837 MR diagnosis of atypical-location meningioma

Objective: To investigate the MR features of meningioma in atypical location. Methods: The MR features were analyzed retrospectively in 32 cases of meningioma in atypical location(lateral ventricular, brain, cerebellum, cistern, pineal region, orbit, jugular foramen region, paranasal sinus) proved by operation and pathology. Non-enhanced and contrast enhanced MR scanning were performed in all cases. Results: Malignant meningioma was found in 4 cases, benign in 28 cases; multiple meningiomas in 2 cases, single lesion in 30 cases(intra- extracranial communication meningioma in 2 cases). The MR imaging feature of atypical location meningioma showed intraparenchymal tumor; homogeneous isointense or slightly hyperintense or slightly hypointense signal on T1WI and T2WI; the shapes of mass were round, round-like or lobulated, homogeneous enhancement or target ring sign were found on Gd-DTPA enhanced MR scanning. Conclusion: Meningioma has certain imaging characteristics, when combined with clinical data, most of the atypical location meningioma can be made qualitative diagnosis, and have a certain degree of significance in guiding the selection of the mode of surgical operation.

841 The comparative study between ICA and SPM in the detection of activated brain regions

Objective: To access the concordance of the ICA and SPM in detecting task-induced activated brain regions. Methods: Twenty-eight healthy subjects(15 males, 13 females) performed fMRI scanning,which was block-designed with word generation tasks. SPM5 and GroupICATv2.0a software were used to process the data, then comparison of the results were made. Results: The activated brain region of traditional SPM were left lateral inferior frontal gyrus, left lateral insula, right lateral inferior frontal gyrus and insula, supplementary motor area, right lateral cerebrum, right lateral sriatum and left lateral sriatum. The result of ICA were anterior cingulate gyrus, both sides of middle frontal gyrus, inferior frontal gyrus, insula, left lateral superior parietal lobule and left lateral inferior parietal lobule. Conclusion: The activated brain regions identified by ICA were not completely the same as that identified by SPM, and the ICA result might be an important complement for the SPM result.

844 Left atrium function in patients with paroxysmal atrial fibrillation: analysis from two-dimensional speckle tracking echocardiography

Objective: To observe the changes of left atrium(LA) function in patients with paroxysmal atrial fibrillation(AF) using two-dimensional(2D) speckle tracking echocardiography(STE). Methods: The studied population consisted of 33 patients with paroxysmal AF and 30 age, sex-matched subjects in sinus rhythm(SR) referred to our echocardiography laboratory served as control group. All of the patients were in sinus rhythm when they were checked. Left atrial diameter(LAD), left atrial area(LAA), interventricular septum thickness(IVST); left ventricular end-diastolic diameter(LVEDD) were measured in 2D echocardiography imaging. Left ventricular ejection fraction(LVEF) was determined. Mitral valve A wave velocity time integral(VTI-A) and maximum velocity(VA) were measured. LA wall strain in the longitudinal direction were obtained by using 2D-STE. Peak atrial longitudinal strain(PALS) and atrial contraction longitudinal strain(ACLS) were measured in apical 4-chamber view and apical 2-chamber view. Time to peak longitudinal strain(TPLS) were measured. △-TPLS was defined as the difference between the TPLS in apical 4-chamber view and apical 2-chamber view. Results: Compared with control group, △-TPLS were significantly increased in AF group(P<0.05). In patients with AF, ACLS were significantly decreased than that in the control group(P<0.05). Conclusions: LA reservoir function were remained but there were conductive asymmetry between atrial walls in patients with paroxysmal AF. AF also hurt LA cardiac muscle, LA pump function decreased significantly. 2D-STE can effectively and easily measure LA △-TPLS and ACLS, speckle tracking echocardiography could be a method to non-invasively assess LA function in paroxysmal AF patients.

848 Application value of dual-source CT in the diagnosis of coronary artery origin anomalies

Objective: To study the application value of dual-source CT(DSCT) in the diagnosis of coronary artery origin anomalies. Methods: Three thousand five hundred and fifty patients undergone dual-source CT coronary artery angiography(DSCTA), the data were analyzed retrospectively. Results: Eighty-nine cases(2.50%, 89/3550) of coronary artery origin anomalies were detected by DSCTA. The origin anomalies consisted of 31 cases of high takeoff, 14 cases of multiple ostia, 5 cases of single coronary artery, 37 cases of coronary artery or branch from the opposite or noncoronary sinus, 2 cases of coronary artery from pulmonary artery. Conclusions: The DSCTA is a convenient, fast, and non-invasive method for detecting coronary artery origin anomalies, and it is an important and effective method for the diagnosis of coronary artery origin anomalies.

