77 Research on right ventricular systolic function in pulmonary hypertension by tissue Doppler imaging techniques

Objective: To evaluate ventricular systolic function of pulmonary hypertension by tissue Doppler imaging techniques, and compare with the control group. To observe the changes of ventricular systolic function parameters in pulmonary hypertension patients. Methods: Sixty-two pulmonary hypertension patients and 30 normal subjects were observed. Color Doppler echocardiograph were performed and ventricular free wall thickness and end-diastolic area were observed. Right ventricular ejection fraction(RVEF) were measured with double plane Simpson formula. Peak systolic velocity of tricuspid valve ring(anterior leaflet, posterio leaflet, septal leaflet) was measured with tissue Doppler echocardiography. Peak systolic myocardial strain, strain rate of free wall of right ventricular and basal segment of interventricular septum, intermediate segment, apical segment were analysed off-line. Results: The right ventricular systolic function of pulmonary hypertension patients was decreased. The peak systolic myocardial velocity, strain, strain rate were lower than those of control group in different degree. Peak systolic myocardial strain of free wall of right ventricular basal segment, intermediate segment were lower than those of control group. Conclusion: The right ventricular peak systolic myocardial strain and strain rate, peak systolic velocity of tricuspid valve ring of pulmonary hypertension patients could be used as markers for evaluating ventricular systolic function. The ventricular systolic function could be evaluated more accurate when combined with right ventricular geometry indices and RVEF.

80 Evaluation of left atrial function in patients with atrial fibrillation using real-time triplane strain rate imaging

Objective: To evaluate the alteration of left atrial function in patients with atrial fibrillation using real-time triplane strain rate imaging and to investigate its clinical significance. Methods: Thirty-five patients with paroxysmal atrial fibrillation, 47 patients with persistent atrial fibrillation and 33 healthy volunteers were enrolled. Systolic peak strain rate, early diastolic peak strain rate and late diastolic peak strain rate were measured in different left atrial walls using real-time triplane strain rate imaging. Results: There was a significantly step-down change among control group, paroxysmal atrial fibrillation group and persistent atrial fibrillation group in mean systolic peak strain rate and mean early diastolic peak strain rate(P<0.05 or 0.01). Conclusions: Real-time triplane strain rate imaging is able to assess left atrial dysfunction due to atrial fibrillation.

83 An experimental study of impact on biliary pressure after stent implantation in distal common bile duct in dogs

Objective: To discuss the mechanism on the changes of the biliary pressure after metallic stent placement in distal common bile duct in dogs. Materials and Methods: Six healthy adult dogs were randomly selected, and then an animal model was accomplished to implant the metallic stent in distal common bile duct through percutaneous transhepatic biliary puncture. With biliary manometry, measured results of duodenal pressure(DP), sphincter of Oddi basal pressure(SOBP), sphincter of Oddi contractive amplitude(SOCA), sphincter of Oddi duration(SOD), common bile duct pressure(CBDP) were compared before and after stent implantation of distal common bile duct. Results: All of the six dogs were successfully performed with biliary puncture and pressure measurement. Five dogs were healthy and survived during the observation. DP was regarded as a zero, and then the relative results were obtained. Before stent placement, the results of SOBP, SOCA, SOD, CBDP were (13.69±4.29)mmHg, (42.65±8.50)mmHg, (6.69±1.46)s, (12.98±2.86)mmHg respectively. After 5 weeks of stent implantation, the results of which were as follows, (10.58±3.98)mmHg, (31.95±9.00)mmHg, (4.47±1.21)s, (7.43±2.20)mmHg respectively. Compared with those before stent implantation, SOCA was lower and SOD was shortened, and there were significant differences(P<0.05). CBDP was significantly lower than that before stent implantation, and had statistically significant difference(P<0.01). Conclusion: After 5 weeks of stent implantation in distal common bile duct in dogs, the changes of sphincter of Oddi motor function is manifested mainly in the weakened phasic contraction, but there is still the basic contraction. Although descending, the CBDP is still higher than normal DP, and maintains with the pressure gradient to the duodenum. So it is possible to prevent duodenobiliary reflux.

87 The best b value of MR diffusion-weighted imaging for pancreatic cancer

Objective: To investigate the best b value for pancreatic cancer with DWI. Methods: Routine MR, DWI with different combination of b values [(0, 50), (0, 400), (0, 700), (0, 1000)s/mm2] were obtained in 73 cases(41 pancreatic cancer, 32 normal). The maps and the value of ADC in pancreatic cancer, tissue near the cancer, normal pancreatic tissue were analyzed and measured respectively. Results: The high image quality of ADC was the combination of (0, 400)s/mm2. There was a significant difference among tissue near the cancer, normal pancreatic tissue and pancreatic cancer. Conclusion: The (0,400)s/mm2 is the best b value for pancreatic cancer with DWI.

90 Analysis of the relationship between the contrast-enhanced ultrasound and pathology of renal cell carcinoma

Objective: To observe the perfusion characteristics of contrast-enhanced ultrasound(CEUS) in renal cell carcinoma(RCC) by a new contrast agent SonoVue, and to analyze the pathological characteristics of RCC after operation. Methods: Forty-two cases of RCC were examined by the CEUS before operation, we observed the beginning of contrast agent enhancement, the peak and the dissipation as a whole, and analyzed the time-intensity curve(TIC) with ACQ software. Results: Twenty-eight clear cell renal cell carcinoma(CCRCC) cases showed rich blood supply, the results of TIC showed ΔAT and ΔTTP were smaller than the other two groups, the ΔPI and ΔI60s were significantly higher than the other two groups, that was “fast-in and slow-out and high enhance” contrast mode. Other 3 CCRCC cases, all the chromophobe carcinomas and papillary carcinomas showed low blood supply performance. The AT and TTP of chromophobe carcinomas and papillary carcinomas were slower than the surrounding renal tissue, the PI and I60s were lower than the surrounding renal tissue, that was “slow-in and low enhance” contrast characteristics. Conclusion: Real-time CEUS can provide more micro-perfusion characteristics in RCC. It provides more reference of the diagnosis and differential diagnosis in RCC, which has a higher clinical value.

