533 Imaging findings of pleomorphic xanthoastrocytomas

Objective: To investigate the imaging features of pleomorphic xanthoastrocytoma(PXA). Methods: A retrospective study was based on 12 cases of PXA proved by operation and pathology. Ten cases only underwent MRI scanning, 2 cases underwent both CT and MRI scanning. Results: Thirteen lesions were detected in the 12 cases. Six lesions located in the temporal lobe, 4 lesions mainly located in the temporal lobe but involving multiple cerebral lobes, 2 lesions were in the insular lobe and 1 lesion in the frontal lobe. Thirteen lesions could be divided into 3 types of imaging findings: cystic-solid lesion(n=5), cystic lesion with nodule(n=6) and solid nodule(n=2). The solid components and mural nodules enhanced intensely. All the cyst walls had no enhancement in the 5 cystic lesions with nodule. The cerebral piamater closed to the lesion was enhanced in 2 cases. One lesion showed calcification. The apparent diffusion coefficient(ADC) values of solid portions were higher than contralateral tissue. ADC values and relative apparent diffusion coefficient(rADC) values of 2 cases of WHO Ⅲ were obviously lower than 4 cases of WHO Ⅱ. Cho peak ascended and NAA peak declined, with the Cho/NAA values of 0.84 and 1.06 respectively on 1H-MRS of 2 cases. Lac peak was detected in 1 case. Conclusion: There are some imaging characteristics in PXA. Recognizing these characteristics with multi-functional imaging technology of MRI is helpful to improve accuracy of diagnosis.

538 The CT and MRI diagnosis of totally calcified meningioma

Objective: To explore the characteristics of totally calcified meningioma in CT and MRI images, analyze its valueable information, so as to improve the diagosis. Methods: Twenty-three cases(6 males and 17 females, 33~82 years old with the mean age of 61.5) with calcified meningioma proven by operative pathology were retrospectively analyzed. All of them underwent CT scans, while 19 of them were examined by MRI(17 underwent enhanced scans). Results: CT of all the 23 cases showed high density masses. Twenty-one masses were heterogeneous in density on bone-window. Sixteen masses were round in transverse-section. MRI of the tumors showed iso-intensity or slight hypo-intensity in 20 cases, and heterogeneous slightly hyper-intense in 3 patients on T1WI. On T2WI, hypo-intensity were detected mainly in 12 cases with a little iso-intensity or slight hyper-intesity; 6 tumors showed iso-intensity mainly and 5 showed slight hyper-intensity. Among them, 8 cases showed thin circular iso-intensity on both T1WI and T2WI. Twelve cases showed significant enhancement on T1WI, of which progressive enhancement were demonstrated in larger lesions. Dural enhancement was seen in 11 cases. Five cases were found crossing the cerebral falx. Conclusion: CT and MRI examination on completely calcified meningioma can be complementary for each other. A combination of both can reflect the characteristics of the tumor more objectively and comprehensively, and improve the diagnostic accuracy.

541 Early evaluation of carotid elasticity function in patients with systemic lupus erythematosus by wave intensity technique

Objective: To observe the clinical value of early diagnosis of atherosclerosis in patients with systemic lupus erythematosus(SLE) using wave intensity technique and to evaluate changes of elasticity of carotid artery in pre-menopausal SLE patients. Materials and Methods: Sixty patients with SLE were divided into two groups according to different clinical courses: SLE group 1(duration<5 years), and SLE group 2(duration≥5 years). A Wave intensity method was used to measure and analyze instantaneous acceleration Wave intensity(W1), instantaneous deceleration Wave intensity(W2), negative area(NA), stiffness constant(β), one-point pulse wave velocity(PWVβ and PWV-WI), pressure strain elastic modulus(Ep), arterial compliance(AC), augmentation index(AI) and intimamedia thickness(IMT) of the right carotid arteries in 60 patients with SLE and in 30 normal control individuals. Results: Compared with the normal group, W2, β, Ep, PWV-WI, PWVβ of SLE group 1 and SLE group 2 were higher than those of the control group, but AC was lower than that of the control, with a statistically significant difference(P≤0.01). W1, AI, NA values were different in the three groups, but the difference was not statistically significant(P>0.01). W2, PWV-WI, β, Ep, PWVβ significantly increased in SLE group 2 comparing with SLE group 1, but AC decreased, with a statistically significant difference(P≤0.01). Conclusion: WI technique can detect the early changes of carotid elasticity in systemic lupus erythematosus patients. It may be useful for prevention and treatment of the complicated cardiovascular disease.

