Volume change of the mesial temporal structure and the relationship between the volume variety and clinical factors in temporal lobe epilepsy

Objective: To explore the volume change of hippocampus, amygdala and entorhinal and the relationship between the volume change of the mesial temporal structure and clinical factors in temporal lobe epilepsy(TLE). Methods: To measure the volume of hippocampus, amygdala and entorhinal of 117 TLE patients with MRI and explore the relationship between the volume change and clinical factors. Results: The ipsilateral volume of hippocampus in unilateral TLE were significantly lower than the contralateral side. In TLE with hippocampal sclerosis the volume of the ipsilateral entorhinal cortex were significantly lower than the contralateral. The volume of amygdala were not significantly different. The degree of hippocampus, amygdala volume reduction was not related with the frequency of simple partial seizures, age of seizure onset, duration of seizure and history of febrile convulsions. The degree of entorhinal cortex reduction was related with the frequency of simple partial seizures. Conclusions: The vulnerability of mesial temporal structure were different.

Research on carotid arterial elastic function change in diabetes patients using pulse wave velocity

Objective: To explore the value of carotid pulse wave velocity(CPWV) in evaluating the arterial elastic function changes in diabetes patients. Methods: The carotid arteries of both 123 diabetes patients who were divided into hypertensive group(45 cases) and normotensive group(78 cases) and 50 subjects as normal control group were examined with TDI and M-mode ultrasound. The electro-mechanical time(EMT), pulse wave time(PWT), and the systolic and diastolic diameters were recorded. The CPWV, the stiffness index and compliance were calculated. Results: In comparison with the control group, there were longer CPWV in each diabetic group(P<0.001). CPWV correlated with age stiffness index and compliance. Conclusions: CPWV can be used to early reflect the arterial function changes in diabetes patients, and it may be used to evaluate the elastic function as a new index.

Useful value of dynamic enhanced CT in differential diagnosis of solitary pulmonary nodules

Objective: To evaluate the value of dynamic enhanced CT in differential diagnosis of solitary pulmonary nodules. Material and Methods: Forty-two cases of SPN collected in this study all accepted dynamic enhanced CT scanning. The enhancement pattern included no enhancement, ring enhancement, homogeneous and heterogeneous enhancement. The CT values of SPNs on pre- and post-enhanced images were measured. The dynamic curves for different kinds of lesions were described, respectively. Results: Eighteen cases of lung cancer demonstrated homogeneous enhancement, 2 cases of whom showed spot vascular enhancement, 7 cases heterogeneous enhancement. Eight cases of inflammatory nodules(8/10) showed homogeneous enhancement, 2 cases ring enhancement. Five cases of tuberculosis(5/7) demonstrated no enhancement, 2 cases rim enhancement. Peak heights of lung carcinoma and inflammatory nodules were significantly higher than that of tuberculosis(P<0.001; P<0.01). The dynamic curve of malignant SPNs showed a slowly progress, rapidly progress and then decreased slowly; Inflammatory nodules showed a rapidly increase, slowly decrease and then increase again; The curve for tuberculosis was a very low and smooth pattern. The durations of reaching peak height of malignant and inflammatory nodules were 39s and 36s after injection of contrast media, respectively. Conclusions: Dynamic enhanced CT can supply much more information about dynamic enhancement of SPNs. It possesses the important value in improving the qualitative diagnostic accuracy.

Evaluation of ultrasound diagnosis in biliary atresia

Objective: To summarize and analyse the ultrasonographic findings of 20 infants with biliary atresia(BA), and to evaluate its diagnostic value. Methods: The color Doppler ultrasonography(US) was performed in 20 infants with BA. The size and echo of liver and spleen, fasting gallbladder and its dynamic change after meal, fibrotic mass in hepatic hilum were observed. Results: The fibrotic mass in hepatic hilum was significantly present in 10 of the 20 infants with BA. Three had small fibrotic mass, 2 had no gallbladder, 1 infant’s gallbladder had no lumen, 15 had small gallbladder without dynamic change after meal, 2 had the normal size and shape of gallbladder but no contraction after meal. All infants had different degree of hepatomegaly, 9 had small amount of fluid in abdominal cavity, 14 had splenomegaly. Conclusions: The fibrotic mass in hepatic hilum is an objective and specific criterion for diagnosing BA with US. The little gallbladder, especially no contraction function of gallbladder was valuable for diagnosis and differential diagnosis of BA. Hepatomegaly and coarsening of liver parenchyma echogenicity was positively relative with hepatic fibrosis, which may suggest the course and prognosis of BA.

