761 Correlation study of MRS diagnosis of diffuse axonal injury with clinical prognosis

Objective: To study the imaging features of brain diffuse axonal injury(DAI) by magnetic resonance spectroscopy(MRS), and explore the value of MRS in the diagnosis of DAI and severity assessment. Materials and Methods: Thirty patients with DAI(DAI group) and 10 healthy volunteers(control group) undertook 1H-MRS scanning, to assess the metabolite levels of N-acetyl aspartic acid(NAA), choline(Cho), creatine(Cr), combined with Glasgow coma scale(GCS) and Glasgow outcome scale(GOS) score for correlation analysis. Results: Compared with control group, NAA decreased, Cho increased, NAA/Cho, NAA/Cr value decreased and Cho/Cr value increased in DAI group. The change of NAA/Cr, NAA/Cho, Cho/Cr value showed correlation with GCS and GOS in patients with DAI. Conclusion: MRS can quantitatively measure the brain tissue metabolism of patients with DAI, showing important value for the diagnosis and prognosis evaluation of the severity of DAI.

764 Research in the relationship between the contrast-enhanced ultrasound images and expression of C-erbB-2, nm23 in breast cancer

Objective: To discuss the relationship between the contrast-enhanced ultrasound(CEUS) images of the breast cancers and the molecular biology markers of C-erbB-2 and nm23. Methods: The imaging data of CEUS in breast cancer, C-erbB-2 and nm23 immunohistochemical results after operation were analysed retrospectively. Correlation between CEUS indexes or parameters and the expression of C-erbB-2 and nm23 were also studied. Results: ①The positive expression rate of C-erbB-2 and nm23 in breast cancer were 66.7%(46/69) and 27.5%(19/69) respectively. The positive expression of nm23 in the group with C-erbB-2 positive expression was 13%, which was higher than that in the group with C-erbB-2 negative expression. ②In the group with C-erbB-2 positive expression, lesions showed high enhancement, heterogeneous enhancement or lesions with perfusion defect area, peripheral radial enhancment, relative rising slope and relative peak intensity showed significant differences in the time intensity curves, compared with the group with C-erbB-2 negative expression(P<0.05). ③In nm23 negative group, the lesion showed high enhancement, peripheral radial enhancement, and the time intensity curve showed higher relative rising slope with statistical significance(P<0.05) compared with the nm23 positive group. Conclusion: Some contrast-enhanced ultrasound features correlated with C-erbB-2 and nm23 expression in breast cancer, which could be useful to predict the prognosis of patients to some extent.

768 The CTA manifestations and classification of anomalous origin of coronary artery

Objective: To investigate the role of CTA examination in displaying the anomalous origin of coronary artery, and showing the classification and the clinical significance based on CTA in detecting this disease. Materials and Methods: Coronary CTA examinations were performed in 25 596 cases by 64 multi-slice spiral CT. Volume rendering, multi planar and curved surface reconstruction and maximum intensity projection were used to analyze the origin of the coronary artery. Results: The anomalous origin of coronary artery was found in 263 cases, including 171 cases of anomalous origin of right coronary artery, 86 cases of anomalous origin of left coronary artery, and 6 cases of double sinus malformation. According to the comprehensive analysis of the classification and the clinical manifestation of this disease, we modified the previous classification. Conclusion: CTA could display various types of anomalous origin of the coronary artery clearly, showing great importance in guiding clinical practice.

772 A preliminary study of improving imaging quality on hepatic venography with low voltage andlow concentration of contrast media by energy CT

Objective: To investigate the feasibility of energy CT with low concentration of contrast media and low voltage for improving imaging quality of hepatic vein on hepatic venography. Materials and Methods: 119 patients with body mass index(BMI) lower than 22 kg/m2 who were suspicious of abdominal diseases were recruited in this prospective study and underwent tripple-phase enhanced CT scan of adbomen from March to December 2013 in our hospital. According to the scan time, the patients were randomly divided into two groups: group A(September to December 2013) with high concentration of contrast medium(350 mgI/mL) and routine tube voltage(120 kVp), including 41 cases(male 26 cases, female 15 cases, aged 59.20±11.69, group B(March to August 2013) with low concentration of contrast medium(270 mgI/mL) and low voltage(80 kVp), including 78 cases(male 55 cases, female 23 cases, aged 61.96±11.90). At portal phase, 1.25 mm-slice conventional images and optimal monochromatic images were obtained, and reconstruction was conducted by volume rendering(VR) and maximum intensity projection(MIP). The image quality of hepatic venography and the intrahepatic branches by MIP and VR was evaluated in a double-blind manner on a five-point scale by two radiologists with 8 years and 6 years of experience in abdominal diagnostic radiology. Two independent sample T-test was used to compare the CT value of hepatic veins and the liver parenchyma, the image noise, the vein-to-liver contrast-to-noise ratio(CNR) and the figure of merit(FOM) in the two groups. Inter-observer agreement was assessed by Kapppa value in image quality scoring. This study was approved by the institutional review board of our hospital. Results: The Kappa value of the image quality of left, middle and right hepatic veins by portal venography was good in group A(0.808, 0.852, 0.851, respectively) and group B(0.923, 0.937, 0.899, respectively). The CT values of the hepatic veins and liver parenchyma, image noise, CNR, FOM in group A were significantly lower than those in group B. Conclusion: Monochromatic images of energy CT with low concentration of contrast media and low voltage are feasibile for improving imaging quality of hepatic veins.

