Measurement of hippocampal T2 relaxation time on MRI in idiopathic temporal lobe epilepsy

Abstract: Objective: To determine the value of hippocampal T2 relaxation time(HCT2) on MRI for the assessment of temporal lobe epilepsy(TLE), and provide primary quantitative data to predict its operational prognosis by HCT2. Methods: Thirty-six cases of generalized epilepsy(GEP), 23 non-temporal lobe partial epilepsy(NTPE), 26 temporal lobe epilepsy(TLE) and 60 neurologically healthy people(CL) were chosen to get the HCT2 on oblique coronary MRI obtained by SE dual echo sequence, and the differences of HCT2 between different groups were analyzed. Results: The mean HCT2 of high side in TLE was 7.5518ms higher than that in CL(P=0.000), and 4.1639ms and 3.7293ms higher than that in GEP and NTPE respectively(P=0.000, 0.000). The mean HCT2 of low side in TLE, low side and high side in GEP and NTPE, were 2.7547ms, 3.1347ms, 3.2829ms, 3.4669ms, 3.9016ms higher than those in CL respectively(P=0.000, 0.000, 0.000, 0.000, 0.000). The mean HCT2 ratio of low side to high side in TLE was 0.0482, 0.0456, 0.0425 lower than those in CL, GEP and NTPE respectively(P=0.000, 0.000, 0.000). Conclusion: HCT2 of focus side in TLE is significantly higher than those in CL, GEP and NTPE. The lower HCT2 ratio of low side to high side is relatively specific change in TLE.

Comparison of the value of ultrasonic E-Tracking imaging and the method of flow-mediated vasodilation in evaluating vascular endothelial function in type 2 diabetes mellitus

Abstract: Objective: To study the value of ultrasonic E-Tracking imaging technology and the method of flow-mediated vasodilation in evaluating vascular endothelial function in type 2 diabetes mellitus(T2DM) comparatively. Methods: Forty-six T2DM and thirty normal controls were included. E-Tracking imaging technology was performed to follow the movement tracks of the right brachial arterial wall, parameters including internal systolic diameter(Ds), internal diastolic diameter(Dd), stiffness parameters(β), pressure-strain elastic modulus(Eρ), arterial compliance(AC), and one-point pulse wave velocity(PWVβ) were assessed. The dilation of brachial artery during reactive hyperaemia(with increased flow causing endothelium-dependent dilatation) and after sublingual nitroglycerin application NTG, causing endothelium-independent dilatation were measured using high resolution ultrasound. The same parameters were assessed after three months treatment with melbine in 24 cases of T2DM. Results: In the T2DM group, β, Eρ and PWVβ were significantly higher, and AC was significantly lower than those in the control group(P<0.01, comparatively). In T2DM, flow-mediated dilatation(FMD) decreased conspicuously compared with that in controls(P<0.01). The dilation by NTG was not significantly different between the two groups(P>0.05). Conclusion: E-Tracking imaging technology and the method of FMD can be used to evaluate vascular endothelial function in T2DM. Two methods have satisfactory concordance.

CT features of lymph node metastases of thoracic esophageal carcinoma

Objective: To explore the CT features and distribution characteristics of lymph node metastases of neck, mediastinum and abdomen in patients with thoracic esophageal carcinoma. Methods: To analyse the CT materials of 200 cases of histopathologically proved esophageal cancer retrospectively with three fields of lymph node metastases. The CT imaging criteria of lymph node metastases are maximum short axis diameter of lymph node equal to or larger than 1cm. According to the lymph node location, the three fields of lymph node were classified as 19 groups. CT imaging was evaluated with special attention to the location, shape, size, number and enhancement. Results: The lymph node metastases of three fields were observed mainly around the right inferior paratracheal nodes(40.0%), the aorticopulmonary window nodes(34.0%), and the inferior tracheal bifurcation nodes(27.0%). The posterior crural nodes(2.5%), and the splenic nodes(1.5%) were the least location where metastases occurred. Conclusion: Lymph node metastases of thoracic esophageal carcinoma are mainly located around the right inferior paratracheal nodes, the aorticopulmonary window nodes and the inferior tracheal bifurcation. CT imaging can clearly reveal the location of lymphadenopathy in patients with esophageal carcinoma.

Application of diffusion tensor tractography in brain astrocytomas

Objective: To evaluate the value of diffusion tensor tractography(DTT) in brain astrocytomas. Materials and Methods: Thirty-three patients with histologically confirmed astrocytomas underwent routine MRI, DTI, DTT, analyzing the changes of white matter fibres around the tumors and specific conditions in different grade of astrocytomas. Results: There is displacement of white matter fibres around tumors whether low-grade tumors or high-grade tumors(77.78%~100%). Disruption and interruption of white matter fibres around tumors are related to malignancy of astrocytomas. The higher the grade of tumor, the higher incidence rate of white matter disruption and interruption are(7.69%~72.73%). Conclusion: DTT plays an important role in revealing the grade of brain astrocytomas.

