761 Correlation of internal carotid artery stenosis and risk of stroke inpatients with transient ischemic attacks

Objective: To evaluate the relationship between internal carotid artery stenosis, plaques properties and stroke occurrence in transient ischemic attacks(TIA). Materials and Methods: Conventional CT, CT angiography(CTA) and CT perfusion imaging were performed in 26 patients with clinically diagnosed TIA. The time to peak(TTP), cerebral blood flow(CBF) and cerebral blood volume(CBV) were calculated. The stenosis of carotid artery was also assessed. Results: In 26 cases with TIA, CTA showed 19 cases of internal carotid artery stenosis, mild stenosis in 7 cases(37%), moderate stenosis in 8 cases(42%), severe stenosis in 4 cases(21%). Carotid plaques in 44 places, including 9 mixed plaques, 22 calcified plaques, 7 fibrous plaques, 6 lipid plaques. Old-age incidence was obviously higher than middle-aged group(P<0.05). In 26 cases with TIA, there were 21 normal cases and 5 cases with lacunar infarction, 2 cases had multiple arteries involvement on plain CT scan. Seventeen patients had abnormal CT perfusion and 9 had normal CT perfusion in 26 cases. The TTP of affeted side (11.25±1.94)s compared with that of the contralateral side (8.64±0.96)s was significantly prolonged(t=9.418, P<0.01). The difference of the CBF and CBV was not statistically significant between both sides. Follow-up of 6 months in 3 cases had cerebral infarction. Conclusion: Cerebral apoplexy and internal carotid artery stenosis has close relationship in TIA patients. CT perfusion and CTA can provide valuable diagnostic informations for patients with TIA.

766 Clinical value of 18F-FDG dual head coincidence imaging and 131I-SPECT/CT inthe diagnosis of patients with metastatic after resection of papillary thyroid carcinoma

Objective: The aim of this study was to evaluate the value of 18F-FDG dual head coincidence imaging and 131I-SPECT/CT in the diagnosis of patients with metastatic papillary thyroid carcinoma(PTC). Methods: Fifty-six patients who had thyroidectomy and radioiodine ablation of residual normal thyroid tissue were included. They all had 18F-FDG dual head coincidence imaging and 3 days after giving 3.7~7.4GBq 131I 131I-SPECT/CT were performed. We had comparative analysis of the two imaging results and the variable parameters between the 18F-FDG dual head coincidence imaging positive group and 18F-FDG dual head coincidence imaging negative group. Results: The sensitivity, specificity, accuracy, positive predictive value and negative predictive value were 84.21%(32/38), 94.44%(17/18), 87.5%(49/56), 96.97%(32/33), 73.91%(17/23). The difference was significant between 131I-SPECT/CT and 18F-FDG dual head coincidence imaging in sensitivity, accuracy, negative predictive value(χ2=16.114, 14.175, 6.285, all P<0.05). Metastatic size of 18F-FDG dual head coincidence imaging positive group and negative group were (1.83±0.30)cm, (1.29±0.26)cm, respectively, the difference between them was statistically significant(P<0.001). Conclusion: 131I-SPECT/CT was likely to disclose more metastasis than 18F-FDG dual head coincidence imaging in patients with PTC. 18F-FDG dual head coincidence imaging is an important supplement of 131I-SPECT/CT in detecting metastatic PTC, the metastatic size affected the sensitivity of 18F-FDG dual head coincidence imaging.

770 Assessing the value of multi-slice CT in the preoperative classification and staging of laryngeal carcinoma

Objective: To evaluate the significance of multi-slice CT with multiplanar reconstruction(MPR) and enhanced scanning in preoperative classification, staging of laryngeal carcinoma, and to analyze the factors of influence. Methods: Forty-one patients with laryngeal carcinoma diagnosed by electrical laryngoscope were studied by preoperative CT examination and MPR. Its images were used to investigate the classification and staging of tumor, the findings were compared with pathological results. Results: The overall positive predictive value of CT in assessing tumor invasion was 81.9%, false negative rate was 9.3%, the sensitivity was 90.7%. In preoperative T staging, T1, T2, T3 and T4 were 80%, 66.7%, 52.6%, 72.7% respectively. In preoperative N staging, N1 was 58.3%, N2 was 100%. Conclusion: The CT images with MPR and enhanced scan can display shape, size, invasion and lymph node metastasis, and it is of essential value in typing, staging and establishment of reasonable treatment.

