305 MRI diagnosis and differential diagnosis of lateral ventricular astrocytoma

Objective: To investigate the MRI features of lateral ventricular astrocytoma, and their relationship to the circumventricular organs. Methods: The MRI features were retrospectively analyzed in 14 cases of astrocytoma in lateral ventricle proved by operation and pathology. Nonenhanced and contrast enhanced MRI scanning were done in all cases, including 8 males and 6 females. The patient’s age ranged from 13~62 years old with average age of 26 years old. Result: One case (1/14) as pilocytic astrocytoma; 7 cases (7/14) were of WHO grade Ⅱastrocytoma, 4 cases (4/14) were of WHO grade Ⅲ astrocytoma, 2 cases(2/14) were of WHO grade Ⅳ astrocytoma. In 11 cases, MRI appearance agree with the operations: 2 cases arised from the septum pellucidum, 3 cases situated near the foramen of Monro, 2 cases had obscure boundary with the corpus callosum, and 4 cases had obscure boundary with the thalamus. Conclusion: Base on tumor’s location, patient’s age, and their MRI appearance, it is possible to improve the diagnostic accuracy of lateral ventricular astrocytoma.

309 Study on imaging characteristics and regularity of adult corrected transpositionof the great arteries in color Doppler echocardiography

Objective: To probe into the imaging characteristics and regularity of color Doppler echocardiography(CDE) with adult corrected transposition of the great arteries(CTGA). Methods: Forty patients with adult CTGA were examined by CDE, 35 cases had angiocardiography and 38 cases were proved by surgical operations. Result: Thirty-nine cases were properly diagnosed based on the CDE characteristics with diagnostic accuracy of 97.5%, 1 case was misdiagnosed as complete transposition of the great arteries. The CDE characteristic and regularity of adult CTGA were obvious: ①Apical four-chamber view of two-dimensional echocardiography(2DE) showed atrioventricular discordance in all patients with atrial situs solitus and an L-loop ventricle in SLL and atrial situs inversus and an D-loop ventricle in IDD. ②The two great arteries appeared as two circular echo signals in the parasternal short-axis view, namely, “double circle” sign of 2DE. The type of transposition of the great arteries can be concluded according to the relationship between the two circles. In SLL, the two circles were positioned in the left anterior-right posterior direction and in IDD, they were positioned in the right anterior-left posterior direction. Pulmonary artery stenosis(PS) and pulmonary hypertension(PH) could be distinguished according to the internal diameters of the two circles. In PS, the posterior circle was smaller than the anterior one and in PH, the posterior circle was bigger. ③Color Doppler flow imaging(CDFI) showed unclear shunt flow signals through the interatrial septum or interventricular septum when patients associated with atrial septal defect or ventricular septal defect. In patients associated with atrioventricular valve insufficiency, CDFI showed obvious reflux signals through atrioventricular valve and in those with PS CDFI showed obvious jet flow signals. Conclusions: Adult CTGA has obvious CDE features and regularities. Identification of atrioventricular and ventriculoarterial discordance with 2DE is the key point to diagnose CTGA. Diagnosis of the associated anomalies of CTGA by 2DE is somewhat difficult but is of great clinical value in deciding operation methods.

313 Echocardiographic hemodynamics and imaging regularity of unroofed coronary sinus syndrome

Objective: To explore the hemodynamic characteristics and regularity of unroofed coronary sinus syndrome(UCSS) on echocardiograph. Methods: The composite echocardiograph including transthoracic echocardiography(TTE), transesophageal echocardiograph(TEE), real-time three-dimensional echocardiograph(RT3DE) and contrast echocardiograph(CE) was performed in 24 patients with UCSS that were confirmed with CT or surgical findings. Results: Twenty-four cases of UCSS includes 14 cases of typeⅠa, 2 cases of type Ⅰb, 2 cases of type Ⅱa, 2 cases of type Ⅱb, 4 cases of type Ⅲb. Color Doppler flow imaging(CDFI) or CE showed persistent left superior vena cava(PLSVC) draining into the left atrium on type Ⅰa UCSS, PLSVC draining into the right atrium and “spillway-like” shunting into the left atrium and atrial level left to right shunt on type Ⅱb or type Ⅲb. Conclusion: The hemodynamics of UCSS varies greatly depending not only upon its type but also upon various associated congenital anomalies. Its accurate diagnosis is sometimes difficult by TTE due to its atypical hemodynamic manifestations and may reliably be drawn by TEE, RT3DE and especially CE via finding out atrial level shunt or abnormal PLSVC draining into left atrium.