545 The clinical study of differentiating benign and malignant breast tumors by diffused optical imaging with ultrasound

Objective: To assess the diagnostic value of diffused optical imaging with ultrasound(OPTIMUS) in differentiating benign and malignant breast tumors and the correlation between the quantitative parameters from the report of OPTIMUS and pathological results. Methods: We selected 52 patients with suspected malignant breast tumors by ultrasound(excluding those with typical cysts and fibroadenomas) in Wuhan Union Hospital from April to May of 2011. The breast tumors of the patients were examined by OPTIMUS. The diagnostic accuracy of the OPTIMUS and the correlation between the quantitative parameters and pathological results were analyzed by comparing the reports and pathological results. Results: Forty-five patients had accepted surgery treatments before November 2011. OPTIMUS reported 23 tumors with BI-RADS 1~3 and 22 tumors with BI-RADS 4~5, and pathological results showed that 26 tumors were benign and 19 tumors were malignant. The sensitivity, specificity, positive predictive value, negative predictive value of the diagnosis of OPTIMUS were 89.5%(17/19), 80.8%(21/26), 77.3%(17/22), 91.3%(21/23), respectively. There were significant differences in the comprehensive diagnostic parameters(P=0.000), total hemoglobin(P=0.025), tumor direction(P=0.009), shape factor(P=0.001) between benign and malignant tumors. In sub-parameters, total hemoglobin and tumor direction entered the Logistic regression equation, and total hemoglobin(standardized regression coefficient 2.817) was more relevant to tumor direction(standardized regression coefficient 1.180). Conclution: OPTIMUS has effective identification of benign and malignant breast tumors. Total hemoglobin, as a new quantitative parameter, is directly related to the nature of tumor pathology, and some sub-parameters are related to the nature of tumor pathology. OPTIMUS provides quantitative information for clinical diagnosis.

549 Research on rotation and torsion of subendocardial and epicardial myocardium in acute myocardial ischemia and reperfusion by speckle tracking imaging

Objective: To evaluate the heart function through measuring the rotation and torsion of subendocardial myocardium, epicardial myocardium and transmural gradient in acute myocardial ischemia and reperfusion in dogs by speckle tracking imaging. Methods: The left anterior descending(LAD) coronary artery of 20 adult healthy hybrid dogs were ligated for 180 minutes followed by reperfusion for 120 minutes. Rotation and torsion of myocardium on apical and basal level right before, immediately after and 60, 120 and 180 minutes after LAD occlusion, 0, 60 and 120 minutes after reperfusion were compared by speckle tracking imaging. Results:①Rotation of the basal level: with the extension of ischemie time, the change of transmural gradient decreased and recovered rapidly at the time of reperfusion. The change of the transmural gradient was the lowest at the time of reperfusion for 120 minutes. ②Rotation of the apical level: the change of the transmural gradient decreased immediately after occlusion, and decreased to the lowest at the time of occlusion for 180 minutes. It increased immediately after reperfusion and increased smoothly at the time of reperfusion for 60, 120 minutes. ③Torsion: with the extension of ischemie time, torsion angle value of transmural gradient decreased rapidly. It decreased to the lowest at the time of occlusion for 180 minutes and recovered rapidly at the time of reperfusion. Conclusion: Quantitatively analyze local and overall rotation and torsion of subendocardial and epicardial myocardium can evaluate the heart function by speckle tracking imaging. The peak values of rotation and torsion of subendocardial myocardium, epicardial myocardium and transmural gradient are the useful method to evaluate the wall motion and heart function in acute myocardial ischemia and reperfusion dogs.