774 Differential diagnosis of imaging findings between patients with triple-negative breast cancer andpatients with idiopathic granulomatous mastitis

Objective: To analyze the imaging findings of triple-negative breast cancer(TNBC) and to improve the understanding and diagnostic performance of the disease, by comparing it with idiopathic granulomatous mastitis(IGM). Methods: The imaging findings in 63 randomly assigned women with surgically and pathologically confirmed TNBC(n=45) and IGM(n=18) were retrospectively analyzed. All cases underwent a mammographic examination. An ultrasonic examination was performed in 37 cases of TNBC and 18 cases of IGM. MR examination was performed in 33 cases of TNBC and 14 cases of IGM. The mammographic imaging findings included lesion shape, density, margin, calcification, axillary adenopathy and thickening of the skin. The ultrasound imaging findings included shape, margin, size, echo, axillary adenopathy and thickening of the skin. The MR imaging findings included tumor shape, size, margin and internal enhancement, signal intensity within the tumor as well as peripheral signal intensity on T2-weighted, then the type of time-signal intensity curve(TIC) was analyzed. Results: The results of comparing TNBC group with IGM group showed as follows: ①Mammography imaging findings: mass type accounts for 67%(30/45) in TNBC group and 39%(7/18) in IGM group. Smooth margin of the lesion accounts for 60%(18/30) in TNBC group, while 29%(2/7) in IGM group.②Ultrasound imaging findings: mass type accounts for 92%(34/37) in TNBC group and 89%(16/18) in IGM group. Smooth margin of the lesion accounts for 62%(21/34) in TNBC group, while 37%(6/16) in IGM group.③MR imaging findings: mass type accounts for 91%(30/33) in TNBC group and 86%(12/14) in IGM group. Rim enhancement accounts for 67%(20/30) in TNBC group and 75%(9/12) in IGM group. For the rim enhancement mass type, smooth margin of the lesion accounts for 70%(14/20) in TNBC group, while 11%(1/9) in IGM group; peripheral hyperintense pattern on T2-weighted accounts for 85%(17/20) in TNBC group and 21%(2/9) in IGM group. Conclusion: We believe that through careful observation of the lesion site, shape, edge of the tumor mass and peripheral signal intensity on T2-weighted, enhancement pattern and the TIC types, might be useful for the identification of TNBC and IGM.

780 A study of the color Doppler ultrasonographic characteristics and regularity in levoversion deformity

Objective: To explore the image characteristics and regularity of color Doppler echocardiography(CDE) on levoversion deformity. Methods: Fifty-two patients with levoversion deformity were examined by CDE to explore their image characteristics and regularity. All cases with levoversion deformity underwent angiocardiography examination after CDE. Thirty-four cases were proved by surgical operations. Results: Forty-eight cases were diagnosed correctly by the features of CDE imaging. The diagnostic accuracy rate was 92.3%. The CDE characteristics of levoversion were: ①Levoversion can be associated with 8 kinds of complicated congenital heart disease, among which corrected transposition of the great arteries(34.6%) were common, bilocular heart(19.2%), double-inlet ventricle(15.4%), double outlet right ventricle(13.5%), and mitral atresia(11.6%) were less common, tricuspid atresia, complete transposition of the great arteries and persistent truncus arteriosus(1.9% each) were rare. ②Each kind of complicated congenital heart disease combined multiple congenital heart deformity, among which were transposition of the great arteries(98.1%), pulmonary stenosis(88.5%), ventricular septal defect(60.0%), atrial septal defect(38.5%) and patent ductus arteriosus(7.7%). ③With pulmonary stenosis, color Doppler flow image showed obvious signal of colorful mosaic jet through the pulmonary artery. With ventricular septal defect, atrial septal defect and patent ductus arteriosus, color Doppler flow image showed mild colorful flow signal of shunt. ④Despite the diversity of combined congenital heart disease, all cases of levoversion had a common CDE characteristic of atria situs inversus. Conclusions: The image characteristics and regularity of CDE on levoversion deformity were obvious. CDE can be used to correctly diagnose levoversion deformity, only if the operators of CDE have a clear idea of the anatomy of the levoversion. Before examination, reading front view chest X-ray image would be helpful to improve accuracy and efficiency of diagnosis.

783 A preliminary study of myocardial contrast echocardiography for coronary microvascular endothelial dysfunction inpatients with angiographical normal coronary arteries

Objective: To evaluate myocardial contrast echocardiography for the coronary microvascular endothelial dysfunction in patients without showing coronary obstruction by angiography. Methods: Twenty subjects with angiographically normal coronary arteries were involved in group 2. Twenty healthy volunteers without risk factors for coronary artery disease were control group 1. Real-time myocardial contrast echocardiography(MCE) were performed and flash imaging were applied to evaluate myocardial microcirculation perfusion. The peak video intensity(PI), the time to peak concentration(TP) and the duration time(T) were recorded and analyzed in two groups. Results: The PI of group 2 were lower than that in group 1(P<0.05). The TP and the T of group 2 were longer than those in group 1(P<0.05). Conclusions: Coronary microvascular endothelial dysfunction could be detected early in groups with risk factors for coronary artery disease by MCE.

