685 CT and MRI findings of the primary NK/T cell lymphoma in nasal cavity

To study the CT and MRI features of primary NK/T cell non-Hodgkin’s lymphoma in nasal cavity so as to promote the diagnostic accuracy. Materials and Methods: CT and/or MRI findings of 16 cases with nasal cavity NK/T cell lymphoma verified by pathology were analyzed retrospectively. Sixteen cases were examined with CT plain scans and 2 of them were done with CT contrast scans. Eight of the 16 cases were examined with MRI plain scans and 1 of them were examined together with MRI contrast scans. Results: Of all the 16 cases, the tumors involving unilateral nasal cavity in 6, bilateral in 10. The tumor was confined in the nasal cavity in 4, and with extranasal extension in 12. The tumor involvement of adjacent structures in 13 and into subcutaneous soft tissue of the nasal labial fold or face in 8, with the bony destruction in 9. CT showed the nasal cavity was filled with soft tissues and spread along the nasal mucosa, there were mild-to-moderate heterogeneous enhancement on post contrast CT. On T1WI showed the tumors were homogeneous and isointense, the signal intensity was similar to or slightly lower than muscle. On T2WI the tumors showed slightly heterogeneous and hyperintense, the signal intensity was higher than muscle but lower than sinonasal mucosa, there were mild-to-moderate heterogeneous enhancement after gadolinium injection. Conclusion: Primary NK/T cell lymphoma of nasal cavity has certain characteristics in its imaging appearances, the imaging findings can suggest the diagnosis and show ranges and degrees of the lesions and provide more accurate informations for clinical staging.

688 The value of spiral CT pulmonary angiography in the diagnosis of pulmonary embolism

To evaluate the efficacy and technique of spiral CT in the diagnosis of pulmonary embolism(PE). Materials and Methods: Two hundred and forty-nine consecutively registered patients with clinically suspected of PE were analyzed retrospectively. The technique of sprial computed tomography pulmonary angiography(SCTPA) and the appearance of PE were studied. Results: The peak time of enhancement in aorta is about 10.8s. There were 68 patients and 705 pulmonary arteries with embolism, including eccentric embolism in 506 pulmonary arteries(71.8%), centric embolism in 147 pulmonary arteries(20.8%), wall thickened in 19 pulmonary arteries(4.7%) and complete obstruction in 33 pulmonary arteries(2.7%). The other 181 had no PE. But there were other pulmonary lesions found in 105 patients. Conclusion: SCTPA is important in diagnosing PE, and it can detect other pulmonary lesions simultaneously. In patients with clinical suspicion of PE, SCTPA is the primary imaging technique.

691 The effects of Carvedilol in treating chronic heart failure evaluated by Tei index

To evaluate the effect of Carvedilol on the cardiac function in patients with chronic heart failure(CHF) using Tei index and other cardiac function index. Methods: Sixty-three cases of CHF(NYHA Ⅱ~Ⅲ) were divided randomly into two groups: one group received regular treatment, while the other group was treated with Carvedilol on the basis of regular treatment. All the patients were followed up for at least six months. Left ventricular ejection fraction(LVEF), Tei index, left ventricular internal diameter at end-diastole(LVIDd), interventricular septal thickness at end-diastole (IVSd), left ventricular posterior wall thickness at end-diastole(LVPWd), and isovolumic contraction time(ICT), isovolumic relaxation time (IRT), ejection time(ET) of left ventricle were measured by echocardiography. Results: Compared with pre-treatment, LVIDd, ICT, IRT and Tei index were significantly decreased and IVSd, LVPWd, LVEF, ET were significantly increased after treatment in both regular treatment group and Carvedilol treatment group. After treatment of six months, LVEF and ET in Carvedilol treatment group were significantly higher than those in regular treatment group(P<0.05,P<0.01), Tei index, and ICT in Carvedilol treatment group were significantly lower than those in regular treatment group(P<0.01, P<0.05). Conclusions: Carvedilol treatment together with regular treatment is more effective than regular treatment only in improving cardiac function of CHF patients and Tei index is more sensitive than LVEF and NYHA classification in evaluating cardiac function.