776 CT and MRI findings of intrahepatic biliary papillomatosis

Objective: To investigate the clinicopathological features and the imaging characteristics of intrahepatic biliary papillomatosis(IHBP). Methods: Five patients with surgically and pathologically confirmed IHBP were included in the study. The imaging findings and pathological features were analyzed with the review of literature. Results: The clinical symptoms of these cases included intermittent obstructive jaundice(2 cases), abdominal pain and fever(1 case), and left upper quadrant pain(1 case). One asymptomatic patient was detected during a routine examination. MRCP images showed dilatation of the entire biliary tree in all 5 cases. There was no mass or biliary stone in the distal common bile duct. When the tumor involved a segment or a hepatic lobe, the degree of bile duct dilatation was thus particularly severe and the involved bile ducts dilated cystically like an aneurysm. Multiple round, linear and papillary signal voids were presented within the lumen or attached at the wall of the bile duct on MRCP. Three cases located in the right intrahepatic bile duct, 2 in the left intrahepatic bile duct. The papillary tumors had a diameter range of 0.3 to 2.5 cm. The intraductal papillary tumors appeared as hypo- or isoattenuated soft tissue masses within the dilated duct compared with the hepatic parenchyma, and MRI images showed hypo- or isointense lesions on T1WI, hyper- or isointense lesions on T2WI, hyperintense linear or papillary lesions on DWI. The lesions demonstrated slight enhancement on contrast enhanced imaging. Conclusion: The CT and MRI findings of IHBP can show some of its imaging characteristics, especially on MRCP, DWI and contrast enhanced images, which are helpful for its diagnosis and differential diagnosis.

781 The value of multimodal MRI to identify solid benign and malignant splenic tumors

Objective: To explore the value of multimodal MRI to identify solid benign and malignant splenic tumors. Materials and Methods: The clinical and medical imaging data of 21 cases of benign splenic tumors and 14 cases of malignant tumors were analyzed retrospectively. Scanning sequences included: T1WI, T2WI, liver acquisition with volume acceleration(LAVA), diffusion weighted imaging(DWI) and enhanced T2 star weighted angiography(ESWAN). The phase, magnitude and R2* maps were generated from the original ESWAN data. The signal intensity on T1WI, T2WI, DWI, and in phase, magnitude, R2* maps as well as tumor characteristics on LAVA sequence of the two groups were tested by Chi-square test. The values of apparent diffusion coefficient(ADC), phase and R2* of the two groups were measured by two observers, and the consistency measured by two observers was tested by the intra-class correlation coefficients(ICC). Mann-Whitney U test was used to compare various parameters of the two groups. Receiver operator characteristic(ROC) curve was used to calculate the cut-off value of ADC, phase, R2* and performance of ADC, phase, R2* value for distinguishing these solid benign and malignant tumors. Results: The signal intensity of the solid benign and malignant tumors on T1WI and T2WI had no difference. However, the signal intensity of malignant tumors on DWI, phase, magnitude on ESWAN and R2* maps had more complicated mixed signal intensity; the two groups showed differences in the enhanced pattern on LAVA sequence, but with overlap. The ADC values, phase values and R2* values of the two groups obtained from the two observers were in good consistency(ICC value >0.75. The ADC of the benign and malignant tumors were (1.80±0.44)×10-3 mm2/s and(1.40±0.40)×10-3 mm2/s with phase of 0.071±0.037 and -0.003±0.119, R2* values of (13.46±9.00) Hz and (42.09±19.82) Hz respectively, all showing statistically significant differences(all P<0.05). The AUC of the ADC, phase and R2* values were 0.754, 0.721 and 0.941, and the best cut-off values for ADC, phase and R2* in characterizing benign and malignant tumors were 1.23×10-3 mm2/s, 0.440 and 20.47 Hz, respectively. Conclusion: The solid malignant splenic tumors show more complicated signals than benign tumors on DWI and ESWAN. The ADC, phase and R2* values can be used as quantitative indexes to identify benign and malignant splenic tumors, and R2* values were more valuable.