Horizontal visual hemi-field stimulation on single eye in patients with hemianopia: a functional MRI study

Objective: To observe response of primary visual cortex to horizontal visual hemi-field stimulation on single eye in subjects with hemianopia and to discuss the probability of evaluating the activation of visual cortex with functional MRI(fMRI). Materials and Methods: Thirteen patients with pituitary adenoma and fifteen normal volunteers as control were recruited in this study. Block-designed fMRI was performed with 1.5T MR scanner. Black and white check board(horizontal unilateral 40 degree visual field) and “+” were used as stimulus and control respectively. Bilateral eyes received nasal and temporal side stimulus individually. The fMRI data were processed with SPM 2. Average mappings of subjects were obtained. The relationship between visual field deficits and activation in visual cortex was analyzed for patients group. Results: Ten patients and 10 normal subjects were selected for group analysis after head motion effect was ruled out. In normal subjects, the contralateral occipital lobe showed activation to temporal visual field stimulation and ipsilateral occipital lobe showed activation to nasal visual field stimulation. In patients group, bilateral visual cortex showed no activation to temporal visual field stimulation and ipsilateral occipital lobe showed activation to nasal visual field stimulation. Conclusion: There was a corresponding relationship between visual field and activation of visual cortex in normal subjects. Patients with hemianopia mainly presented with function decrease in contralateral visual cortex. fMRI is an effective method for studying affection of optic chiasm abnormality on visual cortex.

The diagnosis of congenital pulmonary vein stenosis by Doppler echocardiography

Objective: To evaluate the diagnostic value for congenital pulmonary vein stenosis(PVS) by Doppler echocardiography. Methods: The echocardiography of 15 patients with PVS confirmed by catheterization, MRI or surgical operation were reviewed retrospectivey. Results: Among 15 PVS patients, six had right pulmonary vein stenosis, in whom 4 patients(66.7%) were diagnosed correctly by echocardiography, 2 patients(33.3%) were missed. Four patients had left pulmonary vein stenosis, all patients were diagnosed correctly. Five patients had bilateral pulmonary vein stenosis, four patients(80%) were diagnosed correctly and one patient with left pulmonary vein stenosis was missed. Four patients(26.7%) were simplex PVS, eleven patients(73.3%) were found to accompany with other cardiovascular anomalies. The most common associated anomalies were atrial septal defect, ventricular septal defect, patent ductus arteriosus, complete atrioventricular septal defect, double outlet of right ventricle. Conclusion: The reflux of pulmonary vein could be demonstrated and PVS could be diagnosed accurately by Doppler echocardiography.

Value of ADC and EDC at 3.0T in predicting hemorrhagic transformation after acute experimental cerebral infarction

Objective: To investigate the value of diffusion weighted imaging(DWI) in predicting the hemorrhagic transformation(HT) after acute experimental cerebral infarction. Methods: Autologous thrombus was injected into unilateral internal carotid arteries of 75 rabbits to set up acute cerebral infarction model. These rabbits underwent DWI with 3.0T MR equipment at 3, 6, 12, 24, 48 and 72h after arterial occlusion. Apparent diffusion coefficient(ADC) and exponent diffusion coefficient(EDC) map were calculated at work station by using software of function. According to the method of European cooperative acute stroke study(ECASS), the HT of infarcted animals were classified into non-HT and HT groups, the latter was further divided into hemorrhagic infarction(HI) group and parenchymal hematoma(PH) group. The difference of ADC and EDC between non-HT group and various HT groups were assessed by using SPSS 11.5 software. Results: Cerebral ischemic model was successfully set up in 59 rabbits(78.67%). No pathologic findings of HT was found in 21 animals(35.59%, 21/59). Thirty-eight rabbits(64.41%, 38/59) who had HT included 31 rabbits(52.54%, 31/59) with HI and 7(11.87%, 7/59) with PH. The ADC value of non-HT groups and HT group were (0.601±0.128)×10-3~(0.813±0.060)×10-3 and (0.432±0.091)×10-3~(0.618±0.060)×10-3 respectively. There was obvious difference of ADC value between the two groups from 3h to 72h after occlusion(t=2.417~5.986, P=0.000~0.031). Similarly, significant difference of EDC value was found between the non-HT group[(0.415±0.039)~(0.502±0.009)] and HT rabbits[(0.559±0.040)~(0.583±0.064)](t=4.002~6.995, P=0.000~0.002). However, no obvious difference of ADC value was detected between HI group[(0.435±0.091)×10-3~(0.631±0.045)×10-3] and PH group[(0.390±0.180)×10-3~(0.586±0.104)×10-3](t=0.057~1.996, P=0.171~0.956) at every time point, except at 24h(t=4.664, P=0.002). There was no prominent difference of EDC value between HI[(0.545±0.039)~(0.638±0.042)] and PH groups[(0.575±0.054)~(0.649±0.019)](t=0.051~1.698, P=0.150~0.960). Conclusion: There is correlation between the decrease of ADC, the increase of EDC and the occurrence of HT in acute cerebral infarction. The abnormalities of ADC and EDC can probably predict HT. But DWI may play little role in the discrimination between HI and PH.

