77 The value of 1H-MRS in cerebral glioma

Objective: To approach the clinical application value of correspondance quantitative analysis with 3.0T magnetic resonance spectroscopy(1H-MRS) in grading cerebral glioma before operation. Methods: Thirty-two patients with cerebral glioma confirmed by pathological results were performed with 1H-MRS on GE 3.0T magnetic resonance system. The spectral line in the solid area of tumour, peritumor edematous area and normal-area of the opposite side were observed with 1H-MRS. Cho/Cr, NAA/Cr, Cho/NAA, Cho/nCho, Lac/Cr were analyzed. The results of 1H-MRS were compared with the results of pathology. Another 11 subjects were normal control. Results: There were significant differences in Cho/Cr, NAA/Cr, Cho/NAA between solid area of glioma and normal area of the opposite side(2.10±1.04, 0.84±0.44, 2.05±0.70 and 0.87±0.13, 1.43±0.14, 0.61±0.10 respectively). There were significant differences in Cho/Cr, NAA/Cr, Cho/NAA, Cho/nCho, Lac/Cr between solid area in the low grade glioma group and that in the high grade glioma group that is(1.58±0.63, 0.96±0.57, 1.74±0.61, 1.04±0.81, 1.58±0.10 and 2.62±1.45, 0.78±0.31, 3.35±0.89, 2.14±1.77, 2.61±2.75 respectively)(P<0.05). There were also significant differences in Cho/Cr, NAA/Cr, Cho/NAA, Cho/nCho(i.e. 1.43±0.32, 0.82±0.45, 1.22±0.19, 1.23±0.36 and 2.21±0.49, 1.12±0.23, 2.09±0.82, 1.47±1.13) between edematous area in the low grade glioma group and that in the high grade glioma group(P<0.05). Cho/Cr, Cho/NAA, Cho/nCho, Lac/Cr(r=0.42, 0.62, 0.41, 0.26) of solid area of tumor was of positive correlation with the grade of tumor, and NAA/Cr(r=-0.35) was of negative corretation. The correlative coefficient of Cho/NAA(r=0.62) was the maximum and had maximal dependability with grade of tumor. Conclusion: 1H-MRS had very important clinical application value in providing diagnostic support for grading of cerebral glioma before operation.

83 Comparison of ultrasonography and MR imaging in diagnosing and staging of laryngeal carcinoma

Objective: To compare the value of ultrasonography and MR imaging in diagnosing and T-staging of laryngeal carcinoma. Methods: Ultrasonography and MRI were performed in thirty-six patients with laryngocarcinoma. Histopathologic and surgery findings were performed to validate imaging findings. Results: In 36 patients with laryngeal carcinoma, 34 cases were found by ultrasonography, 32 cases were detected on MRI. The accuracy of ultrasonography and MRI in the diagnosis of T1, T2, T3, T4 staging laryngocarcinoma were 90.9%, 60.0%, 87.5%, 85.7% and 63.6%, 70.0%, 75.0%, 100%, respectively. There were no statistical differences observed in accuracy rating between ultrasonography and MRI(χ2 were 2.329, 0.220, 0.410, 1.077, respectively, all P>0.10). The overall accuracy rating in T-staging was 80.6% for ultrasonography, and was 75.0% for MRI. There was no significant difference between the two methods in overall T-staging accuracy rating(χ2=0.321, P=0.778). Conclusions: Ultrasonography can provide adequate information with the location, size and infiltration of laryngeal carcinoma, it is a complementary method for laryngoscopy, CT and MR imaging assessment of laryngeal carcinoma.