553 The study of post-processing of hepatic proton MRS in vivo on 3.0T MR scanner

Objectives: To investigate the post-processing of hepatic MRS findings in vivo. Materials and Methods: A total of 58 single-voxel proton spectra of the liver were acquired on 3.0T scanner using the eight-channel phased array abdomen coil. Consecutive stacks of breathhold spectra were acquired using the PRESS technique with a short echo time of 30 ms and a repetition time of 1 500 ms. The spectra was processed with the software package “SAGE”. Areas and heights for metabolites resonances were obtained. Results: The method Spectrum of raw MRS data showed more metabolites resonances peaks than the method Probe. The group using the parameter 5 Hz had better SNR than that using the parameter 1.25 Hz. The lipid peak height was wider. Conclusions: The parameters 5 Hz is better in post-prosessing of hepatic proton MRS at 3.0T.

556 Application of contrast-enhanced ultrasound in spleen-preserving operation for splenic trauma

Objective: To investigate the application of contrast-enhanced ultrasound(CEUS) in operation of splenic trauma. Methods: Forty-six patients with splenic trauma underwent spleen-preserving operations. Both ultrasonography and CEUS were applied for preoperative and postoperative monitoring. Results: Forty-six patients with splenic trauma were classified as follows: 5 patients in Grade Ⅱ, 32 in Grade Ⅲ and 9 in Grade Ⅳ. Conventional ultrasound successfully detected trauma in 42 patients but failed in 4 patients; CEUS diagnosed 46 patients correctly, among which contrast agent overflow was seen in 9 cases. Postoperatively, one patient was found with progressive increase of seroperitoneum, yet two patients with complications were detected by CEUS: one patient’s splenic artery trunk was pressed, while the other suffered uncontrollable postoperative infection whose volume was less than 1/5 of the original volume after spleen surgery. Conclusions: CEUS offers preoperative basis for the spleen-preserving operation for splenic trauma and is also applied for monitoring and following-up after operation. Thus it has great significance in discovering complications, traumatic state and prognosis.

559 The diagnostic value of MSCT in atypical renal cell carcinoma

Objective: To study the CT features of atypical renal cell carcinoma(RCC) and the value of MSCT. Materials and Methods: CT findings of 16 cases of atypical RCC confirmed pathologically were analyzed retrospectively. All the cases received plain CT scan and dual-phase contrast-enhanced CT scan by MSCT. Then three-dimensional reconstruction was performed. Results: All of the tumours were single and located in unilateral kidney. Among the 16 cases, 8 showed solid appearance, 6 showed cystic-solid appearance, and 2 showed cystic appearance; 10 cases displayed the characteristic of lack of blood supply. Solid RCC lack of blood supply was slightly-moderately enhanced or not enhanced in cortical phase, then slightly reduced in the following parenchyma phase, and no trend of progressive enhancement was found. Among the 8 cases of solid RCC, 5 cases intruded into renal sinus, and 2 cases had circular calcification. The solid portion of 4 cystic-solid RCC were rich in blood supply and enhanced according to the rhythm of solid RCC which was rich of blood supply. The 2 cases of cystic RCC showed uneven thickness of cyst wall with shallow lobulation. In one case, soft tissue with slight enhancement was seen in the junctional zone between the cystic lesion and normal renal parenchyma. The diffuse infiltrative RCC with ill-defined margin was slightly enhanced, which was similar to the inflammatory lesion, and the acanthoid protuberance was found on the edge of kidney, accompanied with thickened perinephric fascia, widened bridge interval and enlarged lymph nodes adjacent to renal hilus. Conclusion: Atypical RCC shows various forms and must be analyzed comprehensively according to shape and pattern of enhancement. The lesion can be displayed well by MSCT which has the advantage in showing the detail of lesions and contributes to differential diagnosis.

