MR perfusion-weighted imaging in the differentiation of solitary cerebralmetastatic tumors and high-grade gliomas

Objective: To determine the value of MR perfusion-weighted imaging(PWI) in the differentiation of solitary cerebral metastatic tumor and high-grade gliomas. Methods: MR PWI features of solitary cerebral metastatic tumors(n=12) or high-grade gliomas(n=22) were compared respectively. Results: After the first pass of contrast agent in PWI, high-grade gliomas demonstrated a rapid recovery of signal intensity; while time-signal-intensity curve(TSIC) of metastatic tumors showed a placid and incomplete recovery of signal intensity. There was no significant difference between the rCBV values of high-grade gliomas and metastatic tumors. However, the para- and peri-tumoral rCBV values of high-grade gliomas were significantly higher than that of metastatic tumors. Conclusion: MR perfusion-weighted imaging is a potential tool to differentiate solitary cerebral metastatic tumor from high-grade gliomas by depicting the hemodynamic features of tumors and peritumoral areas.

Application of T2-star weighted angiography detecting point-like short-T2 signal amongthe aged with lacunar infarction

Objective: To explore the clinical application of T2-star weighted angiography(SWAN) in detecting lacunar infarction in aged patiens with point-like short-T2 signal. Methods: Fifty-eight cases with lacunar infarction and 58 cases healthy elderly population above 60-years old as control group were examined using SWAN and GRE-T2*WI sequence. The number, location and occurrence of point-like short-T2 signal and location, and number of lacunar infarction were calculated. Result: Seventy-seven point-like short-T2 signal in 33 cases were demonstrated by SWAN and 38 point-like short-T2 signal in 17 cases were found by GRE-T2*WI sequence, including 25 cases of lacunar infarction group and 6 cases of control group. Slight, moderate, severe degree of point-like short-T2 signal were found in lacunar infarction group. Only slight point-like short-T2 signal were seen in control group. Incidence and grade of point-like short-T2 signal were significantly different in the two groups. Conclusion: SWAN sequence was much better than GRE-T2*WI in the sensitivity to show point-like short-T2 signal. Most point-like short-T2 signal was considered as cerebral microbleeds. It was revealed that the occurrence of point-like short-T2 signal was significantly high in lacunar infarction patients. Enough attention should be paid to those point-like short-T2 signal if necessary SWAN sequence should be performed before thrombolystic therapy, so this may guide and improve the clinical treatment plan.

Diagnostic value of magnetic susceptibility weighted imaging in occultcerebrovascular malformation

Objective: To evaluate the diagnostic value of susceptibility weighted imaging(SWI) in angiographically occult vascular malformations(AOVM). Methods: The imaging data of MRI were retrospectively analyzed in fifty-two cases(including 27 cases cavernous hemangioma and 25 cases venous angioma). Imaging findings and number of AOVM on SWI was compared with conventional MR sequences. Results: In all cavernous hemangioma cases, SWI detected more cavernous hemangioma lesions than those in other sequences. In all venous angioma cases, the dilated draining veins and medullary veins were shown well on SWI, and assimilated to a “Caput Medusae”. Conclusion: SWI could detect lesions of cavernous angioma and venous angioma more sensitively than other sequences, which is a reliable diagnostic technique for AOVM. SWI has higher clinical value, and should be the routine sequence in the MRI examination.

A comparative study of the intracerebral hematoma dynamic absorption rate under different treatments

Objective: To quantitatively assess the dynamic changes of hematoma absorption rate after different treatments with hematoma volume measurement software. To provide support for clinical treatment programs. Method: The changes of hematoma volume of conservative treatment group and drilling skull drainage group were measured by hematoma volume measurement software, the results were calculated, compared and analyzed. Result: The conservative treatment group was rechecked at the first, third and seventh day, and the average rates of hematoma absorption separately were 9.31%, 16.04%, 28.05%. The absorption rates of hematoma were reduced by turns with the volume of the first intracerebral hemorrhage: <10ml, 10~20ml, 20~30ml. In the drainage group the absorption rates of hematoma after operation were 30.19%, 34.11%, 58.38% at the first, third and seventh day. There was significant deviation in the reduced rates of hematoma volume at the third and seventh day between the conservative treatment group and drilling skull drainage group(P<0.0001). Conclusion: When the volume of hemotoma is more than 30ml, the hematoma cured after the drilling skull drainage treatment would be absorbed quicker than conservative treatment if the patient could tolerate the operation. The hematoma absorption rate is associated with the first amount of bleeding, and it is quickly absorbed if the amount of bleeding is small, otherwise, opposite.

The optimal b value in DWI of stomach neoplasm: a preliminary study

Objective: To choose the optimal b value in diffusion weighted imaging of stomach neoplasm. Material and Method: Thirty-two cases were proved to be stomach neoplasms by operation and pathology. Four DWI sequences were performed with different b values, including 400s/mm2, 600s/mm2, 800s/mm2, 1000s/mm2. The ADC value of different stomach neoplasm and free water in gastric lumen were measured. SIR of DWI with different b value was evaluated and compared. Then using image analysis software, the density of cells in pathologic specimens were calculated. Finally, use of statistical analysis software, correlation between the density of cell and ADC of stomach tumor under different b value was calculated. According to correlation of the above-mentioned calculations and SIR(ratio water-background noise), optimal b value was determined. Result: When b value was 400s/mm2, 600s/mm2, 800s/mm2, 1000s/mm2, correlation between the density of cell and ADC of stomach tumor under different b value was: -0.3458, -0.3789, -0.5865, -0.5231 respectively. There was significant difference between different b value(P<0.05). The SIR of different b value was: 2.036, 1.541, 1.148, 1.109 respectively(F=40.12, P<0.05). When b value was 400s/mm2, 600s/mm2, 800s/mm2, there were significant difference between them (P<0.05). When b value was 800s/mm2, 1000s/mm2, SIR approximate 1, there were significant difference between them(P<0.05). Conclusion: Diffusion weighted imaging for diagnosing gastric tumor is feasible, the best optimal b value is 800s/mm2. as b value as 800s/mm2, correlation between the density of cell and ADC of stomach tumor was the best, and SIR approximate 1. We can approximately think T2 influence was eliminated, and it can reflect the real situation of water molecules diffusion.

