229 MRI diagnosis of the pediatric fourth ventricular tumors

Objective: To discuss the MRI diagnostic value of the fourth ventricular tumors in children. Materials and Methods: Eighteen cases of pediatric fourth ventricular tumors proved by surgery and pathology were reported. The routine and post-contrast images were obtained by a 1.5T MR scanner. Results: There were 7 medulloblastomas, 7 astrocytomas, 3 ependymomas, and 1 schwannoma. Most tumors demonstrated as low signal intensity on T1WI and high signal intensity on T2WI. Medulloblastomas originated from cerebellar vermis. Six of them had small cysts and one of them had bleeding within the tumor. They all enhanced strongly. Five astrocytomas originated from cerebellar vermis and one from brain stem. Most of them had big cysts with semi-rim like circular enhancement or rim-like circular enhancement. Enhanced nodules can also be seen. The poor differentiated ones were irregular in shape with more nodules. Two ependymomas originated from fourth ventricle with small cysts inside. One ependymoma originated from cerebellar vermis with big cyst formation. One schwannoma originated from fourth ventricle. The upper part enhanced strongly and lower part was a big cyst without enhancement. Conclusion: MR examination is very useful in the assessment of pediatric fourth ventricular tumors.

232 Clinical value of the different distribution types of high metabolic hilar and mediastinal lymph nodes in the aged individuals

Objective: To evaluate the clinical value of the distribution types of the hilar and mediastinal high metabolic lymph nodes in the aged by PET/CT image. Materials and Methods: Sixty-one patients ≥60 years old underwent FDG PET/CT images. According to the distribution of the hilar and mediastinal high metabolic lymph nodes(HML), all patients were divided into 4 groups: patients without HML as group A, with bilateral HML as group B, with HML at the same side of the lung nodule or mass as group C, other patients as group D. Results: Of the 61 patients, 35(57.4%) patients had HML, 7 patients and 54 patients with and without hilar and mediastinal metastatic lymph nodes respectively. In group A, B, C and D, there were 0, 3, 4 and 0 patients with hilar and mediastinal metastatic lymph nodes respectively, and there were 26, 13, 2 and 13 patients without hilar and mediastinal metastatic lymph nodes respectively, P<0.001. Conclusion: The inccident rate of hilar and mediastinal high metabolic lymph nodes is high. The distribution types of HML is useful to find the malignant HML from the benign ones.

235 Applications of cardiac MRI after repair of congenital heart disease

Objective: To evaluate the value of MRI in the follow-up of patients with repaired congenital heart disease. Methods: Ninty-six cases with repaired congenital heart disease(mean age 5.71 years; age range 5 days to 24 years) were evaluated by cardiac MRI including FIESTA, double IR, phase-constrast cine imaging and CE-MRA. Thirty-two of the 96 cases underwent corrected repair and 64 had palliated operation. Results: Of the 96 cases, 21 residual excardiac anomalies and anastomosis narrowing, which was missed or not clearly visualized by echocardiography, was detected by MRI. Dilated azygos vein with retrograde flow was found in 5 of 23 cases after bidirectional Glenn operation. In 2 of 5 cases with Fontan operation, hemdynamics measured by PC-MR showed the similar physiology as that of post-Glenn surgery. Varied amount of pulmonary regurgitation occurred in all of the 5 cases with corrected TOF repair. Of the 7 cases after repaired TAPVC, 3 cases with residual pulmonary vein stenosis was found. Decreased systolic velocity and reversed flow during diastolic phase in the ipsilateral branch pulmonary artery with branch pulmonary vein stenosis was measured by PC-MRI in 1 case. Conclusion: MRI can provide high resolution, high contrast, and anatomical images in freely selectable 2D planes. Anatomical images, combined with the functional information provided by cine, velocity-mapping techniques make MRI ideally suited to the evaluation of congenital heart disease in patients after operation.