565 Application of color Doppler ultrasound in the diagnosis of cesarean scar pregnancy

Objective: To evaluate the value of color Doppler ultrasound in diagnosis of cesarean scar pregnancy. Methods: We analyzed retrospectively the data of 21 patients who were diagnosed as a cesarean scar pregnancy from December, 2008 to October, 2011 at affiliated hospital of Jining medical college and analyzed the ultrasonographic features of cesarean scar pregnancy. Results: Eighteen of 21 cases were correctly diagnosed of cesarean scar pregnancy by ultrasound. The accuracy of ultrasound diagnosis of cesarean scar pregnancy was 85.7%(18/21). One case misdiagnosed as cesarean scar pregnanly was trophoblastic disease confirmed by pathology. The misdiagnose rate was 4.8%(1/21). Two cases of cesarean scar pregnancy were missed and diagnosed as cervical pregnancy and incomplete abortion respectively by ultrasonography. The missed diagnose rate was 9.5%(2/21). The ultrasonography of cesarean scar pregnancy can be divided into three types: gestational sac type(10 cases), gestational sac-like structure type(5 cases) and mixed mass type(5 cases). Conclusion: Color Doppler ultrasound is important in the diagnosis of cesarean scar pregnancy, and can provide important reference information for early clinical diagnosis and treatment.

568 Comparison of TOF-PET and PSF-PET image reconstruction technique in clinical application

Objective: TOF-PET(Time of flight-positron emission tomography, TOF-PET) and PSF-PET (Point spread function-PET, PSF-PET) were used to improve the PET performance of the latest technology. The purpose of this study was to compare TOF-PET and PSF-PET on normal tissue of brain, liver, as well as the thoracic and abdominal tumor lesions on 18F-FDG standard uptake value maximum(SUVmax) effect, so that to guide the clinical use of TOF-PET and PSF-PET technique better. Materials and methods: Thirty-five patients(male 19, female 16 cases, age 58.69±12.88 years old) with BMI 25.18±4.32 were studied. The 35 patients had 29 pulmonary lesions and 12 abdominal lesions. CT measurement of lesion diameter was in the range of 5~40 mm. GE Discovery PET/CT Elite and AW workstation was used. The SharpIR+VUE Point HD, TOF+VUE Point HD and SharpIR+VUE Point HD+TOF reconstruction method of PET images were selected. SharpIR and VUE Point HD were commercial names for PSF and iterative reconstruction respectively. Results: Compared with NO-TOF reconstruction SUVmax value of chest and abdominal lesions with TOF were increased by 7.1%(n=29, P<0.001) and 5.54%(n=12, P<0.006). In the group of BMI>25, SUVmax values of chest and abdominal lesions with TOF increased by 15.6%(n=12, P<0.001) and 20.6%(n=4, P<0.035) compared with NO-TOF. For the same group of lesions(n=6), SUVmax value by VUE HD+SharpIR, VUE HD+TOF, VUE HD+SharpIR+TOF were increased by 17.69%, 2.3%, 22.54% compared with VUE HD. For all the lesions and normal brain tissue, SUVmax values were increased by both TOF-PET and PSF-PET, but for patients with BMI greater than 25 lesions, TOF-PET raised SUVmax more obviously. Compared with TOF-PET, PSF-PET enhanced SUVmax more significantly than TOF-PET. Normal liver tissue was not affected by TOF-PET technology. Conclusion: Our results indicate that TOF-PET and PSF-PET can improve the brain lesions and normal tissue SUVmax, but PSF-PET is superior to TOF-PET. For the patients with BMI greater than 25, TOF technology increases the SUVmax value of chest and abdominal lesions more obviously.
