1 MR features of atypical meningiomas

Objective: To summarize the MR imaging features of atypical meningiomas. Materials and Methods: There were 11 cases with atypical meningiomas proved by pathology, both plain and post-contrast MRI were performed in all the cases. Four cases had diffusion weighted imaging(DWI). The MRI manifestations including location, size, shape and margin of tumors, the signal intensity of lesions, the peritumor edema, dural tail sign, blood vessel flow void sign and signal intensity of DWI were analyzed. Results: Most of the lesions located in the cerebral convexity with average diameter larger than 5cm, and showed inhomogeneous signal intensity before and after contrast administration with irregular shape and blurred margin. In the lesion there were multiple flow void blood vessels and on DWI the tumor showed high and slightly high signal intensity. Two cases had invasion of sinus sagittalis superior, adjacent skull and soft tissue respectively. Peritumor edema and dural tail sign are not specific signs in the diagnosis of atypical meningiomas. Conclusion: When a tumor located in the common sites of meningioma with a larger size and atypical features including irregular shape, blurred margin between cerebral parenchyma and tumor, inhomogeneous signal intensity before and after contrast administration, blood vessel flow void sign in tumor and/or invasion of adjacent tissue, the diagnosis of atypical meningiomas should be considered.

5 Magnetic susceptibility weighted imaging of FLASH-T2*WI sequence in diagnosing cerebral cavernous malformation: a comparison with TSE-T2WI

Objective: To evaluate the value and accuracy of magnetic susceptibility weighted imaging(MSWI) of FLASH-T2*WI sequence in the diagnosis of cerebral cavernous malformation(CCM) by comparing with TSE-T2WI. Methods: Twenty-seven patients performed with routine MR examination and dual echo FLASH-T2*WI(TE=15, 35ms) were analyzed retrospectively, all cases were confirmed by pathology or other methods. Results: ①All lesions on SE-T1WI and TSE-T2WI were depicted iso-, hyper-, hypo- or heterogeneous signal intensity, 99 lesions were detected by TSE-T2WI, and magnetic susceptibility artifact(MSA) was found in 84 lesions, 63(75%) of them were depicted as “hoop sign” and 21(25%) were completely hypo-intense; 234 and 423 lesions were found orderly by dual echo FLASH-T2*WI(TE=15, 35ms), MSA was present in all focus, 14 lesions were described as “hoop sign” and 220(94.02%), 409(96.69%) were absolutely hypo-intense, there were great difference of image signs, size and contrast to noise ratio(CNR) of MSA between the three weighted imagings. ②Seventeen lesions of 19 patients with “hoop sign” or MSA were confirmed as thrombus, hemosiderin or gliosis by pathology. Conclusion: “Hoop sign” or MSA was one of the important signs for CCM, MSWI of FLASH-T2*WI sequence is helpful in the diagnosis of CCM, and FLASH with long TE is better than short TE to show small lesions.

9 The application of 64 multidetector CT angiography in the evaluation of proximal vertebral artery stenosis

Objective: To evaluate the diagnostic value of 64 multidetector CT angiography(CTA) for proximal vertebral artery stenosis compared with digital subtraction angiography(DSA). Methods: All patients with interpretable CTA and DSA of the proximal vertebral arteries from Dec. 2005 to Apr. 2008 were included in this study. This yieled a total of 796 vessels. Two senior neuroradiologists graded the CTA and DSA images referring to the distal vertebral artery, according to the North American Symptomatic Carotid Endarterectomy Trial guidelines. Comparison of CTA with DSA in detecting the soft plaque of vertebral artery. Statistical significance was determined using χ2 test. Results: Referring to the DSA, the overall sensitivity, specificity, positive predictive value and negative predictive value to diagnose proximal vertebral artery stenosis by 64 MSCT were 100%, 97.6%, 93.9% and 100% respectively, and were 100%, 97.3%, 82.1%, 100% respectively when proximal vertebral artery with stenosis≥50%, and were 92.0%, 100%, 100%, 99.5% respectively when proximal vertebral artery had central stenosis<50%, and were 92.0%, 100%, 100%, 99.5% respectively when proximal vertebral artery had eccentric stenosis<50%. There was statistically significant difference in detecting soft plaque between CTA and DSA(P<0.05). Conclusions: CTA has a high sensitivity, high specificity, high negative predictive value for proximal vertebral artery stenosis, and has a higher accuracy in evaluating soft plaque and proximal vertebral artery eccentric stenosis than DSA. CTA appears to be an excellent evaluating test for proximal vertebral artery stenosis.

