381 Exploration of diagnostic value of three-dimensional contrast-enhanced magnetic resonance angiography for intracranial aneurysms

Objective: To assess the clinical value and accuracy of three-dimensional contrast-enhanced magnetic resonance angiography(3D CE-MRA ) in the detection of intracranial aneurysms. Methods: Three-dimensional time of fight magnetic resonance angiography(3D TOF-MRA) and 3D CE-MRA was performed in 27 of 51 patients with intracranial aneurysms, all patients were confirmed by DSA. 3D CE-MRA images and 3D TOF-MRA images were compared for detection of aneurysms and morphology of the aneurysm, neck and the relationship of the parent artery. Results: Twenty-nine aneurysms were detected in 27 patients, the sensitivity in the detection of aneurysms was 100%(29 of 29) for 3D CE-MRA and 86%(25 of 29) for 3D TOF-MRA. In the depiction morphology of aneurysm, neck and parent artery, 3D CE-MRA was significantly better than 3D TOF-MRA(P<0.01); In the demonstration of the thrombus in aneurysmal sac, there was no significant difference between 3D CE-MRA and 3D TOF-MRA(P>0.05). Conclusion: 3D CE-MRA is a fast, efficient, and noninvasive imaging method in diagnosing intracranial aneurysms.

384 CT and MRI characteristics of cerebral venous sinus thrombosis

Objective: To discuss the imaging feature of cerebral venous sinus thrombosis(CVST). Methods: The finding of CT, MRI and magnetic resonance venography(MRV) in 35 patients with CVST were retrospectively analysed. Results: The thrombosis of superior sagittal sinus, inferior sagittal sinus and straight sinus, transverse-sigmoid sinus was seen in 15 cases, 8 cases and 7 cases respectively. Some cortical drainage and large internal cerebral veins were also involved in 7 cases. In the acute phase, 10 cases showed the direct and indirect CVST signs by CT scanning, but subacute and chronic patients had no abnormality. MRI showed the disappearance normal flow void in the dural sinus, the abnormal high signal intensities were showed in 32 cases instead. MRV demonstrated that there was no blood flow in these involved dural sinuses or veins. Conclusion: CT is a simple and effective method to diagnose acute CVST. Conventional MRI combined with MRV are effective and noninvasive methods in the diagnosis of subacute and chronic CVST.

387 MRI characteristics of primary central nervous system lymphomas

Objective: To analyse MRI findings of primary central nervous system lymphoma(PCNSL) in immunocompetent patients. Materials and Methods: MRI findings in 21 patients with pathologically proved PCNSL were analyzed retrospectively, 14 male and 7 female, mean age, 49.6 years old. Results: Thirty lesions were identified in 21 patients. The MRI findings of PCNSL were as follows: ①hypo- or iso-intense on T1WI and iso- or slightly hyper-intense on T2WI, solitary or multiple well-defined lesions; ②slight or moderate peritumoral edema, and characteristic flame-like edema; ③enhancement is intense and homogeneous, the presence of “incision sign” and “angular sign” are more specific. Conclusion: Conventional MRI plays an important role in the diagnosis of PCNSL.

390 Pulmonary inflammatory pseudotumor: correlative study of HRCT features and pathological findings

Objective: To study HRCT features of 37 cases of pulmonary inflammatory pseudotumor and assess their value in differential diagnosis. Materials and Methods: Thirty-seven cases pulmonary inflammatory pseudotumors(PIP) confirmed at resection and pathology with diameter≤3cm were performed HRCT scan after common CT scan. The morphologic features of PIP with HRCT were analyzed and compared with pathological findings. 104 cases of peripheral lung carcinoma(PLC) were selected at the same time in random sampling as control. Results: The most common locations of PIP were at the margin of both lower lobes and associated with local pleural thickening and adhesion. The density of nodule in which CT attenuation value was 15~60HU(26.9±13.5HU) was lower than that of PLC. Most of them were round/round-like and irregular in shape. The interior density of lesion was inhomogeneous. The presence of calcification or multiple cavitations, straight margin sign, hole sign and without hilar and mediastinal adenopathy were of great significance in the diagnosis of such kind of disease. Conclusion: Some HRCT features of PIP could reflect their pathological findings. It is necessary to increase the diagnostic rate by comprehensive analyzing the HRCT features of PIP with the history of infection.