239 Findings of MSCT in Castleman’s disease of the abdomen

Objective: To analyse the most suggestive multi-slice CT(MSCT) imaging findings of Castleman’s disease, and to improve the recognition of this disease. Methods: MSCT imaging materials of nine patients with Castleman’s disease were retrospectively reviewed, diagnosis of Castleman’s disease was done by surgery and pathology. Results: Nine lesions were located in the abdomen and pelvis(n=5) and the retroperitoneal zone(n=4) respectively. The lesions were localized in 8 cases and disseminated in 1 case. Seven cases were hyaline vascular type of Castleman’s disease, on plain CT images, the lesions manifested as homogeneous soft masses, which was isoattenuating to muscle. On dynamic contrast CT scan, most of the masses show marked enhancement at arterial phase and delayed scan and the attenuation was similar to that of aorta. The pathologic subtype of 2 localized and disseminated cases was plasma cell type. Imaging findings showed several well-defined nodules lied in the retroperitoneal zone and enhanced lightly. Conclusion: CT findings of Castleman’s disease are closely associated with its pathyology. For localized type(hyaline vascular type) of Castleman’s disease have typical MSCT signs. For disseminated type(plasma cell type) carries no specific CT signs, its verification is mainly based on pathologic examination.

243 Analysis of 1H-MRS changes in temporal lobe epilepsy

Objective: To explore the 1H-MRS change of temporal lobe epilepsy and its value in lateralizion of temporal lobe epilepsy(TLE). Methods: To analyze the NAA/Cr, NAA/(Cr+Cho), Cho/Cr, mI/Cr of mesial temporal lobe in 147 TLE patients. Results: NAA of the involved side in TLE patients were significantly lower than that of the contralateral side and the control group(P<0.05). There were no statistical difference between TLE group and control group in the ratio of NAA to Cho and NAA to mI. The bilateral asymmetry of NAA in unilateral group was more significant than bilateral TLE. The positive lateralization rate of NAA reduction were 70.45% and 50% in the hippocampus sclerosis(HS) and the cryptogenic TLE respectively. The concordance between 1H-MRS(NAA) and EEG were 66% and 50% respectively in these two groups. There are no significant difference between the two methods. Conclusions: 1H-MRS(NAA) may provide TLE lateralizion information, while Cho and mI may not provide this information.

247 Application study of 16-slice spiral CT in diagnosing atherosclerotic ulcer of the aorta

Objective: To investigate 16-slice spiral CT in diagnosing atherosclerotic ulcer of the aorta. Methods: Fifteen patients with atherosclerotic ulcer of the aorta, who underwent 16-slice spiral CT, were studied retrospectively. Of the 15 patients, 14 patients had acute aortic syndromes, 1 patient with other chronic diseases. More attention was put on the size, shape, depth, number and adjacent structure of the atherosclerotic ulcer. Results: Twelve patients had 1 lesion, 3 patients had multiple lesions. Fourteen patients with acute aortic syndromes had fairly long intramural hematoma and 10 patients with middle pleural, mediastinal, pericardial effusion; however 1 patient with other chronic diseases had local pseudoaneurysm without intramural hematoma and effusion. Conclusion: 16-slice spiral CT is of high value in diagnosing atherosclerotic ulcer of the aorta and may be the first choice diagnostic device for emergency.

