685 The role of differential diagnosis between radiation necrosis and glioma recurrenceusing multi-modality magnetic resonance imaging

Objective: To investigate the value of DTI, T2*-PWI, 1H-MRS in the differential diagnosis between the glioma recurrence and radiation necrosis. Methods: The imaging features on the above sequences in 15 cases of radiation necrosis and 9 cases of glioma recurrence were studied retrospectively. The nrCBV values, Cho/Cr, NAA/Cr, and Cho/NAA ratios in the enhancing part, peri-local edema, and contralateral normal white matter were measured, and the nerve fiber of the lesions and normal contralateral white matter was reconstructed. Results: The nrCBV values, Cho/Cr and Cho/NAA ratios in the enhancing part and peri-local edema of glioma recurrence were higher than those of radiation necrosis. The sensitivity and specificity of the Cho/Cr ratios in the enhancing part were 93.3% and 100%, and in the peri-local edema, they were 66.7% and 88.9%. The Cho/NAA ratios, in corresponding location, were 86.7% and 100%, 73.3% and 77.8%. There were no statistically differences in the NAA/Cr ratios(P>0.05). Conclusion: The multi-modality magnetic resonance imaging, including DTI, T2*-PWI, 1H-MRS can significantly improve the differential diagnosis between glioma recurrence and radiation necrosis.

691 Preliminary study of 3D magnetic susceptibility weighted imaging in the diagnosis of cerebral venous malformation

Objective: To evaluate the display ability of 3D susceptibility weighted imaging(SWI) on cerebral veins(sinuses). And to evaluate the value of 3D SWI in the diagnosis of cerebral venous malformation(CVM). Methods: Twenty healthy volunteers were examined by 3.0T MR, including T1WI, T2WI and 3D SWI. The vascular image quality of cerebral veins on T1WI, T2WI and SWI images were analyzed on several preset anatomical regions. The display of veins was classified into 4 grades(grade 0~grade 3). The difference among T1WI, T2WI and SWI in venous display quality was evaluated. Thirty patients of CVM were examined by 3.0T MR, including conventional MRI(T1WI, T2WI), 3D SWI and T1WI contrast-enhanced scan. The display of venous malformations was classified into 4 grades(grade 0~grade 3). The difference among conventional MRI, SWI and enhanced MRI in displaying CVM was evaluated. Results: In 20 healthy volunteers, SWI, T1WI and T2WI were almost the same in displaying the great cerebral venous system(straight sinus, superior sagittal sinus, inferior sagittal sinus, transverse sinus, sigmoid sinus, the great cerebral vein). The difference was not statistically significant(P>0.05). For the displaying of cerebral deep veins(internal cerebral vein, the basal vein) and small drainage veins(striate vein, the septum pellucidum vein, island vein), SWI was superior to T1WI and T2WI. The difference was statistically significant(P<0.05). In 30 cases of CVM, the display quality of conventional MRI, SWI and T1WI enhanced scan showed significant difference(P<0.05), and furthermore comparison between two of them showed that SWI and T1WI enhanced scan obtained consistent display effect. The difference between the two techniques showed no statistical significance(P>0.05). The two sequences were better than conventional MRI. The difference was statistically significant(P<0.05). SWI showed more details than T1WI enhanced scan. Conclusion: SWI is a more sensitive, feasible and effective method to show cerebral veins. It can be used as a routine sequence for cerebral venous system. SWI is highly sensitive to CVM and can be used as the preferred method for CVM instead of enhanced MRI.

696 MSCT features of thyroid nodules and correlation with pathology

Objective: To summarize the MSCT features of 105 cases of thyroid nodules and the correlation with pathology, and to explore the CT signs of benign and malignant thyroid nodules. Methods: The CT features of 105 cases of thyroid nodules(220 nodules) were analyzed retrospectively including density of nodules, cystic formation, degree of enhancement, margin of nodules, and correlated to pathology. Results: Compared with pathology, the CT features of sand-like calcification, cystic formation, and degree of enhancement had statistical significance in the differentiation between benign and malignant lesions(P<0.05). The CT features of density and number of nodules had no statistical significance in the differentiation between benign and malignant thyroid nodules(P>0.05). Conclusion: There is important value of MSCT in the differentiation of the nature of thyroid nodules.

700 Correlation of shear-wave elastography with clinicopathological and immunohistochemical features in breast invasive lobular carcinoma

Objective: To compare the mean elasticity value, as measured by shear-wave elastography(SWE), with clinicopathological and immunohistochemical features of invasive lobular breast carcinoma. Methods: SWE was performed in 52 cases of breast invasive lobular carcinoma before operation. Mean elasticity values in kPa were measured for each lesion. The clinicopathological and immunohistochemical features were analyzed and correlated with Emean. Results: The result of one-way analysis variance showed that the tumor size, histological grade, axillar lymph node state, clinical tumor stage and the expression of ER and C-erbB-2 were related to the mean elasticity value of invasive lobular breast carcinoma. The result of multivative regression showed that the tumor size, histological grade and clinical tumor stage were independently correlated with the mean elasticity value. Conclusion: The clinicopathological feature of invasive lobular breast carcinoma is significantly associated with the mean elasticity. Biological characteristics of breast cancer influence the change of elasticity value.

