The correlative study between MR classification of hemorrhagic transformation of non-thrombolytic therapy brain infarction and clinical outcome

Objective: We intend to ①establish a MR classification method of hemorrhagic transformation(HT) of brain infarction; ②investigate the correlative relation between MR classification of HT of brain infarction and clinical outcome. Then investigate the applicative value of MR classification method of brain infarction HT on assessment clinical outcome. Materials and Methods: Review 421 non-thrombolytic therapy for acute brain infarction cases, who were examined and cured in our hospital from October 2007 to November 2009. All cases had high signal on DWI images performed at 3~7 days after stroke onset. Thirty-one HT cases were classificated by our own MR classification method. The correlative relation between MR classification of HT of brain infarction and clinical outcome were investigated. Disability and function repaired were assessed after 3 months using the modified Rankin score(mRs). Outcomes in patients with and without HT were compared by χ2 test. t test was used between two groups. Results: In all 421 cases, 31 cases(7.4%) had HT, grade Ⅰ: 10 cases(2.34%), grade Ⅱ: 4 cases(0.95%), grade Ⅲ: 17 cases(4.0%). 3-month mRs: 165 cases(39.2%) had disability(mRs 3~5) or died, 35 cases(8.3%)died. In 31 HT cases, 21 cases(67.7%) had disability or died. Grade Ⅱ HT, grade Ⅲ HT had significant relationship with poor outcomes. Conclusions: 7.4% non-thrombolytic therapy for acute brain infarctions had HT, brain stem or cerebellum HT, large area HT had significant relationship with poor outcomes; MR classification method of HT of non-thrombolytic therapy for brain infarction had significant relationship with outcomes.

The clinical and CT findings of adenolymphoma of the parotid gland

Objective: To investigate the characteristics of clinical and CT findings of adenolymphomas of the parotid gland. Methods: The clinical and CT characteristics of 10 cases of adenolymphomas of the parotid gland confirmed by pathology were studied retrospectively. Results: Seven cases were found in unilateral side(2 cases each with 2 lesions); bilateral side in 3 cases(1 case with 2 lesions in right side), altogether 16 lesions. Twelve lesions were of small size, which 10 lesions located in lower posterior and 2 in lower anterior quadrant of the parotid gland; 4 were of large size which involved multiple quadrants. Thirteen lesions were round or oval, 3 were lobulated. Thirteen lesions were well-defined, a ring of interface could be seen between the lesion and the normal tissue in which 3 lesions; the margins in 3 lesions were ill-defined, with ipsilateral parotid gland swelling and were increased in density. Lesion size was 0.6~4.8cm[(2.6±1.5)cm]. The density of the lesions were homogeneous on plain CT. Twelve were homogeneous and 4 were heterogeneous after enhancement. CT value of the lesion was (58.2±8.1)HU and (89.1±18.7)HU on plain CT and arterial phase(delayed 30s) after enhancement respectively. Four lesions were mildly, 7 were moderately and 5 were significantly enhanced. CT value of venous phase(delayed 90s) of the lesions was (92.6±16.8)HU. Small vessels could be seen in or around the lesions in 5 cases(31.3%). No enlarged cervical lymph node was noted. Conclusion: There were some characteristics of clinical and CT findings in the adenolymphomas of the parotid gland. Most cases could be correctly diagnosed before operation by analyzing the clinical and CT findings carefully.