13 Functional MR study of task-induced deactivations in schizophrenic patients

Objective: Task-induced deactivations(TIDs) of cortical regions may reflect elements of a “default-mode” of brain function associated with self-directed mental processes at rest. TID phenomenon in schizophrenia and its relevance to patients’ emotional state and task performance were examined. Materials and Methods: Fifteen schizophrenic patients and matched control subjects were studied in a fMRI design of 2-back working memory task. Positive and negative affect scale scores were calculated for every individuals. Results: Relative to control subjects, patients showed significantly prolonged reaction time, lower TID of the medial prefrontal cortex and posterior medial cortex, and higher negative emotional scores. The magnitude of the TID for medial prefrontal cortex was associated negatively with patients’ reaction time and negative emotional scale. Conclusion: Shift efficiency of attention resource between cognitive and emotional state is impaired, which is probably one of the neural mechanism of cognitive function impairment for schizophrenic patients.

17 Assessment of left ventricular long-axis systolic function in patients with essential hypertension using real-time triplane quantitative tissue velocity imaging

Objective: To evaluate the alteration of left ventricular long-axis systolic function during left ventricular geometric remodeling in essential hypertension(EH) using real-time triplane quantitative tissue velocity imaging and to investigate its clinical significance. Methods: Totally 101 patients with EH and 31 healthy volunteers were enrolled. Systolic peak velocity(Vs) was measured at basal and middle segments in different left ventricular walls using real-time triplane quantitative tissue velocity imaging. Results: Based on Ganau, patients with EH were divided into 4 groups: normal geometry(32 cases), concentric remodeling(22 cases), concentric hypertrophy(28 cases) and eccentric hypertrophy(19 cases). Vs in each hypertensive groups were significantly lower than in control groups(P<0.05, 0.01). There was a significantly step-down change among normal geometry, concentric remodeling, concentric hypertrophy and eccentric hypertrophy groups in Vs(P<0.05, 0.01). Conclusions: Real-time triplane quantitative tissue velocity imaging can assess left ventricular long-axis systolic function correctly in patients with EH and its alteration during left ventricular geometric remodeling.

20 Liver fibrosis: diffusion-weighted MR imaging and correlation analysis with blood biochemiology

Objective: To evaluate the diagnostic efficacy of diffusion-weighted MR imaging for liver fibrosis. To investigate the correlation between apparent diffusion coefficient(ADC) value and serum markers of liver fibrosis. Methods: Thirty-eight Wistar rats developed experimental model of liver fibrosis by carbon tetrachloride(CCl4) hypodermic injected and ten rats as control group were performed MR scans and MR-DWI. Grouping according to different stage of fibrosis, the results of DWI signal intensity(SI) and ADC were compared with different b values(333, 666 and 1000s/mm2). Blood samples were obtained after MR examination to observe serum markers of liver fibrosis including hyaluronic acid(HA), procollagen type Ⅲ(PC Ⅲ), type Ⅳ collagen(C-Ⅳ) and laminin(LN). The differences of results at various stages of liver fibrosis were compared. Analysis of relationships among serum markers and stage of liver fibrosis were performed. Results: Grouping according to different stage of fibrosis, S0(n=3), S1(n=5), S2(n=9), S3(n=13), S4(n=8). When b values equal to 333, 666 and 1000s/mm2 respectively, SI increased and ADC decreased gradually as the stage of fibrosis progressed(P<0.01). There was a good relationship between ADC and stage of fibrosis, especially when b value was 1000s/mm2. When b value equal to 333 and 666s/mm2, S0~S2 can be distinguished from S3 and S4 stage, and S3 can be distinguished from S4 stage according to SI and ADC values. Moreover b value equal to 1000s/mm2, S0~S2 can be distinguished from S4 stage. There was excellent negative correlation between ADC and serum indices(P<0.05). Conclusions: DWI would be a useful criterion for detecting liver fibrosis on S3 and S4 stage, but having limitation to detect early fibrosis(S0~S2 stage). Excellent correlation between ADC value and serum indices reflects the characteristic pathologic state of abnormal synthesis and deposition of extracellular matrix.

