309 The value of different MR sequences in the diagnosis of hand-foot-mouth disease complicated with encephalitis

Objective: To investigate MRI with different sequences in the diagnosis of encephalitis in hand-foot-mouth disease, and correlated with pathology. Methods: In 100 children with hand-foot-mouth disease who had early clinical manifestations of encephalitis underwent axial routine T2WI, T1WI, T2FLAIR, DWI sequences, 56 cases had MRI positive expression, in the 56 cases, 30 cases in the first MRI scan, 13 cases had a second repeat MR examination on the second week(13/30), 17 cases(17/30) had a repeat MR examination on the third week, scanning sequence the same as above. The detection rate was compared between 4 imaging sequences in different periods of disease. Results: In 56 cases there were 147 lesions, the lesion was symmetrical on both sides, MRI showed slightly low signal/low signal on T1WI, and on T2WI, T2FLAIR, DWI showed slightly high signal/high signal mottled shadow, blurred edges, some lesions on T2FLAIR or DWI sequence showed more clearly. T1WI showed 50(50/147), T2WI showed 65(65/147), T2FLAIR showed 120(120/147), DWI showed 109(109/147). DWI and T2FLAIR compared with T2WI, T1WI(P<0.05), T2FLAIR compared with DWI, T1WI compared with T2WI(P>0.05). Thirty cases on repeat MR examination showed the 73 lesions. T1WI showed 55(55/73), T2WI showed 57(57/73), T2FLAIR showed 68(68/73), DWI showed 15(15/73). T2FLAIR compared with DWI(P<0.05), DWI compared with T1WI, T2WI(P<0.05), T2WI compared with T2FLAIR, T1WI compared with T2WI(P>0.05). Conclusion: The combination of T2FLAIR, DWI sequence has important clinical value early diagnosis in for children with hand-foot-mouth disease complicated with encephalitis, T2FLAIR has important clinical value in the diagnosis of children with hand-foot-mouth disease complicated with encephalitis recovery. T2FLAIR, DWI can be used as the preferred sequence for hand-foot-mouth disease complicated with encephalitis.

313 Analysis of the relationship between large vestibular aqueduct syndrome and hearing by SCT

Objective: To evaluate the relationship between large vestibular aqueduct(LVA) and hearing loss, also obtain the threshold of LVA on coronal SCT of temporal bone. Methods: The SCT features of temporal bone from patients with enlarged vestibular aqueduct in the third affiliated hospital of Zhengzhou university were studied retrospectively. The midpoint diameter and width at the outlet of vestibular aqueduct were measured both on the axial and coronal SCT in normal individuals as well as in patients. The hearing tests from two different times were also performed to assess the evalution of hearing loss in patients with LVA. Results: In patients, the average midpoint diameter of the aqueduct was (2.4±0.93) mm and the average width of the aqueduct outlet was (6.21±1.4) mm on transverse view, the midpoint average diameter was (5.48±1.76) mm and the average width of the outlet was (6.87±1.61) mm on coronal view. The threshold value of the transverse diameter of the aqueduct for diagnosis of LVA was 2.5 mm, and the width of the outlet of the aqueduct was 4.25 mm on coronal view. There were no significant differences of both the air conduction and bone conduction from the two different times in patients, P>0.05. Follow-up study showed that 64.3% patients had mild to moderate hearing loss, only 2 cases had hearing improvement, 5 cases had hearing loss. There was no correlation of the vestibular aqueduct diameter with the bone conduction threshold value, while there was strong correlation between air conduction threshold and vestibular aqueduct diameter at low frequencies. Conclusion: Air conduction is the main factor in LVA patients with conductive or mixed hearing loss. SCT coronal scans can provide additional information to assess the LVA, especially when the axial scan diagnosis is insufficient.

318 3.0T MR examination on parotid gland in Sjogren’s syndrome

Objective: To evaluate the MRI characteristics of parotid gland in Sjogren’s syndrome(SS). Materials and Methods: MRI features of parotid gland were studied in 22 patients with SS(Group A) and 21 controls without SS(Group B). Analyze the MRI features of parotid gland in each group, calculate the volume of each parotid gland, to summarize the MRI special manifestations of parotid gland in SS. Results: The typical MRI appearance of the parotid gland in group A was characterized by a loss of homogeneity in signal intensity on T1- and T2-weighted images with speckled to nodular structures in the parotid gland, some glands developed fatty degeneration. The mean volume of parotid gland in group A was smaller than that in group B. Conclusions: MRI of parotid gland with SS has typical appearance, and MRI can provide objective imaging evidence for diagnosing the disease.

