153 Localization of epileptogenic foci caused by hippocampal sclerosis using MSI

Objective: To evaluate the value of magnetic source imaging(MSI) in localization of the epileptogenic foci in temporal lobe epilepsy(TLE) caused by hippocampal sclerosis(HS). Methods: Sixteen patients with HS were evaluated with magnetoencephalography(MEG) before surgical excision, whose epileptogenic foci were located by MSI. Results: In the follow-up according to Nanjing Military Hospital, 14 patients(87.5%) received satisfying curative effect; 1 patient(6.25%) was partially relieved and 1 patient(6.25%) got poor effect, with a total effective rate of 93%. The concordance of MSI and Video-EEG in localization of epileptogenic foci was 62.5%. Conclusion: MSI could be a new valuable noninvasive functional neuroimaging technique for identification and delineation of epileptogenic foci in presurgical evaluation.

156 Study on relationship between volume of pituitary gland and changes of hormone levels after delivery

Objective: To explore the changes of morphology of pituitary gland and hormone levels after normal delivery. Methods: Fifty volunteers underwent MRI examination and clinical biochemistry analysis on the 7th day, in the 3rd, 6th and 9th month after natural delivery respectively. 3D-SPGR with 3D T1WI sequence was employed in the scanning of pituitary. We measured the volume and signal intensity of gland in each period of time in the image workstation. To determine the hormone levels, the radioimmunoassay method was adopted. Results: The mean volume of the pituitary was (548±44)mm3 on the 7th day, (526±42)mm3 in the 3rd month, (518±41)mm3 in the 6th month, and (516±41)mm3 in the 9th month after delivery. Comparing the signal intensity of anterior lobe of the pituitary with the pon, there were significant differences in their signal contrast between 7 days and 3 months, as well as 3 and 6 months after delivery. There was no significant difference between 6 and 9 months. Serum level of PRL was the highest on the 7th day and decreased to normal after 6 months; levels of TSH and β-HCG reached the highest on the 7th day and returned to normal after 3 months; the level of E2 was normal after 7 days. Conclusion: The volume of pituitary reduces gradually after delivery. Signal intensity of the anterior lobe declines gradually, and there is a certain correlation between the morphologic changes and levels of hormones.

160 Synchronous application of MRA and dynamic contrast-enhancement inone station examination of hepatic tumor with 3.0T MR

Objective: To investigate the value of synchronous application of MR angiography(MRA) and dynamic contrast-enhancement in one station scanning of hepatic tumor with 3.0T MR for clinical diagnosis and surgical decision. Methods: Forty-one cases, including 10 normal volunteers and 31 patients with malignant and benign hepatic lesions, were scanned with multi-phase dynamic contrast-enhanced 3D MR imaging adopting 3D fast spoiled gradient recalled(3D FSPGR) and array spatial sensitivity encoding technique (ASSET) and zero-filling interpolation(ZIP). The image data was reconstructed with maximum intensity projection(MIP) on the ADW 4.2 workstation. The signal intensity of hepatic solid lesions was measured and signal intensity-time curve was derived. Results: Common hepatic artery, proper hepatic artery, right and left hepatic arteries, artery branches of grade 4 and above were clearly showed in 10 volunteers, so it was with the portal vein trunk and branches of grade 4 to 6. The diagnosis of 31 patients, including 23 cases of hepatocellular carcinoma and 8 cases of hemangioma, were made correctly with multi-phase dynamic contrast-enhanced 3D MR imaging. Destruction and deformity of hepatic arteries caused by tumor invasion, cancerous embolus of portal vein, and formation of collateral circulation were demonstrated clearly, too. Conclusion: The diagnosis of hepatic tumor and the display of hepatic vessels can be finished synchronously in multi-phase dynamic contrast-enhanced MR scanning with 3D FSPGR, which is of great value for clinical diagnosis and surgical decision.

