381 MRI diagnosis of intracranial pilocytic astrocytoma

Objective: To investigate the MRI features of intracranial pilocytic astrocytoma(PA) to further enhance the understanding of the disease. Methods: The pre-and-post contrast MR images of 18 cases of pathologically confirmed PA were analyzed retrospectively. Results: Of the 18 PA, 9 located in the midline structure(5 in the optic chiasm, 2 in the brainstem, 1 in the hypothalamus and 1 in the fourth ventricle), 5 in the cerebellum and 4 in the cerebral hemisphere. The size of lesions ranged from 2.1cm to 5.9cm. On MRI, 10 cases were predominantly solid mass with cystic degeneration or necrosis or micro-cystic components, all had markedly inhomogeneous enhancement; Eight cases were solid-cystic mass, 4 with intensely enhancing mural nodule and cystic wall, 3 with intensely enhancing mural nodule and non-enhancing cystic wall and 1 with mildly enhancing mural nodule and cystic wall. Thirteen cases showed no obvious peritumoral edema and 5 showed slight peritumoral edema. Conclusion: The MRI appearance of PA has some characteristics. According to the clinical manifestations, it is helpful for accurate preoperative diagnosis. Meanwhile, it also offers significant reference value for treatment.

385 Clinical application of CTA coupled with whole brain CT perfused blood flow after onset of aneurismal subarachnoid hemorrhage

Objective: To investigate the clinical application of CTA coupled with whole brain CT perfused blood flow after onset of aneurismal subarachnoid hemorrhage(SAH). Methods: Perfused blood volume(PBV) values were confirmed in 29 patients with SAH by dual source computed tomography. CTA maps and PBV maps were reconstructed. In the meantime we detectd the aneurysm, we evaluated the change of vasospasm and the cause of cerebral ischemia by the perfusion characteristics. Results: Twenty-eight aneurysms in 27 patients were detected. PBV value in the frontal, temporal, parietal, occipital lobe and cerebellum is 11.17±3.13; 10.92±2.91; 11.08±2.53; 9.88±3.01; 19.83±5.12 respectively in the SAH group, and 17.10±3.60; 15.98±5.03; 15.84±3.53; 16.14±1.86; 23.54±4.64 respectively in the control group. Significant difference was showed in the frontal, parietal and occipital lobes of the brain(P<0.05), but the PBV values had no significant difference in the survival group and the death group. It showed no significant difference of hypoperfusion between bleeding dominant hemisphere and the other one. Conclusions: The cause of the SAH could be detected and the cerebral ischemia due to vasospasm could be showed simultaneously by CTA coupled with whole brain CT PBV.

389 MR spectroscopy of human astrocytoma and the correlation with the expression of Ki-67 and bcl-2

Objective: To investigate the relationship between the MRS and the Ki-67, bcl-2 expression of human astrocytoma. Methods: Fifty-two patients of astrocytoma proved by surgery and pathology were included. MRS was performed in all the patients. After surgical operation, the specimen was examined for Ki-67 and bcl-2. The NAA/Cho, Cho/Cr and NAA/Cr were recorded and the relationship with tumor grade was analyzed. The relationship between the above indices and Ki-67, bcl-2 were investigated respectively. Results: There was significant correlation between NAA/Cr, Cho/Cr, NAA/Cho and the pathological grading of astrocytoma. Positive expression of Ki-67 and bcl-2 in astrocytoma. Positve expression of Ki-67, bcl-2 increased with increasing clinicopathologic grades(r=0.600, P<0.01). The Cho/Cr was strongly positively correlated with expression of bcl-2 and Ki-67(r=0.736, P<0.01; r=0.809, P<0.01 respectively). The NAA/Cho was markedly negative correlated with the expression of bcl-2(r=-0.253, P<0.01), and had markedly negative correlation with Ki-67(r=-0.501, P<0.01). The NAA/Cr of astrocytoma was negatively related with the expression of bcl-2(r=-0.369, P<0.01) and Ki-67(r=-0.416, P<0.01). Conclusion: MRS can reflect the protein manifestation of Ki-67 and bcl-2 of astrocytoma noninvasively, being helpful in the preoperative assessment of apoptosis, metabolic information, reproductive activity, invasive activity and can predict prognosis of astrocytomas. The Cho/Cr, NAA/Cho reflected pathologic grade of astrocytoma relatively steadily. The Cho/Cr and NAA/Cho are more useful than NAA/Cr.