250 Diagnostic value of MSCT and EUS in gastrointestinal mesenchymal tumors

Objective: To investigate the manifestation of gastrointestinal mesenchymal tumors(GIMTs) in 16-row spiral CT and EUS, and the diagnostic value of MSCT and EUS in GIMTs. Methods: Thirty-seven patients with pathologically proved GIMTs were analyzed retrospectively. Plain scan in 37 cases, two-phase dynamic enhanced spiral CT in 35 cases and EUS in 17 cases were made before surgery. Results: Among those there were 31 GISTs(19 in the stomach and 1, 6, 1, 4 in the esophagus, duodenum, colon and outsides gastrointestinal tract respectively), leiomyoma 4 cases(3 in the esophagus and 1 in the stomach), stomach schwannoglioma 2 cases. ①The important CT features were as follow: tumors with abundant vascularity in submucosa inclined to exophytic, showed necrosis, cystic degeneration, hemorrhage and calcifications, with no lymphnode metastasis. The benign and potential malignant GISTs displayed homogeneous enhancement with well-defined borders. The diameter of most tumor was 1.0~3.0cm(mean 2.16cm) in benign GIMTs and 1.2~3.5cm(mean 2.24cm) in potential malignant GIMTs. The malignant GIMTs exhibited inhomogeneous enhancement and invasion to adjacent organs. The diameter varied from 3.5cm to 16.0cm(mean 6.9cm) in malignant GIMTs. Accuracy of CT diagnosis for qualitative analysis was 20/37(54.05%) and for location was 30/37(81.08%) of GIMTs. ②Endoscopic and EUS characteristics: endoscopically, GIMTs had the following appearances: submucosal protuberance. They all showed low echo images originated from the muscularis propria or muscularis mucosa on EUS. Benign tumors had homogeneous internal echoes with smooth margin, and the diameter varied from 0.5cm to 3.5cm(mean 1.75cm). The malignant lesions showed heterogeneous internal echoes, with or without fluid low echo, irregular border, and infiltration of the mesentery. The diameter ranged from 6.0cm to 10cm(mean 9.2cm). The accuracy of diagnosis for qualitative analysis was 15/17(88.24%) and for location was 16/17(94.12%) of GIMTs. Conclusion: MSCT and EUS were the two most valuable imaging modalities in assessing GIMTs and they could provide useful informations for exactly showing the layer of origin, location, tumor diameter, shape, border characteristics, internal echo patterns and invasion of GIMTs, these were useful to differentiate benign from malignancy, to direct management and to predict prognosis in patients with GIMTs.

255 Correlative study of VEGF, bFGF and angiogenesis with MSCT multi-phase scanfeatures of benign prostatic hyperplasia

Objective: To investigate the correlation of the expression of vascular endothelial growth factor(VEGF), basic fibroblast growth factor(bFGF) and angiogenesis with MSCT features of benign prostate hyperplasia(BPH). Materials and Methods: Thirty-five BPH cases proved by histopathology were examined with multi-phase contrast-enhanced MSCT scanning. Each pathologic specimen was stained with hemetoxylin and eosin and immunohistochemical technique to detect the level of MVD, VEGF, bFGF and related microvascular indexes. The CT features and the expression of MVD, VEGF, bFGF were analyzed to investigate the relationship with each other. Result: The peak value of 71.43%(25/35) BPH was seen in delayed phase(180s), 28.57%(10/35) in venous phase(75s). The positive rate of VEGF and bFGF was 31.43%(11/35) and 14.29%(5/35) respectively. There was significant positive correlation between VEGF, bFGF and MVD(P<0.01), and between MAV and MVD, VEGF, bFGF(P<0.01), between VEGF and bFGF(P<0.01), and negative correlation of MAV with average diameter, perimeter and area. There was no correlation between MAV and heterotypic index(P>0.01). Conclusion: The expression of bFGF, VEGF and MVD in BPH was different. The MSCT multi-phase scan features of BPH have significant correlation with the expression of VEGF, bFGF and angiogenesis.

259 Application of CE-MRA in radiofrequency catheter ablation of atrial fibrillation

Objective: To evaluate the clinical application of MRI, particularly the contrast enhanced magnetic resonance angiography(CE-MRA) in the treatment of atrial fibrillation by radiofrequency catheter ablation. Methods: The MRI materials of 23 patients(16 men, 7 women; age from 38 to 75 years; mean age 60.4 years) with atrial fibrillation underwent radiofrequency ablation were analyzed retrospectively. The demonstration of pulmonary veins and left atrium, the variation degree of the pulmonary veins were depicted. Results: All MRI images can be used to demonstrate the left atrium and pulmonary veins clearly. The majority of patients(n=19, 82%) had two ostia on both sides of the left atrium(classical type). Four patients(18%) had variations of pulmonary veins. Among them 3(13%) had one common ostium on the left side of the left atrium. One(4%) had three veins on the left side. Of the 3 patients with a common ostium on the left side, 1 patient had three veins on the right side. The demonstration rate of pulmonary ostium by CE-MRA(100%) is higher than that of Double-IR(61.5%) and FIESTA(71.4%). Conclusion: MRI and CE-MRA is the most useful imaging modality to demonstrate the pulmonary veins. It can provide a valuable road map and precise data of pulmonary veins in the treatment of atrial fibrillation by radiofrequency catheter ablation.

