505 The value of MSCT in the diagnosis of parotid basal cell tumor(an analysis of 15 cases)

Objective: To review MSCT imaging features of 15 cases of basal cell tumor in parotid gland and explore its diagnostic value. Methods: Fifteen patients with pathologically confirmed basal cell tumor underwent preoperative MSCT plain-scan and dual-phase contrast scan. The change of CT value and enhancement patterns in the tumors were assessed. Quantitative analysis was performed to determine CT value changes of the tumors in different enhanced-phases as compared with the plain-scan. Compare these imaging features of basal cell tumor with those of other common benign tumors: pleomorphic adenomas and adenolymphomas. Analysis of variance was used for the statistical analysis. Results: Basal cell tumors showed isodense nodule with significant cystic area on CT plain-scan. At dual-phase scan, basal cell tumors showed a pattern of persistent strong enhancement with characteristic cystic areas. At arterial phase, the increase of CT value was the highest for basal cell tumors, which had statistical difference from pleomorphic adenoma(P<0.01). At venous phase, the increase of CT value was statistically different between basal cell tumor and the other two histopathologic types(P<0.05). Conclusion: Basal cell tumors showed hetergeneous isodense nodule on CT plain-scan, which showed a pattern of persistent strong enhancement with characteristic cystic areas. The CT features of basal cell tumors were different from those of the other two bengin tumors.

509 Correlation of HRCT manifestation of temporal bone lesion with surgical operation and the coincident rate

Objective: To explore the diagnostic value of HRCT in the detection of lesions in the temporal bone and ossicular chain, by comparing the coincident rate between temporal bone HRCT diagnosis with operation. Methods: This retrospective study included 50 ears of 48 chronic suppurative otitis media patient that have complete clinical and CT data, HRCT features were correlated with surgical operation to explore the accuracy of HRCT in detecting ossicular chain in otitis patient. Result: HRCT had good coincident rate of ossicular chain with surgical operation, incus Kappa value is 0.8286, more than 0.75, indicated that HRCT had excellent coincident rate with surgical diagnosis. Kappa values of malleus, upper structure of stapes, incudomalleolar joint and incudostapedial joint are more than 0.40, indicated good coincidence between HRCT and surgical diagnosis. Conclusion: Temporal bone HRCT before operation can make accurate judgment of ossicular chain pathological change by use of different reconstruction methods, which help to do accurate judgment of ossicular change, have benefit to the choice of surgical pathway and method, and help to assess the surgical risk and prognosis.

512 High frequency ultrasonography in the diagnosis of carotid atherosclerotic plaque in aged patients with diabetes

Objective: To evaluate the application value of high frequency ultrasonography in the diagnosis of carotid atherosclerotic plaque in aged patients with type 2 diabetes. Methods: Carotid artery ultrasonography in 111 aged patients with diabetes were compared with that of 97 normal controls. Results: The intima-media thickness(IMT) and the incidence rate of the atherosclerotic plaques in the diabetic group was higher than that of the control group(P<0.05). There were no statistical difference in the echo and site of the plaques between diabetes and control group(P>0.05). Conclusion: The main lesion of carotid artery in aged diabetic patients was thickened IMT and carotid atherosclerotic plaques. Color Doppler ultrasonography can show the size and site and echo of the plaques, so it is important to early prevention and treatment for the complicaiton in diabetic patients.