394 AQP4 expression and diffusion tensor imaging in rat brainstem after diffuse axonal injury

Objective: To investigate the changes of magnetic resonance diffusion tensor image(DTI) after diffuse axonal injury(DAI) in rats brainstem. At various stages, diffusion tensor image was used to measure apparent diffusion coefficient (ADC) and fractional anisotropy(FA), with immunostaining being used to determine Aquaporin 4(AQP4). Methods: Forty-eight rats were randomly divided into 6 groups. Five separate groups with eight rats in each group were injured and imaged at 3h, 6h, 12h, 24h, 72h post-injury by a weight drop trauma model. One group as contral. Rats in DAI group were sacrificed for histology immediately after the scan. The control group of 8 rats was imaged and sacrificed for histology but not injured. Each group was studied by conventional MR imaging, diffusion tensor imaging and histologically examination including HE, silver staining, and immunohistochemistry of AQP4. The results of conventional MRI and DTI were compared with the changes of histopathology. Results: Histological examination showed axonal damage and brain edema in rats of DAI groups. AQP4 expression increased, reached the peak values at 24h and decreased gradually afterwards. ADC value increased and reached the peak values at 12h, and also decreased afterwards. FA value decreased consecutively. There was significant correlation of ADC value and AQP4 expression in brainstem between DAI group and control subjects within 12h and between 24 to 72h(P<0.01) respectively. There was no significant correlation between 12h to 24h(P>0.05). AQP4 expression was strongly correlated with FA value(P<0.01) within 72h. Conclusion: The expression of AQP4 protein after DAI is time-dependent and possibly implicated in the formation and resolution of DAI-induced cerebral edema. ADC measurements in brainstem were correlated with AQP4 after DAI. Combining AQP4 expression and ADC value is helpful for reflecting the classification between vasogenic brain edema and cytotoxic brain edema.

399 Evaluation of left ventricular regional myocardial systolic function in patients with diastolic heart failure using real-time triplane strain rate imaging

Objective: To evaluate the left ventricular regional myocardial systolic function in patients with diastolic heart failure using real-time triplane strain rate imaging and investigate its clinical significance. Methods: Twenty-nine patients with diastolic heart failure, 26 patients with systolic heart failure and 30 healthy subjects as control group were enrolled. The systolic peak strain rate(SRs) were measured at basal and middle segments in different left ventricular walls using real-time triplane strain rate imaging. Results: SRs in diastolic heart failure and systolic heart failure groups were significantly lower than in control group(P<0.05 or P<0.01). There was a significantly step-down change among normal, diastolic heart failure and systolic heart failure in SRs(P<0.05 or P<0.01). Conclusions: The left ventricular regional myocardial systolic dysfunction existed in patients with diastolic heart failure. Thus, assessing left ventricular regional myocardial systolic function using real-time triplane strain rate imaging has important clinical value in the comprehensive evaluation of cardiac function in heart failure.

402 Compare image quality, diagnostic accuracy and radiation dose of prospective and retrospective electrocardiogram gated dual-source computed tomography