703 MRI typing from architectural and dynamic features: a retrospective study in the differential diagnosis of breast lesions

Objective: To retrospectively evaluate the diagnostic ability of MRI typing from architectural and dynamic features in the differential diagnosis of breast lesions. Methods: 46 patients with breast lesions(54 lesions) underwent MRI before biopsy or operation. MRI findings were scored and rated into five types from both architectural features(enhancement diameter, shape and quantity of masses; enhancement distribution; border and surrounding structures) and dynamic features(enhancement pattern and time-signal intensity curve). Pathology results were taken as gold standard to evaluate MR imaging types. Results: Pathology revealed 22 benign masses(21 of type 3 and 1 of type 4), 14 malignant masses(10 of type 4 and 4 of type 5), 10 benign lesion without mass(9 of type 4 and 1 of type 5) and 8 malignant lesion without mass(5 of type 4 and 3 of type 5). When type 4 and 5 were considered malignant, MRI showed a sensitivity of 100% and a specificity of 93.8%(30/32). Only type 5 taken as malignant, the specificity was 100% but with a sensitivity of 31.8%(7/22). Conclusion: In the differential diagnosis of breast lesions in benign and malignant qualities, MR imaging types from architectural and dynamic features are of good diagnostic value and types 4 plus 5 as malignancies are of high diagnostic efficacy.

707 Utility of low kVp technique and contrast agent with reduced iodine content in enhanced chest CT: a preliminary study

Objective: To evaluate the utility of low kVp technique and reduced iodine contrast agent in enhanced chest CT by comparison of radiation dose, enhancement, image quality and diagnostic accuracy. Methods: 200 patients with BMI<28 underwent enhanced chest CT examination, and were randomly divided into four groups(n=50 for each group): group A with 120 kVp and iohexol(Omnipaque, 300 mgI/mL), group B with 120 kVp and iodixanol(Visipaque, 270 mgI/mL), group C with 100 kVp and iohexol(Omnipaque, 300 mgI/mL) and group D with 100 kVp and iodixanol(Visipaque, 270 mgI/mL). The volume CT dose index(CTDIvol), the dose length product(DLP) were recorded and the effective rediation dose(ED) was calculated. Two radiologists evaluated subjective image quality with 4-points score. CT value and SD value for pulmonary artery were measured and the contrast to noise ratio(CNR) was calculated. The diagnostic accuracy was compared with pathological results for each group. Results: For 300 mgI/mL iohexol, the CT value of pulmonary artery in group A((204.9±32.4) HU) was 19.1% higher than that in group C((172.1±24.4) HU), for 270 mgI/mL iodixanol, the CT value of pulmonary artery in group A((188.7±26.0) HU) was 21.8% higher than that in group C((154.9±21.4) HU). The effective radiation doses for 100 kVp(group C (3.9±0.6) mSv and group D (3.9±0.8) mSv) were lower than 120 kVp (group A (4.8±1.3) mSv and group B (5.1±1.3) mSv)(P<0.01). Subjective image qualities for four groups all achieved the standards for diagnostics, and the scores for group A and group B were a little better than group C and group D. The scores of two radiologists were highly consistent(kappa=0.76±0.05). Compared with pathological results, the consistent rates for four groups ranged from 82.1% to 86.8% and had no significant difference. Conclusion: Combination of 100 kVp and 270 mgI/mL iodixanol in enhanced chest CT examination met the requirement of clinical diagnosis, and reduced 18.8% radiation dose.

712 Evaluation of right ventricular systolic function in children with ventricular septal defect by speckle tracking imaging

Objective: To explore the value of two-dimensional speckle tracking imaging(2D-STI) strain in evaluating right ventricular systolic function in children with ventricular septal defect(VSD). Methods: Forty simple VSD patients and thirty healthy children were included in this study. According to VSD, the patients were divided into two groups: VSD without pulmonary hypertension(PH) group and VSD with PH group. Conventional parameters of right ventricular were measured. Right ventricular peak strain, systolic strain rate application with 2D-STI were measured. Results: Compared with control group and VSD without PH group, right ventricular end-diastolic area and end systolic area increased in VSD with PH group(P<0.05), and the difference was statistically significant. Peak strain and peak strain rate in each section and the globle right ventricle decreased, the difference was statistically significant(P<0.05). Compared with control group, it showed no signigicant differences in VSD without PH group(P>0.05). Conclusions: The VSD with PH has decreased right ventricular systolic function. To evaluate right ventricular systolic function of VSD in children, 2D-STI parameters of strain and strain rate are of great significance.

