609 Application of arterial spin labeling and intra-voxel incoherent motion imaging in preoperative glioma

Objective: To investigate the three-dimensional arterial spin labeling(ASL) and intra-voxel incoherent motion(IVIM) value in preoperative grading of cerebral glioma. Materials and Methods: Forty-five patients with glioma proved by pathology(26 cases of high grade glioma and 19 cases of low grade glioma) underwent 3D-ASL and IVIM-DWI examination. Cerebral blood flow(CBF) in the most obvious perfusion area of the solid part of tumor and also in the contralateral normal area were measured using cerebral blood flow map. By fitting double exponential model, got the fast ADC map. On 3D-ASL, the ADC value in the solid part of the tumor and also in the contralateral normal area of the same section were measured. Results: CBF ratios of the lesion and contralateral normal area in high and low grade gliomas were 3.11±1.45 and 0.86±0.28 respectively, the fast ADC value were 1.88±0.51 and 0.96±0.31 respectively. There was significant differences between the two groups(P<0.05). Conclusion: 3D-ASL and IVIM techniques are safe and noninvasive methods to extract information of perfusion in gliomas and thus can be used as methods for determining the grade of gliomas before operation.

612 MRI appearances of the early period of tuberculous meningitis in children

Objective: To improve identification of MRI appearances in the early stage of tuberculous meningitis in children. Methods: MRI appearances of 24 cases with early stage of tuberculous meningitis were retrospectively analyzed. All patients had conventional MRI and contrast-enhanced T1WI scans by Siemens Avanto 1.5 Tesla scanner. Four cases had conventional contrast-enhanced FLAIR scans. Results: Eight cases showed widening of cerebral sulci or cerebral fissure slightly widened and deepened and 16 cases had no abnormality on MRI plain scan. Twenty cases with diffuse linear enhancement of the meninges and 4 cases had no abnormal enhancement on MRI contrast-enhanced T1WI scans. Contrast-enhanced FLAIR images were superior to contrast-enhanced T1WI in the delection of meningeal lesions in 4 cases. All cases had no abnormal enhancement in the brain parenchyma. Conclusion: MRI contrast-enhanced scanning is an effective approach to detect the early period of tuberculous meningitis in the early stage, the MRI features of the early period of tuberculous meningitis are diffuse linear enhancement of the meninges, recognize the MRI characteristics is important in clinical diagnosis.

615 Application value of BI-RADS in mammography

Objective: To investigate the application value of breast imaging reporting and data system(BI-RADS) in mammography. Methods: One hundred and seventy patients with 175 masses were discovered by digital mammography and confirmed by surgical, biopsy and pathology from January 2010 to January 2013. All patients were female, the average age was 46.30±8.53 years old. ACR BI-RADS standard was used for classification. Correlation of the images with pathologic results were carried out and the positive predictive value(PPV) was analyzed. Results: In the 175 masses, pathologic diagnosis after operation: benign lesions n=103, malignant lesions n=72. On X-ray, appearances of simple mass n=128, among which 32.3%(41/128) were malignant, masses with calcification n=47, among which 65.96%(31/47) were malignant. Benign lesion appeared oval in shape 40.6%(71/175), obscured margin 26.3%(45/175), isodensity 53.1%(93/175). Malignant lesions mainly appeared as irregular in shape 29.8%(52/175), ill defined border 20.6%(36/175), hyperdensity 24.6%(43/175). X-ray diagnosis by BI-RADS 2, 3 in 29 lesions, BI-RADS 4(a, b, c) and 5 in 146 lesions. Accuracy rate of benign lesion was 100%(29/29). PPV was 49.32%. Conclusion: Application of BI-RADS can effectively evaluating and predicting benign and malignant masses discovered by mammography.

