457 The study of MR PWI in the brain of children

Objective: To study the feasibility and method of magnetic resonance perfusion weighted imaging(MR PWI) in children’s brain. Methods: GE 1.5T HDx echo speed plus MRI scanner were used and double dosage(0.2mmol/kg) Gd-DTPA was given. In the study group, 45 children underwent MR PWI with the injection speed ranged from 2~4ml/s according to the limit of the vein. In control group, 20 adults underwent MR PWI with the injection speed of 2~4ml/s. After the MR PWI scan, negative enhancement integral(NEI) maps were obtained. NEI values and relative NEI(rNEI) were studied in NEI maps. Results: The study showed that the children with body weight <20kg and Gd-DTPA injection speed of 3ml/s or with body weight >20kg and injection speed 3~4ml/s could get satisfactory PWI curves. Children <10kg with injection speed of 2ml/s could get available PWI curves. The NEI values of the studied group had a marked standard deviation(13.69~22.54) and there were significant statistical differences between the two groups(P<0.001). But rNEI of studied group had a slight standard deviation(0.05~0.08) and there were no significant statistical differences between the two groups(P>0.001). Conclusion: MR PWI could be used in the research of children brain perfusion if the injection speed was high enough. This study is very important to the research of cerebral MR PWI in pediatrics.

461 CT and MRI appearances of Warthin tumor of the parotid gland

Objective: To probe the CT and MR imaging appearances of Warthin tumor of parotid gland for improving the diagnostic recognition. Methods: Thirty-seven consecutive patients of Warthin tumor in parotid glands confirmed by the surgical pathology were retrospectively evaluated based on the clinical and imaging data. Results: Of the 37 patients(33 men and 4 women; age ranged from 45~82 years, mean age 55.5 years), 29(78.4%) patients were over 55 years old and 27(73%) cases had smoking habit over twenty years. Of the 37 patients with 53 tumors, 25 cases had solitary lesions, 8 cases had multiple lesions on one side, 4 cases had bilateral tumors. Most Warthin tumors(79.2%) located in the posterior and inferior quadrant of the parotid gland. On CT and MRI, Warthin tumors usually were spherical or ovoid in shape, had a smooth surface, the average maximal cross-sectional diameter was 2.8cm. On CT scan(21 tumors), the lesions showed solid(11 lesions) or cystic(10 lesions) and the solid part of tumor showed hyperdensity compared with normal parotid gland. All the 32 tumors detected by MRI examination in our series showed low SI on T1WI and heterogeneous high SI(27 lesions) on T2WI with fat-suppression. Cystic area showed long T1, T2 signal intensities. Sixteen lesions had high-SI area on T1WI. Most of the lesions showed a rapid early moderate enhancement and rapid washout later after contrast administration. Some tumors showed enhanced capsule. Conclusion: With the characteristic clinical and imaging features of Warthin tumor in parotid glands, it is helpful to preoperative diagnosis.

464 Correlation study of CT features, pathology and the serum CEA concentration in peripheral lung cancer

Objective: To discuss the correlation of pathology, CT features and serum CEA concentration in peripheral lung cancer. Materials and Methods: The pathology, CT features and the serum CEA concentration in 48 cases of peripheral lung cancer, confirmed by operation and pathology, were analyzed. Results: The serum CEA concentration was (13.23±20.47)ng/ml(adenocarcinoma, n=30), (3.62±2.48)ng/ml(squamous carcinoma, n=11). There was no correlation between the serum CEA concentration and the tumor differentiation degree in lung cancer(r=-0.017). The serum CEA concentration were higher in lung cancer with deep lobulation sign or diameter≥3cm. There was no significant difference between lung cancer with pleural indentation sign, spiculation sign, increased CT value≥20HU after enhancement, calcification or enlargement of hilar or mediastinal lymph nodes and serum CEA concentration. There was positive correlation between the percentage of Ki-67 antigen and the serum CEA concentration in lung cancer(r=0.576). The serum CEA concentration was (19.37±24.40)ng/ml(p53 positive group), (3.64±2.11)ng/ml(p53 negative group). Conclusion: There was positive correlation between serum CEA concentration, the percentage of Ki-67 antigen, expression of p53, deep lobulation sign and diameter≥3cm of peripheral lung cancer. There was no significant difference between tumor differentiation degree and serum CEA concentration in peripheral lung cancer.

468 CTPA in evaluating the degree of acute pulmonary obstruction and effect of treatment

Objective: To explore the value of CTPA in grading the severity of acute pulmonary embolism(PE) and to investigate the change of the cardiovascular parameters before and after treatment. Method: 152 cases of clinical suspicion of PE, included 118 cases explored by CTPA were studied. The patients were separated into three different groups according to clinical severity and treatment decision: included patients without PE(WPE)(n=40); included patients with severe PE(group SPE) who were treated by fibrinolysis(n=42); and included patients with non-severe PE(group NSPE) who received heparin(n=36); then reviewed the results of CTPA after the treatment of 14 days. With the diagnosis of CTPA as a basis, we analyzed the difference of cardiovascular parameters among the three groups; analyzed the obstruction index between group SPE and group NSPE, the correlation between obstruction index and cardiovascular parameters; explored the difference of cardiovascular parameters before and after treatment for PE. Result: CT obstruction index in patients with SPE was higher than that of patients with NSPE. The cardiovascular parameters in patients of the three groups, showed significant difference. The correlation between CT obstruction indexes and cardiovascular parameters was significant. Spearman’s rank correlation coefficent was higher between right pulmonary artery diameter and CT obstruction index. The cardiovascular parameters between patients with group SPE and group NSPE before and after treatment showed significant difference. Conclusion: CTPA not only can diagnose PE also can evaluate the degree of pulmonary arterial obstruction and the change of right heart function, and then reflect the prognosis and observe curative effect.

