Clinical value of follow-up MR scans after radiotherapy for laryngeal carcinoma

Objective: To explore the clinical value of follow-up MR scans after radiotherapy for laryngeal carcinoma. Materials and Methods: To analyze follow-up MR appearances of the larynx in 37 patients of laryngeal cancer with pathological evidences after radiotherapy. Results: Eighteen patients received merely radiotherapy. Seventeen patients received laryngectomy and post-operation radiotherapy 6 to 8 weeks after operation. Two patients received radiotherapy because of recurrent carcinoma two years after operation. MR appearances showed edema with different degree at the sixth week, and extinct at the third to sixth months after radiotherapy. The affected range of tumor also reduced or disappeared gradually. Four patients showed chronic edema after 1 year, 1.5 years, 3 years and 6 years respectively. Recurrent tumors were found in two patients after merely radiotherapy and in five patients after operation and post-operation radiotherapy. One patient showed laryngeal stenosis with infection. Conclusion: Follow-up MR scans can provide reliable images about reactive edema, chronic lymphedema, laryngeal stenosis, improvement of the primary tumors and recurrent laryngeal carcinoma.

Evaluation of 99Tcm-MIBI myocardial perfusion imaging in patients of essential hypertension with microvascular damage heated with NTG intervention

Objective: To describe the radionuclide myocardial perfusion imaging(MPI) features of myocardial microvascular damage in the patients with essential hypertension and discuss the relationship between image characteristics and the severity of hypertension. Method: The 99Tcm-MIBI MPI features of resting imaging and nitroglycerin(NTG) intervention was described in 102 patients with essential hypertension by the method of qualitative and semi-quantitative scoring. Result: ①After NTG intervention, 68 and 170 MPI segments were reversed redistributed separately in CAG normal group and in CAG abnormal group. They accounted for 11.5% and 28.8% separately of all abnormal segments. The ratio of reverse redistribution had significant difference between the two groups(χ2=10.53, P<0.001). ②The MPI features in resting state had no significant difference with the image after NTG intervention in the patients with hypertension grade Ⅰ(t=1.81, P>0.05), but the MPI features of the resting state had significant difference with the image after NTG intervention in the patients with hypertension grade Ⅱ(t=8.23, P<0.001) and grade Ⅲ(t=7.55, P<0.001). ③ The average scores of SRS, SSS and SDS were increased separately from 2.83±1.25, 5.67±2.29 and -4.35±2.11 to 15.32±6.54, 24.19±7.36 and -14.62±3.82 with aggravation of high blood pressure. The average scores of SRS, SSS and SDS had significant difference among the three groups(F=25.77, P<0.05; F=27.69, P<0.01; F=26.34, P<0.01). The scores had significant difference between SSS and SRS in each hypertension group(t=4.35, P<0.001; t=7.78, P<0.001; t=6.17, P<0.001). Conclusion: ①The segmental number and severity of myocardial ischemic in 99Tcm-MIBI MPI can reflect the extent of coronary microvascular injury in the patients with essential hypertension. ②Reverse redistribution is the characteristic 99Tcm-MIBI MPI feature in the patients with myocardial microvascular damage after NTG intervention.

Evaluation of the effect of chronic pressure overload on rabbit’s left ventricular function using two-dimensional speckle tracking echocardiography

Objective: To assess left ventricular longitudinal, radial, circumferential strain and twist in rabbit models of left ventricular hypertensive hypertrophy and diastolic heart failure induced by abdominal aortic constriction using two-dimensional speckle tracking echocardiography(STE). Methods: Thirty-four male New Zealand rabbits were randomly divided into three groups: sham-operation group(control group, n=10), diastolic heart failure group(DHF group, n=12) and left ventricular hypertrophy group(LVH group, n=12). LVH and DHF were induced by suprarenal abdominal aortic constriction. After rabbit models were established, left ventricular strain and twist parameters were measured by STE. And left ventricular catheterization was performed. Results: Compared with sham-operation group, in LVH group, only longitudinal strain decrease significantly, in DHF group, longitudinal and radial strain both decreased significantly. Ptw increased significantly in LVH group, but increased nonsignificantly in DHF group. Conclusions: STE is useful in evaluating left ventricular performance in pressure-overload rabbit.