516 Value of multi-spiral CTA in the diagnosis of arteriovenous malformations in oral and maxillofacial regions

Objective: To determine the value of computed tomography angiography(CTA) in the diagnosis of arteriovenous malformations in the oral and maxillofacial regions using imaging reformating techniques. Methods: A total of 8 patients with surgically and pathologically proved arteriovenous malformations in the oral and maxillofacial regions were enrolled. The imaging features of the lesions especially the CTA findings were retrospectively analyzed. Results: In the 8 patients, lesions were located in soft tissues in 5 patients and mandible in 3 patients. All the 5 patients lesions located in soft tissues had an ill-defined boundary without calcification. Hemorrhage was seen in 1 patient. All the 5 patients had obvious enhancement. An obvious expansive osteolytic lesions was found in 3 patients located in mandible. Also, an obvious enhancement was seen in the 3 patients. Through reformating techniques such as VR and MIP, CTA could well detect the feeding and draining vessels of the lesions. Conclusion: MSCT can directly, comprehensively show the lesion details and vessels of AVM located in oral and maxillofacial by means of different 3D image reformating technique.

519 Localization diagnosis of hyperfunctional parathyroid gland using 99Tcm-MIBI and 99TcmO4- imaging

Objective: To study the localization diagnostic value of nuclide imaging by 99Tcm-MIBI double phase parathyroid combined with 99TcmO4- thyroid imaging in patients with hyperparathyroidism(HPT). Methods: Thirty-six cases of primary HPT were included in this study. Control group included 10 cases with thyroid disease. Early phase imaging was performed in 15 minutes after injecting imaging agent 99Tcm-MIBI. Delay phase imaging was performed in 2 hours. Thirty-six HPT patients and 10 cases control group were also performed 99TcmO4- thyroid imaging 24 hours later. The two kinds of imaging were compared with each other, were analyzed, and were underwent imaging subtracted by eyes and imaging results judgement. Results: In 36 cases of HPT patients, 34 cases showed positive result imaging with 99Tcm-MIBI/99TcmO4- imaging. Two cases showed false negative result. Positive rate was 94.44% with 99Tcm-MIBI/99TcmO4- imaging. The positive result imaging showed abnormal increased uptake of 99Tcm-MIBI. In 36 cases of HPT patients, 12 cases showed positive result with single 99Tcm-MIBI imaging. Four cases showed false negative imaging. Twenty cases showed ambiguous positive results. Positive rate was 88.89%(55.56% was ambiguous positive results) with single 99Tcm-MIBI imaging. In 10 cases of control groups, 3 cases showed false positive results with single 99Tcm-MIBI imaging. One case showed ambiguous false positive results. No case showed false positive result with 99Tcm-MIBI/99TcmO4- imaging. Conclusion: 99Tcm-MIBI parathyroid double phase imaging combined with 99TcmO4- thyroid imaging have important value for diagnoses and localization of HPT. 99Tcm-MIBI/99TcmO4- imaging have higher positive rate and higher specificity than single 99Tcm-MIBI imaging for diagnoses and localization of hyperfunction parathyroid gland.

523 The application of dynamic enhanced spiral CT in the diagnosis of renal cell carcinoma

Objective: To investigate the signs of renal cell carcinoma(RCC), and improve the imaging diagnosis of renal malignant neoplasm with multi-phase dynamic enhancement CT scanning. Materials and Methods: The multi-phase contrast enhanced helical CT scan data of 72 patients with surgical-pathologically proved renal malignant neoplasm were analyzed retrospectively. Fifty-seven cases of RCC, confirmed by pathological techniques, according to WHO classification, these included 38 clear cell carcinomas, 10 papillary, 8 chromophobe and 1 spindle cell RCC, were analyzed. Enhancement degrees and patterns in unenhanced, corticomedullary, nephrographic phase and excretory phase were compared. Results: CT attenuation on unenhanced scan was not statistically different. CT attenuation of clear cell carcinoma was higher than that of papillary and chromophobe RCC in corticomedullary, nephrographic and excretory phase(P value is 0.01, 0.01 and 0.03 respectively), while that of papillary and chromophobe RCC was not statistically different in all three phases. Homogeneous and heterogeneous enhancement was most commonly seen in chromophobe RCC and clear cell carcinoma. Conclusion: Dynamic enhanced CT scan was effective for differentiating clear cell carcinoma from non-clear cell carcinoma, and was useful in differentiating RCC. It could be used in formulating therapeutic plan preoperatively.

