Imaging diagnosis of suprasellar germinoma and diagnostic errors analysis

Objective: To study imaging features of suprasellar germinoma, and analyse the reasons of diagnostic error. Methods: The imaging findings of 14 misdiagnosed and pathologically confirmed suprasellar germinoma cases were retrospectively reviewed. The most common causes of diagnostic error were analysed. Results: Teenagers(mean age 15.5y) and female patients(n=8) were more common in these suprasellar germinoma patients, clinical features including visual loss(n=7) and hypothalamic-pituitary dysfunction symptoms, such as polydipsia, polyuria(n=8) and amenorrhea(n=3). Sometimes serum or cerebrospinal fluid β-HCG elevation could be recognized(n=4). The tumors were mainly solid(n=10), cystic change(n=4) could be seen in some cases. Calcification(n=2) was rare. They showed slightly high density on CT scan, iso-high signal on MRI scan. The lesions were usually obviously and homogenously enhanced(n=7). The common causes of misdiagnosis include: ①unconfirmed diagnosis because of incomplete imaging examination(n=6), ②craniopharyngioma due to calcification or age(n=6), ③pituitary adenoma because of "snowman sign"(n=3), ④glioma because of age or other reasons(n=3). Conclusion: Suprasellar germinoma tends to be misdiagnosed as craniopharyngioma, pituitary adenoma, or glioma. Correct diagnosis of suprasellar germinoma should base on complete imaging examination and in the meanwhile, patient's age, clinical symptoms, laboratory examination should be taken into consideration.

Applications of CT in the differential diagnosis of low-risk thymomas, high-risk thymomas, and thymic carcinomas

Objective: To assess the CT imaging findings of each type of thymic epithelial tumors and to determine useful findings in differentiating the main subtypes of low-risk thymomas, high-risk thymomas, and thymic carcinomas. Methods: CT features of 62 low-risk thymoma, high-risk thymoma, and thymic carcinoma cases confirmed by operation and pathology were reviewed retrospectively. The imaging characteristics of each group were compared and the main different characteristics among the three groups were analyzed. Results: On CT imaging, lobulation and irregularity of contour, necrotic or cystic component within tumor, great vessel invasion, presence of mediastinal lymphadenopathy and extrathymic metastasis are more common in thymic carcinomas than thymomas. High-risk thymomas usually have pleural metastasis. Clusters calcification are usually seen in thymic carcinomas, nodosity and punctiform and eggshell calcification are more common in thymomas. Conclusion: The presence of irregular contour, necrotic or cystic formation, mediastinal lymphadenopathy and extrathymic metastasis, great vessel invasion are suggestive of thymic carcinomas. The fiber diremption in tumor and eggshell calcification only seen in thymomas. Pleural metastasis are suggestive of high-risk thymomas.

Function of PET-CT in 125I seeds permanent braehytherapy treatment for lung cancer

Objective: To explore the location value of PET-CT in 125I seeds permanent braehytherapy treatment for non-small cell lung carcinoma(NSCLC) patients. Methods: From 26 patients with NSCLC underwent whole body PET-CT scans, PET-CT and CT images were input into the treatment plan system(TPS), radiotheapy dose were calculated, compared with gross tumor volume and the numbers of 125I seeds. Results: The gross tumor volume, the mean number of seeds were (51.03±16.02)cm3, 43.88±12.30, in PET-CT group and (58.10±19.11)cm3, 49.46±14.74 respectively, in CT group, gross tumor volume of PET-CT group reduced 12.17%, number of seeds reduced 11.28%(P<0.001). Conclusion: PET-CT is more accurate than CT alone in the assessment of the radiation target volume for NSCLC patients.

Evaluation of rheumatic mitral valve area using real-time three-dimensional xPlane imaging

Objective: To determine the feasibility and accuracy of real time three dimensional xPlane imaging(RT3DxPlane) for the evaluation of mitral valve area(MVA) in patients with rheumatic mitral stenosis. Methods: MVA were measured using two dimensional echocardiography(MVA2D) and RT 3DxPlane (MVAxPlane) in 36 patients. The stenotic mitral specimens were acquired in mitral valve replacement surgery, and MVAOP as the golden standard of MVA was calculated with photoshop and Matlab software. Results: There was no significant difference between MVAOP and MVAxPlane(P>0.05), but statistical difference was found between MVAOP and MVA2D(P<0.05). MVA2D, MVA3D and MVAOP correlation coefficient were 0.95 and 0.97, respectively(P<0.05). Bland-Altman analysis demonstrated that both MVA2D and MVAxPlane had good agreement with MVAOP. Conclusion: RT 3DxPlane imaging can provide an easy, feasible and accurate method for quantifying MVA.

