481 CT and MR features of supratentorial intraparenchymal ependymoma(analysis of 15 cases)

Objective: To analyse the CT and MRI features of supratentorial intraparenchymal ependymoma and to improve the diagnostic accuracy of atypical supratentorial intraparenchymal ependymoma. Materials and Methods: The CT and MR images of 15 cases with supratentorial intraparenchymal ependymoma confirmed by postoperative pathology were analyzed retrospectively. Among the 15 cases, 9 were male and 6 female. Two cases were under 10 years old, 2 cases were 11~20 years old, and 11 over 30 years old. Results: Supratentorial intraparenchymal ependymoma frequently involves the boundary sites of parietal lobe, temporal lobe and occipital lobe. At most, its growth closed to the brain surface, and in the affinity of the supratentorial ventricles, and the occurance of cystic changes were usually high. After administration of contrast medium, vasculature were found in the solid parts of the tumor. Peritumoral edema was absent or slight. The tumor was demonstrated slightly hyperdense with marked inhomogeneous enhancement after administration of contrast medium. Conclusion: The finding of vascular image in the solid parts of tumor was also the characteristic of supratentorial intraparenchymal ependymoma. Identifying the manifestation of supratentorial intraparenchymal ependymoma in CT and MRI was helpful in correct preoperative diagnosis of supratentorial intraparenchymal ependymoma.

484 CT and MRI findings of primary orbital lymphoma and review of literature(report of 7 cases)

Objective: To study CT and MRI findings of primary orbital lymphoma, to improve the diagnostic ability of this disease. Methods: Plain CT, enhanced CT and delayed CT scans were performed in all 7 patients with primary orbital lymphoma proved histopathologically. In 3 of the 7 patients, the plain and enhanced MRI were performed. Both CT and MRI findings were analyzed retrospectively. Results: Bilateral lesions were found in 2 patients, located in bilateral conjunctivae and lacrimal gland respectively. Unilateral lesion was found in 5 patients. The lesion located in lacrimal duct in 1 patient, in the other 4 patients the lesion crossed orbital septum and molded around orbital structures with irregular shape, and the lacrimal gland and lacrimal duct were involved, in 3 of the 4 patients the diffuse lesion infiltrated extraconal and intraconal space. There were no displacement of extraocular muscle, thickening and deformity of ocular ring in all patients. Destruction of orbital bone was evident in 2 patients. All the lesions presented soft tissue density similar to extraocular muscle and with distinct margin on plain CT scan, the density of lesion which showed mild enhancement on enhanced CT in 4 patients was found to decrease on delayed phase, there was no obvious change of density in the other 3 patients on delayed CT scans, compared to enhanced CT. MRI findings in 3 patients demonstrated isointense signal compared to extraocular muscle on T1WI and T2WI. However, the lesion in 2 patients was hyperintense compared to extraocular muscle after enhancement. All the lesions showed homogeneous on CT and MRI. Conclusion: The CT and MRI findings of primary orbital lymphoma are characteristic, which can highly suggest the diagnosis of this disease.

488 The emptying time of contrast medium during parotid sialography in patients with Sjogren’s syndrome

Objective: To study emptying time of contrast medium during parotid sialography with Sjogren’s syndrome(SS). Methods: The patients were divided into normal group without symptom of oral cavity dryness, normal group with symptom of oral cavity dryness and SS group. The images of parotid sialography at 15min and 60min were analyzed, and some of the patients were randomly selected for long term follow up observation. Results: In the normal group without symptom of oral cavity dryness, most of the contrast medium was excreted(9/12 sides), completely excreted(3/12 sides) at 60min images. In the normal group with symptom of oral cavity dryness, contrast medium was not excreted in most cases at 60min images. In the SS group most cases showed no obvious excretion(44/58 sides). In cases with no oral cavity dryness the emptying of contrast medium was well. In the follow up observation from 1.5 hours to 6 months only 2 sides of SS patient showed bilateral complete excretion. Conclusion: When there is no obvious excretion of contrast medium on the 60min film it is considered to be abnormal as seen in most of the SS patients. Excretion time is correlated with the symptom of oral cavity dryness. The emptying of contrast medium on parotid sialography has high sensitivity in the diagnosis of SS, but its specificity is relatively poor.

491 The clinical application of bronchial arterial embolization in massive hemoptysis

Objective: To study the clinical effect of bronchial arterial embolization in massive hemoptysis. Methods: Medical therapy was unsuccessful in 32 patients(19 men, 13 women) with massive hemoptysis(>300ml of blood in 24 hours). The main etiologies of hemoptysis were bronchiectasis(15 patients), tuberculosis(11 patients), pneumonitis(4 patients), lung cancer(2 patients). Selective bronchial arterial angiography and bronchial arterial embolization were performed. In selective bronchial arterial embolization gelfoam was used, in superselective bronchial arteries embolization, PVA was used. Results: Embolization was performed once in 28 patients, twice in 3 patients, emergency lobectomy was required for massive hemoptysis recured three days after twice embolization in 1 patient. Conclusion: Bronchial arterial embolization is an effective and safe treatment in patients with massive hemoptysis for medical therapy.