787 Diagnostic value of 18F-FDG PET/CT imaging in primary small intestine tumors

Objective: To investigate the diagnostic value of PET/CT with fluorine-18-labeled fluorodeoxyglucose(FDG) for primary small intestine tumors. Method: 18F-FDG PET/CT imaging findings were retrospectively analyzed in 38 cases with the small intestine diseases, of which 37 cases were proved by pathology after operation or biopsy and one case was confirmed after a 3-year follow-up. The images were interpreted and compared with the histopathologic findings to assess the value of 18F-FDG PET/CT in detecting primary small intestinal malignant tumors. Results: Among 38 patients, 33 cases had high FDG uptake, 87.9%(29/33) of which were small intestinal malignant tumors. There was no statistical significance in SUVmax value between malignant neoplasms and benign lesions. The main manifestations of small intestinal tumors were focal thickening of intestines and soft tissue masses on CT images. Three cases were false positive and one case was false negative. The sensitivity, specificity, diagnostic accuracy, false positive rate and false negative rate of 18F-FDG PET/CT were 96.8%, 57.1%, 89.5%, 42.9% and 3.2%, respectively. Conclusion: 18F-FDG PET/CT was of great value for the diagnosis of primary small intestinal tumors.However, the benign lesions with high uptake and the malignant lesions with low uptake were easy to be misdiagnosed, demanding more attention for diagnosis.

793 Acute and chronic appendicitis: comparison of ultrasonography with CT in pediatric patients

Objective: To compare the diagnostic value of ultrasonography(US) with CT for pediatric acute and chronic appendicitis. Methods: Clinical data for 134 patients pathologically verified appendicitis at the age of 5 to 12 years old were retrospectively analyzed to compare the value of preoperative CT(n=134) and US(n=134) in the diagnosis and antidiastole of this disease from January 2013 to March 2014. The sensitivity of the two modalities was compared in the diagnosis of acute appendicitis and chronic appendicitis, acute simple appendicitis and acute non-simple appendicitis. Results: The diagnostic value of CT for acute appendicitis was significantly better than that of US. The sensitivity of US vs. CT were 69.75% vs. 81.51% in diagnosing acute appendicitis, respectively. Higher sensitivity was shown by CT in diagnosing acute simple appendicitis compared with US(84.38% vs. 62.50%, respectively; P<0.05) while no difference was found in diagnosing acute non-simple appendicitis(87.36% vs. 82.76% respectively, P>0.05) or chronic appendicitis(66.67% vs. 40.00% respectively, P>0.05). Conclusions: Ultrasonography can be considered as the first choice for the diagnosis of pediatric acute or chronic appendicitis. When US revealed negative or equivocal results with progressively aggravated symptoms, a CT scan was then needed.

797 Sonographic findings of low-grade endometrial stromal sarcoma

Objective: To summarize the sonographic characteristics of low-grade uterine endometrial stromal sarcoma(LGESS) for reducing the preoperative misdiagnosis rate. Methods: Sonographic findings were retrospectively analyzed in 11 cases with LGESS confirmed by surgery and pathology. Results: The tumors appeared as intramural masses in 7 cases and endometrial masses in 4 cases. The maximum diameter was 20 mm to 130 mm with a mean diameter of 50.7 mm. The lesions were oval(n=8), irregular (n=2) or almost round(n=1). The margins were smooth(n=7) or ill-defined(n=4). Eight lesions were heterogeneously hypoechoic and 3 were heterogeneously intermediate echoic. There were 3 types of ultrasonic menifestations of LGESS: patchy echogenicity(n=6), with tiny cystic spaces(n=3) and hyperechoic center with hypoechoic surroundings(n=2). The echo texture of the original lesions was different from their complications in 4 cases. Blood flow was seen by color Doppler imaging in all cases with resistance between 0.41~0.78. Conclusion: The sonographic charactersitics of LGESS included ill-defined margin with possible infiltration to the myometrium, heterogeneous patchy echogenicity, tumor echo different from the complications and a medium vascularity. When detecting these findings in premenopausal women, especially with hypermenorrhea, the diagnosis of LGESS should be considered with very high probability.

800 Imaging characteristics of tuberous sclerosis complex with multiple system impairment

Objective: To analyze the imaging characteristics of tuberous sclerosis complex(TSC) with multiple organ impairment in multiple systems which might help improving the diagnostic efficacy. Methods: Clinical presentation and imaging data of 10 TSC with multiple organ impairment confirmed in clinic were retrospective analyzed, so as to study the extracranial manifestations and pathological types. Results: All the 10 cases were accompanied with typical subependymal nodules, one with subependymal giant cell astrocytoma. Bilateral multiple renal angiomyolipomas(AML) were proven in seven cases with one case of bilateral multiple cysts found concomitantly, intratumoral aneurysm in one case, multiple hepatic AML in one case, pulmonary lymphangioleiomyomatosis in two cases, cardiac rhabdomyosarcomas in two cases with one had congenital anal malformation, pancreatic endocrine neoplasm in one case and knee subcutaneous fibroma with joint effusion in one case. Conclusion: TSC involves multiple organs besides subependymal nodules. Hence, better understanding of these imaging manifestations permits a more specific and accurate diagnosis.