Application of

F-FDG PET/CT in patients with biopsy proven tumor metastases from unknown primary site

Objective: To evaluate the role of whole body PET/CT scan with (18)F-fluorodeoxyglucose(FDG) in the detection of the primary tumor in patients with metastatic cancer from unknown primary origin. Methods: Fifty-four consecutive patients, with biopsy proven tumor metastases from unknown primary site, underwent a whole-body FDG-PET/CT study. Results: The primary tumor site was correctly identified by FDG-PET/CT in 41 patients(41/54, 75.9%). In 13 patients(13/54, 24.1%), the primary tumor site was not localized. However, in 35 cases(35/54, 64.8%) of them, FDG-PET/CT scan identified further unexpected metastases, modifying the stage of disease. Conclusion: Our data strongly support that the whole body FDG-PET/CT is a useful tool in the evaluation of patients with cancer with unknown primary site and PET/CT is able to find more metastatic lesions.

Abnormalities with negative 18F-FDG uptake detected using non-enhanced CT of PET/CT in non-small cell lung cancer

Objective: To evaluate the clinical value of abnormalities that do not show increased 18F-FDG uptake on non-enhanced CT of integrated PET/CT in patients with non-small cell lung cancer. Materials and Methods: The study consisted of a retrospective analysis of consecutive PET/CT scans obtained at our PET center between May and December 2006. During the eight-month period, there were 119 patients had a diagnosis of non-small cell lung cancer. All scans were obtained with integrated PET/CT scanner(Siemens Biograph Sensation 16). Each case was interpreted independently by two experienced PET/CT physicians. All abnormalities with negative 18F-FDG uptake were recorded. The abnormalities were classified as being of major, moderate or minor significance according to the definitions previously by Ford. Results: There were 119 patients with non-small cell lung cancer underwent initial staging(66/119, 55.5%), restaging or treatment monitoring(53/119, 44.5%) PET/CT imaging during the study period. The abnormalities were located in the head and neck(n=40), thorax(n=97), abdomen and pelvis(n=128), and bony skeleton(n=18). The clinical importance of these abnormalities was classified as major n=9(3%), moderate n=121(42%), or minor n=153(54%). Major abnormalities noted in the study included 4 cases of small lung nodules, 1 case of osteolytic lesion and 1 case of brain lesion suggestive of metastasis, as well as 1 case of liver mass and 2 cases of renal mass. Conclusions: CT scan in PET/CT can not be only a tool for attenuation correction and anatomic landmarks alone. Even low-dose CT can provide important diagnostic information. There is a high prevalence of CT abnormalities in non-small cell lung cancer that do not show 18F-FDG uptake, but small proportion of which has clinical significance. Diagnostic CT should be done to get more significant information in PET/CT interpretation.

Quantitative evaluation of left ventricular myocardial systolic synchrony in normal subjects by real-time three-dimensional echocardiography and tissue velocity imaging

Objective: To evaluate left ventricular myocardial systolic synchrony in normal subjects by real-time three-dimensional echocardiography(RT-3DE) and tissue velocity imaging(TVI). Methods: Twenty-seven normal subjects were selected randomly. The time to the point of peak velocity(Tv) during systolic phase, to the point of minimal volume(Tmv) and cardiac cycle were obtained by the on-line Qlab soft ware. Then the Rst was calculated. Results: There was no significant difference in Rst among the 12 segments of left ventricle in normal subjects by RT-3DE and TVI(P>0.05). Significant correlation existed between the Rst obtained by RT-3DE and the one calculated by TVI in normal subjects(r=0.814). Conclusion: Both RT-3DE and TVI can evaluate left ventricular myocardial systolic synchrony in normal subjects noninvasively and quantitatively, but RT-3DE can provide a novel, quick and convenient technique.

Characteristics of MDCT enhancement scan in T staging of gastric cancer

Objective: To analyze the characteristics of multi-detector spiral computed tomography(MDCT) enhancement scan in gastric cancer T staging. Methods: Thirty-seven gastric carcinoma patients were scanned by MDCT. After plain scans, patients were performed enhancement scans by arterial phase and portal venous phase. We reconstructed the stomach to three-dimension analysis, and combining the source images, we evaluate the scope and extent of the gastric carcinoma. Compared the CT scans with operations and pathological sections, to analyze the T staging characteristics. Results: The enhancement scopes in portal venous phase scan was much bigger than arterial phase scan in all patients. The slightly enhancement appeared both in portal venous phase and arterial phase scans and had the same enhancement scopes in T1 stage patients. In T2 stage, most patients enhanced in the way of slightly arterial phase and obviously portal venous phase enhancement, and enhancement scopes in portal venous phase scans was much bigger. In T3 stage, more than half of patients’ enhancement way was same as T2 stage, but enhancement scopes in two phases scans was the same. In T4 stage, most patients were obviously enhanced in all phases. Conclusion: Combining the reconstruction technique, MDCT double-phrase scans can evaluate the scope and extent of the adenocarcinoma invasion, and be of some use in T staging of gastric carcinoma.