619 The diagnostic value of the pulmonary perfusion SPECT/CT in detecting pulmonary embolism forpatients due to malignant tumor

Objective: To retrospectively study the diagnostic value of pulmonary perfusion SPECT/CT in patients suspected of having pulmonary embolism due to malignant tumor. Methods: All the 55 patients suspected of having pulmonary embolism due to malignancy received pulmonary perfusion SPECT/CT in our department from July 1, 2011 to December 1, 2013. The pulmonary perfusion SPECT/CT images were analyzed retrospectively. The standard for diagnosing embolism lung segment were sparse or defective radioactive distribution in a wedge shape lung segment or sub-segment, but no lesions that caused lung perfusion reduced in the corresponding lung on CT images. The final diagnosis of pulmonary thromboembolism(PTE) was confirmed by comprehensive analysis of the patients’ clinical data. The positive rate, sensitivity, specificity, positive predictive value, negative predictive value and accuracy of lung perfusion SPECT/CT in the diagnosis of PTE were calculated. The datas diagnosed by lung perfusion SPECT/CT and by comprehensive analysis of the patients’ clinical data were analyzed by matching four-fold table chi-square test(McNemar test). Results: Among the 55 patients with malignant tumor, 42 cases were diagnosed PTE by pulmonary perfusion SPECT/CT, and 41 cases were diagnosed PTE by comprehensive analysis of the patients’ clinical data. There was no statistical difference of the results diagnosed by pulmonary perfusion SPECT/CT and by comprehensive analysis of the patients’ clinical data (χ2=0.049, P>0.05). The sensitivity, specificity, positive predictive value, negative predictive value and accuracy of lung perfusion SPECT/CT in the diagnosis PTE was 94.4%, 85.2%, 89.5%, 92.0%, 90.5% respectively. Conclusion: PTE in patients with malignant tumor can be accurately diagnosed by pulmonary perfusion SPECT/CT which provides a reliable method for diagnosis.

623 A comparative study of RECIST1.0 and mRECIST in evaluating TACE treatment response forpatients with hepatocellular carcinomas

Objective: CT was used to compare using the response evaluation cirteria in solid tumor 1.0(RECIST1.0) and modified RECIST(mRECIST) in evaluation the treatment effect of TACE for patients with HCC, so as to choose a more optimal cirteria for evaluating the treatment effect. Methods: Seventy patients diagnosed as HCC had at least 3 chemoembolization procedures, and at 1 week before the procedure; 1 month; 3 months after the procedures had CT or MR examinations respectively. At least two radiologists according to RECIST1.0 and mRECIST criterion to assess the result of TACE therapy. The response rate of the two criterion was evaluated with χ2 test. Results: Use RECIST1.0 to evaluate the therapeutic effect of TACE: CR was 0, PR was 9, SD was 38, PD was 23, response rate was 12.86%. If use mRECIST for evaluation: CR was 12, PR was 15, SD was 22, PD was 21, response rate was 38.57%(χ2 test, P<0.05), There was significant differences between the two criterion. Conclusion: To use mRECIST criteria in evaluating the response rate of TACE treatment for HCC, emphasis is put on measuring the enhanced viable tumor in the arterial phase by enhanced CT, it can play a decisive role for subsequent treatment planning.

627 The value of “one-stop” comprehensive MR evaluation in living donor liver transplantation

Objective: To explore the value of "one-stop" comprehensive MR evaluation in living donor of liver transplantation(LDLT). Methods: Sixty-one living liver donors underwent "one-stop" MRI before LDLT. All the primary data were reconstructed by maximum intensity projection(MIP) in post-procession workstation. The anatomy and variation of hepatic arterial, portal venous branching, hepatic veins and biliary system were evaluated and compared with the result of surgery. Results: Of the 61 the living donors, 48 had type Ⅰ, 7 type Ⅲ, 2 type Ⅷ and 1 type Ⅸ hepatic arterial anatomy according to Michel’s hepatic artery classification. Two had dominant artery to segment Ⅳ arising from the right hepatic artery(RHA) and 1 RHA from the celiac trunk(CT). Fifty-eight had conventional portal vein anatomy and 3 had “three bifurcation” type of portal vein. 41 had normal draining pattern of the hepatic veins. Left hepatic vein(LHV) and middle hepatic vein(MHV) formed a common trunk before draining into the inferior vena cava(IVC) were found in 18 cases. 2 inferior right hepatic vein(IRHV) were detected. Fifty-eight cases had normal anatomy of the hepatic duct, 2 had “three bifurcation” type bile duct, and 1 posterior right hepatic bile duct(RPHD) draining into the left hepatic duct(LHD). The diagnostic accuracy of MR in hepatic artery, portal vein, hepatic venous and biliary system was 96.7%, 100%, 95.1% and 93.4%, respectively. Conclusion: "One-stop" comprehensive MR evaluation can successfully delineate the anatomy and variations of hepatic vascular and biliary system and can be used in the preoperation evaluation of living donor in living donor liver transplatation.

