The value of interictal 18F-FDG combined with 13N-NH3·H2O PET imaging in localizing epileptogenic zone in presurgical mesial temporal lobe epilepsy

Objective: The aim of this study was to evaluate the value of interictal 18F-FDG and 13N-NH3·H2O PET imaging in localizing the epileptic foci in patients with mesial temporal lobe epilepsy(MTLE). Methods: Preoperative MRI, 18F-FDG and 13N-NH3·H2O PET data in 17 MTLE patients confirmed by electrocorticogram(ECoG) and depth electroencephalogram(DEEG) were retrospectively analysed. The PET images were visually assessed with ROI-based analysis. The 18F-FDG and 13N-NH3·H2O PET images were compared. Results: According to the ROI-based analysis, the FDG uptake and the CBF(cerebral blood flow) correctly determined the side of the epileptic temporal lobe in twelve cases(70.6%) and nine cases(52.9%) respectively, correctly determined the side of the epileptic focus on the mesial temporal structure in five cases(29.4%) and four cases(23.5%), respectively. The diagnostic performance of the two agents were in moderate concordance by Kappa test(Kappa=0.638, P=0.005). And there was no statistically significant difference in the localization accuracy between the two agents. FDG uptake combined CBF correctly determined the side of the epileptic focus on the mesial temporal structure in eight cases(47.1%). Conclusion: Positron emission tomography measurement of FDG uptake was most sensitive in detecting the side of the epileptic temporal lobe, CBF PET also can lateralize the epileptic temporal lobe effectively. When combine FDG uptake with CBF imaging, the specificity may be improved.

The study of functional magnetic resonance imaging by different color light stimulating the visual cortex

Objective: To study the change of signal strength of visual cortex by stimulating it with different color. Materials and Methods: Forty-eight volunteers accepted the stimulation of red, green and blue light. Functional imaging analysis AFNI software was used for processing the raw data, computer automatically generated time signal intensity curves. And using statistical methods to compare them. Results: Occipital lobe in the district of cortex were excited in 48 cases, the average signal intensity of green stimulation was higher than that of red stimulation by 17.5%(P<0.05), the average signal intensity of blue stimulation was higher than that of green by 9.6%(P<0.05). The average signal intensity on the right was higher than that of the left by 18%(P<0.05). It was discovered that other brain excited areas were also found in the corpus geniculatum laterale and superior colliculus, anterior tectum of brain stem, parietal cortex. Conclusion: The statistical analysis of fMRI suggests that the signals of occipital lobe cortex is increased through three color stimulation, among them, blue stimulation is the most sensitive, and the change is more obvious in the right side than that in the left side.

E-tracking technology for evaluating the elasticity of common carotid artery in kidney transplant patients

Objective: To explore the value of echo-tracking(ET) technology in evaluating vascular function of kidney transplant patients. Methods: Using ET technique, the carotid elastic index Ep, β, AC in 91 kidney transplant patients and 60 normal adults were calculated and analyzed. Results: Compared with normal adults, the carotid artery Ep, β, and AC of transplant patients were different. There was difference between kidney transplant patients with and without hypertension. Conclusions: The carotid artery elasticity was decreased in kidney transplant patients, espespecially with hypertension, so that ET can be used to detect arterial elasticity early and may assess it quantitatively.

The role of CT imaging and X-ray barium examination in the evaluation of esophageal inlet lesions

Objective: To analyze the radiological features of esophageal inlet lesions by CT and barium study in order to improve the diagnostic accuracy. Methods: The clinical and radiological data of 34 cases with esophageal inlet lesions were retrospectively analyzed. All cases were pathologically confirmed by surgery or biopsy. Twenty-seven patients underwent CT scan with contrast and 7 patients underwent plain CT scan. Twenty-eight patients underwent barium study. Results: Surgery or biopsy revealed 14 cases with primary esophageal tumor(12 cases of squamous carcinoma and 2 cases of undifferentiated cancer), 3 cases were hypertrophy of sphincter, 5 cases were diverticulum, 12 cases were due to trauma. All patients presented with dysphagia. In cases with late stage esophageal cancer, barium study showed mucosa destruction. CT scan may have negative findings in patients with early stage cancer while irregular thickening of esophagus or intra-luminal mass can be found in patients of late stage. In cases of hypertrophy of esophageal sphincter, CT scan showed localized wall thickening of esophagus with smooth mucosa. In cases of esophageal diverticulum, CT scan revealed unenhanced mass of esophageal inlet. Barium study showed niche filling with contrast. In patients with esophageal perforation, barium study showed contrast dissemination into cervical soft tissue. In 2 cases of esophageal perforation with superior mediastinal abscess, CT scan showed low density lesion with typical ring-like enhancement. Conclusion: Concerning low-cost, convenience and easy-to-perform, barium study remains irreplaceable in diagnosis of esophageal inlet lesions. CT is significant in staging of esophageal tumor and evaluation of esophageal trauma and its complications. Combining both methods can improve the diagnostic accuracy of esophageal inlet lesions.

