153 MRI diffusion tensor imaging in patients with heterotopic gray matter: a quantitative study

Objective: To discuss the imaging characteristics of cerebral heterotopic gray matter tissues and the white matter fiber bundle on peri-heterotopic gray matter in MRI diffusion tensor imaging(DTI). Methods: Ten patients with heterotopic gray matter confirmed by MRI were examined DTI. To observe the white matter fiber bundles in peri-heterotopic gray matter, and the fractional anisotropy(FA) values were measured on the heterotopic gray matter and the surrounding white matter. In addition, the FA value of the contralateral normal cerebral white matter and gray matter were measured, respectively, to compared the FA values between the white matter fiber bundle on peri-heterotopic gray matter and the contralateral normal cerebral white matter. The heterotopic gray matter were compared with the normal gray matter area. Results: Ten patients with Ⅰ type in 2 cases; Ⅱ type in 5 cases; Ⅲ type in 1 case; Ⅳ type in 2 cases. There was basic change of the white matter fiber bundle at peri-heterotopic gray matter appeared as displaced or gathering together damage; there was no significant difference for FA value between the white matter fiber bundle at the peri-heterotopic gray matter and the contralateral normal position cerebral white matter(T=-0.435, P=0.669); the FA value between ectopic location of gray matter and normal gray area had no significant difference(T=-0.000, P=1.000). Conclusion: The white matter fiber bundle on peri-heterotopic gray matter has morphological abnormality, FA value has no significant decrease compared with that of the contralateral white matter fiber tracts; DTT can clearly show the direction and morphology of white matter in the ectopic gray matter.

157 The application of multi-slice spiral CT quantitative imaging technology research in COPD

Objective: To correlate MSCT pulmonary function parameters and pulmonary function test parameters. To compare quantitative parameters of MSCT between COPD patients and normal volunteers, and to define the clinical feasibility parameters of MSCT in patients with COPD. Materials and Methods: Thirty cases of normal control group, forty cases of COPD with clinical pulmonary function test and MSCT scan at the end of the maximum inspiratory and maximum expiratory phase. All data was analysed by the lung analysis software of computer-aided inspection system, correlation analysis with pulmonary function parameters and comparison of the normal volunteers were made. Results: There was significant correlation with Vin/Vex and residual volume(RV)/total lung capacity(TLC)(r=0.90, P<0.01). There was significant correlation with Vin, Vex and TLC, RV(r=0.70, 0.81, respectively, P<0.01). (Vin-Vex) showed correlation with FVC(r=0.76, P<0.01). Expiratory EV-910 correlated with FEV1/FVC%(r=-0.84, P<0.01). Expiratory PI-910 had a good correlation with FEV1 and FEV1/FVC%(r=-0.86, -0.85, respectively, P<0.01). At inspiratory phase, the average Vin was (5318.15±1200.74)ml, lung density was (-898.04±35.36)HU, the density per unit volume was (0.102±0.035)g/cm3. There was no significant difference among the three parameters in normal group and COPD group(P>0.01). At expiratory phase, the average Vex was (3878.21±1202.75)ml, lung density was (-855.04±50.41)HU, the density per unit volume was (0.144±0.051)g/cm3. There was significant difference among the three parameters in normal group and COPD group(P<0.01). Conclusion: MSCT pulmonary function test can evaluate the situation and provide an objective basis for clinical diagnosis of the patients with COPD.

162 3D sonographic measurement of lung volume in fetuses with congenital diaphragmatic hernia

Objective: To evaluate the accuracy of 3D ultrasound in estimating lung volume of fetuses with congenital diaphragmatic hernia(CDH), and to determine their volume change by comparing with the normal fetuses. Methods: Fetal lung volumes were measured during 3D ultrasound examination using VOCAL technique in 10 cases with CDH, 1~3 days before termination. Postmortem autopsy was conducted within 6 hours after labor and the lung volume was achieved by water displacement. Using VOCAL technique, lung volumes were also measured in controls and compared with those in CDHs. Results: ①The correlation between measured volume by 3D ultrasound and postmortem examination was good, with correlation coefficient 0.79 in CDH fetuses. The mean bias, 95% limits of agreement were, -0.68cm3 and(-4.59~3.24)cm3, respectively. ②In fetuses with CDH, lung volumes were substantially lower than in normal fetuses. Conclusion: The lung volumes were significantly reduced in CDH fetuses, due to pulmonary compression. Prenatal 3D ultrasound can estimate accurately fetal lung volumes in CDHs, which may become a potential predictor for the degree of pulmonary hypoplasia.

