantom colonic polyps and find the best single spectral images. Materials and Methods: Two segments of twenty artificial polyps sized from 1 mm to 10 mm(ten sized smaller than 5 mm and ten sized over 5 mm) were established by ligation in a segment of porcine colon. The phantom colon were inflated and immerged into 0.1% iodine solution. Volume data were acquired on a 64-multi detector gemstone CT scanner single spectral images at 45, 60, 75, 90, 105, 120 and 135 keV were reconstructed. CT virtual endoscopy(CTVE), multiplanar reformation(MPR) and virtual dissection(VD) models were reconstructed. Two radiologists blinded to the polyps location assessed every image setting. Contrast to noise ratios(CNR) as well as detecting rates of the polyps were calculated and compared statistically among different acquisitions. Results: The phantom study revealed that the detecting rates of colonic polyps were higher than conventional for those polyps less than 5 mm(P>0.05). The bset CNR is in 75 keV’s single spectral image. Conclusion: Spectral CT colonography is efficient in detecting small colonic polyps, the best single spectral images is 75 keV.

17 The value of 18F-FDG PET/CT imaging in the follow-up of ovarian cancer aftersurgery and the relationship between PET/CT imaging and serum CA125 assay

Objective: To evaluate the value of 18F-FDG PET/CT imaging in the follow-up of ovarian cancer after surgery and the relationship between PET/CT imaging and serum CA125 assay. Methods: 18F-FDG PET/CT and serum CA125 assay were performed in 45 postoperative patients with ovarian cancer. The diagnostic value was determined according to the results of surgical pathology or clinical follow-up. Results: The sensitivity, specificity, accuracy, positive predictive value, negative predictive value of 18F-FDG PET/CT imaging for detecting ovarian cancer recurrence or metastasis were 90.3%(28/31), 92.8%(13/14), 91.1%(41/45), 96.5%(28/29) and 81.5%(13/16), respectively. And the sensitivity, specificity, accuracy, positive predictive value, negative predictive value of serum CA125 assay for detecting ovarian cancer recurrence or metastasis were 74.2%(23/31), 78.5%(11/14), 75.5%(34/45), 88.4%(23/26) and 57.9%(11/19), respectively. There was a correlation between 18F-FDG PET/CT imaging and serum CA125 assay. Among 26 patients with increased serum CA125, recurrent lesions were detected by 18F-FDG PET/CT in 23 patients. However, among 19 patients with normal CA125, only 8 patients were detected to have recurrent lesions by 18F-FDG PET/CT. The recurrence detected by 18F-FDG PET/CT was statistically different between increased serum CA125 group and normal CA125 group(χ2=11.007,P=0.001). Fewer recurrent lesions were found in 10 patients with serum CA125≤100 U/mL. In our study, 90%(9/10) of patients with serum CA125≤100 U/mL had 1~3 recurrent lesions. More than 3 recurrent lesions were found in 21 patients with serum CA125>100 U/mL. The number of recurrent lesions was statistically different between the group with serum CA125≤100 U/mL and that with serum CA125>100 U/mL(χ2=14.006, P=0). Conclusion: 18F-FDG PET/CT possesses an important clinical priority for following-up patients with ovarian cancer after surgery. There was a correlation between 18F-FDG PET/CT imaging and serum CA125 assay. 18F-FDG PET/CT and serum CA125 assay can complement each other.

21 64-MSCT low-dose scanning in cervical spine trauma

Objective: To compare the scanning dose of two different acquisition patterns of 64-slice spiral CT in the cervical scan. Materials and Methods: 60 patients with cervical injury were assigned to undergo either cervical pattern(n=30) or pelvis pattern(n=30) scan on a 64-slice spiral CT. A water phantom was scanned 30 times with each acquisition pattern as control(cervical pattern and pelvis pattern). We recorded the average volume CT dose index(CTDIvol), scanning time, effective mAs and calculated the dose-length product(DLP), mean effective dose(E) each time respectively. All images were evaluated by 3 experienced radiologists blindly. Results: The CTDIvol of cervical pattern group was larger than that of pelvis pattern group. The reduction of E value in the group of pelvis pattern is up to 59.0% compared with the group of cervical pattern. There was no significant difference in image quality score between two acquisition pattern groups which was acceptable for clinic need(P>0.05). Conclusion: Considering the optimization of dose, the pelvis pattern of 64-slice spiral CT has lower radiation dose to evaluate the patients with cervical trauma.

Objective: To investigate relationships between the degree of the cervical spinal canal stenosis and lower cervical segmental stability in patients with cervical spondylotic myelopathy(CSM). Methods: Sixty-two patients with CSM for prepared surgery were examined with the lateral dynamic radiographs(flexion-extension) and MR scanner. Radiographic evaluation included the C2~C7 Cobb angle and FSU-Cobb angle at C3~C4, C4~C5, and C5~C6. Cervical spinal stenosis was classified by means of the T2-weighted MR images into four grades. Radiological change was analyzed according to the degree of the spinal stenosis during flexion-extension motion. Results: The segmental activity of the C4~C5, C5~C6 was greater than that at the C3~C4 in Grade Ⅰ spinal stenosis, and statistical difference was observed between the C4~C5 and the C3~C4. The segmental activity at the C4~C5 decreased significantly in Grade Ⅲ compared with Grade Ⅰ and Grade Ⅱ spinal stenosis, and implied the segmental units at the C4~C5 contributed the majority of the mobility on the cervical spine and motion decreased in the severe spinal stenosis. There was no statistical difference on the range of the motion of the overall cervical spine between grade 0~Ⅱ and grade Ⅲ groups. Conclusion: The functional spinal unit(FSU) is the smallest functional unit of the cervical spine and exhibits the generic characteristics of the spine. The mobility of the FSU was significantly reduced in the severe cervical spinal stenosis, from an unstable phase to an ankylosed stage, with the progression of the degenerative change. Hence, combined application of the common lateral dynamic radiographs and MRI in judging the activity of the cervical spine and spinal canal stenosis plays an important role in the clinical practice.