316 CT imaging diagnosis of fibrous dysplasia of vertebrae

Objective: To analyse the CT imaging findings of fibrous dysplasia of vertebrae and evaluate the imaging characteristics. Materials and Methods: CT imaging findings in 6 cases with pathologically confirmed fibrous dysplasia of vertebrae were reviewed retrospectively. Among all the patients, 4 cases were male and others were female. The range of age was 31~49 years. All cases were examined by CT plain scan, 1 case was processed by MPR image reconstruction of coronal and sagittal plane. Results: Of 6 cases, 5 patients had monostotic fibrous dysplasia including 3 patients with solitary lesion and another 2 with multiple lesions, 1 patient had polyostotic fibrous dysplasia. Among them, the lesions of 2 cases were located in thoracic vertebra, 4 in lumbar vertebra. CT showed monostotic with focal lesion, monostotic with multiple lesions and polyostotic cystic radiolucent lesions with sclerotic margin, internal “ground glass”-like density or osteoid high density, but the contour of vertebral bodies was normal. The lesions located in vertebral body can be seen in all cases, one case of monostotic fibrous dysplasia with multiple lesions had involvement of appendage whereas the lesion of the polyostotic fibrous dysplasia case affected appendage, proximal rib and articulation. Conclusion: Fibrous dysplasia of vertebrae shows distinctive CT imaging features that are different from fibrous dysplasia of bone in typical sites. And it could display some collective features of fibrous dysplasia of bone. These features can be helpful to make a correct diagnosis.

319 The study of high resolution blood oxygen level dependent MR venography in intracranial masses

Objective: To investigate the value of high resolution blood oxygen level dependent MR venography(HRBV) in diagnosing intracranial masses. Methods: Thirty intracranial masses performed by histology, 6 caverous angiomas, 2 arteriovenous malformations, 6 meningiomas, 6 glioblastomas, 2 acoustic neuromas, metastases and arachnoid cysts, 1 of each of the following trigeminoma, medulloblastoma, hemangioblastoma and schistosomal granuloma. Routine MRI plain scan, contrast-enhancement and HRBV were performed in all patients. Results: Copious petty veins distributed in 6 caverous angiomas. The sign of iron rims and hemorrhagic focus were detected in 4 caverous angiomas. Profuse gross veins distributed in 2 arteriovenous malfomations. Many veins distributed in the high grade glioblastomas, acoustic neuromas, trigeminoma, schistosomal granuloma and 4 meningiomas. Many patchy hemorrhagic focus distributed in the high grade glioblastomas. A big fistular vein with branches located in the medulloblastoma. Armillary vein circled the metastases. Few vein were detected in the low grade glioblastoma, arachnoid cysts and 2 meningiomas. The edema around masses on HRBV were coherent with FLAIR. Conclusion: HRBV is useful to detect the venography of intracranial masses and can provide the information about blood supply, hemorrhagic focus and edema around masses. HRBV is an important tool to definite the histology of masses and direct the clinical therapy.

323 Comparison of 18F-FDG PET-CT and 99mTc-MDP bone scintigraphy in detection of bone metastasesof lung cancer

Objective: To compare the diagnostic value of 18F-FDG PET-CT and 99mTc-MDP bone scintigraphy for the detection of bone metastases in patients with lung cancer. Methods: We examined 49 patients, 28 men, 21 women with lung cancer, mean age (58.9±10.8)years. All patients underwent both whole-body 18F-FDG PET-CT and 99mTc-MDP bone scintigraphy within 10 days. Bone metastases were verified either by pathological results or MR findings or follow-up more than half a year. Results: Bone metastases were confirmed in 228 of the 259 foci. The sensitivity for bone metastases using 18F-FDG PET-CT and 99mTc-MDP as 90.8% and 56.6% respectively, the specificity was 93.5% and 45.2% respectively. The accuracy was 91.1% and 55.2% respectively. Conclusion: The specificity and the overall accuracy of 18F-FDG PET-CT for the diagnosis of bone metastases in patients with lung cancer are higher than those of 99mTc-MDP bone scintigraphy.

327 Paraganglioma: pathologic and CT findings

Objective: To investigate pathologic characteristics and the value of CT in diagnosing paraganglioma. Methods: The imaging findings in 42 cases examined by CT with pathologically-proved retroperitoneal paraganglioma were collected. The image characteristics were retrospectively analyzed and compared with histological features. Results: Paragangliomas are tumors that arise anywhere from the neck to the pelvis paralleling the sympathetic ganglion chain. Pre-contrast CT appeared iso- or hypo-dense and the solid component of tumors were markedly enhanced after contrast administration. The tumor size, extent, location, surrounding vessels involvement can be well demonstrated on MPR and VR by CT reconstruction. Conclusion: CT is an effective imaging modality in the localization and qualitative diagnosis of paragangliomas, moreover, it can provide more anatomical information for surgery.