164 Analysis of CT findings in bronchioloalveolar carcinoma

Objective: To develop the diagnostic level of bronchioloalveolar carcinoma(BAC) by analyzing the clinical features and CT presentations of BAC. Materials and Methods: All the cases of BAC comfirmed by operation and pathology were retrospectively analyzed from June 2002 to June 2007. These cases were review using the 2004 revised WHO classification of BAC and separated them into two groups, pure BAC and mixed subtype adenocarcinoma with BAC components. The CT presentations of pure BAC were analyzed. Evaluated findings included the number(solitary or multiple), size, morphous, contours, density and gas-containing space of lesions. Other CT findings such as air bronchogram, vascular converging and lymphadenopathy were also assessed. Results: 145 surgically resected cases with thin-section CT images were histologically diagnosed as BAC from 2002 to 2007. Of these, 32(22.1%) proved to be pure BAC using the 2004 revised WHO classification. There were 20 men(62.5%) and 12 women(37.5%). The positive rate of tumor markers CEA and CA125 was 10% and 5%, respectively. The mean diameter of BAC was less than 3cm and all of the cases presented pure ground glass opacity. 82.9% of them showed inhomogeneous density, 97.1% well-defined margin. The rate of lobulated shape, air bronchogram and gas-containing space were 62.9%, 60%, 57.1%, respectively and lymphadenopathy in none of these BAC. Conclusion: In the diagnosis of early stage of BAC, some morphologic features of lesions on thin slice CT may be more important.

167 Clinical study of stereoscopic echocardiography in assessment of atrial septal defects

Objective: To evaluate the clinical value of stereoscopic echocardiography(Stereo Echo) in the diagnosis of secundum atrial septal defect(ASD). Methods: Twelve patients with secundum ASD were examined using real time 3D echocardiography. Stereo Echo was generated with a high-performance volume renderer with stereoscopic glasses. Stereo Echo was compared with 3D echocardiography with standard display for assessment of the shape, size, and the surrounding tissues of ASD. Results: Stereo Echo could accurately measure the diameter of ASD, there was no significant difference between stereoscopic vision display and standard display[(2.89±0.73)cm vs (2.85±0.72)cm, P>0.05]. The appearance rate of coronary sinus for Stereo Echo is higher compared with the standard display(100% vs 93.3%). The mean time of the IVC, SVC for Stereo Echo was slightly shorter than those of the standard display[(10.33±3.59)s vs (11.03±3.76)s, P>0.05]. Conclusions: Stereo Echo improved the visualization of 3D echocardiography ultrasound images, potentially improving the safety of the surgical interventions.

170 Value of endoluminal convex array probe in detecting internal carotid arterial disease

Objective: To investigate the value of endoluminal convex array probe in detecting internal carotid arterial(ICA) disease. Methods: 146 ICA of 73 cases were examined with endoluminal convex array probe and high-frequency linear probe respectively to collect following data: visible length, color Doppler appearance and peak systolic velocity(PSV). PSVICA/PSVCCA ratio was calculated in 18 cases of unilateral ICA stenosis diagnosed with DSA to compare the accuracy of evaluating stenosis with two different probes. Result: Visible length and PSV from high-frequency linear probe are (2.05±0.35)cm and (89.8±20.9)cm/s and those from endoluminal convex array probe are (4.41±0.32)cm and (116.8±35.1)cm/s, the difference between them is significant(P<0.05). Satisfied grey and color Doppler images can be obtained with endoluminal convex array probe in 41 ICA which were difficult to be examined with high-frequency linear probe. Thirty-one ICA were detected twisting in “V” or “S” appearance with endoluminal convex array probe, while only 19 ICA were detected in abnormal appearance with high-frequency linear probe. For 18 ICA with 70%~99% stenosis diagnosed by DSA, the accuracy of diagnosis is 72.2% with high-frequency linear probe, but 94.4% with endoluminal convex array probe. Conclusion: Endoluminal convex array probe is an effective complement for ICA inspection with high-frequency linear probe because visible length of ICA could be significantly increased with endoluminal convex array probe which is valuable for detecting pathologic changes in middle or distal parts of ICA.

