Using perfusion and proton spectroscopic MRI in the differentiation of high-grade gliomas and cerebral metastases

Objective: To assess the clinical application value of dynamic susceptibility contrast(DSC) MR perfusion and 2D-Multivoxel 1H MR spectroscopy in the differentiation of high-grade gliomas and cerebral metastases. Methods: Retrospective analysis all patients’ data who with histopathologic diagnosis of high-grade gliomas(n=16) and metastases(n=9) underwent DSC-MR perfusion imaging and 2D-Multivoxel 1H MR spectroscopy before surgery. The data were analyzed retrospectively. The ratios of relative cerebral blood volume(rCBV) were measured in the regions of tumor, peritumoral and marginal regions with T2 hyperintensity respectively. Simultaneously, the interesting voxel Cho/NAA, Cho/Cr, NAA/Cr ratios were calculated in the peritumoral area and in the maximum perfusion area of tumor in the MRS slice. The difference of the these data have been analysed statistically. Result: There are no significant difference of rCBV in the maximum perfusion area of tumor and marginal zone with T2 hyperintensity between high-grade glomas group and metastases group. But, the value of rCBV of high-grade glomas group is significantly higher than metastases group(P=0.023). The results of 2D-Multivoxel 1H MR spectroscopy demonstrated that the ratio of Cho/NAA, Cho/Cr in the maximum perfusion area of high-grade glomas group tumor and the Cho/NAA in the peritumoral area were more higher than those of the metastases group significantly. Conclusion: DSC-MR perfusion imaging and 2D-Multivoxel 1H MR spectroscopy can offer the information of angiogenesis and metabolin in tumor and peritumoral area, so these techniques will be usefull in differentiating metastases form high-grade gliomas.

The research of influence factor for breast sentinel lymphoscintigraphy with 99Tcm-phytate

Objective: Variable success rates for identifying axillary sentinel lymph node(SLN) in breast cancer patients using preoperative lymphoscintigraphy have been reported. We evaluated whether tumor location, size, method of interval and age affect its success in identifying SLN. Method: Preoperative breast lymphoscintigraphy was performed in consecutive breast cancer patients. Imaging at 5 minuties and 2 hours after peritumoral injection of 99Tcm-phytate. Chi-Square test for statistics was used with SSPS 11.5 software. Results: SLN were obtained in 204(97.1%) at 5 minuties and?蛐or 2 hours of 210 patients. Success in identifying SLN was not affected by tumor location, size, method of interval; SLN were identified significantly more frequently in patients under 50 years old(59.4%) than those over 50 years old(44.7%)(χ2=4.03, P<0.05) at 5 minutes; 98.5% and 90.7%(χ2=4.49, P<0.05) at 2 hours. Conclusion: 99Tcm-phytate SLN scintigraphy is an easy technique with an acceptable success rate. No effect of tumor size, location, method of interval was found. Patient age affected its success in identifying SLN.

The value of X-ray attenuation correction for myocardial perfusion imaging

Objective: To evaluate the clinical value of X-ray attenuation correction for myocardial imaging in SPECT/CT. Methods: Total 65 cases, 35 cases(Group 1), with no coronary artery disease by clinical history, ECG, ultrasound and other necessary clinical examinations; 30 patients(Group 2) with coronary artery disease. All underwent 99mTc-MIBI myocardial perfusion tomography imaging and X-ray attenuation correction and were compared with their angiograms. Results: ①The uptake ratios of inferior and posterior wall in Group 1 and Group 2 increased from (77.0±4.77)% and (76.0±5.7)%, (31±4.3)% and (35±5.5)% without X-ray attenuation correction to (85.0±4.5)% and (83.0±5.2)%, (38±5.1)% and (42±5.5)% with X-ray attenuation correction(P<0.001), respectively. ②With qualitative analysis, Group 1: 82.8% subjects showed obvious improvement in the defects in inferior and posterior wall; 14.3% subjects with medium-level improvement. Group 2: 73.7% subjects showed improvement. ③The coherence of myocardial perfusion angiographic images of Group 1 and Group 2 after X-ray attenuation correction improved to 8.6%, 15.4% separately. Conclusion: X-ray attenuation correction in SPECT/CT is a simple and valid method. It corrects most of the attenuation artifacts in myocardial perfusion imaging, especially in the inferior and posterior wall. It helps to avoid the false positive diagnosis and improve the diagnostic confidence of nuclear cardiologists. It is important for coronary artery disease especially with myocardial perfusion images.

Dynamic CT and MR in the diagnosing intraductal papillary mucinous carcinoma of the pancreas and correlation with pathology

Objective: To explore the clinical value of MSCT and MRI findings in the diagnosing intraductal papillary mucinous carcinoma of the pancreas. Materials and Methods: Nine cases of intraductal papillary mucinous carcinoma of the pancreas underwent plain and contrast MSCT and among these 5 cases had plain and contrast MRI scanning. All cases were confirmed by surgical and pathology. The CT and MRI data were reviewed and analysed retrospectively and correlated with surgical and pathologic results. Results: Among the 9 cases, 5 cases were of the main duct type, showed obvious dilation of the main duct of the pancreas, most cases with mural nodules in the duct, which showed high density on CT and inhomogeneous long T1 nad long T2 signal on MRI, the main duct showed long T1 and long T2 signal. Three cases were of the branch-duct type, showed solitary or multiple cystic lesions with septum or nodules within the duct, CT showed high density, MRI showed long T1 long T2 signal. Two cases showed the lesion linked with the main duct. One case belonged to the mixed type showed solitary cystic lesion with nodules in the duct, together with dilation of the main duct, the lesion showed long T1 and long T2 signal. Six cases showed dilation of the main duct(4~45mm). Six cases showed atrophy of the pancreas, 3 cases with dilation of the duodenal papilla, 2 cases with irregular calcifications. After injection of contrast medium the intra-cystic septum was markedly enhanced and the mural nodules within the duct were mildly enhanced. Conclusion: Dynamic thin slice MSCT with MPR, CPR and MRI with MRCP have important value in diagnosing intraductal papillary mucinous carcinoma of the pancreas, small nodules can be showed clearly, and correlated well with pathologic findings. Connected with clinical signs, most cases can be distinguished from benign intraductal papillary mucinous neoplasms of the pancreas.