694 Correlation of intravesical prostatic protrusion with acute urinary retention in BPH patients

To investigate the relationship between the degree of intravesical prostatic protrusion(IPP) and acute urine retention(AUR). Methods: According to the occurrences of AUR, 97 benign prostatic hyperplasia(BPH) patients were divided into 3 groups. Group A included 54 patients who had never underwent AUR, and group B included 36 patients who had surffered AUR once or twice, while group C, the remaining 7 had suffered AUR at least three times. The IPP and prostatic volume(PV) was measured by transabdominal ultrasound(TAUS). We compared the clinical data of the three groups. Results: In the clinical data, there was statistical significance in the PV between A and B or A and C groups(P<0.05). But there was no significance in the PV between B and C groups(P<0.05). The degree of IPP has strong statistical significance between the three groups(P<0.05). There was positive correlation between IPP and PV(r=0.681, P<0.01). Conclusions: The IPP can reflect the changes of PV, the presence and severity of bladder outlet obstruction. The IPP assessed by TAUS has important clinical significance to determine the severity of BPH and occurrence of AUR.

697 Experimental study on spreading cortical depression in cat model using functional MRI

To study the brain functional activation of cat model with spreading cortical depression(SCD) induced by KCl stimulation using blood oxygenation level dependent functional magnetic resonance imaging(BOLD-fMRI). Methods: Six adult female cats were anesthetized with ketamine(25mg/kg, i.m.) and chlorpromazine(10mg/kg, i.m.). Craniotomies were performed in both frontal sinuses, which were then filled with agar(2% in sterile saline) to reduce image susceptibility effects between brain and adjacent tissue. A second craniotomy with durectomy 5mm lateral to the midline and 5mm posterior to the bregma exposed the surface of the suprasylvian gyri for KCl/agar and sterile saline/agar application respectively. Images were acquired using a 1.5T super-conducting magnet(Siemens Vision 1.5T). The functional imaging of brain cortex by BOLD technique was obtained in resting phase, KCl stimulated phase and sterile saline stimulated phase respectively. Results: The activated regions included the local areas which contacted KCl of the suprasylvian gyrus and adjacent areas of the marginal gyrus, and spread to posterior part of the marginal gyrus, but there were no activated regions on the opposite hemisphere. The signal of fMRI increased significantly in the local activated areas immediately after the application of KCl and sustained thereafter, showed on time-signal intensity curve. In contrast, the application of sterile saline did not cause the signal of fMRI increase. Conclusion: BOLD-fMRI is reliable in detecting the changes of cerebral blood flow(CBF) associated with SCD and is valuable for evaluating SCD.

701 Changes of superior mesenteric venous perfusion and splenic venous perfusion after hepatic cirrhosis: experimental study in canines

To discuss the changes of superior mesenteric venous perfusion(SMVP) and splenil venous perfusion(SpVP) respectively before and after cholestatic cirrhosis by CT perfusion of portal vein in dogs. Methods: Only partial liver was involved in this cholestatic cirrhosis model. Twelve dogs survived from the process of model building. By selective catheterization in superior mesenteric artery and splenic artery by femoral Seldinger technique, SMVP and SpVP were available after portal CT perfusion. SMVP and SpVP of the normal part and cirrhotic part, before and after cirrhosis, were compared respectively. Results: SMVP and SpVP of the involved lobe decreased significantly from those of normal state, and the decrease ratio of SMVP differed significantly from that of SpVP(P=0.003). After model building, there was significant difference between SMVP and SpVP of the involved lobe and those of the normal lobe, and the decrease ratio of SMVP differed significantly from that of SpVP(P=0.001). Conclusion: After cholestatic cirrhosis, SMVP and SpVP decreased significantly, and the decrease of SMVP contributed more.

705 Analysis of noncompaction of the ventricular myocardium by echocardiography

To observe characteristic presentation of noncompaction of the ventricular myocardium(NVM) by echocardiography. Methods: Echocardiographic images of 31 patients in our hospital who were definitely diagnosed as NVM were reviewed retrospectively from April 2005 to December 2007. Results: Family history was observed in 7 cases of 31 patients(22.6%), but was not observed in 24 cases(77.4%). Twenty-one cases had lesion impaired in left ventricle(67.7%), 2 cases had lesion in right ventricle(6.5%) and 8 cases in both ventricles(25.8%). Furthermore, involved ventricles in 31 cases(100%) were enlarged and ejection fraction of left ventricle in 29 cases(93.5%) were less than 55%. The most impaired areas in sequence were left ventricle apex in 29 cases(93.5%), papillary muscles in 26 cases(83.9%), chorda tendineaes in 6 cases(19.4%) and valve level in 1 case(3%). Ratios of the impaired ventricular wall thickness between the noncompacted trabeculated layer and the nontrabeculated compacted layer were from 2.2 to 3.1. And 31 cases(100%) indicated color Doppler blood flowing into intertrabecular recesses with dim blood in them and blood filling defect in trabecular muscles. Conclusions: NVM was difficult to diagnose definitely because of its various kinds of clinical manifestations. But echocardiographic images of NVM were characteristic, so echocardiography could be an excellent tool to diagnose NVM.