630 Preliminary study of acoustic structure quantification technology in normal adult liver

Objective: To evaluate the diagnostic value of acoustic structure quantification(ASQ) technology in normal adult liver and to analyze the factors that make the differences between the groups. Methods: Four hundred and forty-nine healthy subjects, who underwent quantitative assessment of liver tissue using the acoustic structure quantification technology, were included in the study and according to gender and age were divided into elderly group(60~80 years old), middle-aged group(40~59 years old), and youth group(20~39 years old). In each group were further divided into male and female group. ASQ quantitative parameters were analyzed, including Mode, Average, and SD. Results: There was no significant difference in Mode among the age groups(P>0.05). There were significant differences in Average and SD between female elderly group and male elderly group(all P<0.05). There were significant differences in Average and Mode between female middle-aged group and male middle-aged group(all P<0.05). There were no significant differences in Mode, Average and SD between female youth group and male youth group(all P>0.05). Conclusion: Age and gender factors play important roles in the assessment of ASQ technology.

634 The clinical value of high frequency ultrasound in the diagnosis of mesenteric lymphadenitis

Objective: To investigate clinical value of high frequency ultrasonography in the diagnosis of mesenteric lymphadenitis. Methods: The children’s abdomen were carefully scanned by using Toshiba Aplio 500 high frequency ultrasound(5~12 MHz), special attention was paid to the umbilical area and the right lower abdomen. The number, size, shape and internal echo of the mesenteric nodes were observed and recorded, meanwhile also observed whether there were other abdominal abnormalities. The longitudinal diameter of the lymph nodes above 0.5 cm was considered enlarged. Results: In 130 children with acute abdominal pain, the demonstration rate of lymphadenitis was 100%. Enlarged lymph nodes were located in the right lower quadrant or in the navel area. In 124 cases(95.4%) the number of enlarged lymph nodes more than 5. In 50 cases a second ultrasound examination were performed after two weeks, the mesenteric lymph nodes showed no change in 30 cases, size of the enlarged nodes reduced in 20 cases. Conclusion: High frequency ultrasound not only can display the pediatric mesenteric lymph node clearly, but also can provide reliable evidence for clinical diagnosis and differential diagnosis of mesenteric lymphadenitis.

638 Demonstration of rectal cancer with the extent of wall invasion as an criteria forimproving accuracy of MRI-T staging

Objective: To investigate the value of conventional T staging for rectal cancer and which combined with the extent of tumor invasion of the circumference of rectal wall in T staging for rectal cancer. Methods: The conventional T staging in 79 cases with rectal cancer and that combined with tumor invasion of the circumference of the bowel wall based on MRI were analysed and compared retrospectively. On low field MRI system, when there was difficulty in discriminating stage T2 and stage T3 rectal cancer, then try to find tumor invasion of intestinal wall circumference based on MRI. The cirteria of invasion of bowel wall circumference set for T2 and T3 was <1/2~1/2 the circumference was considered as stage T2, >1/2 of the circumference as T3. Use postoperative pathologic T staging as standard, the value of conventional T staging and which combined with invasion of the circumference of the rectal wall was compared. Results: Based on conventional T staging and that combined with invasion of the circumference of rectal wall and only conventional T staging, the Kappa pathological T staging values were 0.688 and 0.331 respectively(P<0.05). The parameters for staging≤T2 and T3/T4 based on tumor invasion of rectal circumference in T staging are superior to those based on conventional staging alone, the former AUC(area under the ROC curve) value is higher than the latter(0.905 vs 0.662, P<0.05). Conclusion: The conventional T staging for rectal cancer combined with invasion of the circumference of rectal wall can improve the accuracy of T staging for rectal cancer, especially improving the discrimination between T2 and T3 so as to provide effective information for clinicians and surgeons.

