Differential diagnosis of space occupying lesions in parotid gland with CT

Objective: To evaluate the value of CT in the diagnosis and differential diagnosis of parotid gland space occupied lesions. Methods: Fifty-one patients with proved parotid occupying lesions were analyzed retrospectively. All patients were scanned with plain CT and contrast CT. Results: There were 38 benign masses and 13 malignant lesions. Benign lesions demonstrated regular shape and clear margin. Among the 38 benign lesions pleomorphic adenomas(18/38) are most common, which were often found in the superficial lobe, with homogenous density; followed by adenolymphoma (10/38), which were often found in the posterior-inferior part of the parotid gland(9/10). There were 7 primary malignant lesions among the 13 malignant masses, which were often showed as irregular masses with unclear margin. The most common primary malignant lesions were mucoepidermoid carcinoma(5/7). Metastatic tumors often came from nasopharynx(5/6), which was easy to find enlarged lymph nodes and primary leisons. Conclusions: CT is sensitive in detecting and diagnosing the occupying lesions of the shallow lobe of parotid gland, it was not good enough to detect or diagnose the occupying lesions of the deep lobe and lesions with infection.

Study of 3D CEMRA technique of hepatic vessels

Objective: To investigate the clinical value of 3D CEMRA of hepatic vessels. Methods: 3D CEMRA was performed in 82 cases. When contrast reached descending aorta, 3D scan started. Arterial phase, portal venous phase, vena cava phase were acquired. The images were subtracted and reconstructed by MIP or MPR. The quality of image was evaluated. Results: With this technique high quality of images were obtained. Contrast volume and flow rate correlated with the time when bolus reached the target vessel. Contrast volume and flow rate have linear correlation with the signal of hepatic vessel. Conclusion: 3D CEMRA technique can grasp the exact time when contrast reached target vessel. Combined with optimal contrast volume and injection rate can improve the image quality obviously.

Ultrasonic characteristics and clinical feature of breast phyllode tumor

Objective: To study the ultrasonic characteristics and clinical features of the breast phyllode tumor(PTB). Methods: Clinical and imaging findings were retrospectively reviewed in 10 cases with 10 phyllode tumors identified at preoperative ultrasonography. Results: All patients were female, age ranged from 21 to 59 years old, each of them had a history ranged from 1 week to 5 years. Painless breast lump was the common presentation. On clinical examination, all lumps were firm and mobile. Ultrasonography display 10 lumps and the upper outer quadrant was the most commonly involved site(7/10). The ultrasonic characteristics of images were divided into three types, their shape including lobulated(n=6), round and oval(n=3), irregular in shape(n=1, as mixed echoes of cystic and solid). The tumor diameter varied between 1.5cm and 9.5cm. According to the histologic criteria of WHO, of all the cases, 6 were benign PTB, 2 cases were borderline and 2 cases were malignant PTB. Most of the borderline and malignant PTB showed blood flow grade II~III. Endotumoral calcifications and secondary sign of malignancy were absent. Conclusion: Although many of the phyllode tumors are indistinguishable from fibroadenoma on the basis of clinical and ultrasonic characteristics, in large breast tumors with rapid growth, and show typical ultrasonic characteristics, the tentative diagnosis of phyllodes tumors should be considered.

Evaluation of SPECT/CT scan for differential diagnosis of benign and malignant single spinal disease

Objective: To evaluate the value of SPECT/CT imaging in differentiating malignancy from benignancy in single spinal disease. Methods: There were 74 patients(male 34, female 40, mean age 55.2yrs) underwent bone scanning and SPECT/CT. The diagnosis was made by two doctors from Nuclear Medicine Department and one doctor from Radiologic Department based on interpretation of the nuclear imaging combined with CT. Results: Of the 74 lesions detected in whole body scan, 30 were malignant lesions and 44 were benign lesions. The malignancy or benignancy of the single spinal disease not only relate to the location, but also to the characteristic appearance of the lesion. Conclusions: SPECT/CT scan is clinically valuable in differentiating malignancy from benignancy in single spinal disease.

Study of China-made peripheral arterial sirolimus-heparin eluting stent and its pharmacokinetics: in vitro experiment

Objective: To evaluate the China-made sirolimus-heparin eluting stent’s physical performance, biocompatibility and pharmacokinetics in vitro. Methods: Coated PLGA-heparin complex to the sirolimus-coated stents with self-made printing equipment to gain the multiple membrane. Evaluated the physical and degrading efficiency, blood compatibility and pharmacokinetics in vitro of the stents coated with multiple membrane. Results: The China-made sirolimus-heparin stent had good surface performance and had good combining power with polymer. The test showed that degradation time of PLGA and PLGA with sirolimus membrane was 3 months; The blood compatibility test of the china-made stent show that platelet adhesion on PLGA membrane was reduced, and no obvious aggregation. The PLGA membrane with drugs had even less platelet adhesion. The clotting time of drug coating was prolonged and blood coagulation factors were activated in a low level. The pharmacokinetics test in vitro showed that the sirolimus slowed down within 3 months and the heparin slowed down within 50 days. Conclusion: The China-made sirolimus-heparin eluting stent had good biocompatibility, sirolimus and heparin could release slowly and could inhibit in-stent stenosis and thrombosis.