Multivariate feature analysis of power Doppler sonographic findings of residual-relapse lymph node in post-therapy of nasopharyngeal carcinoma

Objective: Sonographic criteria of the lymph node have been found to be good indicators for metastatic lymph nodes. We determined which sonographic features are most predictive of residual-relapse in cervical lymph nodes among patients with post-therapy nasopharyngeal carcinoma. Methods: Power Doppler sonography were analyzed in 179 cervical nodes(114 residual-relapse and 65 reactive nodes) from patients with post-therapeutic nasopharyngeal carcinoma after neck lymph node radical dissection or selective dissection, All cases were proven by post-surgery pathology. The gray-scale sonographic features of the presence or absence of hilar structure, the even or uneven of lymph node echo, short axis length(≥10mm), long axis length, long-short axis ratio(L/S 2), the boundary, the blood flow were evaluated. Multivariate logistic regression analyses were conducted to determine the relative value of each sonographic feature. Results: Multivariate analysis suggested that the absence of hilar structure, increase in short axis length(≥10mm) were the sonographic features for prediction of residual-relapse lymph nodes. Specificity, sensitivity, accuracy and Kappa index was 90.4%, 84.6%, 88.2% and 0.75 for node short axis diameter(≥10mm). 95.6%, 86.5%, 92.1% and 0.82 for the absence of hilar structure. The sonographic features of absence of hilar structure was the better index than the features of short axis diameter(≥10mm). Conclusions: The sonographic criteria most predictive of metastatic cervical lymph nodes were absent hilar structure and increase in short axis length(≥10mm), as assessed by logistic regression analysis. It can be used as an clinical criteria to diagnosis residual-relapse lymph nodes in post-therapy nasopharyngeal carcinoma.

Studies on the methodology of 18F-FDG cell binding of ovarian cancer SKOV3

Objective: To establish a useful and stable methodology of 18F-FDG cell binding of ovarian cancer SKOV3. Methods: Several binding conditions were tested. ①The cell number was from 5×104 to 5×106 cells/bottle. ②The reaction time was from 20 to 120min. ③The radioactivity of 18F-FDG was from 1.85 to 29.6KBq. ④The glucose concentration was from 0 to 11.1mmol/L. The CPM(B) in cells and CPM(F) in supernatant were measured by the γ detector. The binding efficiency of ovarian cancer cell line SKOV3 with 18F-FDG was calculated. Results: ①There was linear regression relationship between 18F-FDG cell binding efficiency of human ovarian cancer SKOV3(y) and the cell number(x1), reaction time(x2), glucose concentration(x3). Regression equation: y=(7.395+5.18E-0.06x1+0.094x2-2.040x3)×100%, F=106.9, P<0.05. The P values of partial regression coefficient were less than 0.05. R2c=0.728, P<0.05. The Pearson correlation coefficient of cell number, reaction time, glucose concentration with 18F-FDG cell binding efficiency was 0.581、0.179、-0.599 respectively, P<0.05. ②The basic conditions of 18F-FDG cell binding of ovarian cancer SKOV3: the cell number was 1×106 cells/bottle, the radioactivity of 18F-FDG was 3.7KBq, the glucose concentration was 0mmol/L, the reaction time was 100min. The binding efficiency was (25.45±1.66)%. Conclusions: The methodology of 18F-FDG cell binding of ovarian cancer SKOV3 was established successfully. It contributed to evaluate early the effect of radiotherapy and chemotherapy on inhibiting ovarian cancer cell line SKOV3.

The pathological basis of nodular goiter and correlative with specific signs of spiral CT

