761 CT manifestations analysis of Wegener granulomatosis in lung

Objective: To evaluate CT manifestations and the value of diagnosis of Wegener granulomatosis of lung. Methods: All the CT findings of the 10 cases with Wegener granulomatosis in lung confirmed clinically or by operation were analyzed retrospectively. Results: Nodules and cavitations were seen in 6 patients. Masses with cavitations were seen in 2 patients. Wedge-shaped areas of peripheral consolidation without uniform density abutting the pleura were seen in 2 patients. A feeding vessel sign to a nodule or mass were seen in 5 patients. Ground-glass attenuation and consolidation were seen in 4 patients. Bronchial wall thickening was discovered in 4 patients, tracheostenosis was seen in 1 patient, bronchiectases were found in 3 patients. Pleural irregularities were seen in 1 patient and pleural effusion was seen in 1 patient. Conclusion: Nodules and cavitations, wedge-shaped areas of peripheral consolidation, ground-glass attenuation and consolidation in lung are specific signs in diagnosing Wegener granulomatosis in lung. A feeding vessel sign to a nodule or mass is also one of its important characteristics, and all these special developments are more important for us to diagnose Wegener granulomatosis in lung.

764 Initial experience with 16-slice helical CT of extracardial vascular deformities in patients with congenital heart disease

Objective: To study the value of 16-slice helical CT(MSCT) and three-dimensional(3D) reconstructions in the diagnosis of extracardial vascular deformities in patients with congenital heart disease(CHD). Methods: To analyse extracardial vascular abnormalities in 26 patients confirmed by surgery. All patients were examined by contrast-enhanced MSCT and subsequent 3D reconstruction at work-station. Correlative evaluations were performed with 26 echocardiograms(UCG) and 13 cineangiocardiograms(CAG). Results: Eighty-three extracardial vascular abnormalities were confirmed. The denifite rate of diagnosis of MSCT, UCG and CAG were 98.8%, 89.2%, 100% respectively. MSCT and CAG were superior to UCG in the identification of extracardial vascular abnormalities(χ2=4.76, P<0.05); (χ2=4.11, P<0.05), the diagnostic accuracy of MSCT and CAG showed no difference(χ2=0.43, P>0.05). Conclusions: MSCT(short acquisition time of image) with 3D reconstruction were superior to UCG for the assessment of extracardial vascular abnormalities in patients with CHD and could demonstrate the pathological morphology. This combination was a good noninvasive diagnostic way in preoperative evaluation and selecting operative modality. The combination of MSCT and UCG was a substitution for invasive CAG in great extent.

768 Study on characteristics and regularity of ruptured mitral chordae tendineae in color Doppler echocardiography

Objective: To probe into the imaging character and regularity of ruptured mitral chordae tendineae(RMCT) in color Doppler echocardiography(CDE). Methods: Thirty patients of RMCT were examined with CDE. Seven cases underwent angiocardiography. All cases were confirmed by operation. Results: Thirty cases were accurately diagnosed by imaging character of CDE. The features of CDE in RMCT: ①The ruptured chordae tendineae of mitral valve seems like flail on 2DE, named 2DE “flail sign”. ②M-mode echocardiography showed superimposed curve echo and trembling wave of mitral valve. ③For patients with mitral valve prolapse, a hammock-like prolapsed mitral valve can be seen on M-mode echocardiography and the prolapsed mitral valve protrude into left atrial on 2DE during systole. ④Pulse Doppler and continuous Doppler could record turbulent flow spectra from left atrial on top of mitral valve during systole. ⑤Color regurgitation flow through the mitral valve could be observed on CDFI during systole. ⑥The incidence rate of RMCT is much higher in male than in female and partial rupture accounts for larger proportion than complete rupture. Conclusions: The feature of CDE image in RMCT was obvious. CDE was valuable in diagnosing RMCT, but it should be differented from mitral valve prolapse and mitral valve vegetation.

771 Effect of left bundle branch block on interventricular asynchrony in chronic heart failure

Objective: To assess the effects of left bundle branch block on interventricular asynchrony in chronic heart failure. Methods: There were three groups. Chronic heart failure without intraventricular conduction delay and chronic heart failure with complete bundle branch block were compared with healthy individuals. Pulse tissue Doppler imaging(TDI) was performed to measure the time of electromechanical motion of left and right ventricular lateral wall in systole on atrial-ventricular annulus with mapping techniques. Pre-ejection contraction time of left and right ventricles were recorded with pulsed-wave Doppler(PW). Results: Interventricular electromechanical delay time in systole and interventricular ejection delay time increased in chronic heart failure with complete bundle branch block compared with healthy individuals and chronic heart failure without interventricular conduction delay. Conclusions: Interventricular asynchrony can be assessed with TDI and PW Doppler, which can provide good evidence for treatment of chronic heart failure.