321 Clinical application of prospective electrocardiographic-gated dual-source CT coronary angiography

Objective: To evaluate the value of clinical application of prospectively electrocardiography(ECG)-triggered sequential acquisition for coronary computed tomographic angiography by dual-source computed tomography. Methods: The images of prospective ECG-triggered sequential acquisition for coronary computed tomographic angiography by dual-source computed tomography of 114 patients were analyzed, 18 of them underwent coronary angiography within two weeks after CT angiography. Results: The mean heart rate of the 114 patients was (63±8) beats per minute(bpm). Image quality average score was (1.65±0.59). The rate of excellent and good image quality and rate of assessable patients was 90.35% and 99.12%. 1 297 coronary artery segments were explored. The rate of assessable segments was 96.53%. The average effective radiation doses was (6.27±1.81) mSv. Thirty-eight patients were diagnosed as coronary artery disease by dual-source CT. Forty-eight stents of 21 patients were evaluated. Forty-three stents were easy and smooth. Four stents were in-stent restenosis. One stent was in-stent occlusion. Set the results of coronary angiography as criteria, the sensitivity, specificity, positive predictive value and negative predictive value of dual-source CT coronary angiography for the diagnosis of coronary artery were respectively 95.45%, 96.00%, 91.30% and 97.96% in 72 arteries of 18 patients. Conclusion: Prospective electrocardiographic-gated transverse dual-source CT is a kind of safe, effective, low-dose and noninvasive CT coronary angiography. It is also a valuable modality for the follow-up of coronary stent patency.

325 Investigation of 3D-STI for the assessment of left ventricular regional longitudinal systolic function in patients with different graded coronary artery stenosis

Objective: To investigate the feasibility of three-dimensional speckle tracking imaging(STI) for the assessment of left ventricular regional longitudinal systolic function of patients with different graded coronary artery stenosis. Materials and Methods: One hundred and ten patients suspected of coronary heart disease(CHD) in Beijing Hospital of the Ministry of Health were selected from June, 2010 to Aug, 2012. Men 68, women 42, average age was (63.2±8.9) years old. All patients received the examination of regular and three dimensional echocardiography before the CAG. The materials of the three dimensional all volume dynamic cardiac graph were collected and stored. The 3D dynamic graph using the 3D-STI offline were analyzed. The systolic peak longitudinal strain(LS) and the time of peak longitudinal strain(T-LS) of the 16 myocardial segments of the left ventricle(consist of 6 segments at the level of mitral orifice, 6 segments at the level of papillary muscles and 4 segments at apex section) were obtained. According to the results of the CAG, the subjects were divided into 5 groups: normal group(stenosis extent=0%), slightly narrowed group(0%<stenosis extent≤25%), mild narrowed group(25%<stenosis extent≤50%), moderate narrowed group(50%<stenosis extent≤75%), severe narrowed group(stenosis extent>75%). Using statistical software of SPSS 16.0 to compare the peak LS strain value and T-LS in different graded coronary artery stenosis. Results: In different grade coronary artery stenosis groups, LS of part of the myocardial segments corresponded to the lesion vessel were descending, while T-LS were ascending along with the increasing of the grade of setenosis, especially on the severe narrowed group. Conclusion: 3D-STI is feasible for the assessment of left ventricular regional longitudinal systolic function in patients with different graded coronary artery stenosis and can provide some supports for the diagnosis of CHD by noninvasive technique.

329 Evaluation of left ventricular long-axis diastolic function in patients with essential hypertension associated with left ventricular hypertrophy using real-time triplane strain rate imaging

Objective: To evaluate left ventricular(LV) long-axis diastolic function in essential hypertension(EH) patients with hypertrophy using real-time triplane strain rate imaging. Methods: Sixty-two EH patients and 35 age-matched healthy subjects as control group were included. The patients with hypertension were divided into 2 groups: 32 patients without LV hypertrophy(NLVH) and 30 patients with LV hypertrophy(LVH). Using real-time triplane stain rate imaging measured early diastolic peak strain and late diastolic peak strain rate(SRe and SRa) at basal and middle segments in different LV walls. Results: SRe of all wall in LVH and NLVH group were significantly lower than that in the control group(P<0.05 or P<0.01), significant difference was found between LVH and NLVH group, SRa of lateral wall(LW) and posterior wall(PW) was lower in LVH than that in the control group(P<0.05). Conclusions: Real-time triplance strain rate imaging can quantitatively analyse LV long-axis diastolic function accurately and objectively in EH patients with LVH.