472 The application of 320-row volume CT scan in coronary artery bypass grafts imaging

Objective: To investigate the application of 320-row volume CT scan in coronary artery bypass grafts(CABG) imaging. Method: Fifty-five cases(male 36) after 3~9 months of the operation of CABG were enrolled in this study and underwent coronary artery graft imaging using 320-row volume CT scan. Total 126 grafts(52 arterial grafts, 61 venous grafts and 13 sequential grafts) were evaluated. Grafts images were reconstructed using multiple planar reformations(MPR), curved planar reformations(CPR) and volume rendering(VR). The image quality(good, moderate and poor) and stenosis of grafts(non-stenosis, slight, mild and severe stenosis) were evaluated. Result: All the 55 patients(126 grafts) accepted 320-row volume CT scan after CABG operation. 116 grafts were filled with contrast agent and the other 10 grafts which no contrast agent in lumen were considered as total occlusions. The image quality of 116 grafts was good(97 grafts, 83.6%) or moderate(19 grafts, 16.4%). No poor image quality was observed. Calcifications of wall were found in 8 grafts. Lumen stenosis was found in 5 arterial grafts, 5 venous grafts and 2 sequential grafts. Conclusion: 320-row volume CT scan can image grafts clearly and can be used to evaluate stenosis of graft. It is a good noninvasive method for graft imaging after CABG operation.

475 Comparative study of dynamic change in 3D-Watsc and pathologic change in rabbit’s osteoarthritis

Objective: To study the imaging feature and pathologic basis of osteoarthritis(OA) of the rabbit by comparing the relationship between MRI grade and pathological grade of OA. Materials and Methods: Thirty-five healthy New Zealand rabbits were divided into five groups randomly, including normal control group, 2-weeks, 4-weeks, 6-weeks and 8-weeks groups respectively after transaction of anterior cruciate ligament to build OA model. MRI and pathological observation were carried out at corresponding time point. HE was performed to observe the histopathologic changes in the degenerative cartilage. Results: The cartilage showed high signal and high contrast on 3D-Watsc sequence. The damage of cartilage became more severe with time. The MRI grades showed good consistency with pathological grades(Kappa=0.80). The MRI grades were different between each group(P<0.05). No significant differences were found between control and 2-weeks group, 4-weeks and 6-weeks groups, 6-weeks and 8-weeks groups by multiple comparison(P>0.05). The differences between the other pairs were significant(P<0.05). Conclusion: The pathological grades and MRI grades of OA increase gradually with postoperative time. MRI 3D-Watsc is able to observe signal features which changes in early stage of OA.

479 MRI evaluation of bone marrow mesenchymal stem cells in the treatment of cartilage damage in RA rabbit model

Objective: To observe the effects of bone marrow stem cells(BMSCs) transplantation in treating rheumatoid arthritis(RA) in rabbits by measuring the thickness of knee joint articular cartilage using MRI. Methods: Ovalbumin was used to induce right knee rheumatoid arthritis in 42 New Zealand white rabbits. Four wks later, MRI and pathologic examination were carried out in 6 rabbits, the remaining 36 rabbits were randomly and equally divided into the RA group as control and the BMSCs transplantation group. Normal saline was injected into the right knee joint of the former group and same volume of BMSCs was injected into the right knee joint of the latter group. At 1, 2, 3 months after injection, 6 rabbits at each time point of both groups were given MRI and pathologic examination respectively, the thickness of knee articular cartilage was measured by MRI and the pathologic changes were observed and scorings were recorded. Results: After 1, 2, 3 months the cartilage in the RA group showed thinning and damaged with high pathologic score. While the cartilage was thickened with lower pathologic scorings in the BMSCs group. MRI measurements and pathology scoring correlated well. Conclusion: MRI can be used to evaluate the effect of BMSCs in the treatment of cartilage in early RA.

483 Diagnostic value of color Doppler ultrasound in giant cell tumor of bone

Objective: To evaluate the diagnostic value of color Doppler ultrasound in giant cell tumor of bone(GCT). Methods: Thirty-nine patients with GCT were examined with 2D ultrasound and color Doppler ultrasound. The echo characteristics, distribution of blood flow and hemodynamic changes of blood flow in the lesions were observed retrospectively. The hemodynanic parameters were studied according to the Campanacci scale. Results: The peak systolic velocity(PSV) did not differ(P>0.05), end diastolic velocity(EDV) in band I were significantly lower than those in band III(P<0.05). Resistance index(RI) in band I were significantly higher than those in band II or III(P<0.05). There was no significant difference between band II and III(P>0.05). Conclusion: Observing the features of lesion and hemodynamic parameters by 2D ultrasonography and CDFI has important value in diagnosing GCT and distinguishing the clinical band.