The technology of first-pass perfusion MR imaging and delayed enhanced MR imaging in the swine myocardial infarction model: a preliminary study

Objective: To investigate the dose of first-pass perfusion and the best delayed scanning time. Thereby, the purpose is to get a better cardiac perfusion MRI images and the best delay-enhanced images. Method: Five experimental pigs with myocardial infarction underwent two MR myocardial first-pass perfusion imaging, the contrast agent dose were 0.05mmol/kg and 0.1mmol/kg respectively. Ten experimental pigs with myocardial infarction underwent delayed enhanced MR imaging at 5min, 10min, 30min, 40min time point. The results were analyzed statistically. Result: There was statistical difference(P<0.05) between the two groups(0.05mmol/kg and 0.1mmol/kg) in detection of the myocardial perfusion decrease or defect on the first-pass MR perfusion imaging. The data of the delayed enhancement MR imaging in different scanning time showed. ①The signal values were statistically significant between 10 minutes and 5 minutes, 40 minutes and 30 minutes; ②The signal values of myocardial infarction showed rise in the stage of 5~10 minutes; and the signal values were relatively stable in the 10~30 minutes; During the stage of 30~40 minutes, the signal value of myocardial infarction showed a clear downward trend compared with the previous time. Conclusion: The perfusion imaging quality of the dose of the 0.1mmol/kg group is better than the 0.05mmol/kg group. It can detect more myocardial perfusion defects. Delayed time between 10~30min is the best scanning time to the delayed enhancement MR imaging. These are helpful in the assessment of myocardial viability using MR first-pass myocardial perfusion imaging and delayed enhancement MR imaging.

18F-FDG PET/CT imaging in the diagnosis of postoperative of gastric cancer recurrence and metastasis

Objective: To assess the value of 18F-fluorodeoxyglucose positron emission and computed tomography(18F-FDG PET/CT) in the post-operative patients with gastric cancer recurrence and metastasis. Methods: Of all the 112 patients who had received PET/CT examination for 150 times, the image analysis using visual and semi-quantitative analysis(SUVave), lesions confirmed according to the results of pathological examination, a variety of imaging techniques and clinical follow-up examination, follow-up time for more than half a year. Results: Sixteen cases showed tumor recurrence and metastasis, PET/CT diagnosis of gastric remnant recurrence sensitivity was 81.2%, specificity was 100%, accuracy was 97.3%. SUVave of recurrent lesions was 4.4±1.2, SUVave of anastomotic inflammatory lesions was 2.6±0.5, difference was statistically significant(t=3.9370, P=0.0005). PET/CT diagnosis of gastric cancer metastasis: sensitivity was 94.1%, specificity was 96.7% and accuracy was 95.5%. Twenty-six cases who had received PET/CT examination for two or more than two times, 15 cases who received the first PET/CT examination and 6 cases showed improvement after radiotherapy and chemotherapy. These patients survived after 6 months to 2 years of follow-up; PET imaging in 9 cases after treatment showed no significant change in conditions or the emergence of new lesions, the patient survival time was 3 months to 1 year. Conclusions: 18F-FDG PET/CT in the postoperative recurrence and metastasis of gastric cancer have a good diagnostic value, and have certain value in monitoring postoperative chemotherapy of gastric cancer.

Imaging findings of benign renal angiomyolipoma with minimal fat content

Objective: To analyze the imaging findings of benign renal angiomyolipoma(AML) with minimal fat. Materials and Methods: Ultrasonography, CT and MRI findings in 20 patients(6 males, 14 females) of benign renal AML with minimal fat confirmed by surgery and pathology were analyzed retrospectively. All patients underwent ultrasonography. Unenhanced and biphasic CT scans were performed in 12 patients, 1 patient underwent biphasic CT scan only. Unenhanced and dynamic contrast-enhanced MRI were performed in 12 patients, 1 patient underwent unenhanced MRI only. Results: Ten lesions(50%) were of irregular or ellipsoidal shape. The tumor-kidney interface was straight in 9 lesions(45%). Fifteen lesions protruded from the renal margin, and 7 lesions(46.7%) with restricted growth pattern or “fungus sign”. Compared with the surrounding renal parenchyma, 9 lesions(75%) showed homogeneously high attenuation on unenhanced CT images. Of the 13 patients examined with MR imaging, all were hypo-intense on T2WI, homogeneous(4 lesions, 30.8%) or with micro hyper-intense spot(9 lesions, 69.2%). On double-echo GRE chemical shift imaging, focal or diffuse signal suppression were found in 5 lesions(41.7%). All lesions showed homogeneous or reticular enhancement on biphasic CT and dynamic contrast-enhanced MRI images. Twelve lesions(60%) showed hyperechogenicity on ultrasonography, and 18 lesions(90%) were homogeneous. Tortuous and enlarged vessels were found in 3 lesions(15%). Conclusion: Benign renal AML with minimal fat usually has irregular or non-expansive growth pattern, homogeneous hyperechogenicity on ultrasonography, homogeneously high attenuation on unenhanced CT images, homogeneously hypointense on T2WI with micro hyperintense spots, homogeneous or reticular prolonged enhancement on contrast-enhanced CT and MRI images, female predominance, and tortuous or enlarged vessels were found occasionally.