Objective: To evaluate the diagnostic value of 18F-FDG PET/CT for the patients with ascites of unknown causes. Methods: Twenty-two patients with ascites of unknown cause refered to our department for whole-body 18F-FDG PET/CT. The causes of their ascites could not be confirmed by ultrasound, endoscope and/or radiographic imaging, CT and/or MRI, and diagnostic paracentesis for biochemical content and cytology. The images were interpreted by visual inspection and semi-quantitative analysis(standardized uptake value, SUV). Results: According to the results of follow-up and pathology, 14 patients were confirmed as malignant ascites, 4 ascitic patients were caused by other reasons, the causes of ascites were still unknown in 4 patients. In the 14 patients of malignant ascites, in 7 patients primary focus were found by 18F-FDG PET/CT and 7 patients were false negative. 10/22 cases(45.5%) were correctly diagnosed. Conclusions: 18F-FDG PET/CT was a more sensitive and effective method in finding the causes of ascites compared with traditional imaging methods.

Application of delayed enhancement MSCTU scanning and image processing in congenital urinary obstruction lesions in children

Objective: To evaluate the value of delayed enhancement MSCTU scanning and image processing in the diagnosis of congenital urinary obstructive lesions in children. Methods: Delayed enhancement MSCTU scanning, image processing and ultrasonography were performed in 108 patients with congenital urinary obstruction lesions before operation, all cases were confirmed by operation. The cortical-phase images and the parenchymal-phase images were reconstructed with multiplanar reconstruction(MPR), the excretory-phase images were reconstructed with maximum intensity projection(MIP), curved planar reconstruction(CPR) and shade-surface displayment(SSD). The results of the two methods(MSCTU and ultrasonography) were then compared. Results: Ureteric pelvic junction obstruction(UPJO) of the left kidney is 76.19% in boys, and it is obviously higher than other urinary anomalies; the morbidity of hydro-duplex kidneys with hydro-duplex ureter is 84.00%, and most of the patients are girls. Renal function of the diseased kidneys are better in obstruction at ureteropelvic distalis, secretory function of the diseased kidneys exist in severe UPJO, and excretory function is disturbed; renal function of the diseased kidneys is the worst in severe renal hypoplsia and hydro-duplex kidneys. The accordance rate of MSCTU and ultrasonography with operation is 96.3%, 60.04%, respectively. MPR is a facile technique and it is for viewing function and construction of kidneys; CPR, MIP and SSD are for observing whole shape of collecting system, evaluating excretory function of kidneys, defining the site of obstruction and making qualitative diagnosis. Conclusion: Grasping best low-dose delayed enhancement MSCTU scanning time and adopting reasonable methods of image processing can diagnose congenital complex urinary obstruction lesions accurately in children, and it could be selected as one stop choice in diagnosing urinary lesions before operation.

Endometrial contractility during the menstrual cycle in the patients with infertility

Objective: To evaluate the changes of the endometrial contractility during the menstrual cycle in patients with infertility. Methods: Real-time transvaginal ultrasound and advanced audio-visual and computer technology were used to evaluate the endometrial contraction throughout menstrual cycles in 84 patients with infertility. Movement type, frequency and speed were evaluated. Results: The percentage of scans with endometrial movements and non-movement and obscurity was 82.74%, 15.18%, 2.08% in the 336 examinations of 84 patients. The detection rate of endometrial movements was increased during the menstrual cycle(P<0.05). Seven activity patterns were distinguished, which varied throughout the cycle(P<0.05). The frequency and speed of the endometrial movement were increased during the follicular phase and decreased after ovulation(P<0.05). Conclusions: Transvaginal ultrasound and advanced audio-visual and computer technology is effective and practicable in evaluation the endometrial movement. The movement of endometrial varied in accordance with the menstrual cycle.