Initial exploration of assessing normal left ventricle segmental volumetric parameters by real time three dimensional echocardiography

Objective: To explore the characteristics of normal left ventricle(LV) segmental volumetric parameters using real-time three dimensional echocardiography(RT-3DE) combined with semi-automatic boundary detection method. Methods: Thirty-six healthy adults were studied. Using RT-3DE combined with semi-automatic boundary detection method, a series of segmental LV volumetric parameters were measured, which were segmental end diastolic volume(sEDV), segmental end systolic volume(sESV) and segmental stroke volume(sSV). Then all the parameters in different segments were compared respectively. Results: ①Segmental volume-time curves of normal LV are “U-shaped” and uniform, and the coordinates of curve peak and valley are nearby. ②Comparing between segments of corresponding LV wall, sEDV, sESV and sSV decrease from basal segments to apical segments. However, there are no statistical differences between middle segments and apical segments of most LV walls. ③Comparing groups along LV longitude, segmental volumes of basal-segment group are larger than those of mid-segment group and apical-segment group. However, there are no statistical differences between mid-segment group and apical-segment group. ④Comparing groups along LV circumference, segmental volumes of anterior, anteroseptal and lateral segment group are largest, those of posterior and septal segment group take 2nd place, and those of inferior segment group are smallest. Conclusions: RT-3DE can conveniently and accurately evaluate normal LV segmental volumetric parameters, and volumetric parameters of different LV segments are heterogeneity.

The imaging characteristics of 18F-FDG accumulated by brown adipose fat(report of six cases)

Objective: To analyze and discuss the characteristic pattern of 18F-FDG uptake in brown adipose tissue(BAT). Methods: All the 18F-FDG PET/CT scans between Nov. 2005 and Jan. 2007 were reviewed, and the patients with BAT 18F-FDG accumulation were selected according to criteria. Results: Six patients whose BAT accumulated 18F-FDG were qualified among 493 cases(6/493, 1.2%), one man and five women. The distribution of BAT was symmetrically situated in the neck, the supraclavicalar area, axillary area, paravertebral thoracic area. The SUVmax in BAT ranged from 1.4 to 9.2. Five cases(5/6, 83.3%) presented 18F-FDG accumulated in BAT in cold season. In one patient who underwent PET/CT scans twice, BAT was visualized once, but not for the other time. There was no distinct difference for the distribution and radioactive intensity of BAT between early scan and delay scan among six cases. The BMIs of 4 cases were in the lower level. Conclusion: There are multiform patterns of 18F-FDG accumulated in BAT, which should be familiar with and mastered by the PET diagnostician in order to decrease the rate of false positive diagnosis.

Value of CT in the diagnosis of exophthalmos

Objective: To study the etiologic factors, the value of CT diagnosis and differential diagnosis of exophthalmos. Method: The CT appearances and clinical features of 82 cases who had exophalmos confirmed by operation, pathology and follow-up of treatment were analyzed retrospectively. Result: This study included Graves disease 23 cases (hyperthyroidism 17 cases, normothyroidism 6 cases), inflammatory pseudotumor 20 cases, angioma 16 cases, metastatic tumor 3 cases, retinal glioblastoma 4 cases, cranial neurogliocytoma 5 cases, lymphangioma 3 cases, nasosinusitis 3 cases, carotid cavernous fistula 5 cases. Their CT appearances were exophalmos, tumor behind the eyeball and within the eyeball, thickening muscular apparatus. Conclusion: The CT scanning has important value for etiological diagnosis and displaying the circumsciption of pathological changes of exophalmos.