26 Design and application of external fiducial frame in pelvic and abdominal SPECT/CT image fusion

Objective: To design pelvic and abdominal external fiducial marker frame for SPECT/CT rigid image registration and to complete image fusion. Methods: Ⅳ-shaped external marker frame was designed. Then the frame, combining the interactive general software that was based on external marker, which was empoldered by ourselves, was used to register and fuse validating the external marker frame and the recurrent lesions SPECT/CT transaction of patients with colorectal cancer. Result: The Ⅳ-shaped external marker frame could be easily fixed to the body, and the relative position of external marker point maintains in the same station during the different scanning procedure by SPECT/CT. With combination of marker frame and the image fusion software based on that, the recurrent lesions of colorectal cancer SPECT/CT image fusion was accomplished on a low cost PC. Conclusion: The Ⅳ-shaped external marker frame suited to the image fusion software of pelvic and abdominal SPECT/CT we designed could be easily fixed to the body, which was qualified to multi-modal transaction image fusion.

29 The practical value of transcatheter closure in the interventional treatment of congenital heart diseases under transthoracic echocardiographic guidance

Objective: To explore the practical value of transthoracic echocardiography(TTE) guiding transcatheter closure in the interventional treatment of congenital heart diseases including atrial septal defect(ASD), ventricular septal defect(VSD), patent ductus arteriosus(PDA). Methods: After transthoracic echocardiography, 33 patients with congenital heart diseases (including 16 cases of ASD, 8 cases of VSD, 9 cases of PDA) underwent Amplatzer occlusion operation. Echocardiography was used before, during, immediately after the operation and one month, six months later not only to diagnose and monitor the condition of the patients but also to access the clinical effect and to follow up patients after the operation. Results: Thirty-two patients were treated successfully, failure in one patient with ASD. No shunt was found after the occlusion operation. The effect of the occlusion was proved to be good by rechecking the condition after the treatment(the pulmonary artery pressure of 14 patients with PAH was ameliorated, the tanspulmonary valve pressure gradient of one ASD patient with PAS was lowered down, for those patients with enlargement of ventricular cavity, recovery of ventricular chamber size was made one month, six months after the operation. Slight shunt still existed in aortic valve of a VSD case after the treatment). Conclusions: The transthoracic echocardiography is very important in selecting the cases for operation, guiding the device Amplatzer during operation and evaluating the efficiency after operation.

32 Application of multi-slice CT in the diagnosis of esophageal atresia and tracheoesophageal fistula in neonate

Objective: To study the value of multi-slice CT(MSCT) in the diagnosis of esophageal atresia and tracheoesophageal fistula(EA-TEF) in neonate. Methods: Twelve patients suspected to have EA-TEF were examined by MSCT. Multi-planar reformation(MPR), minimum intensity projection(MinIP), volume-rendered image(VR), and virtual endoscope(VE) were reconstructed at workstation. Two experienced radiologists overviewed the images and were related measurements. Results: There were 11 cases of type Ⅲ EA-TEF(including 1 case of type Ⅲa, 10 cases of type Ⅲb) and 1 case of type Ⅳ. Six patients had 6 types of congenital malformations. Conclusions: MSCT and reconstruction images has great value in diagnosing, typing, searching fistula of EA-TEF disease and could evaluate lung condition and related malformations.