786 Evaluation of rectum cancer neoadjuvant therapy using MSCT hypotonic air inflation of rectum

Objective: To evaluate rectum cancer preoperative neoadjuvant therapy using hypotonic rectal air inflation MSCT. Methods: Imaging findings in 33 cases of pathologically confirmed colorectal cancer after neoadjuvant therapy were analyzed retrospectively. The assessment included: intestinal wall thickness at tumor site, the extent of tumor invlovement of intestinal circumference, the length of intestinal tumor, the features after enhancement, the peri-vascular fascia, lymph node enlargement, and T stage of tumor. Results: Hypotonic rectal air inflation MSCT pre- and post-neoadjuvant therapy can show the rectal lesion satisfactorily. Before therapy: the intestinal wall thickness at the tumor site was 4~10mm in 7 cases, 11~30mm in 15 cases, beyond 31mm in 11 cases. Involvement of the intestinal circumference: less than 1/3 in 6 cases, 1/3~1/2 in 11 cases, more than 1/2 less than whole circumference in 8 cases, whole circumferential involvement in 8 cases. The length of invlovement: less than 10mm in 4 cases, 10~30mm in 8 cases, 30~50mm in 14 cases, more than 50mm in 7 cases. All cases had moderate enhancement. Invasion of mucosal and submucosal layer in 7 cases, invasion of serosa in 15 cases, invasion beyond serosa in 11 cases. In 11 cases the peri-intestinal fatty tissue and fascia were invaded. Peri-intestinal, peritoneal and posterior peritoneal cavity lympy node enhargement were seen in 15 cases. Distant metastases in 4 cases. Occurrence of T1 stage was 15.15%, T2 stage was 30.30%, T3 stage was 42.42%, T4 stage was 12.12%. After therapy: the intestinal wall thickness became normal in 10 case, 4~10mm in 5 cases, 11~30mm in 10 cases, beyond 31mm in 8 cases, involvement of intestinal circumference less than 1/3 in 15 cases, more than 1/3 less than 1/2 in 12 cases, more than 1/2 less than whole circumference in 6 cases. Tumor length less than 10mm in 13 cases, 10~30mm in 6 cases, 30~50mmin 10 cases, beyond 50mm in 4 cases. All cases had mild enhancement. Invasion of mucosal and submucosal layer were seen in 10 cases, invasion of serosa layer in 17 cases, invasion beyond serosa in 6 cases. Peri-intestinal fatty tissue and fascia involvement in 2 cases. Peri-intestinal, peritoneal cavity and post-peritoneal cavity lymph node enlargement seen in 6 cases. Distant metastases were seen in 3 cases. After neoadjuvant therapy the changes of T stage were: T0 stage 30.30%, T1 stage 24.24%, T2 stage 30.30%, T3 stage 9.09%, T4 stage 6.06%. Conclusion: Hypotonic rectal air inflation MSCT is useful in evaluating the effect of neoadjuvant therapy and can accurately assessing the changes of T staging pre- and post-therapy.

790 Applications of MR whole body DWI in multiple bone tumors

Objective: To investigate the value of whole body DWI(WB-DWI) in differential diagnosis of multiple bone metastases and multiple myeloma. Materials and Methods: Fifty-seven cases with multiple bone tumors and 12 cases normal volunteers underwent WB-DWI. Furthermore, the signal characteristic and ADC values of the thoracic, lumbar vertebrae, and llilac bone were analyzed and measured respectively. In addition, the detection frequency for searching primary tumors in patients with multiple osseous metastases was calculated. Results: Multiple osseous tumors demonstrate abnormal high signals on WB-DWI. The ADC value of multiple osseous metastases were higher than that of multiple myeloma. Meanwhile, the ADC value of multiple myeloma and multiple osseous metastases were markedly higher than that of normal bone. The difference was significant in ADC values between multiple myeloma and multiple osseous metastases(P<0.01). The detection frequency for searching primary tumors in patients with multiple osseous metastases was 74.5%(35/47). Conclusion: Multiple osseous metastases and multiple myeloma can be differentiated by quantitative analysis with ADC value. The WB-DWI can demonstrate primary tumors, which also can help to differentiate multiple osseous metastases and multiple myeloma.