393 The appearance of bronchioloalveolar carcinoma on high resolution CT

Objective: To study the appearance of bronchioloalveolar carcinoma(BAC) on high-resolution CT. Methods: HRCT scans of 41 patients with pathologically proven BAC were retrospectively analyzed. 7 HRCT findings were separately recorded into 3 types(solitary nodule, multi-nodular, diffusion type) and were assessed with pathologic correlation. Result: HRCT findings of solitary nodule(n=18) are: typical peripheral or subpleural location in 15; pulmonary lesion with irregular margin forming a stellate or patchy pattern 8; vacuole sign and/or brochiologram 14; pleural retraction 12; The features of single lesion in multi-nodular(n=9) were the same as the former; Diffuse consolidative type(n=14): lobar or segmental homogenous consolidation (n=7); “withered tree” sign(n=4); “honey comb” sign(n=6); ground-glass opacity(n=8); acinar nodules(n=6). Conclusion: HRCT is an important method to determine the diagnosis and differential diagnosis of BAC.

396 The clinical values of coronary artery imaging with 64-slice spiral CT

Objective: To evaluate the diagnostic accuracy for the assessment of coronary artery diseases with 64-slice CT coronary angiography. Methods: Totally 562 patients suspected of coronary artery diseases(CAD) were examined by 64-slice spiral CT, 48 of them also compared with conventional coronary angiography(CCA) regarded as the gold standard. The visibility of coronary branches was evaluated. The left main artery(LM), left anterior descending artery(LAD), circumflex artery(CX), and right coronary artery (RCA) were screened for the presence of over 50% stenosis. Results: Among the 562 patients, 48 patients were consisted with CCA. 720 coronary arteries of them can be detected. 687 coronary segments were applied for quantitative analysis. Ninty-eight lesions were found by SCTCA, 92 lesions were found by CCA. The sensitivity, specificity, positive predict values and negative predict values were 94.5%, 98.1%, 88.7%, and 99.2% respectively for the detection of coronary artery stenosis with SCTCA. Conclusion: 64-slice CTCA has excellent sensitivity and specificity in detecting coronary artery stenosis. It is a useful non-invasive method for screening coronary artery diseases, an important role in clinical diagnosis.

399 Analysis of 18F-FDG PET uptake and prognosis of patients with malignant liver tumor

Objective: To evaluate the various levels of FDG uptake for patient with malignant liver tumor. Methods: FDG PET were performed for 28 patients with malignant liver tumor. Region of interest(ROI) was placed over the tumor regions and the standardized uptake value(SUV) was calculated. Then the tumors were divided into the following two types: FDG uptake increased(type A) was seen in 20 patients, including 19 HCC and one liver metastasis of gastrointestinal carcinoid tumour. FDG uptake similar to the surrounding nontumor liver tissue(type B) was seen in 8 patients, including 6 HCC and one liver metastasis of gastrointestinal carcinoid tumour and one liver metastasis of HCC. Result: After following up to 18~84 months, the cumulative survival rate was found to be singnificant higher in group B than in group A(44.8±19.7 months vs 20.2±16.8 months.)(P<0.05). The survival time was significant different between patient of liver metastasis from gastrointestinal carcinoid tumor according to the uptake degrees. Conclusions: HCC is heterogenous in terms of its glucose metabolism. The degree of FDG uptake is useful to predict the outcome of malignant liver tumors.

401 MRCP diagnosis of biliary stricture after liver transplantation

Objective: To discuss the feasibility of MRCP in the diagnosis of biliary stricture after liver transplantation. Methods: MRCP was performed in 19 patients with suspected biliary stricture in 3 to 10 months after orthotopic liver transplantation(OLT). Diagnostic confirmation was obtained with endoscopic retrograde cholangiopancreatography(ERCP) during one week after MRCP(n=16), two months’ clinical and laboratory follow-up(n=3). Combined with ERCP and clinical follow-up, the value and limitation of MRCP were analysed. Results: Fifteen patients were diagnosed as biliary stircture and had the stent implantation, in one case of whom had normal findings on MRCP and stent was implanted 15 days later because of the aggravation of the biliary obstruction. Two were anastomotic stricture(AS) and 13 nonanastomotic stircture(NAS). One case with donor-to-recipient common bile duct disproportion was misdiagnosed as anastomotic stricture. Three cases were confirmed to have cholangitis finally. For diagnosis of biliary stricture, MRCP had 93% sensitivity, 75% specificity. Our study showed that the bilary stricture were usually accompanied by the elevation of Alkaline phosphatase and Gamma glutamyl. Conclusion: Compared with ERCP, MRCP can show the biliary stricture correctly after OLT, but cannot discribe the lesions in detail due to the lower resolution and usually exaggerates the extent of stricture. Combined with the monitoring the level of Alkaline phosphatase and Gamma glutamyl, MRCP will provide the diagnosis early.