174 Preliminary study of PWI in rabbit models of hepatic VX2 tumors after high intensity focused ultrasound

Objective: To discuss the basic characteristic of perfusion-weighted imaging(PWI) in rabbit models of hepatic VX2 tumors after treatment by high intensity focused ultrasound(HIFU). Methods: VX2 tumor in tissue block were implanted into the liver of 20 new Zealand rabbits to make VX2 liver tumor model. After routine scans with 1.5T MR scanner, PWI was performed in 14 days before and after tumor implantation in all animals by using EPI-SE series and SENSE technology. Choose the ROI of rabbit liver VX2 such as central district, marginal area and paratumor parenchyma. Blood flow degrees were measured and analyzed statistically. Maximal signal reduction slope(SRSmax) of the signal intensity versus time curves were created as quantitative variable. Analyze the change of SRSmax in different ROI. Results: There were difference in SRSmax between the central district and the marginal area(P<0.05). No difference in SRSmax in paratumor parenchyma before and after HIFU(P>0.05). Conclusion: It is feasible to study VX2 of rabbit liver perfusion imaging by 1.5T MRI scanner. PWI can be used to evaluate the situation of the liver tumor angiogenesis and angioruin. The findings of VX2 carcinomas on PWI are matched with the levels and scopes of necrosis.

178 Research on BMD measurement of femoral neck with no-external reference phantom CAD based on QCT data

Objective: To study the values of bone mineral density(BMD) measurement of femoral neck with and without external reference phantom computer-aided detection(CAD) based on QCT data, and DXA, in order to accurately and conveniently study BMD and set up large sample BMD database. Materials and Methods: Volumetric CT scanning(64-slice CT, Philips) of femoral neck were initially performed with external reference phantom(approved by Ethics Committee in our hospital) in 53 health-volunteers(10 men, 43 women, 38~70 years, mean 57 years), scanning ranging from L5 to femoral neck, and then did DXA examination. Doing BMD measurement of femoral neck without external reference phantom with OsteoCAD1.2 software. Medial vastus muscle(MVM) and subcutaneous fat, lateral vastus muscle(LVM) and subcutaneous fat, erector spinae(ES) and subcutaneous fat respectively were determined as internal references. Linear Regression correlation analysis was made by SPSS 10. Results: The correlation of integral, trabecular, cortical BMD measured with and without external reference phantom CAD, DXA were at a high level of significance. The correlation coefficients of integral, cortical, trabecular BMD selected MVM as internal references and that with external reference phantom were 0.956, 0.904, 0.974 respectively. The correlation coefficients of integral, cortical, trabecular BMD selected LVM as internal references and that with external reference phantom were 0.943, 0.870, 0.967 respectively. The correlation coefficients of integral, cortical, trabecular BMD selected ES as internal references and that with external reference phantom were 0.955, 0.886, 0.976 respectively. The correlation coefficients of DXA and trabecular BMD without external reference phantom in MVM, LVM, ES as internal references were 0.911, 0.868, 0.906 respectively. Conclusion: BMD measurement of femoral neck without external reference phantom based on QCT data is an effective and feasible method, both MVM and LVM selected as internal reference is more convenient and has no difference comparing with erector spinae as internal reference.

184 Accuracy of PET/CT on lymph nodes staging in non-small cell lung cancer—a Meta-analysis

Objective: To estimate the diagnostic accuracy of PET/CT for lymph nodes staging in patient with non-small-cell lung cancer using a Meta-analysis with the gold standard of pathological results. Materials and Methods: Search in the studies in English literature in PubMed and Chinese CBM database between 1999 and 2007. Criteria for inclusion and exclusion were according to the criteria of diagnostic research published by Cochrane center. Characteristics of the included studies were evaluated. Subsequently pooled weighted sensitivity, specificity, positive predictive value, negative predictive value were analyzed, and the corresponding 95%CI were calculated. Summary receiver operating characteristic(SROC) curve was performed and the area under the curve(AUC) was calculated. Finally, a sensitivity analysis was performed. Results: Altogether 13 articles that met the standard were included. The pooled weighted sensitivity was 84% respectively and the pooled weighted specificity was 87%. The area under SROC curve was 90.84%. Conclusion: PET/CT is an accurate noninvasive imaging test for staging of lymph nodes in patient with non-small cell lung cancer, with high sensitivity and specificity.