86 The clinical value of metallic stents for malignant airway obstruction

Objective: To evaluate the feasibility and efficacy of metallic stent for malignant airway obstruction. Methods: Forty patients with airway obstruction and dyspnea caused by malignant carcinoma underwent metallic stents placement. Twenty-three patients underwent additional chemotherapy and radiotherapy. Results: Stenosis of 32 patients were located in the trachea, 33 metallic stents were placed. Stenosis of 8 patients were located in the inferior segment or bifurcation of trachea, Y-shaped metallic stents was placed in each of these patients. The stents expanded sufficiently in 28 patients after immediate placement, and in other 12 patients, stents expanded about 40% to 80%. All stents expanded sufficiently after a week and did not have displacement. The stent disruption happened in 4 patients after 2~4 months of placement, they underwent replacement of stent again. The survial time of 17 patients without any additional therapy was 2~6 months. For the 23 patients who had chemotherapy and radiotherapy, the life span was obviously extended as long as 6~15 months. Single-factor survival analysis of overall survival showed that there was statistical significance between the two groups(P=0.002). Conclusion: Metallic stents are useful for relieving respiratory distress and improving life quality in patients with malignant airway obstruction, additional chemotherapy and radiotherapy can further lengthen existence.

89 Research on the detectable ability of MicroPure system on breast micro-calcifications

Objective: To research the detectability of MicroPure system on breast micro-calcifications. Materials and Methods: Forty-three patients with 96 separate micro-calcifications in breast confirmed by the molybdenum target X-ray system were given MicroPure system detection and conventional ultrasound imaging technology respectively. Results: Ninety-four micro-calcifications were detected by MicroPure system, however, there were only 66 micro-calcifications found by conventional imaging technology. There was significant difference between them(P<0.05). Micro-calcifications were found in 30 patients with breast cancer of all the 31 patients with breast cancer by MicroPure system. When compared with conventional ultrasound imaging technology(18 patients), there was significant difference between them(P<0.05). In addition, the results also showed that MicroPure system detected more micro-calcifications(62 lesions) within hypoechoic lesions(64 lesions) than conventional ultrasound imaging technology(40 lesions) in patients with breast cancer. At the same time, the detectability of MicroPure system for micro-calcification in benign and malignant breast tissue was very high. Conclusion: MicroPure system can detect micro-calcifications and has a better visibility for micro-calcifications within hypoechoic lesions, which suggests that the MicroPure system has the ability to discover early breast cancer.

92 MSCT findings in severe influenza A(H1N1) virus pneumonia

Objective: To explore the MSCT features in patients with severe influenza A(H1N1) virus pneumonia. Methods: The 64 multi-slices spiral CT images in 20 patients diagnosed as severe influenza A(H1N1) in our hospital were studied retrospectively. Results: ①Most of the lesions in all cases were distributed diffusely and bilaterally, the others located in the middle and lower lobes. ②The CT findings including airway thickening(21/21, 100%), centrilobular nodules(12/21, 57.1%), interlobular septal thickening(9/21, 42.9%), intralobular thin reticulation and micro-nodules(3/21, 14.3%), ground-glass opacity(14/21, 66.7%), single-lobular inflammation(7/21,33.3%), large consolidation(9/21, 42.9%), airway dilatation(5/21, 23.8%). ③Pleural effusion(8/2, 38.1%); lymphadenopathy(9/21, 42.9%); pleural thickening(12/21, 57.1%); effusion of pericardium(1/21, 4.76%). ④The 9 re-examed cases, the imaging findings of the pheumonia changed quickly during the follow-up accompanied with the improvement of clinical symptoms. Three death cases showed centrilobular nodules, consolidation and ground-glass opacity. The others resolved quickly after anti-virus treatment. Conclusions: The 64-slice spiral CT findings of patients with S-OIV(H1N1) are diffuse distribution of bilateral ground-glass opacities and consolidations of lung, most of them showed abnormalities of parenchymal and interstitial tissues. Pleural effusion, lymphadenopathy and pleural thickening were observed. The increasing of ground-glass opacity and consolidation’s range could be the marker of progression of H1N1 infection.