165 The clinical application of 64-MSCT in evaluating the stability of plaque in coronary artery

Objective: To evaluate the feasibility and diagnostic accuracy of 64-slice spiral computed tomography(MSCT) in differentiating the stability of plaque in coronary arteries. Methods: Seventy-six patients diagnosed of having CAD clinically had undergone 64-MSCT angiography. They were divided into two groups according to clinical materials: ACS and SA. The number, morphology and CT value of plaques obtained by MSCT were compared between the two groups, as well as the change of CT value after delayed scanning. The remodeling index had also been analyzed. Results: The number of plaques did not differ between the two groups, however, in the culprit lesion of ACS group, the minimum CT density[(29±24)HU] was lower than that of SA group[(69±29)HU](P<0.0001). Similarly, in ACS patients with multiple plaques, the minimum plaque density[(22±25)HU] was lower in the culprit coronary segment than in the non-culprit segment[(50±28)HU](P<0.002). The reduction of CT density in SA group was significantly lower than ACS group after delayed scanning. With respect to remodeling index, the rate of positive remodeling in ACS group was 58.1%, while this rate was 24.7% in SA group. The chi-test demonstrated significant difference between the two groups. Conclusion: 64-MSCT can sufficiently detect vulnerable plaques from coronary plaques in patients with coronary artery disease through determining their density and morphology et al, but its clinical use may presently be limited due to image quality and evaluation means to a certain extent.

170 Clinical relevance of dual source CT angiography in the detection of visceral artery aneurysms

Objective: To quantitatively evaluate the clinical value of contrast-enhanced axial CT images and post-processing CT images at dual source CT in the detection of visceral artery aneurysms. Methods: Fourteen patients with visceral artery aneurysms demonstrated by dual source CT angiography were retrospectively analyzed. Two well-experienced radiologists evaluated the axial and post-processing images, they knew the patients’ history but not the location and numbers of aneurysms. Location and numbers of aneurysms were recorded if one or more aneurysms in a vessel were found, it was regarded as positive. With the comprehensive diagnosis as reference standard, the sensitivity and specificity of enhanced axial images and post-processing images to detect aneurysms were calculated. The time to draw a conclusion for the enhanced axial images and post-processing images was recorded and analyzed. Results: Of the 14 patients, aneurysms were found in 23 vessels and 61 vessels had no aneurysms. With the comprehensive diagnosis as reference standard, axial images correctly diagnosed 17 vessels with aneurysms, 4 false positive vessels, 6 false negative vessels, and 61 true negative vessels, corresponding to sensitivity and specificity of 74% and 93%. Post-processing images correctly diagnosed all aneurysms in 23 vessels and 61 vessels have no aneurysms, corresponding to sensitivity and specificity of 100% and 100%. Axial images under- or over-diagnosed more aneurysms than post-processing images. The analyzing time of axial images[(58.5±15.6)s] was longer than that[(30.6±8.2)s] of the post-processing images(P<0.001). Conclusions: Dual source CT angiography, especially VR and MIP, can clearly visualize the morphology, location, and number of visceral artery aneurysms. It has an important value in the detection of small aneurysms and definitive diagnosis and clinical management and saving reading time.

174 Ultrasonography in the diagnosis and analysis of 205 cases non-tumor intestinal obstruction

Objective: To investigate the value of color Doppler ultrasonography in the diagnosis of intestinal obstruction result from different non-tumor causes and summarize the features of those ultrasonograms. Methods: To analyze retrospectively the abdominal ultrasonography results of 205 cases of non-tumor intestinal obstruction which were later confirmed by surgery, which included the diameter of intestine pipes, the thickness of intestinal walls, the echo of intestinal walls, the scope of lesion, the intestinal peristalsis, the blood supply of intestinal walls and mesentery etc. Results: To diagnose accurately the causes of non-tumor intestinal obstruction 180 cases among 205 cases by ultrasonography, diagnose accordance rate 87.8%(180/205); missed diagnose and misdiagnose rate 12.2%(25/205); all cases were later confirmed by surgery. Conclusions: Abdominal ultrasonography is accurate and dependable to diagnose non-tumor intestinal obstruction and those causes because it can provide objective imageology evidences for clinician, so it can serve as a routine examination for non-tumor intestinal obstruction.

178 Morphologic study of normal nerve roots of brachial plexus within spinal canal by balance SSFP sequence

Objective: To delineate normal morphology and anatomy of nerve roots of brachial plexus within the spinal canal on the multiplanar reconstruction images of 3-D balance steady state free precession(balance SSFP). Methods: Twenty cases of normal volunteers and 28 cases in which no abnormal appearance of CTM coming from a consecutive 283 cases were evaluated by 3-D balance SSFP sequence on coronal, sagittal and oblique axial reconstructive images and some data of the nerve roots, 20 cases of normal volunteers were measured. Results: A coronal image clearly demonstrated all of the nerve roots and described the ventral roots as a wing-like configuration and the dorsal roots as a brush-like configuration. A sagittal image on paraspinal cord demonstrated all of the nerve roots and the locating relationships of the upper and lower nerve roots with the corresponding vertebra body. Oblique axial image might display a more continuous roots than one on conventional axial image. Conclusion: Coronal, sagittal and oblique axial reconstructive of 3-D balance-SSFP sequence images was helpful in obtaining more information about the morphology and anatomy of nerve roots of brachial plexus within spinal canal.