Value of 64-slice spiral CT postprocessing in the diagnosis of malignant tumors of long bone

Objective: To evaluate value of postprocessing of 64 slice MSCT reconstruction techniques in diagnosing malignant tumors of long bone. Methods: Reconstruction techniques were used after 64-slice CT imaging had been completed for 52 patients with long bone tumors. Based on the raw data, two kinds of image reconstruction, including multiple planar reconstruction(MPR) and volume rendering(VR) were carried out. Results: All reconstructed images showed bone tumor. MPR accurately displayed bony damage, changes in periosteum, extent of bony damage and tumor invasion to surrounding soft tissues; VR displayed changes in bone stereographically; VR on basis of enhancement, displayed supplying arteries, veins and neoplastic blood vessels. The two techniques combined with cross-sectional images were able to display bony damage, type of periosteal reaction, extent of tumor and invasion of tumor to surrounding tissues along different axes. Conclusion: The two kinds of 64-slice CT postprocessing images can fully and accurately demonstrate the malignant tumors of long bone, combined with the axial images correct evaluation of the malignant tumors of long bone can be made.

Imaging diagnosis of 13 cases of bone malignant lymphoma

Objective: To explore the imaging diagnosis of bone malignant lymphoma. Materials and Methods: Thirteen cases of bone malignant lymphoma verified by operation and pathology were analyzed retrospectively. Thirteen cases were examined by X-ray, 9 cases were also checked by CT and 2 cases examined by MRI. Results: In 12 cases of the primary bone malignant lymphoma, the lesion of 5 cases was in the mandible, clavicle, ulna and thoracic vertebrae. Two cases were in the tibia and the iliac bone, 3 cases were in the femur. Two cases had metastases in the thoracic vertebrae, 4 cases with pathological fracture, in one case primary mediastinal malignant lymphoma with bone metastases. The CT and X-ray features of 5 cases were osteolytic type, 4 cases were of infiltrating type, 2 cases were of sclerotic type(one case with ivory vertebra), 2 cases of mixed-type. In 2 cases, MRI showed slightly hypointense or homogeneous on T1WI, slightly hyperintense or homogeneous on T2WI, slightly hyperintense on fat suppressed sequence, mild to moderate and homogeneous or inhomogeneous Gd DTPA enhancement with the contrast agent. Conclusion: X-ray examination plays an important role in the diagnosis of the bone malignant lymphoma. Its characteristics are ivory vertebra, multiple level vertebral body involvement and multifocal destruction, osteolytic multifocal destruction of long bones. Compared with X-ray, CT can more clearly discern the lesions such as destruction of bone, the sclerotic reaction, periosteal reaction, the invasion of soft tissues. Compared CT and X-ray, MRI can more clearly identify the lesions such as the early changes of bone mineral content invaded by lymphoma and the early involvement of bone marrow.

The MRI manifestations and diagnostic value in primary and secondary aneurysmal bone cyst

Objective: To analyze the MRI manifestations of aneurysmal bone cyst and to evaluate the value of its imaging diagnosis. Methods: A retrospective analysis was made on MR images of 18 patients who were pathologically proved to have primary or secondary aneurysmal bone cyst. Results: Eighteen lesions all showed expansile and lytic bony destruction(100%), and 15 lesions located in long tubular bones(83.3%), and 12 lesions were eccentric(66.7%). Among all the lesions,there were visible cortical bone defect in 11 lesions(66.1%), periosteal reaction in 2 lesions(11.1%), lobulated margin in 12 lesions(66.7%), multiple cystic spaces in 14 lesions(77.8%), internal septations in 10 lesions(55.6%), hypointense peripheral rim in 11 lesions(61.1%), fluid-fluid levels in 6 lesions(33.3%), soft tissue masses in 5 lesions(27.8%). Conclusion: ①Primary aneurysmal bone cyst showed relatively uniform MRI manifestations. Secondary aneurysmal bone cyst showed complicate and multiplex manifestations, however, most lesions showed collective MRI characters of aneurysmal bone cyst such as multiple cystic spaces and hypointense rims. ②MRI could reveal characteristic manifestations earlier and more sensitive than X-ray and CT. It is helpful to qualitative diagnosis of aneurysmal bone cyst, especially to secondary aneurysmal bone cyst. ③A comprehensive imaging analysis could improve diagnostic accuracy of aneurysmal bone cyst.

Analysis of bone mineral density and the incidence of osteoporosis in 2288 residents in Shenyang

Objective: To analyze the changes of bone mineral density (BMD) and the incidence of osteoporosis in Shenyang residents for diagnosis and treatment. Methods: BMD were measured by dual energy X-ray absorptiometry(DEXA) in 2288 residents aged 20 to 80. BMD values were analyzed by different age groups in 5 years interval, and the incidence of osteoporosis was analyzed by different age groups in 10 years interval. Results: The peak bone mass were detected in 30~35 age group and declined afterwards. A remarkable decrease in BMD was shown in women after 50 years old. The female group has a higher incidence of osteoporosis than the male group, especially in the Ward’s trigonal area. Conclusion: This study provides references for the diagnosis and therapy osteoporosis in Shenyang region.