852 Clinical application of 18F-FDG PET/CT in the staging of gastric carcinoma

Objective: To assess the clinical application value of 18F-FDG PET/CT on TNM staging of gastric cancer. Methods: PET/CT scan were performed in 33 newly diagnosed gastric carcinoma patients within two weeks before operation. Primary lesions, lymph nodes metastasis and distant metastasis of all patients were diagnosed by pathological examination, other imaging findings and clinical follow up for more than 6 months. The value of PET/CT in the diagnosis and staging of gastric cancer was assessed by comparing the findings of PET/CT with the clinical findings. Results: Compared with operative pathology in 33 patients, the sensitivity of 18F-FDG PET and PET/CT for diagnosing primary lesions of gastric cancer were 84.8%and 100%. In 33 patients, the diagnostic concordance of PET/CT with pathological and clinical findings for T-stage, N-stage, M-stage and TNM-stage were 81.8%, 75.8%, 93.9% and 90.9%, respectively(Kappa value was 0.514, 0.557, 0.767 and 0.759 respectively, P<0.01). Conclusions: 18F-FDG PET/CT plays an important role in the diagnosis and TNM staging for gastric cancer with a good concordance of TNM staging with operative pathology. False-negative commonly occurs in patients with signet-ring cell carcinoma and especial attention should be paid on them.

856 MRI imaging features of xanthogranulomatous cholecystitis

Objective: To investigate MRI features of xanthogranulomatous cholecystitis(XGC). Methods: A retrospective analysis was performed on 15 patients with histological confirmed XGC. All patients underwent preoperative MRI scanning. The MRI features were: the presence of intramural nodules, mucosal line, the patterns of wall thickening and enhancement, and the presence of stones in the gallbladder. Peribiliary fat tissue and liver parenchyma changes were also reviewed. MRI features were correlated with pathologic findings. Results: Gallbladder wall thickening occurred in all of the 15 cases. The thickness of gallbladder wall was differ from 0.8cm to 2.6cm, in which 2 cases were local thickening, 13 cases were diffuse thickening; 2 cases were homogeneous thickening, and 11 cases had inhomogeneous thickening. Nodules were found in 10 patients, which were low signal intensity on T1WI and high signal intensity T2WI. Mucous membrane enhanced significantly in all cases, in which 11 cases had intact mucosal lining, 4 cases had interrupted mucosal lining. All patients had cholecystolithiasis. Temporal enhancement of arterial phase in liver parenchyma in all case. Seven cases associated with liver abscess. Six cases had duodenal adhesions. Conclusion: Frequent image manifestations of XGC were asymmetrical thickening of gallbladder wall, nodule in gallbladder wall, intact mucosal lining, and temporal enhancement of arterial phase in liver parenchyma. All of these features were relevant to pathological results.

860 Diagnostic value of morphologic analysis of MRCP for inflammatory pancreatic mass and pancreatic cancer in the head of pancreas

Objective: To study the value of morphologic characteristics of magnetic resonance cholangiopancreatography(MRCP) in the diagnosis and differential diagnosis of inflammatory pancreatic mass(IPM) and pancreatic cancer(PC) in the head of pancreas. Methods: MRCP findings in 30 cases of IPM and 30 cases of PC were analyzed. Results: MRCP characteristics in PC group included: ①dilatation of the pancreatic duct(25 cases), interruption in the lesion(26 cases) and continuous and smooth dilatation of the distal pancreatic duct(21 cases); ②dilatation of bile duct(25 cases), interruption in the head of pancreas(21 cases); ③dilatation of intrahepatic bile duct(24 cases). MRCP characteristics in IPM group included: ①dilatation of the pancreatic duct(18 cases) passing through the lesion(26 cases) and string-of-beads dilatation of the distal pancreatic duct(17 cases); ②dilatation of bile duct(12 cases), tapering(10 cases) or normal (18 cases); ③dilatation of intrahepatic bile duct(11 cases); ④calculus of pancreatic duct(14 cases) and paeudocyst(9 cases). Conclusion: MRCP morphologic characteristics had a great value in the diagnosis and differential diagnosis of inflammatory pancreatic mass and pancreatic cancer in the head of the pancreas.

863 Imaging diagnosis and staging of chordoma

Objective: To investigate the value of imaging in the diagnosis and staging of chordomas by analyzing their image characteristics and stages. Methods: Retrospectively reviewed the MRI and CT appearances of 21 lesions in 20 patients with initial diagnosis of chordoma, and imaging staging before operation, followed the local recurrence after operation. Results: Seventeen(17/21, 81.0%) chordomas located in typical region(skull base and vertebral), 4(4/21, 19.0%) chordomas were in rare location. In CT 19(19/21, 90.5%) chordomas showed expansive osteolytic bone destruction, 15(15/21, 71.4%) chordomas showed bone destruction with surrounding soft tissue masses. On MRI T2WI 90.5% chordomas showed honeycomb appearance. High local recurrence rate associated with advanced stage. Conclusions: Most chordomas with their location and margins could be correctly diagnosed pre-operation by MRI characteristics. MRI appearances also could provide objective basis for surgical resection and other therapy. Some prediction about local recurrence and prognosis may be taken on the basis of imaging staging.
