229 The value of real-time ultrasound elastography in the differential diagnosis of small single solid breast neoplasms

Objective: To assess the diagnostic value of ultrasonographic elastography for identifying suspected malignant breast neoplasms. Materials and Methods: Eighty-five patients with small single solid breast neoplasms first underwent two-dimensional, color Doppler and power Doppler ultrasound. Then elastography was performed, and used an improved 5-point system to diagnose malignant or benign lesions. The ultrasonographic results were compared with pathologic findings. Results: Taking the elasticity score 4~5 as malignant standard, the diagnostic sensitivity was 89.18%, specificity was 89.58%, accuracy was 89.41%. The combined diagnostic sensitivity of two-dimensional, color Doppler and power Doppler ultrasound was 48.65%, specificity was 79.16%, accuracy was 65.88%. Conclusion: Real-time ultrasound elastography is a promising imaging technique that can assist in the differential diagnosis of malignant small single solid breast neoplasm.

232 Application of pulse wave velocity measurements by MRI in the evaluation of aortic compliance in essential hypertension

Objective: To evaluate aortic compliance in essential hypertension by MRI pulse wave velocity(PWV). Methods: Forty-three patients with essential hypertension and 31 normal volunteers(ascending aorta, abdominal aorta) were measured by MR cine phase-contrast. We got the time-velocity curves and PWV. Results: the PWV of essential hypertension is faster than normal volunteers. There were high repetition(r1=0.90, r2=0.85) measured with method of MRI PWV. Conclusion: MRI PWV can be used as a better means to observe the changes of aortic compliance.

234 Evaluation of left atrial myocardial function in hypertensive patients with paroxysmal atrial fibrillation using strain rate imaging

Objective: To evaluate the alteration of left atrial myocardial function in hypertensive patients with paroxysmal atrial fibrillation using real-time triplane strain rate imaging and to investigate its clinical significance. Methods: Thirty-five solitary hypertensive patients, 32 hypertensive patients with paroxysmal atrial fibrillation and 33 healthy subjects were enrolled. Mean systolic peak strain rate, mean early diastolic peak strain rate and mean late diastolic peak strain rate(MSRs, MSRe and MSRa) were measured using real-time triplane strain rate imaging. Results: There was a significantly step-down change among control group, hypertensive group and hypertensive with paroxysmal atrial fibrillation group in MSRs(P<0.0001), MSRe was significantly decreased in hypertensive group(P<0.01), MSRe had no significant difference in hypertension with paroxysmal atrial fibrillation group(P>0.05), MSRa was significantly increased in hypertensive group(P<0.01), while MSRa was decreased in hypertension with paroxysmal atrial fibrillation group(P<0.05). Conclusions: The left atrial myocardial function in hypertensive patients is significantly impaired, which is worsen in hypertensive patients with paroxysmal atrial fibrillation, real-time triplane strain rate imaging provides important applicable significance in the comprehensive evaluation of left atrial myocardial function in hypertensive patients.

237 Extracorporeal cardiac shock wave therapy promotes angiogenesis after acute myocardial infarction in pigs evaluated by real-time myocardial contrast echocardiography

Objective: To evaluate the effect of neovascularization of extracorporeal cardiac shock wave therapy in a porcine model in vivo. Methods: Acute myocardial infarction was created by balloon occlusion of left anterior descending coronary artery and two different groups were divided(n=5 each). Real-time myocardial contrast echocardiography was performed before infarction and 1 day, 1 month after infarction. In group A, the shock wave therapy was started 3 days after acute myocardial infarction and applied 9 sessions on 4 segments with low energy(0.09mJ/mm2) at 200 shoots/spot for 9 spots(-1~0~+1 combination) within 1 month and group B was the control group. Results: Within 1 month follow-up, the death rate was 20% and 40% in 2 groups respectively. Neither arrhythmias nor other complications were observed during or after the shock wave therapy. Before infarction, there was no significant differences of regional blood flow valued by A·K on target segments between 2 groups. After infarction, followed at 1 day and 1 month, A·K decreased significantly before and after self control in both groups(P<0.05). In group A, A·K was higher on 4 segments compared with the group B(P<0.05). And the blood perfusion of the middle region improved better than that of the apex region(86.57% vs 65.54%). Whereas group B had little change. Conclusion: These results suggest that our extracorporeal cardiac shock wave therapy is a safe, non-invasive treatment in promoting angiogenesis that was associated by a substantial increasing of regional blood perfusion in a porcine model of acute myocardial infarction.