262 Analysis of arterial triple phase enhancement characteristic in hepatocellular cancer with 16 slices spiral CT

Objective: To discuss hepatocellular carcinoma enhancement characteristic and value of clinical application of arterial triple phase enhancement. Methods: Twenty-seven patients with hepatocellular carcinoma who had diagnosed clearly, they were all scanned by early arterial, middle arterial and late arterial phase, scanning time was 18s(early arterial), 24s(middle arterial), 30s(late arterial) after the contrast agent was injected. Results: Fourteen cases of hepatocellular carcinoma were enhancement and 13 cases of hepatocellular carcinoma were not enhanced in early arterial phase; which 9 cases had blood vessel-like enhancement and 5 cases had whole tumor parenchymal enhancement. Twenty-four cases of hepatocellular carcinoma had enhancement and 3 cases of hepatocellular carcinoma had no enhancement in middle arterial phase, which 16 cases had blood vessel-like enhancement and 8 cases had whole tumor of parenchymal enhancement. All cases were enhanced in late arterial phase with different degree which 4 cases had blood vessel-like enhancement and 23 cases had whole tumor parenchymal enhancement. Conclusion: Triple arterial phase enhancement can reflect enhancement characteristic of hepatocellular carcinoma, tumor vessels are well enhanced in middle arterial phase and whole tumor enhancement is good in late arterial phase.

265 Study on transhepatic crossed papillary biliary metallic stent placement in malignant biliary strictures(analysis of 32 cases)

Objective: To evaluate the efficacy of transhepatic crossed papillary biliary metallic stent placement in malignant extrahepatic biliary strictures. Methods: Thirty-two patients of malignant extrahepatic biliary strictures confirmed by CT and MRCP, including 26 men and 6 women, the average age was 56.6 years. Among the 32 patients, 12 cases were of carcinoma of head of pancreas, 5 cases of carcinoma of stomach lymphonode metastasis, 1 case of carcinoma of bladder lymphonode metastasis, 6 cases of periampullary carcinoma, 7 cases of common bile duct carcinoma, 1 case of duodenal papilla carcinoma. Thirty-two patients were treated with percutaneous transhepatic cholangiography(PTC), and 32 biliary metallic stents were implanted by crossed papillary methods. Results: The success rate was 100%, with bile ducts patency. There were no hemobilia, leakage, and rupture of bile tract. The TBIL(total bilirubin) dropped from (205.7±16.2)mg/dl to (35.5±6.7)mg/dl after operation. One case of acute pancreatitis, 2 cases of biliary tract infection were encountered 12hrs and 30 days after the procedure respectively, and these patients were discharged after symptomatic treatment. The follow-up periods were 1~26 months(average 15.4 months), 6 patients had biliary tract in stent(5 patients were treated with percutaneous tranhepatic drainage or stent implantation, 1 patient died), 6 patients died because of carcinoma metastasis, 2 patients had duodenal ileus. Conclusion: The crossed papillary transhepatic biliary metallic stent placement in malignant extrahepatic biliary is a safe and effective method.

269 Value of CT angiography of mesenteric vasculature in diagnosing the etiology of small bowel ischemia

Objective: To investigate the value of CT angiography(CTA) of mesenteric vasculature in diagnosing small bowel ischemia. Methods: CTA findings of mesenteric vasculature in 22 patients with small bowel ischemia were reviewed retrospectively. Twelve patients were classified as acute small bowel ischemia and other 10 as chronic. Analysis included presence or absence of stenosis, dilation, intraluminal filling defect, convergence, and displacement of mesenteric vasculature. Results: Of the 12 patients with acute small bowel ischemia, abrupt termination of superior mesenteric artery(SMA) was seen in 2 with SMA embolus, intraluminal filling defect seen in 5 with SMA thrombosis and 3 with mesenteric and portal venous thrombosis, formation of true and false lumen in 2 cases with dissection of SMA. Of the 10 patients with chronic small bowel ischemia, stenosis of SMA was seen in 4 of the 6 cases with atherosclerotic mesenteric ischemia, and occlusion of SMA complicated with Riolan’s arch seen in the other 2, dilation of proximal SMA in 2 cases of aneurysm of SMA, dilation and comblike pattern seen in 2 cases with systemic lupus erythematosis. Conclusion: Etiology of small bowel ischemia can be confirmed with CTA of mesenteric vasculature.