494 Value and method of color Doppler echocardiography in detecting total anomalous pulmonary venous connection

Objective: To explore the imaging characteristics of total anomalous pulmonary venous connection(TAPVC) in color Doppler echocardiography(CDE). Methods: Data of CDE in 16 patients with TAPVC confirmed by surgery were analyzed retrospectively. Result: The CDE characteristics were as follows: ①the pulmonary veins join to form a confluence channel that drains back to the RA rather than to the LA. ②In the supracardiac form of TAPVC it drains to the left vertical vein, to the innominate vein to the RSVC or drains to the SVC directly. In the intracardiac form of TAPVC it drains to the coronary sinus(CS). In the infracardiac form of TAPVC drains to the descending vertical vein, to the hepatic system. Mixed form of TAPVC occur in which the PV drains to two separate systemic venous sets(left vertical vein and CS). ③Right to left shunt at atrial level. Sixteen cases were accurately diagnosed by CDE imaging with the diagnostic accuracy rate of 100%, connection sites were correctly diagnosed 94%. Conclusion: CDE has important clinical value in detecting TAPVC.

496 Evaluation of gated myocardial perfusion imaging for detecting left ventricular function before and after coronary artery stenting

Objective: To compare the changes of global and regional left ventricular function parameter before and after coronary artery stenting by using 99mTc-MIBI gated myocardial perfusion imaging(G-MPI) and to evaluate coronary artery stenting effect. Methods: The values of end-diastolic volume(EDV), end-systolic volume(ESV), left ventricular ejection fraction(LVEF) and regional wall motion were assessed by rest G-MPI before and 1 to 6 months after coronary artery stenting in 27 patients. All patients were divided into two groups: Group I(12 cases) involved patients with left ventricular dysfunction(LVEF≤45%); the rest were included in groupⅡ(15 cases). Results: ①After coronary artery stenting, the values of EDV and ESV were decreased markedly(P<0.01), LVEF was increased(P<0.05) compared with preoperation in group I. On the contrary, there were no significant changes of these indexes(P>0.05) in groupⅡ. ②There were 93(82.3%), segments improved postoperatively in mild and moderate hypokinesia, 12(29.3%) segments improved in severe hypokinesia, and 3(15.7%) in akinesia. Conclusion: G-MPI is an excellent tool to evaluate the effect of stent implantation, and suitable for long term follow-up.

499 The dose-effectiveness of VEGF in focal cerebral ischemia reperfusion injury of rabbits evaluated by MR imaging

Objective: To study the dose-effectiveness of the vascular endothelial growth factor(VEGF) in focal cerebral ischemia reperfusion injury of rabbits by MR imaging. Methods: Rabbit model of focal cerebral ischemia was induced by occlusion of the middle cerebral artery for 2 hours. The VEGF was administered immediately after reperfusion at different doses. We measured infarct volume, neural cell apoptosis, activation of caspase-3 and ADCR in the perifocal regions of cerebral ischemia and neurological deficit score 72h after reperfusion. Results: There was no significant difference in the cerebral edema, neuroprotective efficacy and angiogenesis at the dose of 1.25ng/μl VEGF compared with control group. When administered VEGF at the dose of 2.5ng/μl and 5.0ng/μl after reperfusion, the cerebral edema, the percentage of apoptotic cell and activation of caspase-3 in the perifocal regions were lowered significantly, the ADCR and increase of blood vessels were significantly higher than those in the control group. The neuroprotective efficacy and significant enlargement of the blood vessels were observed in the 2.5ng/μl than that in 5.0ng/μl group, and the infarct volume was significantly reduced in the 2.5ng/μl group(P<0.05). Conclusions: The VEGF at a dose of 2.5ng/μl has displayed excellent increase in vessel density and neuroprotective efficacy for focal cerebral ischemia reperfusion injury of rabbits. Therefore, the dose of 2.5ng/μl was used in the following treatments to further evaluate the effect of VEGF.

505 Application of real-time three-dimensional echocardiography in cardiac space-occupying lesions

Objective: To evaluate the value of real-time three-dimensional echocardiography(RT3DE) in cardiac space-occupying lesions. Methods: Twenty-eight patients with cardiac space-occupying lesions, determined by surgical operation and pathology, undertook two-dimensional echocardiography(2DE) and RT3DE. The three-dimensional images were divided into grade I, II and III according to the imaging qualities. Results: The three-dimensional images were assess as grade I in 93%(26/28), grade II in 7%(2/28) and no grade III images. The location, morphology and relation with neighboring structures of cardiac space-occupying lesions could be displayed more intuitively by RT3DE. Conclusion: Compared with 2DE, RT3DE may provide more abundant morphologic information of cardiac space-occupying lesions and is of great benefit to its treatment planning.