773 Subregion study of ADC maps in piglet model with acute severe hypoxic ischemic encephalopathy

Objective: To study the subregion of MR apparent diffusion coefficient maps(ADC maps) in piglet model with acute severe hypoxic ischemic encephalopathy(HIE), compared with clinical manifestation and pathology. Methods: Seven days old piglets(n=48) were divided into two groups randomly: group A (normal control group, n=5) and group B(HIE group, n=43, perform by bilateral common carotid artery occlusion and then inspirited 4% oxygen about 1 hour). According to the clinical manifestation of experimental animal, group B were divided into two subset: group B1(stable group, n=30) and group B2(aggravated group, n=13). ADC maps were got at the time point of 0, 1, 3, 6, 12, 18 and 24 hours after hypoxic ischemic, and 4 regions of interest(ROI Ⅰ~Ⅳ) were set from the minimum ADC values point to the consecutive normal ADC values area. The ADC values of 4 ROIs and the number of the focus were studied compared with clinical manifestation and pathology. Results: Multiple small patchy regions of hypointensity were found in the bilateral cerebrum by ADC maps, and most focuses were distributed in subcortical white matter, basal ganglia, thalami and periventricular white matter. The evolvement patterns of ADC values in 4 ROIs of two HIE groups were quite different, and markedly correlative with clinical manifestation and pathology. Conclusion: To analyze the evolvement patterns of ADC values in 4 ROIs of ADC maps could evaluate pathogenetic condition and prognosis of acute HIE.

777 Value of MRI features in preoperative diagnosis of malignant meningioma

Objective: To investigate the value of MRI features in preoperative diagnosis of malignant meningioma. Methods: MRI findings in 23 patients with malignant meningioma were analyzed retrospectively, and 34 cases of benign meningiomas in the same period were randomly selected as control. Results: Lobulation, cystic necrosis, tumor-brain interface, tumor-brain interface rim, signal intensity in T1WI, signal inhomogeneity, inhomogeneous contrast enhancement, the degree of contrast enhancement and dural tail sign are related to the gradings of meningioma. Conclusion: Malignant meningiomas do have characteristic MRI features, to evaluate some MRI features has important significance in qualitative diagnosing malignant meningioma before operation.

781 Application of 11C-Choline PET/CT in radiotherapy planning of brain tumor

Objective: To investigate the possibility and the methodology of integrating 11C-Choline PET/CT with 3D-CRT/IMRT treatment planning of brain tumors. Methods: Thirty-four consecutive patients presenting with various intracranial tumors before radiotherapy treatment were investigated. Volume delineation was performed on the CT data and PET/CT data separately. GTV and PTV calculated based on CT and PET/CT images were compared using SPSS software. Results: The GTV increased by 25% or more because of PET in 23.53% of cases. The GTV was reduced >25% in 44.12% of patients. Overall, in 67.65%(23/34) of cases, GTV delineation was changed significantly if information from metabolic imaging was used in the planning process. The modification of the GTV translated into altered PTV changes in 58.52%(20/34) of cases. In 3 cases, 11C-Choline PET/CT revealed distant metastasies, changing the treatment strategy from curative to palliative. In 5 cases, no solid evidence of tumor existence was found and their radiotherapy planning was canceled. Conclusion: Integrated 11C-Choline PET/CT for treatment planning for 3D-CRT improves the accuracy of volume delineation compared with that of CT alone. The impact on treatment outcome remains to be demonstrated.

785 Imaging diagnosis and literature review of diffuse giant cell tumor of tendon sheath

Objective: To review clinical, pathological and imaging features of diffuse giant cell tumor of tendon sheath(D-GCTTS) and to elevate recognization for this rare benign tumor. Materials and Methods: To analyse the clinical, pathological and imaging data from two patients with pathologically confirmed D-GCTTS, who underwent the preoperative CR, CT and MRI plain-scan and enhanced-scan. Results: Radiographic examination showed no bony and joint abnormality, iso-dense mass in the soft tissue. CT scan showed an increased attenuation multilobular mass with clear margin; there were no bony destruction except sclerotic cortex and small cysts were found in the irregularly thickening cortex. MRI showed that the lesions have the similar low-signal intensities in the different sequences, specifically on T2WI. Within the removed specimen, there were two well-encapsulated solid elastic brownish masses, with high cellularity feartures consisting of synovial-like mononuclear mesenchymal cell, multinucleate giant cells, a few foam cells, hemosiderin desposition and inflammatory cells. Conculsion: MRI is a high sensitive and specific examination for diagnosis of D-GCTTS.