642 The ultrasonographic features and pathological characteristics of ovarian malignant tumor

Objective: To study the value of the ultrasonography in the diagnosis and differential diagnosis of malignant ovarian tumors and the coincidence rate with pathological diagnosis. Methods: 185 patients with ovarian malignant tumor got resection. The ultrasonographic characteristics of ovarian malignant tumors before surgery were reviewed and correlated with pathological results after surgery. Results: A total of 185 patients with 195 tumors were collected into this study. Ultrasound showed cystic lesions in 6 (6/195), accounted for 3%; cystic-solid lesions in 141(141/195), accounted for 72.3%; mainly solid 48(48/195), accounted for 24.7%; the ultrasonic characteristics of ovarian malignant tumors were correlated with the postoperative pathological diagnosis. In malignant ovarian tumor there was rich vascular supply, multiple-vascellum distributed diffusively in the tumor or internal parenchymal ingredients, demonstrated as fasciculation, dendritic or ball shape vascular network, 2~3 grade disorder bloodstream in the center of parenchymal, the coincidence rate of the image features with postoperative pathological diagnosis was 95.7%. Conclusion: The coincidence rate of the ultrasound images of ovarian malignant tumors is high. Ultrasonography has important diagnostic value, women in reproductive age should carry out routine ovarian ultrasound inspection regularly.

645 Clinical significance of measuring lumbar spinal stenosis on MRI

Objective: To analyze the cause and morphologic characteristics of lumbar spinal stenosis in the guidance of treatment planning. Materials and Methods: Altogether 120 patients including 62 males and 58 females, age ranged from 45~76 years old, with an average of 57.2 years old. The patients were divided into 4 groups, that is 29 cases in the 40~49 years old group, 42 cases in the 50~59 years old group, 33 cases in the 60~69 years old group, and 16 cases in the 70~79 years old group. All the patients had MRI examinations of the lumbar spine. The morphologic characteristics of spinal laminae, spinal canal, ligamentum flavum, and articular joints were observed. The lamina thickness, anterior-posterior diameter of the spinal canal on sagittal view and transverse diameter of spinal canal on coronal view were measured. The thickness of ligamentum flavum was measured on axial view at the level of L2/L3, L3/L4, L4/L5 articular joints. The changes of the measured parameters at different ages and different spinal segments were analyzed. Results: The type of spinal stenosis: central type 27 cases, lateral recess stenosis 35 cases, mixed type 58 cases, proliferation of articular joints: unilateral 33 cases, bilateral 87 cases; the shape of the spinal canal: “triangle” 76 cases, cloverleaf 44 cases. The average thickness of ligamentum flavum at L2/L3 was 2.4 mm, at L5/S1 was 3.1 mm, which was thinner than 3.7 mm at the level of L3/L4, at the level of L4/L5 was 4.5 mm. The thickness of ligamentum flavum increased with increasing age, the increase of thickness was most obvious at the level of L4/L5. The thickness of spinal lamina was 4.5 mm at L5, which was the minimum and 5.8 mm at L3 which was the maximum. The transverse diameter of the spinal canal on coronal view was 4.7 mm at the level of L4/L5 which was the minimum, and maximum of 10.3 mm at the level of L2/L3. The changes of these parameters was not obvious with increasing of age. As the thickness of ligamentum flavum increased, the transverse diameter of spinal canal on coronal view decreased. Conclusions: ①Increase thickness of ligamentum flavum caused the decrease of the transverse diameter of the spinal canal on coronal view, bilateral articular joint especially the superior articular joint hyperplasia with inner aggregation is the main factor causes spinal stenosis. ②During operation should increase the transverse diameter of the spinal canal on coronal view, excise the thickened ligamentum flavum and the hyperplastic part of the articular process, expand the lateral recess, and preserve the spinal process complex.