The clinical value of 18F-FDG PET/CT for investigating elevation of serum AFP inpatients with hepatocellular carcinoma after treatment

Objective: The aim of this study was to evaluate the value of 18F-FDG PET/CT in investigating hepatocellular carcinoma(HCC) with unexplained serum AFP elevation following surgical resection, transarterial chemoembolization(TACE) or radiofrequency ablation(RFA). Methods: A retrospective study was conducted involving 17 HCC patients who underwent 18F-FDG PET/CT to explain the evaluation of AFP without evidence of HCC recurrence examined by routine imaging. The 18F-FDG PET/CT results were correlated with histological findings, as well as long-term radiological and clinical follow-up(shortest follow-up period after FDG-PET was 6 months). Results: Among 17 patients, 12 patients demonstrated HCC recurrence(6 intrahepatic, 3 extrahepatic and 3 intrahepatic/extrahepatic). Metastatic foci were found in lymph node(3 patients), bone(1 patient), lung and muscle(1 patient), lung and crura of diaphragm (1 patient). The 18F-FDG PET/CT identified 7 intrahepatic and 6 extrahepatic. Overall, 18F-FDG PET/CT had 10 true-positives, 3 false-negatives, 4 true-negatives and no false-positives for the detection of HCC recurrence. The sensitivity, specificity, positive predictive value and negative predictive value were 77%, 100%, 82%, 100%, 57%, respectively. Conclusions: When conventional examinations are normal, the 18F-FDG PET/CT examination is a valuable imaging tool in patients who have elevated AFP value after HCC treatment.

Therapeutic effect of high intensity focused ultrasound in the treatment ofbenign prostatic hyperplasia through perineum

Objective: To explore the clinical curative effect of high intensity focused ultrasound(HIFU) in the treatment of benign prostatic hyperplasia(BPH) through perineum. Methods: The clinical effect of 18 cases of BPH treated by HIFU through perineum were observed and follow-up visit and ultrasonic inspection at 3 months, 6 months and 12 months after treatment were performed. Results: The volume of prostate had no significant change after treatment(P>0.05). Each index of IPSS score, uro-flow rate and residual urine volume was statistically different from prior treatment(P<0.05) but each index at 3 months, 6 months and 12 months had no statistical difference(P>0.05). Conclusions: HIFU through perineum for BPH has the advantage of microtrauma, safe and less side effect with stable curative effect. There are different extent of improvement in clinical symptoms.

Imaging features of Hirayama disease on fully flexed position cervical MRI and lateral cervical plain film

Objective: To explore the imaging features of Hirayama disease on fully flexed cervical spine on MRI and lateral cervical plain X-ray film. Materials and Methods: MRI examinations in fully flexed neck were performed on 23 cases of Hirayama disease and 12 normal control subjects. Lateral cervical plain films were performed on 18 cases of Hirayama disease. We summarized the imaging features of Hirayama disease. Results: ①For the angles, there were difference in total comparison(F=11.22, P<0.05). AC5-6 was the largest. ②23 cases(100%) in the patient group had localized lower cervical cord atrophy. ③23 cases(100%) in the patient group had localized lower cervical cord flattening. ④The levels of invloved cervical cords were localized in C6(23 cases), C5(18 cases),C7(11 cases) and C4(1 case). ⑤The anterior-posterior diameter of the cervical cord showed significant difference between patients and controls(t=-10.1, P<0.01). ⑥14 cases (61%) of patients had crescent-shaped high-intensity mass in the posterior epidural space. ⑦The curvature of the cervical spine was lordotic(3 cases 17%), straight(8 cases 44%) and kyphotic(7 cases 39%). Conclusion: Cervical MRI with fully flexed position has significant values on clinical diagnosis of Hirayama disease. Lateral cervical plain film shows values on the auxiliary diagnosis and screening of Hirayama disease.

Response of osteosarcoma to chemotherapy evaluated by 18F-FDG imaging

Objective: To evaluate potential role of 18F-fluoro-2-deoxy-D-glucose(18F-FDG) in assessing chemotherapy response of primary osteosarcoma. Methods: Seven patients with osteosarcoma confirmed by pathology were examined with 18F-FDG imaging before and after chemotherapy. Two practiced nuclear medicine doctor observed the image and do the semi-quantitative analysis. Tumor to non-tumor radioactivity count ratios(TNR) was used. The radioactivity count variance ratio before and after chemotherapy obtained by using formula(TNR1-TNR2)/TNR1. The histopathological tumor necrosis were obtained by surgery. Result: The radioactivity count before and after chemotherapy(TNR1max and TNR2max;TNR1average and TNR2average) had significant difference(P<0.05). Four of the 7 patients got histopathological tumor necrosis, which had a significant correlation to the radioactivity count variance ratio. The correlation coefficient of (TNR1-TNR2)/TNR1max and (TNR1-TNR2)/TNR1average with the tumor necrosis were 0.94 and 0.84 respectively. Conclusion: 18F-FDG imaging can be used to assess the chemotherapy response of osteosarcoma noninvasively.The semi-quantitative analysis which has consistency with the histopathological tumor necrosis is helpful in evaluating response of osteosarcoma to chemotherapy.
