静脉性出血性脑梗死磁敏感加权成像的初步研究

摘要 目的:探讨磁敏感加权成像(SWI)诊断脑静脉(窦)血栓形成(CVST)并静脉性出血性脑梗死(VHCI)的价值。方法:采用3.0T磁共振SWI序列,回顾性分析经DSA或病理证实的18例VHCI患者的影像学资料,并与病变部位近似的12例单纯性脑出血和8例脑肿瘤伴出血相对照。结果:18例VHCI患者,SWI表现为顶叶皮层或皮层下斑片状低信号,邻近大脑皮层或皮层下可见“线状”或“蚯蚓状”广泛扩张的皮层静脉或髓静脉。12例单纯性脑出血患者,SWI表现为均匀低信号,周围皮层静脉正常或稍减少。8例脑肿瘤伴出血患者,SWI表现为高低混杂信号,其内可见线状低信号小静脉影及斑点状低信号出血灶,周围皮层静脉明显减少。结论:对于顶叶皮层及皮层下出血性病变,SWI能够通过显示病灶邻近皮层扩张的皮层静脉或髓静脉的特殊征象,提高对VHCI的诊断,是常规磁共振成像(cMRI)及磁共振静脉血管成像(MRV)的重要补充。

AbstractObjective: To explore the value of susceptibility weighted imaging(SWI) in the diagnosis of cerebral venous/venous sinus thrombosis combined with venous hemorrhagic cerebral infarction(CVST-VHCI). Methods: The imaging data of the 18 patients with CVST-VHCI proved by DSA or pathology were studied retrospectively on GE 3.0T MR system, and were compared with those of 12 patients with simple cerebral hemorrhage and 8 patients with brain tumor which located in nearly the same region. Result: In the 18 patients with CVST-VHCI, SWI showed patches of hypo-signal intensity in parietal cortex or parietal subcortex. There were wide spread or earthworm-like mesh cortex veins or pulp veins around the cortex or parietal subcortex. In the 12 patients with simple cerebral hemorrhage, SWI revealed lesions of low signal intensity. The cortex veins were normal or reduced slightly. In the 8 patients with brain tumor, SWI expressed clumping mixed-signal and linear hypo-signal small veins and patchy hypo-signal hemorrhage inside the tumor. Conclusion: For the hemorrhagic disease in parietal cortex or parietal subcortex, SWI can reveal the specific feature by showing expanded cortex vein or medullary vein to improve the diagnosis of CVST-VHCI. So SWI is an important supplement of cMRI and MRV.