648 The value of chemical shift MRI for evaluating osteoporosis

Objective: To investigate the value of MRI chemical-shift sequences in evaluating fat content of bone marrow for staging of osteoporosis. Materials and Methods: Seventy patients were given dual energy X-ray absorptiometry examination(DXA), conventional MRI, and chemical shift MRI. According to the T-score of DXA at the lumbar spine and hip, the patients were divided into normal bone density, osteopenia and osteoporosis groups. In order to calculate fat content in bone marrow, the signal intensity of lumbar verbebral bodies(L1~L4) was measured on in-phase and out-of-phase sequences of chemical-shift MRI. Fat content was calculated through signal decline index of the vertebral bodies. The quantitative values were compared with these results of bone density obtained from DXA examination at the lumbar spine and the hip. Correlation analysis was conducted between the signal decline index of the vertebrae and bone density values at lumbar and femoral neck respectively. Results: According to the groups divided by DXA at the spine, the median signal decline index of the vertebral bodies of the 3 groups, normal, osteopenia and osteoporosis groups, was 59.73%(21.84%~72.35%), 59.64%(21.55%~78.42%), 57.04%(34.77%~77.30%) respectively. There were statistical differences between osteoporosis and normal group(P<0.001), and between the osteoporosis and osteopenia group(P<0.001), no statistical difference between normal and osteopenia group(P=0.685). According to the groups divided by DXA at the hips, the median vertebral signal decline index of the three groups was 62.41%(21.84%~77.63%), 58.39%(40.96%~78.42%), and 54.76%(21.55%~77.30%) respectively. There was statistical difference among the groups (P<0.001). The signal decline index of lumbar vertebrae was significantly positively correlated to the value of femoral neck bone mineral density(r=0.351, P=0.004). There was no statistically significant correlation(P=0.379) between the value of lumbar spine mineral density and lumbar spine signal decline index. Conclusion: Vertebral fat content changes in bone marrow of patients with osteoporosis can be evaluated by measuring verbebral signal decline index on chemical shift sequences of MRI which has important and high value in clinical application.

652 The clinical effectiveness of stent placement in the treatment of superficial femoral artery obliterans:a 3-year follow-up experience

Objective: To evaluate the clinical effectiveness after stenting of superficial femoral artery(SFA) obliterans in 3-year follow-up, and to evaluate the relevant factors affecting therapeutic effect. Methods: Seventy-nine patients were diagnosed as superficial femoral artery obliterans and received stent placement from January 2010 to December 2012 in the interventional department, Shengjing Hospital. The clinical data after stenting were retrospectively analyzed. All the patients suffered with different degrees of lower extremity ischemic symptoms. The criteria for observation was primary patency with limb save at 6th, 12th, 24th, and 36th months after stent placement and the restenosis rate. The multiple factor analysis of the survival and effective rate was performed. Results: All the 79 patients received balloon expanding with stent placement and postoperative angiography showed 100% technical success. All the patients received Rutherford classification with the preoperative average score of 3.60±0.89, and postoperative score of 1.41±1.18 at 36th month, there was statistical significance difference. The recurrence of ischemic symptoms was seen in 27 limbs within one month to 3 years after treatment. No recurrence, only in 8 patients who did not have good control of diabetes and 10 still continued smoking. Conclusion: Endovascular treatment with stent plantation for 79 cases of superficial femoral artery obliterans was effective and safe at postoperative within 3 years. The result of primary patency had statistical significance between the length of the occluded SFA segment(5~15 cm) and (>15 cm). Effective control of diabetes and smoking, are the main factors which affect therapeutic results.