331 Experimental study of preoperative volume evaluation in living donor liver transplantation using 64-slice spiral CT

Objective: To study the accuracy of liver volume measurement using 64-slice spiral CT(64-SCT) and analyze sources of such inaccuracies with regard to CT liver volume and actual liver volume through simulating donor liver resected with perfusion of liver transplantation, and analyze relationship of actual liver volume and the weight of the liver. Methods: Thirty rabbits were randomly assigned as experiment group and control group(15 for each); thirty rabbit livers were scanned using 64-SCT at three enhancement phases. The imaging data with two axial 2-D CT slices (1mm and 3mm) at the hepatic venous phase and portal venous phase was used for whole liver volumetry or measured on a dedicated image post-processing work station; Each actual liver volume and their weight were measured by water displacement and scales immediately, experiment group were simulated donor liver perfusion of liver transplantation, control group liver blood vessel were ligated prior to resection respectively. Results: The difference of CT liver volume and actual liver volume in the two groups were 20.2%±0.5%, 6.29%±0.43%, respectively. Liver density was (1.000±0.011)g/ml, (1.003±0.007)g/ml respectively. The CT liver volume were positively of the two groups correlated with actual liver volume(r=0.981, r=0.979, r=0.977, r=0.985, r=0.992, r=0.988, r=0.982, r=0.987, P<0.001), ANOVA indicated liver resected with perfusion of liver transplantation was the main sources of such inaccuracies with regard to CT liver volume and actual liver volume (F=420.380, P<0.05). No significant difference between the difference of CT liver volume in two slices and two phases(P>0.05, P>0.05), independent-sample t-test indicated liver density of two groups has no significant difference(t=-1.076, P>0.05). Conclusion: The positive correlation between CT liver volume and actual liver volume, the main sources of such difference between CT liver volume and actual liver volume was liver resected with perfusion of liver transplantation. 64-SCT can assess the liver volume correctly and can be repeatedly.

335 Applications of MRI in pediatric patients with secandum atrial septal defects

Objective: To evaluate the value of MRI in pediatric patients with secundum atrial septal defects(ASD). Methods: Twenty-one cases with secundum ASD (mean age 6.26 years; age range from 2 to 15 years) were evaluated by MRI including FIESTA, phase-contrast MRI and CE-MRA. Results: Of all the 21 cases with secundum ASD, 2 cases associated with partial anomalous pulmonary venous return, which was missed by transthoracic echocardiography(TTE) were detected by MRI. Nine cases underwent transcather closure of the ASD. Four had surgical repair. Measurements at PC-MRI of ASD size and septal rim distances to superior vena cava, inferior vena cava, tricuspid valve, right upper pulmonary vein in 4 cases agreed fairly well with surgical results. Measurements of defect sizes at phase-contrast MR had good agreement with measurements at TTE. The device size after closure of ASD in 9 cases was little larger than measurements at PC-MRI. Conclusion: MR imaging can enable determination of defect size, rim distances to adjacent structures, and anomalous pulmonary venous connections, which can provide some complementary information to TTE and even can be considered as an alternative diagnostic method to TTE before transcather closure of ASD.

338 Comparative study of coronary CTA using 64 MSCT and delayed-enhanced MRI in evaluating the volume of myocardial infarction

Objective: To assess myocardial hypo-enhancement in demonstrating the myocardial infarction(MI) in coronary artery CT angiography(CTA) performed by 64-MSCT and to compare the results with delayed-enhanced MRI(DE-MRI). Materials and Methods: Twenty-three patients suspected of MI underwent both 64-MSCT and DE-MRI scanning. On 64-MSCT scanning, 60~85ml iodine contrast medium(370mgI/ml) were injected and flushed with 40ml saline, injection velocity is 4~5ml/s. The short axis of the left ventricle was reconstructed with 8mm slice thickness. On MRI scanning, contiguous left ventricular short-axis views were obtained 10 to 15min after the administration of gadopentetate dimeglumine 0.2mmol/kg. The hypo-enhanced volume from CTA and delay-enhanced volume from DE-MRI were measured and compared. Results: The MI volume measured by 64-MSCT and DE-MRI were (7.66±5.95)ml and (15.33±9.71)ml, respectively. The hypo-enhanced volume measured by 64-MSCT was correlated with that measured by DE-MRI(r=0.727). Conclusions: The MI demonstrated as hypo-enhanced region on CTA was underestimated compared to hyper-enhanced volume by DE-MRI.

342 The risk of needle tract tumor seeding after RFA of the liver tumor

Objective: Needle tract tumor seeding was one of the complications after RFA, the risk was not consistent as reported. The present study analysis of a retrospective data base of 117 patients with malignant liver tumor who had undergone RFA, with the aim to determine the incidence and the risk factors associated with tumor seeding after RFA. Materials and Methods: A retrospective data base of 117 patients with liver tumor who had undergone RFA in Shengjing Hospital from April 2005 to March 2008 was reviewed to identify the patients with needle tract tumor seeding. The rate of tumor seeding after RFA was determined and some of the risk factors were evaluated. Results: The tumor seeding was identified in 5 out of 117(4.3%) patients based on CT image findings, which corresponded to a rate of 5 of 128(3.9%) per treated lesion. All the patients with tumor seeding were identified within 6 months after RFA. Conclusion: RFA was an effective therepy for liver tumors with a low risk of needle tract tumor seeding. The risk factors associated with tumor seeding included no electrocoagulation of the electrode tract and subcapsular tumor location. A close follow up and recheck was necessary within half a year after RFA.