93 The feasibility of nuchal translucency measurement at 11+2 to 14+2 weeks of gestation using three-dimensional ultrasound

Objective: To evaluate the feasibility of nuchal translucency(NT) measurement through standard midsagittal section acquired by three-dimensional(3D) ultrasound. Methods: 198 cases of 11+2~14+2 weeks of gestational age fetus underwent 2D and 3D ultrasound, NT was measured, comparative measurements between 2D and 3D ultrasound were performed to see whether there was significant difference. Results: The statistical assessment for all the 198 fetuses showed that the difference of NT values measured by 2D and 3D ultrasound was not significant(t=0.488, P=0.626>0.05), positive correlations were found between NT value and gestational age for both methods(r=0.536, P<0.05; r=0.543, P<0.05). Conclusion: Reconstruction and 3D extended imaging could obtain standard midsagittal section essential for NT measurement, so it is feasible to measure NT by 3D ultrasound, and its clinical value should be confirmed.

96 Value of transrectal ultrasonography in diagnosing gynecological diseases

Objective: To study the application value of transrectal ultrasonography(TRS) in diagnosing gynecological diseases and compare TRS technique with transabdominal ultrasonography(TAS) and transvaginal ultrasonography(TVS) technique. Methods: Eighty-seven gynecological patients without sex life history were selected randomly. They received TAS exam and following TRS exam with transvaginal intracavitary probe. Fifty-four gynecological patients were selected to receive TVS exam only. We compared TRS with TAS and TVS to explore the differences of visualization. Diagnostic accordance rate of TRS were compared with that of TAS in 87 patients by follow-up visiting. Results: The detailed visualizing ability in near field (uterus/ovary-center) of TRS was better than TAS significantly(P<0.01). For female patients without sex life history, total diagnostic accordance rate of TRS was higher than that of TAS significantly(88.14% vs. 42.37%, P<0.001). There were no significant differences of outline and detailed visualizing abilities between TRS and TVS(P>0.05). TRS and TVS had similar effects in gynecological detailed visualizing abilities. Conclusion: TRS, which can be taken as a selection of ultrasonography technique in diagnosing gynecological diseases, has outstanding detailed visualizing abilities for uterus and ovaries. It is more appropriate for the gynecological patients without sex life history and need detailed visualizing data.

99 Multislice spiral CT imaging study of the diseases involved pterygoid canal

Objective: To explore the MSCT appearances and its clinical application values of diseases of pterygoid canal. Materials and Methods: One hundred patients who had pterygoid canal lesions proved by clinical or pathlological materials were studied retrospectively, including 26 cases of bone fracture, 26 cases of fibrous dysplasia, 55 cases of tumor, 3 cases of inflammation. The volume data of all cases were subjected to multiplanar reformation(MPR), curved planar reformation(CPR) in working station after axial scanning. Results: The combinations of axial MSCT images and MPR images fracture lines and the distribution of bone chips were perfectly demonstrated. CPR images can best displayed the influences of fibrous dysplasia on pterygoid canal. When the severe inflammatory pathological changes intruded into pterygoid canal, the fatty tissue in pterygoid canal was blurred, the influence on bone wall in pterygoid canal was only slight. Secondary malignant tumors were the most common lesion of pterygoid canal, the bony wall of pterygoid canal was completely destroyed or canal enlarged. Conclusion: MSCT can accurately diagnose and define the diseases of pterygoid cana1.

103 Clinical value of diffusion tensor imaging fiber tracktography in lumbar disc diseases

Objective: To observe the morphous and integrity of lumbar disc annulus fiber with diffusion tensor imaging fiber tracktography(DTI-FT) and to evaluate the clinical value of the DTI-FT in lumbar disc annulus fibers diseases. Methods: Twenty-eight subjects divided into 3 groups, normal volunteer(n=10), lumbar disc herniation(n=12), lumbar disc extrusion(n=8). All were examined with MR scan. MR sequence contained conventional sagittal T1WI, T2WI and axial PDWI, T2WI and axial DTI. The DTI source images sent to the work station and postprocessed fiber tracking(FT) picture, diffusion coefficient average(DCavg) picture, and FA picture. Results: All subjects obtained FA picture and DCavg picture and used the software of “fiber tracking” to track the three dimensional images of lumbar disc annulus fibrosus. The normal disc annulus fibrosus presented a continuous and integrated circulus in FA picture and DCavg picture, the average ADC value and FA value were uniform. FT picture of the annulus fibrosus appeared continuous and complete. Three cases(30%) in disc herniation group could only see disc hernia on conventional MRI but annulus fibrosus disruption was found on DTI-FT picture. Five cases(62.5%) of lumbar disc extrusion group, annulus fibrosus disruption was found in DTI-FT picture. The DCavg value rised and FA value reduced in disrupted place. Conclusion: DTI-FT can noninvasively display morphous and integrity of the lumbar disc anulus fibrosus and can demonstre anulus fiber disruption. As an effective supplement of conventional MRI, DTI-FT can offer a precise guidance for treatment.