332 The value of the SPECT/CT fusion images in the diagnosis of the chest wall bone invasion of lung cancer

Objective: To evaluate the value of SPECT/CT fused images in the diagnosis of the chest wall bone invasion of lung cancer. Methods: The imaging data of 47 cases of patients who underwent MSCT and SPECT/CT were retrospectively analyzed. The detection rate of different methods was compared. Resluts: MSCT for diagnosis of the chest wall bone invasion of lung cancer had a sensitivity of 68.3%(28/41), specificity of 100%(6/6), accuracy of 72.3%(34/47). SPECT/CT for diagnosis of the chest wall bone invasion of lung cancer had a sensitivity of 97.6%(40/41), specificity of 33.3%(2/6), accuracy of 89.4%(42/47), and there was statistical significance(P<0.05). Conclusions: The SPECT/CT fusion images not only obviously improve the detection rate of the chest wall bone invasion of lung cancer, but the missed diagnostic rate are also lower than MSCT, so have very good value.

336 CT features of primary pulmonary malignant fibrous histiocytoma

Objective: To identify CT characteristics of primary pulmonary malignant fibrous histiocytoma(MFH). Materials and Methods: CT images and clinical data of 15 patients with primary pulmonary MFH proved pathologically were reviewed and analyzed retrospectively. Results: Among 15 patients, MFH appeared as peripheral lesions in 12 cases, located in left lobe 7 cases, right lobe 5 cases and central 3 cases. A total of 20 lesions were identified, in which 12 had single lesion and 3 had mutiple lesions, with mean a diameter of 8.5 cm and ranged between 3.5~15.4 cm, including 12 round-shaped and 8 mild lobulated lesions. Among all the 20 lesions, 14 were well-defined and the others had unclear border, while 5 masses were homogenous in density and the other 15 were inhomogenous with necrosis, cysts, bleeding or calcification. Twelve patients underwent enhanced CT, 17 lesions had various degress of enhancement in arterial phase and venous phase. Six lesions involved pleura and chest wall(among which 3 involved ribs), 1 involved pulmonary artery, 1 involved pulmonary vein, 2 involved pericardium, as well as 5 metastasis on pulmonary hilar or mediastinal lymph node, 4 distant metastasis. Conclusion: Primary pulmonary MFHs have some specific CT manifestations which are helpful to the diagnosis, but final diagnosis depends on the cytopathology and immunohistochemistry.

340 Diagnostic value of percutaneous transhepatic cholangiography on bile duct tumor thrombus

Objective: To analyze percutaneous transhepatic cholangiography(PTC) imaging features of bile duct carcinoma thrombus. Materials and Methods: PTC images of 27 patients(the mean age was 49.81±7.27 years old) with histologically proved to have bile duct carcinoma thrombus were reviewed retrospectively. All 27 PTC were performed under the guidance of ultrasound or DSA by selective unilateral or bilateral intrahepatic bile duct puncture. Internal or external drainage was kept until repeated PTC was performed after 3~5 d to observe the imaging changes. Results: Filling defects in bile ducts were seen in all 27 patients. The filling defects and involved bile ducts showed expanding appearance in 26 cases. Proximal bile ducts to the involved parts were dilated like soft canes in 24 cases and fallen branches in 3 cases. Distal bile ducts showed no dilation in any of the case. The contours of filling defects were clearly shown in 9 cases and partially shown in 18 cases. Twenty-one cases showed constant filling defects while 6 cases had shrunken filling defects after drainage. Three types were defined according to the involved range: Type Ⅰ, 2 case; Type Ⅱ, 22 cases; Type Ⅲ, 3 cases. Conclusion: Bile duct tumor thrombus have some typical PTC imaging features, PTC is useful in detecting and diagnosing bile duct tumor thrombus.

344 Research on producing fully automated one-step (11C) labeled water channel protein inhibitor, the imaging agent

Objective: To research and synthesize the fully automated one-step (11C) labeled water channel protein(AQP) inhibitor, the imaging agent. Materials and Methods: Using the TRACERlab FXC synthesis platform, under 100℃ reaction conditions in alkaline conditions (11C) CH3I reacted with acetazolamide. Then using sodium biphosphate as eluent, through high performance liquid chromatography(HPLC), (11C)N-methylacetazolamide was isolated from reaction solution. Results: The synthesis efficiency of the agent was above 50%, the radiochemical purity was over 98%, the time for the whole producing process was only about 30 min. After intravenous injection of (11C)N-methylacetazolamide into the tail vein of rat, and performing PET/CT scanning, the distribution of the above agent can be seen in rats, after injection of (11C)N-methylacetazolamide and N-methylacetazolamide, the inhibitor, the (11C)N-methylacetazolamide decreased significantly in the rat is liver. Conclusion: This is the first study and report on development of the fully automated one-step (11C) labeled water channel protein(AQP) inhibitor. The (11C)N-methylacetazolamide can be used as AQP PET tracer agent.