96 MSCT in the diagnosis of the rupture crevasse in traumatic diaphragmatic hernia

Objective: To investigate MSCT morphologic characteristics of the diaphragmatic crevasse in traumatic diaphragmatic hernia(TDH). Materials and Methods: The MSCT features of 27 patients with TDH were analyzed retrospectively, which included 21 cases of blunt diaphragmatic hernia(BDH)(all confirmed by surgery) and 6 cases of iatrogenic diaphragmatic hernia(IDH)(2 surgically confirmed). Twenty-seven patients all underwent MSCT scanning, slantwise, projective and linear methods of MPR reconstruction were adopted to observe and compare diaphragmatic crevasse for its display and morphological characteristics. Result: Diaphragm crevasse can display in 21 cases of the 27 cases with slantwise methods of MPR(77.8%), besides 24 cases(88.9%) with projective methods and 20 cases(74.1%) with linear methods, average 77.2%, without difference in coronal or sagittal sites of the last two methods. Of the 27 cases with TDH, the shape of the crevasse was not clearly displayed, 11 ones could not show right side(36.7%, 11/30), 15 ones could not display left side(14.3%, 15/105), t=6.51, P<0.05. Form of diaphragmatic crevasse is mainly classic round, oval or spindle, of which, 16 cases(59.3%), 20 cases(45.5%), 26 cases(59.1%) displayed classic round by tilted method, projective method and linear method in turn, likewise 11 cases(40.7%), 24 cases(54.5%), 18 cases(40.9%) were displayed oval or spindle by three methods above mentioned in turn. Conclusion: The morphological characteristics of break in the most of TDH can be displayed directly or indirectly, the demonstrative situation of tilted method and linear method is similar; and more objective than that of the projective ones.

100 Clinic application of susceptibility weighted imaging in the discovery and diagnosis of focal liver lesions

Objective: To investigate the value of susceptibility weighted imaging in the discovery and diagnosis of focal liver lesions. Methods: Eighteen patients with 25 focal liver lesions were collected, all of them were proved by clinic and imaging. It included liver vascular malformation(n=1), hepatocellular carcinoma(n=4), cholangiocellular carcinoma(n=3), metastatic tumor in liver(n=2), liver haemangioma(n=5), and liver cyst(n=3). MR imaging were performed with conventional T1WI, T2WI, SWI and dynamic Gd-DTPA contrast enhanced MRI. The SWI imagings were compared to conventional sequence imagings. 3D MRA were performed in liver vascular malformation(n=1) and post-gadolinium enhanced SWI performed in one liver haemangioma. Results: In liver vascular malformation(n=1), SWI was better than conventional MRI and 3D MRA in discovering abnormal veins. SWI could discover tumor vascular structure and degradation products of blood in hepatocellular carcinoma(n=4), cholangiocellular carcinoma(n=3) and metastatic tumor in liver(n=2). SWI showed homogeneous slight hyper-signal in liver haemangioma(n=5) and liver cyst(n=3), and in comparatively large lesion of which the adjacent vein was compressed. Conclusion: SWI surpassed conventional MRI sequences in discovering liver venous vascular malformation and vascular structure in tumor. SWI is very helpful in differential diagnosis of focal liver lesions. It is a new approach to investigate the internal structures of focal liver lesions.

105 Preliminary experience of dual source, dual energy CT in detection of gout

Objective: To evaluate the clinical value of dual source, dual energy CT(DECT) in detection of gout. Materials and Methods: Three patients with proven gout and 10 volunteers as controls were included into this study. All subjects underwent foot or hand dual source, DECT scans. The dedicated software of gout was used to analyze the presence of gout and color-coded the gout as green. The presence, number, and distribution of gout were recorded. Results: Multiple intra- and extra-ossicular gouts were discovered in bilateral feet and hands of three patients with clinically proven gout, especially in interphalangeal joint and adjacent soft tissue; while no gout was found in hands and feet of 10 controls. Conclusion: DECT can show and color-coded gout, with potential clinical value.