36 CT findings of myocardial bridge and mural coronary artery on dual-source CT coronary angiography

Objective: To investigate the depiction rate and morphologic features of myocardial bridge and mural coronary artery(MB-MCA) with dual-source CT(DSCT) coronary angiography. Materials and Methods: Four hundred and seventy cases of patients underwent DSCT coronary angiography. The length and thickness of MB, the degree of systolic compression of MCA were measured expressed with the means±standard deviations. The situation of the MCA and the nearby arteries of MB-MCA were observed and recorded. Statistical analysis were performed with Pearson correlation analysis and the Wilcoxon two-sample test. Results: ①MB-MCA was detected with DSCT in 131(27.9%) of 470 patients. Mean MB length and depth at DSCT were (20.8±9.0)mm and (2.3±1.1)mm, respectively. The degree of systolic compression was (37.8±16.9)%. A significant correlation was not found between systolic compression and length(r=0.17, P=0.540) but was found with depth(r=0.60, P=0.000). A significant correlation was not found between depth and length(r=0.18, P=0.101). ②62.7%(82 cases) of the arteries proximal or/and distal to the MB-MCA were tortuous. A significant correlation was found between tortuous degree and the depth of MB(z=-3.15, P=0.002). ③The MCA had cone-shaped change in systole and significantly reduced or disappeared in diastole in 67 cases. These features had significant correlation with depth and length of MB(z=-2.91, P=0.004; z=-2.61, P=0.009). ④Arteriosclerotic signs in 46 cases were found in neighboring arteries of the MCA. The depth and the length of MB were related to the arteriosclerosis(z=-3.42, P=0.001; z=-2.89, P=0.004). Conclusion: DSCT may diagnose correctly the presence and may judge the morphological characteristic of MB-MCA, which can provide more worthy information for clinical diagnosis and treatment.

41 Value of the PROSET sequence in the detection of lumbosacral nerve root anomalies: compare with the MRM sequence

Objective: To explore the application value and findings of MR myelogram(MRM) sequence and PROSET sequence in the detection of lumbosacral nerve root anomalies(LSNRA). Materials and Methods: Conventional MRI, 3D MRM and 3D PROSET sequences were performed in lumbosacral nerve roots of 108 cases with low back pain. 3D maximum intensity projection(3D-MIP) reconstruction was used in PROSET and MRM sequences. The lumbosacral nerve roots were observed in the raw images and 3D-MIP images. Results: In the 105 cases with high-quality images, LSNRA were found in the 7, 14 cases with MRM and PROSET sequence. The detection rate was 6.67% and 13.33% respectively. There were statistically significant differences between MRM and PROSET sequence(χ2=5.143, P=0.016). LSNRA were found in only 3 cases with the conventional MRI sequence. Among the 14 cases with variation, there were 10 cases of type Ⅱ(71.43%), which was composed of 4 cases of type Ⅱ-1(28.57%) and each of the type Ⅱ-2, Ⅱ-3 and Ⅱ-4 had 2 cases(14.29% each). There were 2 cases of type Ⅲ, 2 cases of type Ⅵ(14.29% each). Conclusion: PROSET sequence was superior to the MRM sequence in the detection of extradural LSNRA. So it is a good method for the diagnosis of extradural LSNRA.

46 Improved 18F-FLT synthesizing yield by BOC-precursor

Objective: By the study of the process of synthesizing the tracer of tumor cellular proliferation which was 3’-deoxy-3’-18F-flurorothymidine(18F-FLT), using BAThy-FLT-precursor and BOC-FLT-precursor, demonstrated that the BOC-FLT -precursor played an important role in improving the synthesis yield of 18F-FLT. Method: We cooperated with seven PET centers doing the research on synthesizing the radioactive tracer(18F-FLT) following the standardized protocols of the national multicenter clinical trial. Cyclotron(MINItrace, GE Healthcare) and fully automated synthesis system(TRACERlab FX F-N, GE Healthcare). The (5’-O-Benzoyl-2, 3’-anhydrothymidine) and {3-N-t-Butoxycarbonyl-[5’-O-(4,4’-dimethoxytriphenylmethyl)- 2’-deoxy-3’-the product of 18O(p,n) 18F, and then hydrolyzed. Product were purified by the HPLC, getting the 18F-FLT, radiochemical purity>95%. Results: Using BAThy-FLT-precursor to synthesize 18F-FLT, the mean synthesis yield is 10.1%. While using BOC-FLT-precursor to synthesize 18F-FLT, the mean synthesis yield is 23.2%. Conclusion: We found that BOC-FLT-precursor was easier to be marked by 18F-. The mean product yield was 130% more than BAThy-FLT-precursor. We also studied on the effect of some factors on product yield of 18F-FLT, including the ratio of alkali to precursor, the solvent of precursor, the hydrolyze method and operated method. As the collaborator in the domestic distributed in the different areas, it eliminated the regional difference on the product yield. The results of the research is a good guide.