241 Evaluation of the hepatic vessels in living liver donors before transplantation: comparison of MSCTA and 3D-DCE-MRA

Objective: To determine whether MSCTA and 3D-DCE-MRA will be able to perform a complete and accurate role in preoperative evaluation of the hepatic vascular anatomy for living liver donors. Methods: Thirty living donor candidates underwent preoperative MSCTA and 3D-DCE-MRA. Data analysis was performed by 4 reviewers(2 radiologists and 2 surgeons), based on source images and 3-dimensional postprocessing. Determination of image satisfaction degree was based on 4 grades(1: nondiagonostic; 2: fair; 3: good; 4: excellent). They should also count the numbers of aberrant vessels, including non-Michels Ⅰ type, segment Ⅳ branch from right hepatic artery, trifurcation of portal vein branches, right posterior inferior hepatic vein and segment Ⅷ branch of hepatic vein draining to middle hepatic vein. Six donors were living donor in liver transplantation. Surgical findings were used as a standard of reference and then statistical analysis was performed. Results: Both techniques were proved to be efficient to evaluate hepatic artery and portal vein for potential living donors. There was no significant difference between the two. The main advantage of 3D-DCE-MRA lies in the ability to assess the hepatic vein systems more accurately. There was a significant difference between the two. Conclusion: 3D-DCE-MRA could be the first choice to assess potential living donor’s vessels.

246 Posterior mediastinal ganglioneuroma: CT and MR findings

Objective: To improve recognition of posterior mediastinal ganglioneuroma(PMGN). Materials and Methods: Clinical and imaging materials(five cases were performed CT only, one case was underwent MR only, and four cases were performed both CT and MR scans) of 10 cases histologically proved PMGN were analyzed retrospectively, additionally, CT and MR findings were summarized. Results: On images, vertical diameters of the PMGNs were bigger than both of anteroposterior and transverse diameters, and tumors grew along the vertebrae. Moreover, we classified PMGNs in our study into vertebral canal-associated(n=1) and non-vertebral canal-associated(n=9) GN on the basis of adjacent vertebral canal involved or not. Additionally, we grouped them into unilateral type(n=7) and acrossing middle-line type(n=3) according to the growth pattern. On plain CT, GNs were showed homogeneous or heterogeneous hypointense, scattering punctate calcification and patchy fat-density were seen in one case, respectively. There were non-enhancement or mild to moderate homogeneous or heterogeneous enhancement on enhanced CT. On MRI, T1WI showed homogeneous low signal and T2WI demonstrated obvious heterogeneous high signal intensity. With contrast material, there was heterogeneous enhancement or non-enhancement. Conclusions: PMGNs have some characteristics on CT and MR, we should make right diagnosis after thoroughly analysis.

250 Application of different kinds of TDC trend curves in qualitative diagnosis of prostate nodules

Objective: To study the diagnostic values of 3 different kinds of TDC trend curves by multi-slice CT(MSCT) dynamic scan for prostate nodules. Methods: Nine patients with abnormal PSA level whose diagnosis was confirmed were divided into 3 groups and they all underwent MSCT dynamic scan for prostate nodules retrospectively. To refer the article reports, we conclude 3 different kinds of TDC trend curves. Applied 3 different kinds of TDC trend curves in qualitative diagnosis of prostate nodules prospectively, and compared with the results of pathological diagnosis. Result: Thirty-seven patients underwent MSCT dynamic scan prospectively, 27 cases of them were certified by biopsy or operations, 25 cases of them were in accord with pathological diagnosis. Conclusion: Application of the 3 different kinds of TDC trend curves could improve the accuracy rate of diagnoses in the prostate nodules. It also provides biopsy under guidance of ultrasound and improves the positive rate.

254 Features of MRI in hereditary spastic paraplegia due to spastin mutation and REEP1 mutation

Objective: To investigate features of the magnetic resonance imaging in two pedigrees with hereditary spastic paraplegia type 4 and type 31. Methods: Eight HSP patients in two families were determined as SPG4 or SPG31 by genetic linkage analysis. MRI of brain and the whole spinal cord were performed in these 8 patients and 16 age- and gender-matched control subjects utilizing a 1.5T magnet. Cross-sectional areas of the spinal cord at the levels of C2, C7, T4, T9 were measured and data was statistically analyzed using the student’s t test, an association between the spinal cord size and severity of the disease was analyzed using the spearman rank correlation test. Results: All patients had grossly normal MRI of the brain. The cross-sectional areas of SPG31 subjects at the levels of C2, C7, T4, T9 were (50.13±3.43)mm2, (40.45±2.00)mm2, (22.03±4.55)mm2, (26.10±4.55)mm2 respectively. All of these values were less than those in the healthy controls(t=6.331, 6.889, 4.647, 3.345 respectively, P<0.007). While analysis of the cross-sectional areas in SPG4 subjects had no significant abnormalities. There was no association between the degree of spinal cord atrophy and severity and duration of the disease in these two genetic subtypes of HSP. Conclusions: The degree of spinal cord atrophy varies among different genetic types of HSP. Cervical and thoracic MRI of SPG31 subjects revealed atrophies of the spinal cord and enlarged subarachnoid cavity.