527 The relationship between p53 protein and CT manifestation of the TN-staging in thoracic esophageal carcinoma

Objective: To study the relationship between the CT manifestation of the TN-staging and the expressions of p53 protein in thoracic esophageal carcinoma. Methods: p53 protein expressions in esophageal carcinoma and its relationship with the TN-staging by CT were analyzed by means of SP immunohistochemical technique and CT scanning in 66 patients with esophageal carcinoma proved by pathology. Results: There were 3 cases staged as T1, 10 cases as T2, 41 cases as T3, and 12 cases as T4 by CT. Twenty-one cases were detected to have lymph nodes metastases. The positive-expression rate of p53 protein was 62.12%(41/66). The positive-expression rate of p53 protein in T4-stage was remarkably higher than that in T1~2 and T3-stage(χ2=3.412, P<0.05, and χ2=3.381, P<0.05). The positive-expression rate of p53 protein in cases with and without lymph nodes metastasis was 80.95%(17/21) and 37.78%(17/45) respectively, their difference was statistically significant(χ2=10.686, P=0.001). The high expression rate of p53 protein in group T1~2, T3 and T4 was 23.08%(3/13), 46.34%(19/41), 66.67%(8/12) respectively. The expression intensity of p53 protein was increasing in accordance with the rise of T-staging(rs=0.270, P=0.028). The positive-expression rate of p53 protein in cases with lymph nodes metastases(short axis diameter≤2cm) was 84.62%(11/13), in mediastinal lymph nodes metastases(short axis diameter>2cm) was 75.00%(6/8). Their difference was not statistically significant(P=0.289). Conclusion: The high expressions of p53 protein were correlated to CT TN-staging. The expression intensity of p53 protein is increasing in accordance with the rise of T-staging. There was no correlation between the size of lymph nodes metastases and the expressions of p53 protein.

530 The fMRI research of the vestibular vertigo induced by the ice-water

Objective: To study the increase and decrease of MR signal in the functional cortical regions related to the vestibular vertigo with caloric stimulation, 0℃ water, using blood-oxygen level dependent functional MRI(BOLD-fMRI) technique. Materials and Methods: Twenty right-handed human volunteers, 12 males and 8 females, age from 25 to 39 years old, had nothing wrong with the hearing ability and the cavity of the middle ear after the otoscopic examination, the pure-tone audiometry and the tympanometry tests. They were all examined with a recording electronystagmograph before the fMRI scan to confirm that the caloric stimulus actually induced vestibular vertigo and nystagmus. During the electronystagmograph test, 15ml ice water(0℃) was injected into the right ear of each volunteer within 15s with the subject’s eyes open. The functional images of brain were acquired with a 1.5T Signa Infinity Twin+Excite MR system of GE Co. All the subjects were blindfolded with eyes open in a dark circumstance. All data were analyzed on single-subject level and group level using SPM 99. Result: Several cortical and subcortical regions were activated and deactivated. The activated regions included: the supplementary motor area, paracentral lobule, left precuneus lobe, left posterior insula, left MT/MST complex, the cingulum gyrus, left occipital lobe(BA19) and left cerebellum. The deactivated regions included: bilateral superior and middle frontal gyri, right precentral gyrus(BA2/3), left precuneus lobe, right MT/MST. Conclusion: Cold water stimulation at 0℃ in fMRI confirms a widespread cortical network involved in vestibular signal processing.