Correlative study between quantitative parameters of contrast-enhanced ultrasound and microvessel density in hepatocellular carcinoma

Objective: To explore the correlation between quantitation parameters of blood perfusion with contrast-enhanced ultrasound(CEUS) and microvessel density(MVD) in hepatocellular carcinoma(HCC). Methods: CEUS was examined before operation in 46 patients with HCC. Time-intensity curves(TIC) in 46 cases with HCC were analyzed to get the time to peak(TTP) and peak intensity(PI) of HCC and tissue around HCC. These parameters were analyzed correlatively with MVD counted with immunocytochemistry. Result: The mean TTP of HCC and tissue around HCC were (14.24±5.11)s and (25.34±9.11)s, the mean PI of HCC and tissue around HCC were (43.11±10.25)dB and (20.87±8.21)dB, the mean MVD of HCC and tissue around HCC were 52.32±18.21 and 21.43±8.65. The TTP before operation correlated inversely to MVD (r=-0.75, P<0.05) of histological section after operation. The PI before operation was correlated positively to MVD (r=0.73, P<0.05) of histological section after operation. Conclusions: The quantitative parameters of blood perfusion, TTP and PI, with calculated from CEUS was correlated to MVD. These parameters could be a potential indicator of angiogenesis of HCC before operation.

The clinical and CT diagnosis of lymphangioleiomyomatosis

Objective: To investigate CT imaging features of lymphangioleiomyomatosis. Materials and Methods: CT imaging data of 6 patients diagnosed as lymphangioleiomyomatosis were studied retrospectively including imaging of both chest and abdomen. Results: Multiple thin-walled cysts of different size were seen throughout the lung fields in an even distribution with normal intervening lung parenchyma in all the patients. Pneumothorax coexisted in 3 cases. In 1 case, pleural effusion was detected which was proved as chylothorax. Lymphadenopathy could be seen in 1 case. Three cases of kidney angioleiomyolipoma was found in abdominal CT imaging, among which lesions were multiple and bilateral in 2 cases. In these 3 cases, liver angioleiomyolipoma was found in one case and retroperitoneal angioleiomyolipoma was found in another case. Lymphangioleiomyoma of uterine broad ligament and retroperitoneum was found in 1 case. CT showed retroperitoneal lymphadenopathy in 1 case. Conclusion: lymphangioleiomyomatosis is a disease with characteristic imaging features, which is of great help for its diagnosis.

Analysis of CT misdiagnosis of renal mass(report of 21 cases)

Objective: To analyse the CT findings, misdiagnostic reasons of renal masses and to investigate the value of CT imaging in qualitative diagnosis of these diseases. Methods: Twenty-one cases of renal masses, proved surgically and pathologically, were analyzed retrospectively. Results: Thirteen cases were malignant tumors including 11 cases diagnosed as other malignant tumors and 2 cases diagnosed as benign tumors. Five cases were benign tumors diagnosed as renal cell carcinoma. Three cases were infection disease including 1 case renal tuberculosis diagnosed as angiomyolipoma and 2 cases of renal abscess diagnosed as renal cell carcinoma. Conclusion: Our study comfirmed that CT imaging is still useful in the diagnosis of renal masses. It is still difficult to determine some atypical renal masses. To avoid misdiagnosis, frozen biopsy or percutaneous biopsy may be helpful, functional parameters of lesions with different imaging modalities may be helpful in identifying these diseases in the future.

Differential diagnosis of skeletal tuberculosis and malignant skeletal disease with bone scintigraphy based on pathology

Objective: To search the specific features that can differentiate malignant skeletal disease and skeletal tuberculosis on bone scintigraphy and compare with pathology. Materials and Methods: For 1817 patients with pain of bone underwent bone scan. Fifty-six cases of skeletal malignant disease and skeletal tuberculosis were confirmed by pathology. Patients were divided into 3 groups according to the results of pathology: skeletal tuberculosis; bone metastasis and primary malignant bone tumors. Results: Bone metastasis and primary malignant bone tumors multiple lesions were more than solitary lesions. In bone tuberculosis 73% had joint involvement. The foci of bone metastasis bone disease were found in axial skeleton or extremities or both simultaneously, but skeletal tuberculosis were found in axial skeleton only in this study. Adjacent bone involvement was found in 90% of skeletal tuberculosis and in metastatic bone tumor, were distributed diversely. Seventy percent of bone tuberculosis absorbed imaging agent uniformly. The foci was irregular in 82% of metastatic bone tumors and in 94% of primary malignant bone tumors. Conclusion: Multiple lesions in malignant bone diseases could be seen diversely in axial skeleton and extremity bone. Foci were irregular and radioactive uptake not uniform in most malignant bone diseases. Most foci of bone tuberculosis distributed in axial bones, and they were regular and uniform as well as after accompanied by joint involvement.