708 Left atrial appendage function in patients with paroxysmal atrial fibrillation: a transesophageal pulse tissue Doppler study

The left atrial appendage(LAA) function was evaluated in patients with paroxysmal atrial fibrillation by pulse tissue Doppler echocardiographic examinations. Methods: One hundred and seventy-six patients with paroxysmal atrial fibrillation were enrolled. Twenty-six patients having LAA thrombus or spontaneous echo contrast were selected as group Ⅰ and other 150 patients formed group Ⅱ. Thirty healthy subjects served as the control group(group Ⅲ). Transthoracic echocardiographic parameters included left atrial diameter(LAd), left ventricular ejection fraction(LVEF), left atrial ejection fraction(LAEF) and E/E’. In order to determine the LAA function, LAA peak filling flow velocity(LAAPFV), LAA peak emptying flow velocity(LAAPEV) and percentage of LAA area change(LAAAC) were measured. Pulse tissue Doppler of LAA wall parameters included LAA late systolic wave(LSW) and late diastolic wave(LDW). Results: Compared to group Ⅱ and III, LAAPFV, LAAPEV were lower in group Ⅰ(P<0.05); LAAAC and LAEF was also reduced in group Ⅰ(P<0.05); Pulse tissue Doppler LSW and LDW were found to be quite reduced in group Ⅰ(P<0.05); LAd and E/E’ increased in group Ⅰ(P<0.05). The study parameters were statistically similar in patients between group Ⅱ and Ⅲ(P>0.05). Conclusion: Pulse tissue Doppler may be a complement to flow velocities in evaluating LAA function for risk assessment of thromboembolism.

711 Correlation of CT value on contrast-enhanced CT, MVD and the expression of VEGF in renal pelvic carcinoma and its clinical significance

To investigate the correlation of CT value on contrast-enhanced CT, MVD and VEGF expression in renal pelvic carcinoma and to explore its clinical significance. Methods: The expression of VEGF and CD34 was evaluated by immunohistochemistry and the MVD was counted in 26 cases of renal pelvic carcinoma. CT value on enhanced CT were measured and divided into three groups of CT enhancement, slight, moderate and obvious enhancement. The correlation of CT value, VEGF and MVD was calculated statistically and the relationship between them and clinical stages of the tumor were also analyzed. Results: There was significant difference of MVD in the three different CT enhancement groups. There was no statistical correlation between CT enhancement and VEGF expression; There was no significant correlation between CT enhancement and clinical stages. There was positive correlation between expression of VEGF and clinical stages. There was no significant difference of MVD among different clinical stages and different groups of VEGF expressions. Conclusion: The CT enhancement of the tumor can properly reflect the tumor’s blood supply and neovascularization, and was correlated with tumor’s MVD. The expression of VEGF increased with the clinical stages. But MVD did not increase with the increasing expression of VEGF and clinical stages. The CT enhancement of the tumor did not increase with the clinical stages either, which may be due to the vascular permeability effect of VEGF.

715 Perfusion imaging of human acute pancreatitis using dynamic computed tomography

To describe CT perfusion imaging in patients with acute pancreatitis. Methods: CT perfusion imaging was performed in 60 patients including 30 healthy subjects without pancreatic diseases. The raw data was calculated by Siemens perfusion CT(PCT) and the perfusion values were compared. Results: Blood flow in patients with pancreatitis was (0.95±0.19)ml/(min·ml), in control was (1.63±0.34)ml/(min·ml). Blood volume in patients with pancreatitis was(0.22±0.06)ml/ml, in control was (0.26±0.04)ml/ml. Permeability in patients with pancreatitis was (1.25±0.97)ml/(min·ml), in control was (0.14±0.07)ml/(min·ml)(P<0.05). There was no significance between contrast CT and PCT. Conclusion: Perfusion was reduced and permeability was improved in patients with pancreatitis. The information afforded by CT perfusion imaging can help diagnosis of pancreatitis in the early stage.