Evaluation the effect of simvastatin on vascular endothelial function by high frequency sonography in atherosclerotic rabbits

Objective: To assess the effect of simvastatin on vascular endothelial function by high frequency sonography. Methods: Fifty rabbits were randomly divided into control group(A group) and atherosclerotic group(B group) and simvastatin group(C group). Endothelial dependent dilation(EDD) and non-endothelial dependent dilation(NEDD) were measured with high resolution sonography at different time. Results: After intervention of simvastatin, the blood lipid decreased, EDD function was obviously improved, and NEDD was improved after 12 weeks. There was significant difference between the two groups. Conclusions: Simvastatin can improve endothelial function. High frequency sonography can be used to assess the effect of drug therapy on endothelial function.

Differential diagnostic value of CT in the mass-type chronic pancreatitis in the head of the pancreas and carcinoma of head of pancreas

Objective: To analyze CT characteristics of pancreatic carcinoma and chronic pancteatitis with mass in the head of the pancreas in order to improve its CT differential diagnostic accuracy. Materials and Methods: The clinical data of 30 cases with mass-type chronic pancreatitis in the head of the pancreas and carcinoma of head of pancreas were retrospectively analyzed. Results: The main CT differential points of the mass-type chronic pancreatitis and carcinoma of head of pancreas could be summarized as follows: ①calcification within the mass(17 cases, 0 case); ②contrast: lesions were raltive low density in portal venous phase(2 cases, 30 cases), and raltive equal density(28 cases, 0 case); ③shrink of the tail of pancreas(0 case, 18 cases), pancreatic pseudocyst(13 cases,1 case); ④dilatation of the pancreatic duct(18 cases, 25 cases), string of beads of dilation of pancreatic duct(15 cases, 4 cases), smooth dilation (3 cases, 21 cases), dilated pancreatic duct interrupt in the lesion area(4 cases, 12 cases), dilated pancreatic duct passed through the lesion area(14 cases, 1 case); ⑤dilatation of bile duct(25 cases, 12 cases), interrupt in the lesion area(2 cases, 21 cases), tapering from above downward(10 cases, 4 cases); ⑥no cross sign(0 case, 20 cases). Conclusion: The CT findings of the pancreatic head mass in chronic pancreatitis is characteristic, CT plays an important role in the of diagnosis and differential diagnosis of the two diseases.

Ultrasonographic analysis of retroperitoneal neurogenic tumor

Objective: The pathohistology of retroperitoneal neurogenic tumor is multiplex, by the analysis of the tumor’s ultrasonography, we try to discriminate the benign tumor from the malignant tumor and evaluate the ultrasonographic imaging features of retroperitoneal neurogenic tumor. Methods: The ultrasonographic imaging features of 25 cases were analyzed and compared with the results of pathology. Results: The common sites of retroperitoneal neurogenic tumor included spinal roots and the area surrounding kidney and adrenal; The tumors were large, mainly hypoechoic. Thirteen cases were cystic, 13 cases had calcification; No difference between the benign and malignant tumors on the shape and boundary(P>0.05); No difference between the benign and malignant tumors on the color Doppler flow imaging(CDFI)(P>0.05); The diameter of the malignant tumor was much larger than that of the benign tumor(P<0.025, a=0.05). Close and tiny calcification was the characteristic of the neuroblastoma, the CDFI of pheochromocytoma was plentiful, associated with dispersed small cysts and there were its characteristics. Conclusions: The ultrasonograhic features of retroperitoneal neurogenic tumor is not characteristic and ultrasonograhic accuracy was not high; It is not reliable to distinguish the benign and malignant tumors by the shape, boundary or CDFI only.

Functional anatomy of the posterior cruciate ligament in vivo with MRI

Objective: To research the functional anatomic change of the posterior cruciate ligament(PCL) in vivo. Methods: A total 40 knees from 30 healthy subjects, with mean age of 23 years, were studied to measure the change of the length and the deviation, and the elevation of PCL, and to observe the architecture of PCL on oblique coronal and sagital images at different flexion of the knees(0°, 30°, 60°, 90°, 120°) with MRI 3-dimensional multi-echo data imaging combination(3D-MEDIC) sequence. Results: The PCL elongated from 0° flexion to 90° flexion and shortened up to 120° flexion. The deviation of PCL decreasea from 0° flexion to 90° flexion but increased up to 120° flexion. The elevation of PCL was the smallest at 30° flexion and the largest at 120° flexion. Conclusion: It is feasible to make 3D-MEDIC of MRI to study the functional anatomy of PCL in vivo.

Basic experimental study of vertebral angiography using fibrin glue-based contrast agent

Objective: Preparation of fibrin glue-based contrast agent for vertebral venography, and make a preliminary study of its properties. Methods: Use of fibrinogen according to the volume ratio of 1∶1, dissolved in fibrinolysis inhibitor, thrombin according to the volume ratio of 1∶1, dissolved in calcium chloride solution after the combination of different proportions of the contrast agent, grouped using the X-ray imaging test approach and viscosity test to experimental different groups, in order to get the best volume ratio. Results: Ⅷ group with ensure a high ratio of viscosity have a better imaging performance, which can be more useful to help determine bone cement leakage. Conclusion: Fibrin glue-based imaging contrast agents was obtained through the best ratio of fibrin glue and the water-soluble iodine contrast agents, while maintaining its nature, it has greatly increased the viscosity. Which offers a new option for vertebral angiography.