508 Imaging diagnosis of duplication of the alimentary tract in child

Objective: To investigate the imaging manifestations and the value of various imaging examinations of duplication of the alimentary tract in children. Methods: Imaging findings, clinical data and pathological results of 12 cases with histologically proved duplication of the alimentary tract were retrospectively analyzed. Results: Of the 12 cases, there were 1 case of rectal duplication, 1 case of gastric duplication, 10 cases of intestinal duplication. Abdominal plain film showed complete obstruction in 2 cases, incomplete obstruction in 3 cases and mass in 2 cases. Barium/air GI examination or enema was performed in 7 cases, in which 3 cases had mass effect, 1 case had filling defect, 1 case had mass after relocation of intussusception, 2 cases had normal appearances. BUS was performed in 8 cases in which 6 cases showed cystic lesions, 1 case showed intestinal obstruction and 1 case showed peritonitis. CT revealed fluid density, unilocular cyst in 4 cases, cystic wall was thick, and were enhanced after enhancement. Conclusion: Applications of various imaging examinations had important value to accurately diagnose duplication of the alimentary tract in children.

512 The using of contrast-enhanced harmonic sonography with low mechanical index

Objective: To evaluate the value of contrast-enhanced harmonic sonography with low mechanical index in choosing the treatment indicator of focal liver lesions. Methods: Two hundred and thirty-two patients with 357 focal liver lesions were examined by contrast-enhanced harmonic sonography with low mechanical index, contrast-enhanced CT/MR and/or pathological examination. Results: Ten patients with liver abscess were confirmed by contrast-enhanced harmonic sonography with low mechanical index and treated by percutaneous puncture. One hundred and forty-seven patients with malignant tumor were diagnosed by contrast-enhanced harmonic sonography with low mechanical index. In which, 82 patients were treated by non-operative therapy. Percutaneous ethanol injection was carried out in 7 cases. Radiofrequency ablation or microwave coagulation therapy was performed in 57 cases. Transcatheter arterial chemoembolization or percutaneous ethanol injection combined with radiofrequency or microwave ablation was performed in 14 cases. Four patients were only treated by transcatheter arterial chemoembolization. Conclusions: Contrast-enhanced harmonic sonography with low mechanical index is helpful for choosing the treatment indicator of focal liver lesions.

515 The diagnostic value of multi-slice CT and three dimensional reconstruction in primary ureteral carcinoma

Objective: To investigate the diagnostic value of multi-slice CT(MSCT) and three dimensional reconstruction in primary ureteral carcinoma. Methods: Sixteen cases with primary ureteral carcinoma proved by operation or biopsy pathology were collected in the study. All patients had undergone MSCT scanning with multi-planar reformation(MPR), curved planar reformation(CPR), maximum intensity projection (MIP) and volume rendering technique(VRT). Results: The axial CT images displayed intraureteral soft tissue mass with thickened wall. The foci had mild or moderate enhancement after contrast injection and the peak enhancement was in the venous phase. The MPR and CPR images can clearly demonstrate the location, extent, number, thickened ureteral wall and adjacent invasion of the tumor. CPR image can exhibit the whole range of ureter in one plane, MIP and VRT images were similar to intravenous urography. Conclusion: MSCT and three dimensional reconstruction had very important value in the diagnosis of primary ureteral carcinoma, it can supply valuable information for clinic in treatment planning and prognosis evaluation.

518 MRI analysis of pseudotumor injected into nude mice intracranially

Objective: To evaluate the feasibility of imaging pseudotumor injected into nude mice intracranially at 1.5T MR system and to study the manifestation of MRI. Methods: Thirty nude mice were included in this study and distributed into six study groups: injecting Gd-DTPA(1μl), Gd-DTPA(2μl), Methtlene blue(1μl), Methtlene blue(2μl), mixing of Gd-DTPA and Methtlene blue(1μl), mixing of Gd-DTPA and Methtlene blue(2μl). Animals were anesthetized with thiopental sodium. After injection of corresponding reagent by using steriotatic method, the mice were transferred immediately to the MR imaging unit. Unenhanced SE and FSE images were performed with a 1.5T superconducting magnet. Signal intensity of pseudotumor and normal brain parenchyma was measured with 3 different regions of interest. Relative value(%) of enhancement was measured and compared. Volume of pseudotumor were measured and compared. Results: MR images of quite good quality were obtained in all the 30 mice. No pseudotumor was observed in the Methtlene blue(1μl) group and Methtlene blue(2μl) group. In the other groups, pseudotumor presented as focus of short T1 signals in the edge, long T1 signals in the center and short T2 signals. Relative values between pseudotumor enhancement and normal brain were different statistically in each group and there was no statistical difference in the relative value of pseudotumor across the four groups. The volume of pseudotumor was lowest in the mixing of Gd-DTPA and Methtlene blue(1μl) group than in the other groups. Conclusion: MRI is feasible in imaging nude mice brain and detect abnormal signals focus. Methtlene blue as a potential antitumor reagent had no obvious change of signal intensity of nude mice brain. The characteric signal of pseudotumor arise from the difference of the concentration of Gd-DTPA in local nude mice brain.