Aorto-iliac and lower extremity arteries: contrast-enhanced MR angiographyin healthy volunteers with automated table movement

Objective: To determine the optimal biphasic bolus rate based on automated table movement for aorto-iliac and lower extremity arteries of contrast-enhanced magnetic resonance angiography(CE MRA). Materials and Methods: Fifteen healthy volunteers were given CE MRA examinations in aorto-iliac and lower extremity arteries in three station technique. A 1.5T scanner with a 3D SPGRE was used; the whole body coil was selected for the entire study. Total 30~40ml gadopentetate dimeglumine was administered automatically by a power injector. A biphasic bolus protocol was designed, the first half of the contrast agent volume was injected at a rate of 2.0ml/s, and the second half was injected at a rate of 1.0ml/s in group 1(n=5), 0.8ml/s in group 2(n=5), 0.5ml/s in group 3(n=5), then was flushed with 20ml of normal saline at a rate of 2.0ml/s. For automated table movement and bolus-chase MR angiography, commercially available software was used. The qualitative and quantitative data of image quality were analyzed using student t test analysis. Statistical significance was established at P<0.05. Results: The entire arterial tree from the aorto-iliac arteries to the dorsal calf arteries was displayed. All MR angiographic examinations yielded good results with diagnostic image quality. Signal noise ratio(SNR) and contrast noise ratio(CNR) were calculated on the basis of signal intensity measurements in region of interest. There were no significant differences for SNR and CNR values among all the groups(P>0.05), but the highest SNR and CNR values were found in group 3. The venous enhancement in the calf and thigh was most obvious in group 1, partially seen in group 2, and few showed in group 3. There were statistical differences between venous enhancement scores in group 3 and those in group 1 and group 2(P<0.05). Conclusion: These findings suggest that with the biphasic bolus rate of 2.0ml/s followed by 0.5ml/s, optimal image quality is acquired and venous enhancement is minimized.

Clinical application of multislice spiral CT angiography in lower extremity arterial diseases

Objective: To evaluate the clinical application of multislice spiral CT angiography(MSCTA) in diagnosing arterial diseases of the lower limb. Materials and Methods: MSCTA was performed in 43 cases suspected of arterial diseases of the lower limb. The spiral scans cover renal artery to the artery of the lower extremity, length of scanning 1000~1200mm, slice thickness 2.5mm, pitch 1.35~1.675, slice increment 1.25mm, the volume of contrast agent 100~120ml, flow rate 3.0~3.5ml/s. The method of reconstruction images included MIP, VR and MPR. Conventional digital subtraction angiography(DSA) was performed in 7 patients within 2 weeks before and after performance of MSCTA. The data of DSA and MSCTA in 130 segments with stenosis and occlusion were calculated. The sensitivity and specificity of MSCTA were calculated using DSA as the standard. Results: Forty-three cases suspected to have arterial diseases of lower limb were demonstrated successfully with MSCTA, and the quality of reconstruction images is satisfactory. Using DSA as the standard reference, the sensitivity, specificity, positive predictive value and negative predictive value of MSCTA were 96.7%, 95.7%, 95.2%, 97.1%, respectively in diagnosing stenosis and occlusion of lower limb artery. Kappa statistical analysis was used to compare the results between MSCTA and DSA(K=0.91). Conclusion: As a non-invasive, high accurate, fast-acquisition, high space resolution, low-complication technique for vascular imaging, MSCTA has a higher sensitivity, specificity and clinical application value in diagnosing arterial diseases of the lower extremities and can replace DSA to a certain extent, MSCTA can become a primary method in diagnosing arterial diseases of the lower extremities.

Interstitial 125I seed implantation in the treatment for refractory malignant tumor under the guidance of MSCT

Objective: To evaluate the safety and efficacy of percutaneous MSCT guided interstitial 125I seed implantation treatment for refractory malignant tumor. Methods: Seventy-six lesions(diameter 2.5~12.0cm, mean 6.5cm) in 70 refractory malignant tumor patients underwent percutaneous MSCT guided intratumoral 125I seed implantation. Treatment plan system(TPS) was adopted based on CT imaging 14 days before the procedure. The distribution of 125I seed, complications and efficacy were observed immediately after and 2~8 months after the procedure. Results: The procedure of intratumoral 125I seed implantation in 76 malignant tumors was successful. Ten to ninety particles(mean 45) of 125I seeds were implanted into a single tumor. The distribution of seeds in 71 malignant tumors in 66 patients was satisfactory while the distribution of 125I seeds in 5 malignant tumors of the other 4 cases was not uniform, and the latter 5 malignant tumors of 4 cases were carried out supplementary 125I seed implantation in the tumor cool area 4~8 weeks after the procedure. The clinical CR and PR were achieved in 5 and 45 lesions respectively, with an overall response rate of 65.79%(50/76). Pneumothorax occurred in 4 cases with lung cancer. No acute complications and treatment related radiation damages occurred in the other cases. Conclusions: The MSCT guided percutaneous intratumoral 125I seed implantation treatment for refractory malignancies is a safe and effective method.