535 Clinical and imaging analysis of gastrointestinal stromal tumors(report of 20 cases)

Objective: To analyze the clinical and imaging features of gastrointestinal stromal tumors(GIST) and explore the imaging value in diagnosing the disease. Materials and Methods: The clinical and imaging data of 20 patients with GIST proved by operation and pathology were analyzed. Results: Tumors located in stomach 3 cases, in small bowel 11 cases, in colorectum 1 case and in extra gastrointestinal sites 4 cases. Immunohistochemical study diagnosed the GISTs as benign in 1 case, intermediate in 7 cases and malignant in 12 cases. The common symptoms were gastrointestinal tract bleeding(5/20), vague abdominal discomfort and abdominal pain(13/20), abdominal tumors(6/20) and abnormal bowel movements(3/20). The main imaging appearances included predominating exophytic growth, necrosis and liquefaction of tumors. On enhanced CT scan, the tumor enhanced moderately. Mass border is clear or unclear and metastases to local lymph nodes is uncommon, but it is easy to metastase to liver or seeding in abdominal cavity. Conclusion: The imaging features of GIST have some characteristics. CT, MRI examination have high value in detecting its location, evaluating its characteristics and treatment.

538 CT features of extra-pancreatic phlegmon in acute pancreatitis anatomic-pathological correlation

Objective: To study the CT features of extra-pancreatic phlegmon in acute pancreatitis(AP) and the anatomic-pathological bases of its distribution. Materials and Methods: The CT findings of 126 cases of AP proved by operation or clinical treatment were retrospectively analyzed. The CT features of the phlegmon out side of the organ and the anatomic-pathological bases of its distribution were studied. Results: The CT findings of phlegmon appeared irregular soft tissue mass with CT value within 5~30HU, the marginal zone of the phlegmon appeared low-grade enhancement on enhanced scan. The distribution of phlegmon in 259 lesions more commonly appeared in lesser omental bursa(93 times), left anterior pararenal-space(69 times), right paracolonic groove(26 times) and left paracolonic groove(21 times), others including right anterior pararenal-space(12 times), pelvic cavity(11 times), posterior pararenal space, perirenal space and left suprahepatic space(5 times) respectively, left subhepatic space(4 times), right suprahepatic space, left mesentery sinus and thorax(2 times) respectively, right mesentery sinus and pericardiac 1 time respectively. Multiple lesions appeared in 67 cases. Conclusions: The CT findings of phlegmon out of the organ in AP were characteristic. The distribution of the phlegmon depended on the anatomic bases of the abdomen, but some compartment may be involved due to movement of the fluid and the devastating of the phlegmon.

541 The characteristics of dynamic contrast-enhanced MR imaging in cervical cancer

Objective: To approach the characteristics of dynamic contrast-enhanced MR imaging in cervical cancer. To provide reference to contrast-enhanced phase choice in cervical cancer. Methods: Twenty-five cases with pathologically proved cervical cancer were served as study group, 4 cases with cervical leiomyoma and 10 volunteers as control group. Plain scan and dynamic contrast-enhanced MR scanning were given to the patients in the study as well as to the individuals in the control group. For imaging data analysis, ROI was selected and time-intensity curves(TIC) were derived from dynamic contrast-enhanced MR imaging of ROI, calculate ROI enhanced rate(E)=(the post contrast-enhanced signal intensity-the precontrast-enhanced signal intensity)×100%/the precontrast-enhanced signal intensity. On statistics, we compared the differences of enhanced rate respectively at different time, 16s, 32s, 48s, 64s, 128s. The T-test was used, P<0.05 was considered statistically significant. TIC were divided into four types. Results: Almost all cervical cancer showed marked contrast-enhanced appearance in the early period(0~32s). There were significant difference in early enhanced rate between cervical cancer, normal stroma of cervix and leiomyoma(P<0.05), but there were no significant difference on enhanced rate in the intermediate and late phase(P>0.05). In 25 cases with cervical cancer, 22 cases had type Ⅰ curve, 3 cases had type Ⅱ curve. In the control group, almost belong to type Ⅲ and type Ⅳ curve. Conclusion: Dynamic contrast-enhanced MR imaging can reflect the blood supply of the tumor, to emphasize signal change and enhanced rate in the earlier period(0~32s). Dynamic contrast enhanced study is useful to make diagnosis.