Objective: To compare image quality, diagnostic accuracy and radiation dose of prospective and retrospective electro-cardiogram(ECG) gated dual-source computed tomography(DSCT). Methods: One hundred patients who underwent DS-CT coronary angiography(DS-CTCA) were enrolled in the study, 60 were scanned by prospective scan and another 60 were analyzed by gated CT scan retrospectively. Sixty patients were divided into two groups according to their heart rate, receiving either step-and-shoot(heart rate less than 70bpm) or adaptive sequential(more than 70bpm) ECG gated DSCT separately. Retrospective CT angiography was performed with DSCT in another sixty patients, which were divided into two groups according to their heart rates too. The imaging quality of coronary artery segments in the four groups was evaluated using a four-point grading scale by two independent reviewers. Patient radiation dose was calculated by multiplying dose length product by conversion coefficient of 0.017. Results: There was no significant difference between the two groups for age, BMI, heart rate, heart rate variability and image noise. Of the 2801 coronary segments scored, excellent or good image quality(score of 1 or 2) was achieved 95.9%(702/732), 83.8%(547/653), 97.1%(720/742), 86.9%(586/674), and with no significant differences for A and C, B and D groups(P>0.05). Effective doses of prospective and retrospective gated scan were (3.28±0.23)mSv, (5.49±1.31)mSv, with an average of (4.35±1.44)mSv, and (15.29±0.55)mSv, (13.44±1.11)mSv, with an average of (14.51±1.32)mSv, respectively, and decreased 78.55%, 59.16%, with an average reduction of about 70.03%, respectively(P<0.001). In prospective gated group, there was statistically significant between high heart rate and low heart rate. Conclusion: Prospective gated DSCT scanning can provide adequate coronary diagnostic information, while significantly reduce the radiation dose. It can be used for suspected coronary artery disease screening.

406 The research of dual source CT perfusion parameter in biological behavior of gastric carcinoma

Objective: To investigate the value of dual source CT perfusion imaging parameters, including blood volume(BV), blood flow(BF), and Patlak P value in evaluating the biological behavior of gastric carcinoma. Methods: Dual source CT perfusion imaging was performed in 98 patients of gastric carcinoma. The BV, BF, Patlak P value were obtained to study the relationship between the parameters of perfusion and the biological behavior in gastric cancer, and estimate the value of parameters. Results: The BF could predict gastric carcinoma had no distant metastasis with a sensitivity of 72.13%, a specificity of 64.52%, a positive predictive value of 81.67%, a negative predictive value of 52.63%, and an accuracy of 70.41%. The BV could predict the gastric carcinoma had no distant metastasis with a sensitivity of 68.66%, a specificity of 51.61%, a positive predictive value of 75.41%, a negative predictive value of 43.24%, and an accuracy of 63.27%. The BV could predict gastric carcinoma confined within the serous membrane with a sensitivity of 60.00%, a specificity of 69.57%, a positive predictive value of 86.54%, a negative predictive value of 34.78%, and an accuracy of 62.24%. Patlak P value could predict the gastric carcinoma confined within the muscularis with a sensitivity of 61.54%, a specificity of 75.86%, a positive predictive value of 69.57%, a negative predictive value of 68.75%, and an accuracy of 69.09%. Patlak P value also could predict the gastric carcinoma had protrude beyond the serous membrane with a sensitivity of 14.29%, a specificity of 69.23%, a positive predictive value of 60%, a negative predictive value of 20%, and an accuracy of 27.27%. Conclusion: The dual source CT perfusion imaging parameters could assess the biological behavior of gastric carcinoma, but there still has limitations.

410 Clinical value of ultrasound-guided and fluoroscopy in percutaneous nephrostomy

Objective: To evaluate the clinical value of ultrasound-guided and fluoroscopy in percutaneous nephrostomy(PNT). Methods: By ultrasound-guided and fluoroscopy percutaneous placement of nephrostomy catheters were performed in 53 patients with hydronephrosis or pyonephrosis by using Seldinger technique. Results: Fifty-eight kidneys in 53 patients were punctured by percutaneous way, including 30 right kidneys, 22 left kidneys and 3 bilateral kidneys. Fifty-six kidneys(96.6%, 56/58) were successfully puncture at first attempt. The punctures in contralateral kidneys were performed in 2 patients because of the failure of first attempt. Fifty-six nephrostomy catheters(100%, 56/56) were successfully implanted by fluoroscopy-guidance. The improvement of renal function was obvious after operation, and no severe complications occurred. Conclusion: PNT by ultrasound-guided and fluoroscopy is a fast, safe, microinvasive and effective procedure for patients with obstructive uropathy, and this method is worth generalizing and applying.