Objective: To analyse the specific characteristics spiral CT signs of nodular goiter in order to improve its diagnostic accuracy. Methods: Sixty cases confirmed surgically and pathologically to be nodular goiter which were found by preoperative spiral CT scans plus three phase enhanced scans, 1 case was found by cervical disc CT scan, 4 cases were found by chest CT examination. CT signs were observed, and correlated with pathological findings. Diagnostic criteria and differential points were advocated. Results: ①Site, number, size and density of lesions: no lesion protrudes beyond the thyroid capsule, multiple lesions in 43 cases, single in 17 cases, the size of single lesion was larger than that of the multiple lesions with an average of 3.5cm, mostly cystic; mixed density mostly low in 32 cases, high density in 11 cases. ②Pathological types: Forty-nine cases were diagnosed as nodular goiter pathologically, including nodular goiter with papillary hyperplasia or adenomatous nodules in 18 cases, nodular goiter with adenoma in 7 cases, nodular goiter with papillary carcinoma in 4 cases. ③Characteristics of spiral CT: multiple lesions were more common, mixed dominantly low in density, garland-like; patchy; or homogeneously high density in 11 cases. Calcification seen in 31 cases; septations within lesion in 15 cases; capsule were seen in 24 cases; various degree of cystic formation in 49 cases in which completely cystic changes in 12 cases. On enhanced scan, 22 cases showed multiple papillary enhancement, filling enhangcement seen in 32 cases on balanced phase; various size of slit like low density within the cystic area; isodensity on the balanced phase in 26 cases. Conclusion: Specific CT signs of nodular goiter include well defined margin of thyroid; multiple papillary enhanced nodules on contrast scan; homogeneous filling density on balanced phase; central slit-like low density; morpholy and distribution of calcification; septation within lesion; smooth small round cystic area within lesion. These spectific signs have important diagnostic value.

The imaging analysis of funnel chest coexisting with flat chest and scoliosis

Objective: To investigate the incidence of scoliosis and flat chest in pectus excavatum patients. Methods: One hundred and seventy-one cases of pectus excavatum patients before the operation were included in this study. Postero-anterior and lateral chest radiography, and chest CT were performed. The incidence of flat chest and scoliosis were investigated, the relationship between Haller index, Cobb angle, chest flat index, chest shape index and age were analyzed. The relationship between Cobb angle, chest flat index, chest shape index and Haller index were analyzed too. Results: In 171 cases of pectus excavatum patients, 39 cases were 3~10 years old, 92 cases were 11~17 years old, 40 cases were older than 18 years. The cases of Cobb angle >10° were 20.5%, the extent of scoliosis and age have a certain relationship, and the extent of scoliosis of the adolescent and adult groups were more severe than the group with scoliosis; Scoliosis has nothing to do with the Haller index. The cases of flat chest were 17.0%, the extent of flat chest has nothing to do with age, but there was some correlation with the Haller index. The values of chest flat index and chest shape index in patients whose Haller index ≥3.5 were higher than the two groups whose Haller index <3.5. Conclusions: The incidence of flat chest and scoliosis was higher in pectus excavatum than general population, but the correlation needs further proof. The flat chest and scoliosis should be added to the diagnosis of pectus excavatum in some occasions. We should think some necessary treatments in those pectus excavatum’s surgeries.

Small renal cell carcinoma 4D THRIVE dynamic enhanced scanning parameters and the correlation with tumor angiogenesis

Objective: To investigate the relationship of small renal cell carcinoma 4D THRIVE dynamic enhanced parameters and tumor angiogenesis. Methods: Thirty cases clinically suspected small renal cell carcinoma patients were examined by 4D THRIVE dynamic enhanced scanning, tumor area of interest ROI and tumor-adjacent kidney tissues of dynamic enhanced parameters: the biggest enhanced slope(Smax), peak height(PH) and time-to-peak(Tpeak) were calculated respectively. Pathologic specimens were performed by immunohistochemical staining, microvascular density(MVD) count and vascular endothelial growth factor(VEGF) expressing were examined, small renal cell carcinoma of each dynamic enhance parameters were correlated with MVD and VEGF. Results: The enhancement parameters Smax(r=0.707, P<0.01), PH(r=0.687, P<0.01) correlated positively with MVD. Tpeak(r=-0.413, P<0.05) correlated negatively with MVD. The mean value of MVD(63.07±14.73) field in patients with small renal cell carcinoma were siginificantly higher than tumor-adjacent kidney tissues(P=0.002), 67%(20/30) small renal cell carcinoma VEGF expression was positive, higher than tumor tumor-adjacent kidney tissues(P=0.001). Conclusion: Small renal cell carcinoma 4D THRIVE dynamic enhanced scanning, some parameters correlated positively with MVD, VEGF. It has an important value in small renal cell carcinoma angiogenesis.