715 Evaluation of the degree of mitral regurgitation in dilated cardiomyopathy and mitral valve prolapse using three-dimensional color Doppler echocardiography

Objective: To investigate the value of three-dimensional color Doppler echocardiography(3D-CDE) in measuring the regurgitant orifice area(ROA) of mitral regurgitation(MR) in an en-face view to estimate the MR degree of different regurgitation mechanism. Methods: We selected 30 patients of mitral valve prolapse(MVP) and 30 patients of dilated cardiomyopathy(DCM) with different regurgitation degree. The 2D integrative method recommended by the ASE was used as the reference standard for MR grading in the 60 patients. Each patient’s ROA by 3D-ROA method and 2D-PISA method were measured respectively, and the correlation of the two methods was analysed. Results: The shape of the regurgitation orifice and the three-dimensional morphology of PISA was very different. There were correlations between the 2D-PISA and 3D-ROA method in the DCM group(r=0.783, Y=0.661x-0.45, P<0.001). However, in the MVP group, the relationship between the values by the 2D-PISA and 3D-ROA method was close to the line of identity(r=0.924, Y=1.172x-0.057, P<0.001). The 2D-PISA method underestimated the 3D-ROA by 26% in the DCM group. Conclusion: Compared with traditional 2D methods, 3D-ROA method can be more intuitive, more accurate in evaluating functional MR.

719 Assessment of aquaporins function of early-stage liver fibrosis using multi-b diffusion weighted magnetic resonance imaging

Objective: To investigate the value of aquaporins(AQP) function in the diagnosis of early liver fibrosis by using multiple b-value(multi-b) diffusion weighted magnetic resonancce imaging(MR-DWI). Methods: Using acetazolamide as AQP inhibitor, rats of control group and rats of developed model of liver fibrosis undertook conventional MR scans and a multi-b DWI before and after inhibition. The DWI was carried out with 18 different b values selected from 0 to 4 500 s/mm2. First of all, we obtained the standard ADC value in accordance with conventional methods, and then the multi-b values were divided into low-b, middle-b and high-b domains with 200 s/mm2 and 1 500 s/mm2 as thresholds. According to histological fibrosis stage, rats were divided into four groups: S0, S1, S2, S3+S4. With multiple b values, the result of apparent diffusion coefficience(ADC) and the expression and activity of AQP in four groups were compared. Results: There was no significant difference between each group in standard ADC. The AQP1 expression in the liver endothelial cells significantly increased with aggravation of liver fibrosis in early stage. Before inhibition, significant difference was observed between S0 and S3+S4 at high b value. While after inhibition, each group could be distinguished except S0 and S1. The high-b ADC value decreased 15.5% at S0, 16% at S1, while no obvious decrease was observed at S2 with the addition of inhibitor. The inhibition rate increased to 11.7% at S3+S4. Conclusion: Our preliminary experimental results indicate that the standard ADC value obtained by conventional method can not detect and stage early liver fibrosis, while the high-b ADC values are correlated with AQP expression, so the multi-b MR-DWI technique is capable to detect and stage early liver fibrosis.

724 A novel method for evaluating the superficial femoral vein valve insufficiency degree by CDFI

Objective: To evaluate the diagnostic value of color Doppler flow imaging(CDFI) in lower limb deep venous insufficiency. Methods: Total 63 patients with lower limb deep venous insufficiency(81 affected limbs, patient group) and 24 cases of normal control(48 normal limbs, control group) were included in the study. Color Doppler ultrasound was used to observe various indexes of hemodynamic variations including reflux area(RA), reflux time(RT), and reflux velocity(RV) of the first pair of the superficial femoral vein valve. The grading of lower limb deep venous insufficiency was based on the results of lower extremity venous retrograde angiography. Results: There was a statistical difference in RA and RT between the patient group and the control group(P<0.05). There was a strong positive correlation between the superficial femoral vein valve RA estimated by color Doppler ultrasound and the reflux degree estimated by lower limb deep venous insufficiency(r=0.998). But poor correlation was observed about RT and RV(r=0.574, and r=0.170, respectively). Conclusion: CDFI can accurately measure the superficial femoral vein valve RA, which availably reflects the degree of lower limb superficial femoral vein valve reflux. Whereas, the RT and RV have no diagnostic significance for the grading of valvular regurgitation in the study.

727 Multiple b values in diffusion-weighted imaging of rabbit lumbar vertebra tuberculosis model early appraisal

Objective: To construct the animal model of rabbit spinal tuberculosis, and to discuss applications of multiple b values in diffusion-weighted imaging of spinal vertebra tuberculosis. Methods: Firstly, animal models of spinal tuberculosis were built successfully by surgery and divided into two groups. Then they were examined with imaging and pathology at 4 weeks, 6 weeks, 8 weeks and 10 weeks respectively. Results: ①There was statistical difference in lesion group of ADC values between the lesion group and the normal group(P<0.05); ②With the increase of b value, ADC values decrease gradually. At one time point, there was no statistical difference between ADClow values and ADChigh values(P>0.05), but there was statistical difference among ADC3b values, ADC7b values and ADCperf values(P<0.05); ③At 4 weeks, ADC3b values, ADC7b values and ADCperf values were statistically different from those at 6 weeks, 8 weeks and 10 weeks(P<0.05). Conclusion: MRI can provide early diagnosis of spinal tuberculosis with some information, and DWI in MRI scanning is more sensitive to early diagnosis of spinal tuberculosis. Pathology of rabbit model with spinal tuberculosis is similar to that of human.