272 Clinical application of 64 MSCT in the diagnosis of tetralogy of Fallot

Objective: To evaluate the role of 64-slice CT in the diagnosis of tetralogy of Fallot and accompanied anomalies. Methods: Twenty-one patients diagnosed as tetralogy of Fallot by 64-slice CT were analysed retrospectively. Toshiba Aquilion 64 was used, slice thickness 0.5mm, overlap 0.3mm. Multi-planar reconstruction(MPR), curved-planar reconstruction(CPR), volume render(VR), maximum intensity projection(MIP) and minimum intensity projection(MinIP) were performed in all patients. Meanwhile, ultrasound echocardiography(UCG) was performed on all patients. Fourteen patients were confirmed by operation, 7 by angiocardiography(ACG). Results: Twenty-one patients were diagnosed by 64-slice CT accurately: pulmonary stenosis, ventricular septal defect, overriding of the aorta and hypertrophy of right ventrical were seen. In the 21 patients, 6 patients had patent ductus arteriosus, 4 right aotic arch, 2 double aortic arch, 7 left superior vena cava, 3 aberrant left brachiocephalic vein, 1 single coronary artery, 3 major aorta-pulmonary collateral arteries. Conclusion: 64-slice CT with 3D reconstruction not only show the anatomical structure of the heart, but also show the coronary and pulmonary arteries. 64-slice CT was an important noninvasive method in the diagnosis of tetralogy of Fallot.

276 Acute rupture of achilles tendon diagnosed by ultrasonography

Objective: To evaluate the value of ultrasonography in diagnosing achilles tendon rupture. Methods: Ultrasonographic findings in 32 patients with operatively proved acute achilles tendon rupture were analyzed. The manifestations of ultrasonography and operation were compared. Results: The achilles tendon rupture was diagnosed on ultrasonography in 32 cases. Achilles tendon complete rupture was showed in 27 cases, partial rupture in 5 cases. Ultrasonographic appearance of achilles tendon rupture displayed complete or partial discontinuation of the fibrous bundle. The operation has the same result as that of ultrasonography. Conclusion: Ultrasonography can accurately show the degree and portion of acute achilles tendon rupture, which is very helpful for treatment.

278 Applicable value of spiral CT in facets involvement of intra-articular calcaneal fractures

Objective: To investigate the characteristics of calcaneal fractures with spiral CT multiplanar volume reconstruction(MPVR), surface shaded display(SSD), and to determine the effectiveness of MPVR and SSD in the diagnosis of calcaneal fractures. Materials and Methods: Patients suspected or diagnosed with calcaneal fracture were imaged by conventional scans with SCT, then 47 fractures diagnosed with intra-articular calcaneal fracture were further imaged with the techniques of MPVR and SSD to classify the fractures and to show the condition of involvement of joints. Results: Tuberosity slice showed the most involvement of articular facets in all cases and had difference in showing posterior facets with axial slice. Coronal slice was superior to show the medial and posterior facets while involvment of the calcaneo-cuboid facets were defined most clearly in axial and tuberosity slice. The severity of articular incongruity and rotational displacement of the posterior facet fragments were not well appreciated in coronal CT images of 11 fractures while could be very well showed in sagital construction. About 60% anterior facets were not visualised. The 3D reconstruction offered the spatial allocation of the individual slices, and well evaluated preoperatively the degree of joint injury and depression. Conclusion: Sprial CT 3D and MPVR have important value in the diagnosis of calcanenl fracture, 3D and MPVR play an important role in the preoperative classification of calcaneal fractures, and are useful for the selection of surgical approaches, and for the evaluation of prognosis.