523 Dynamic hepatic sonography analysis of liver cirrhosis in rats after contrast enhancement

Objective: To investigate the characters of the dynamic sonographic changes of liver cirrhosis in rats after contrast enhancement and evaluate its role in detecting liver fibrosis and cirrhosis. Method: Model of liver cirrhosis was established in 80 rats by subcutaneously injecting CCl4, which were divided into one control group and 4 groups according to modeling time(6, 8, 10 and 12 weeks respectively). The time-intensity curve of rat’s parenchyma was drawn with video frequency analysis after ultrasound contrast agent was injected intravenously and time to peak(TP), peak intensity(PI), half time of descent(HT), descending slope(DS), area under the curve(AUC) and mean transit time(MTT) were measured and compared. Results: The models of rat’s liver cirrhosis in different stages were established successfully and they could reflect the pathological progress of the cirrhosis. With the aggravation of cirrhosis, the values of TP, PI and HT tended to rise gradually while DS was contrary(compared between 6~12 weeks and control groups and between 6, 8 weeks groups and 10, 12 weeks groups, the differences were significant, P<0.01). Otherwise, the changes of AUC and MTT were not significant(P>0.05). Conclusion: The changes of rats’ hepatic sonograph present some regularity with the development of cirrhosis, which can be reflected by TIC. It also provides a new possibility for non-invasive diagnosis of liver cirrhosis.

526 Budd-Chiari syndrome: evaluation with multiphase dynamic contrast-enhanced MR and revascularization

Objective: To analyze the images of the Budd-Chiari syndrome(BCS) by multiphase dynamic contrast-enhanced MR and evaluate the clinical value of three-dimensional dynamic contrast-enhanced MR angiography(3D DCE MRA). Materials and Methods: Thirty-three patients with BCS underwent multiphase dynamic contrast-enhanced MR examinations. The relevant vessels were reconstructed respectively with MIP and MPR, i.e. 3D DCE MRA. Compared with the findings of surgery or DSA, we estimated the value of multiphase dynamic contrast-enhanced MR examinations and their revascularization in judging the obstruction level and showing intra/extra-hepatic collateral vessels. Results: Multiphase dynamic contrast-enhanced MR examinations of 33 patients with BCS demonstrated narrowed IVC in 21, obstructed IVC in 12, narrowed or obstructed HV in 15, enlarged accessory hepatic veins in 10, intrahepatic or extrahepatic collateral vessels in 23. The information of vessels and collateral pathways could be showed with MIP reconstruction, and MPR could directly display vessels by multiangles. Liver parenchymal abnormalities displayed by multiphase dynamic contrast-enhanced MR examination showed irregularities of liver contours and hypertrophy of the caudate lobe in 11 cases, heterogeneous patchy enhancement in 25 cases and complicated benign regenerative nodules in 7 cases. Conclusion: Multiphase dynamic contrast-enhanced MR examination can correctly reflect the hepatic hemodynamic changes in patients with BCS, and combined with 3D DCE MRA, can explicitly display the obstruction level of vascular lesions and collateral circulations in BCS. So, it is an effective, noninvasive method for evaluating BCS before surgery or intervention.

530 A study of the effect of X-ray tube voltage on image quality and radiation dose

Objective: To study the correlation among X-ray tube voltage, image quality and radiation dose. Methods: Radiographic images were acquired by employing the phantom with 25 pairs of vessels containing iodine at concentration between 4.0~50mg/ml. Curve fitting was used to find and evaluate the correlation among X-ray tube voltage, image quality and surface dose of the phantom. Results: The decrease of image quality is less significant than that of surface dose while the tube voltage increased from 54 to 85kVp, and the decline of surface dose was not evident whereas the decline of image quality was substantial when the tube voltage increased over 85kV. Conclusion: On the basis of some specific thickness of phantom, the optimized value of tube voltage that both satisfy the image quality and the minimum surface dose of phantom can be obtained by means of curve fitting and studying the correlation among them.