Early diagnosis of prostatic cancer on 3.0T MR system

Objective: To study the early diagnostic value of 3.0T MR in prostatic cancer. Methods: To analyse the MR features in 12 cases of early prostatic cancer confirmed by surgery pathology, and needle biopsy. Conventional sequence, DWI and dynamic contrast enhanced scanning were performed. Results: The lesions of the 12 early prostatic cancer cases were all located in the capsule, T2WI fat suppression showed abnormal low signal within the peripheral zone in 8 cases, the other 4 cases showed slightly higher signal in the central zone, DWI sequence in 10 cases showed high signal, 2 cases showed iso-intense signals. LAVA multi-phase dynamic contrast-enhanced scans showed the lesions were obviously enhanced in the arterial phase, low signal in the balance period, in 8 cases 3 cases were continually and progressively enhanced; one case was not obviously enhanced in the arterial phase. Conclusion: 3.0T MR conventional T2WI sequences, DWI and dynamic contrast-enhanced imaging have characteristic MR features and important diagnostic value for early prostatic cancer.

The value of 1H-MRS in the diagnosis of the soft tissue tumors

Objective: To investigate the proton magnetic resonance spectroscopy(1H-MRS) features of benign and malignant soft tissue tumors in human and to explore its feasibility and application value in the differential diagnosis of soft tissue tumors. Materials and Methods: Forty-four cases were included in the study and divided into three groups. There were 10 cases in nomal group, 14 cases in benign tumor group and 20 casese in malignant tumor group. MRI and 1H-MRS were performed in all cases. The 1H-MRS features of all the 44 cases were analyzed. Cho/Cr and Lipid/Cr were calculated on workstation and a statistic test was done. We caculated the cell density of 30 soft tissue tumors and performed pearson correlation analysis between the cell density and Cho/Cr value. Results: The Cho/Cr value in the malignant tumor was larger than that in the benign tumor and normal muscle, but the Lipid/Cr value was smaller than that in the benign tumor and normal muscle. The differences of Cho/Cr value and Lipid/Cr value between malignant and benign soft tissue tumors were statistically significant(P<0.01). In contrast to benign tumor group, 1H-MRS revealed increased Cho, Lac and decreased Lipid and Glc peak in the 12 cases of malignant tumor. The average cell density was 152/FOV in the malignant tumor group and 57/FOV in the benign tumor group. The value of the cell density had a significant positive correlation with the Cho/Cr value in soft tissue tumors(r=0.891, P<0.001). Conclusion: The benign and malignant soft tissue tumors in human showed great difference in 1H-MRS. 1H-MRS has high application value in the differential diagnosis of soft tissue tumors.

Magnetic resonance imaging features of spinal cord schistosomiasis

Objective: To discuss the MRI findings of spinal cord schistosomiasis and its diagnostic value. Materials and Methods: To review the MRI data of 11 cases of spinal cord schistosomiasis, all cases were diagnosed by the trial treatment and laboratory test or by the surgical-pathological examination, all cases were examined with MRI plain scans and contrast scans. Results: MRI showed mild to moderate expansion of the distal spinal cord and conus in 11 cases, the lesions were iso- to slightly hypo-intense relative to cord tissues on T1- and heterogeneous hyperintense on T2-weighted spin-echo images, the signal intensity was lower than cerebrospinal fluid(CSF) on T2WI, soft tissue mass at intradural-extramedullary with abnormal signal intensity of the distal spinal cord in 2 cases. And intramedullary multiple small nodular or patchy enhancement in 7 cases, irregular masslike enhancement in 2 cases, peripheral linear enhancement on the cord surface in 6 cases and enhancement of the cauda equina roots in 5 cases and mild enhancement of the intradural mass in 2 cases after intravenous administration of gadolinium-DTPA. Conclusion: The MRI of spinal cord schistosomiasis presents some features, combined with the history of exposure to schistosoma-contaminated water, the immunological studies and the trial treatment are helpful to the diagnosis of the disease.