Clinical implications and CTA findings of the pulmonary arterial hematoma due to ruptured aortic dissection

Objective: To study the mechanism, CTA findings and clinical implications of pulmonary arterial wall hematoma secondary to ruptured aortic dissection. Methods: We studied 183 patients with nontraumatic aortic dissection from 2007 to 2010 in our hospital, diagnoses were confirmed by CT angiography. CTA findings, treatment modalities and clinical outcomes were retrospectively analyzed in three patients with pulmonary arterial wall hematoma secondary to ruptured aortic dissection. Results: These 3 patients presented with chest pain on admission. CTA findings included: the intimal tear located in the proximal portion of ascending aorta extending to thoracic-abdominal aorta in one case, aortic arch extending only to ascending aorta in the second case and in the distal portion of aortic arch involving thoracic-abdominal aorta in the third case. The size of tear was 0.9cm, 0.8cm and 0.4cm respectively. The hematoma located in the wall of pulmonary arterial trunk in one case, in the wall of pulmonary artery trunk and bilateral pulmonary arteries in one case, in the wall of pulmonary artery trunk with bilateral pulmonary arteries and alveloar bleeding occurred in one case. On follow-up, the patient who underwent surgery survived now, while the other patients with conservative management deceased within 48 hours in hospital. Conclusion: CTA not only establishes the diagnosis of aortic dissection, but also defines the existence and extension of pulmonary arterial hematoma from the ruptured aortic dissection. According to our data and literatures review, patient's age and the extension of pulmonary arterial hematoma are important factors which affect patient's prognosis.

Establishment and value of multi-slice spiral CT model of hepatic carcinoma in interventional procedure

Objective: To explore the value of multi-slice spiral CTA imaging to interventional transcatheter arterial chemoembolization(TACE) of hepatic carcinoma. Methods: MSCTA and DSA of hepatic artery were sequentially performed in thirty-two patients with malignant tumor of liver, and the interval of these two examinations was 1~7 days. Multi-slice spiral CT model of hepatic carcinoma for interventional procedure was established with volume rendering(VR) technique of CT. According to DSA, the anatomical type of hepatic artery, the origin of the celiac artery and the superior mesenteric artery, the feeding arteries of hepatic tumor were compared with MSCTA. Results: There was good correlation between multi-slice spiral CTA imaging of hepatic carcinoma and DSA in the ability of showing the anatomical type of hepatic artery, the origin of the celiac artery and the superior mesenteric artery, the feeding arteries of hepatic tumor. Multi-slice spiral CTA of hepatic carcinoma before interventional procedure can clearly display the anatomical type of hepatic artery, the origin of the celiac artery and the superior mesenteric artery, the feeding arteries of hepatic tumor. Conclusion: Multi-slice spiral CTA imaging of hepatic carcinoma before interventional procedure can provide effective guidance for TACE of hepatic carcinoma.

Autoimmune pancreatitis: study of MRI manifestation

Objective: To investigate the MR imaging findings of autoimmune pancreatitis(AIP). Methods: The MR data of 8 patients with AIP were retrospectively reviewed. Plain MR and dynamic contrast-enhanced three-phase MR scanning was performed on 8 patients. Fat-suppressed T2WI, T1WI, in- and out-phase T1WI and dynamic contrast-enhanced data were analyzed, focusing on signal intensity of abnormal pancreas parenchyma, enhancement patterns of the abnormal pancreas and microstructure alteration. The signal intensity value of abnormal pancreas parenchyma on plain-scanning and each phase of dynamic contrast-enhanced scanning were measured, compared to that of controls(n=25, with normal pancreas). Results: Five patients showed diffuse swelling of the pancreas and 3 patients showed segmental enlargement of pancreas. 6 patients showed psuedocapsule like around the pancreatic body and tail. Main pancreatic duct in abnormal pancreas were invisible. The signal intensity on each sequence was lower than that of normal pancreas: abnormal pancreas showed homogeneous hypointensity on fat-suppressed(or not) T2WI and not fat-suppressed T1WI, and heterogeneous multiscale hypointensity on fat-suppressed T1WI; Some funicular slightly hyperintensity area could be seen in the abnormal pancreas on fat-suppressed T1WI. Funicular and micronodular enhanced area could be seen in 6 patients with AIP ahead of time. Progressively enhancement was found in 8 patients with AIP, different from the controls. Conclusion: There is typical MR imaging features of AIP. Fat-suppressed T1WI and MR dynamic contrast-enhanced scanning play an important role in discovering the alteration of microstructure of AIP. AIP can be diagnosed correctly through MR examination.

HCT116 colon cancer xenografts: diffusion weighted MR imaging to assess early chemotherapeutic response

Objective: To assess early chemotherapuetic response in a mouse colon cancer model using diffusion weighted imaging(DWI) shown as apparent diffusion coefficient(ADC) and analyze the potentially associated histopathologic characteristics. Materials and Methods: Thirty-two Athymic mice bearing HCT116 xenografts on right rear flank were allocated into four groups: control, 5Fu, oxaliplatin and 5-Fu/oxaliplatin combined. Mice were undergone body MR examination, including T2-weighted, T1-weighted and DW MR imaging, before and 3, 7, 11, 20 days after treatment. Changes in ADC and volume of tumors were recorded and calculated. At each time point after chemotherapy, histopathological characteristics were recorded and analyzed. The correlations between changes in ADC and histopathological characteristics at each time point were analyzed. Results: the mean ADC value in each group decreased 3 days after initialization of therapy which may be associated with cell swelling without necrosis. Seven and eleven days after initialization of therapy, mean tumor ADC value in groups treated with oxaliplatin and 5-Fu/oxaliplatin combined increased and significances were seen(P<0.05). For the group treated only with 5Fu, the mean ADC values 7 to 11 days after initialization of therapy increased but no significance was seen(P=0.07). Necrosis within tumor was detected 7 and 11 days after chemotherapy. However, no significant difference in ADC values between 20 days after and before therapy was shown. Regeneration in tumor was detected 20 days after chemotherapy. Conclusions: DWI shown as ADC values increased has a potential in early prediction of response to chemotherapy.