718 Clinical application of 64-slice spiral CT in preoperative staging of colorectal neoplasms

To evaluate the value of 64-slice spiral CT in preoperative staging of colorectal carcinoma. Methods: Thirty-seven patients, who were strongly suspected to have colorectal carcinoma, underwent 64-slice CT before and after intravenous injection of iodinated contrast agent. CT virtual coloscopy(CTVC), MPR, SSD and Raysum images were obtained by using 4 different softwares in workstation. The results of 64-slice CT were compared with the findings of postoperative pathology. Results: 64-slice spiral CTVC could be helpful in showing the pathologic patterns of carcinoma, its accuracy rate was 91.90%. MPR was helpful for displaying directly the invasion of colonic wall and pericolic organ from colon carcinoma. The accuracy rate was 100%. Raysum was better to display the length of tumor, and its accuracy rate was 100%. MSCT pneumocolon examinations demonstrated 37 colorectal carcinoma. Sensitivity and positive accuracy value for T staging were 100%(37/37) and 89.2%(33/37), respectively; Sensitivity and positive accuracy value for lymph node involvement were 80%(24/30) and 70%(21/30), respectively. Conclusion: 64-slice spiral CT 3D-reconstructive images and virtual colonoscopy is a better method of depicting colorectal carcinoma, it could be more helpful to precise localizing and qualitative diagnosis. And it allows accurate staging for colorectal carcinoma, especially in detecting the invasion of adjacent tissues and distant metastasis.

722 Prostate cancer: a comparison study of 1H-MRSI and DWI

To analyze the characteristics of prostate cancer(PCa) in 1H-MRSI and DWI; to explore the relationship between(Cho+Cr)/Cit ratio and ADC value. Materials and Methods: The clinical and pathologic information of 38 patients with histologically proven PCa were studied. MR spectroscopic imaging(MRSI) and diffusion weighted imaging(DWI) were performed using a 1.5T superconductive magnet scanner(Siemens Sonata) with a pelvic phased array multi-coil. MRSI was acquired using CSI-3D-Prostate sequence. All of the data was transferred to Siemens Leonardo Workstation. The (Cho+Cr)/Cit ratio of PCa(n=38), benign prostate hyperplasia(BPH)(n=21) and peripheral zone(PZ)(n=17) were calculated by spectroscopy software. DWI was carried out with a modified single-shot SE echo planar imaging(EPI) sequence. Results: The (Cho+Cr)/Cit ratio of PCa, BPH and PZ was 3.98±0.12, 0.51±0.19 and 0.38±0.09(P<0.05), respectively. The ADC value of PCa, BPH and PZ was (49.32±12.68)×10-5mm2/s, (86.73±26.75)×10-5mm2/s and (126.25±27.21)×10-5mm2/s, respectively. The (Cho+Cr)/Cit ratio and ADC value of PCa was different from that of BPH and PZ(P<0.05). (Cho+Cr)/Cit ratio correlated positively with Gleason grade(r=0.431, P<0.05), while negatively with ADC value(r=-0.309, P<0.05). Conclusion: The metabolite ratios relate closely with tissue microstructure. Combined utility of MRSI and DWI can improve the detection of PCa.

726 The statistical evaluation of CT diagnosis for solitary pulmonary mass

To Compare the difference of CT diagnostic results for solitary pulmonary mass among different qualification physicians, through analysis there are different CT diagnostic accuracy among observers with different working background and experience. To explore methods and ways for raising CT diagnostic level of solitary pulmonary tumor is important. Methods: Forty-nine cases with pathologically proved solitary pulmonary mass are independently diagnosed with double blind method by three radiologists of different work experience on CT image. This study is to evaluate the sensitivity, specificity, accuracy rate, positive predictive value, negative predictive value and Youden value of solitary pulmonary mass. Result: There were six evaluation results for the same group of solitary pulmonary mass from three physicians. The sensitivity, specificity, accuracy rate, positive predictive value, negative predictive value and Youden value was 83.76%(196/234), 53.33%(32/60), 77.55%(228/294), 87.50%(196/224), 45.71%(32/70), 0.371 respectively. The Kappa value was 0.349(standard error: 0.64, P=0.000) for pathological diagnosis. There were no statistical significance of ROC area under the curve. The CT diagnostic results of stability between each radiologist exists statistical significance. Conclusion: To a certain extent, CT imaging could show the benign and malignant pathological features of solitary pulmonary mass. Follow-up visit, and special training are good methods for improving the diagnostic level of solitary pulmonary mass.