789 Relationship of in vivo 31P MR spectroscopy to serum transaminase in chronic hepatitis B

Objective: To study the feature of spectroscopy and metabolism in chronic hepatitis B by in vivo 31P MR spectroscopy, and investigate its relationship to the serum transaminase. Methods: Twenty healthy controls and 26 patients with etiologic-proven HBV-related liver were studied. Base on the status of hepatic function, patients were divided into mild, moderate, severe and normal liver function hepatitis. Hepatic MR scanning and 31P MR spectra were obtained using 1.5T spectroscopy system. To investigate the feature of the spectrum and metabolism in chronic hepatitis B, and the relationship to the serum transaminase. Results: The %PME and PME/PDE of control group are 9.32±2.74 and 0.31±0.06, the value in normal liver function hepatitis are 9.5±4.2 and 0.32±0.14, in mild group are 10.97±1 and 0.38±0.08, in moderate group are 12.59±3.18 and 0.45±0.13, in severe group are 15.49±4.7 and 0.53±0.14. There were insignificant differences between control group and hepatitis with normal liver function. There were significant differences between control group and severe, mild and severe in %PME. A monotonic increase in PME/PDE ratio with increasing severity of disease was statistically significant differences between the control and moderate, control and severe, mild and severe. The PME/PDE ratio correlated with the alanine aminotransferase(ALT), but not correlated with aspartate aminotransferase(AST). Conclusion: We can detect the status of hepatic metabolism and energy by 31P MRS noninvasively and directly, and is valuable to investigate the status of hepatic function in chronic hepatitis B.

793 Value of selective angiography and placement of localizing mark in arterial hemorrhage of the lower digestive tract

Objective: To explore the value of selective angiography in the diagnosis of arterial hemorrhage of the lower digestive tract and the value of keeping the mark of catheter or guide wire in guiding the surgery, and to decrease the blindness in surgical operation. Methods: The angiographic features in arterial hemorrhage of the lower digestive tract were analyzed in 25 cases, and in 18 cases of which, the mark of catheter or guide wire was placed simultaneously as localizing mark. Result: In 25 cases of arterial hemorrhage of the lower digestive tract, positive angiographic features were displayed in 23 cases, the accuracy rate is 92%; In 18 cases(emergency operation in 13 cases, selective operation in 5 cases) of keeping the mark of catheter or guide wire in place, the bleeding site were all accurately discovered, the rate of localization is 100%. No complications of intestinal ischemia and necrosis resulting from the placement of localizing mark were found during operation in 18 patients. Conclusion: Selective angiography is the best effective method in diagnosis of arterial hemorrhage of the lower digestive tract. Placement of localizing mark of catheter or guide wire can be of help to the surgeon in finding the bleeding site and resecting primary affection of hemorrhage successfully without complication. It is a better localizing method and should be widely used.

808 Application and value of color Doppler ultrasonograghy in the diagnosis of vasculogenic erectile dysfunction

Objective: To analyze the effect of color Doppler ultrasonograghy to the diagnosis and stage of vasculogenic erectile dysfunction(ED). Methods: The hemodynamic changes of the cavernous artery and the deep dorsal vein, before and after having injected vasoactive agents into the cavernous body in 259 patients using color Doppler ultrasonograghy were observed. Results: There were 35 cases(13.5%) with non-vasculogenic ED, 58 cases(22.4%) with arteriogenic ED, 47 cases(18.2%) with venogenic ED, 119 cases(46%) with mixed vascular ED. When penis was erected, PSV(peak systolic velocity), EDV(end-diastolic velocity)and RI(resistance index) in the group of non-vasculogenic ED was obviously higher than those in the groups of arteriogenic ED, venogenic ED and mixed vascular ED(P<0.05). Deep dorsal vein velocity in the group of venogenic ED was obviously higher than those in the groups of non-vasculogenic ED and arteriogenic ED(P<0.05). Conclusion: The result of color Doppler ultrasonograghy examination effectively helps the diagnosis and stage of vasculogenic ED.

811 Ultrasonic diagnostic value of cryptorchidism in children

Objective: To evaluate the value of ultrasonic diagnosis cryptorchidism in children. Methods: 53 cases of children with crytorchidism were examined by two-dimensinal ultrasonography. Results: 53 cases and 65 cryptorchidism were confirmed by operations, of which 63 cryptorchidism were diagnosed accurately by ultrasound, the diagnostic rate is 96.9%. 52 of cryptorchidism were in inguinal tube, 8 were within abdomen and 5 were in scrotum. Conclusion: Ultrasonographic method can be used to diagnose and locate the cryptorchidism prior to operation and insure the success and time saving for operation.