Evaluation of collateral circulation in Budd-Chiari syndrome with CTA technique using 64-slice CT

Objective: To evaluate the computed tomographic angiography(CTA) technique in the diagnosis of collateral circulation in Budd-Chiari syndrome(BCS). Methods: Seventy-four patients with mix-type BCS were examined by CTA technique. The involved hepatic veins(HVs), inferior vena cavas(IVCs) and collateral pathways were observed in detail. Results: Obstructions of HVs and IVCs were found in all the 74 patients at different level and degree, collateral pathways developed and were defined in each patient. The collaterals can be classified as intrahepatic, extrahepatic collaterals and portal vein-systemic vein communication. Intrahepatic collaterals were further classified as the following six types: ①hepatic vein(HV)-accessory hepatic vein(AHV) pathway(n=47, 63.5%); ②HV-HV pathway(n=5, 6.7%); ③HV-AHV/HV pathway(n=6, 8.1%); ④inferior vein cava(IVC)-AHV-HV-right atrium pathway(n=4, 5.4%); ⑤HV-umbilical vein pathway(n=4, 5.4%); ⑥HV-hepatic subcapsular vein pathway(n=8, 10.8%). Blood of obstructed HVs was drained into IVC, right atrium, umbilical veins or hepatic subcapsular veins through communicating branches which were in various amount and diameter. Extrahepatic collaterals include: ①ascending lumbar-azygos/hemiazygos pathway(n=74, 100%); ②left renal-ascending lumbar-hemiazygos(n=74, 100%)/left renal-inferior phrenic vein pathway(n=48, 64.8%); ③renal vein-renal subcapsular vein-superficial collaterals of the abdominal wall pathway(n=26, 35.1%); ④superficial abdominal wall pathway(n=10, 13.5%). Conclusion: Collateral circulations of BCS can be demonstrated clearly and non-invasively by CTA. CTA is valuable for the guidance in choosing right therapy method.

Control study of CT perfusion imaging and histology of rabbit liver VX2 tumor before and after the application of Sorafenib

Objective: To study the CT perfusion parameter change and histology of rabbit liver VX2 tumor before and after the application of Sorafenib, it was supposed to explore the feasibility of CT perfusion imaging in monitoring of the growth of liver tumor, and to provide experimental basis for evaluation the effect of Sorafenib inhibiting tumor angiogenesis and promoting cell apoptosis. Methods: The multi-slice CT perfusion was performed in 12 New Zealand white rabbits at 2 weeks after VX2 hepatic tumor implantation. Two were executed as the control group of pathology. The various perfusion parameters(HBF, HBV, MTT, PS, HAF) were measured in the tumor area and non-tumorous regions in other 10 rabbits. Sorafenib was administered by mouth at the dosage of 20mg/kg/d for one week. The second CT perfusion was performed at 3 weeks, the parameters of tumor region were compared by the previous result. After that, all the rabbits were executed and pathological examination and Caspase-3 staining was performed. Result: Twelve(100%) rabbits were successfully implanted with VX2 tumor in the liver. It was demonstrated that the increased HBF, HBV, PS, HAF and the decreased MTT in the tumor area were differed significantly from the non-tumorous regions at 2 weeks after VX2 hepatic tumor implantation(P<0.01). The statistical analysis also showed significant difference in HBF, PS, HAF of liver tumor at the two CT perfusion(P<0.01). And no significant difference in HBV, MTT. The expression intensity of Caspase-3 was significantly lower than that of normal control group(P<0.05). Conclusion: The multi-slice CT perfusion imaging had advantages in the changes of hemodynamics of the rabbit model of VX2 liver tumor, the effect of Sorafenib inhibiting tumor angiogenesis and promoting cell apoptosis could be more identified if combination with the research of histology.

An experimental study of dual source CT approach measuring bone mineral density

Objective: To evaluate the accuracy of DEQCT in determining bone mineral density(BMD) by using dual source CT(DSCT). Methods: BMD of the lumbar vertebral body of pig was measured by our designing software, DEQCT, at DSCT and by SEQCT software of the Neusoft and DEXA. The lumbar body of pig was then calcined, the lime of the calcined material was measured. The measuring results of DEQCT, SEQCT140kV and DEXA were compared. Results: The BMD of the vertebral body and the calcined material measured by DEQCT was (0.5050±0.0434)g/cm3, and (0.5023±0.0353)g/cm3 respectively, there was no significant difference(P=0.779). BMD measured by SEQCT140kV[(0.4813±0.0314)g/cm3] was significantly lower than the measurement of the calcined material. BMD determined by DEXA[(0.4527±0.0360)g/cm2] was significantly lower than the measurement of calcined material and also significantly lower than the results measured by DEQCT and SEQCT(P<0.05). Conclusion: BMD determined by DEQCT at DSCT is superior to SEQCT and DEXA approach.