405 The value of MSCT urography in the diagnosis of ureteric carcinoma

Objective: To evaluate the diagnostic effect and characteric of MSCT urography in ureteric carcinoma. Materials and Methods: To analyse the MSCT features in 18 cases of ureteric carcinomas confirmed by pathology. Conventional scan and contrast enhanced scan including arterial phase, venous phase multiple-delay-phase scan were performed in all cases. The reconstruction methods such as MPR, CPR, VR, MIP were performed. Results: Axial images of arterial and venous phases together with MPR, CPR show the relationship between the tumor and its tissues around it clearly, especially CPR, it can well and continually show the whole ureter and the tumor through the contrast of urine and contrast medium, VR and MIP reconstruction are also good in showing the irregular stenosis and obstruction by the tumor. Conclusion: MSCT urography shows the ureteric carcinoma clearly from multiple planar, multiple directions . It offers a good method to clinical treatments.

408 Significance of 99Tcm-HMPAO-SPECT in clinical evaluation of glioma

Objective: To evaluate the significance of 99Tcm-HMPAO-SPECT in clinical evaluation of glioma. Methods: Seventy-two patients with primary glioma confirmed by pathology underwent 99Tcm-HMPAO and MRI. The uptake of 99Tcm-HMPAO was semi-quantified by tumor cerebellum index(TCI). In light of WHO classification system, these cases included 33 low-grade and 39 high-grade gliomas. Tumor MRI parameters i.e. enhancement, necrosis and edema were evaluated as three grades. Results: The relationships among 99Tcm-HMPAO uptake index and tumor histological grade and MRI characteristics were evaluated respectively. In the high-grade tumor group, 32.6% had a high uptake of 99Tcm-HMPAO(TCI>1.0). 55.8% low uptake(TCI<0.8), and 11.6% moderate uptake similar to normal brain tissue(TCI:0.8~1.0) In low-grade group, these ratios were 10.3%, 27.6% and 62.1% respectively. The difference between the two groups is significant. Both tumor necrosis and edema had a significant influrence on uptake of 99Tcm-HMPAO. Conclusions: The modification of rCBF in low-grade glioma is usually similar to normal brain tissue, but in high-grade lesion it is great. Tumor necrosis and edema are two factors in relation to low rCBF.

411 Systolic function of left ventricle in patients with myocardial infarction assessed by quantitative tissue velocity imaging and tissue tracking imaging

Objective: To quantitatively evaluate regional systolic myocardium function of left ventricle(LV) in patients with myocardial infarction by means of quantitative tissue velocity imaging(QTVI) and tissue tracking imaging(TTI). Methods: Twenty healthy subjects and 33 patients with myocardial infarction were studied by QTVI and TTI. Off-line LV regional velocity and displacement profiles along LV apical long-axis view, apical two-chamber view and four-chamber view were obtained synchronously, and peak velocities of LV regional myocardium during systole(Vs), and peak systolic displacement(Ds) were measured. LV ejection fraction(LVEF) was determined by 2D echocardiography using Simpson’s rule. Results: Both Vs and Ds of LV segments in myocardial infarction group(MI group) were significantly lower than those in normal control group(control group). Mean Vs(mVs) and Ds(mDs) of different segment level in MI group were lower than those of the control group. mVs and mDs in basal and middle segments level related significantly and in apical segments level did not relate to LVEF. Conclusions: The QTVI and TTI can judge the longitudinal systolic velocities and amplitudes, and is a non-invasive and quantitative method to assess systolic function of left ventricular regional myocardium.

415 CT diagnostic value of large cell lung carcinoma

Objective: To explore CT features of large cell lung carcinoma(LCLC). Methods: CT findings in 15 cases of LCLC confirmed by surgery and pathology were retrospectively analyzed. Imaging were obtained by whole body CT in 10 cases and multi-slice spiral CT(MSCT) in the others. Results: The lesions were all presented as peripheral. Most of them were located in peripheral areas with a mean size of 4.5cm×5.5cm, which presented as round, oval or irregular in shape. The features of shallow lobulation, smooth margin, spicule-like process and heterogeneous density existed in 13 cases, and smooth margin homogenous density in 2 cases. 1 case showed internal grit-like calcification, 1 case showed thick-wall cavities and 1 case showed thin-wall with typical “air-crescent sign”. After contrast moderately heterogeneous enhancement of solid portion existed in 9 cases, homogenous enhancement in 2 cases and ring-shaped enhancement in 1 case. Three cases demonstrated punctiform vascular-like enhancement in the edge and solid portion after 60 seconds enhanced MSCT. Lymph node enlargement in mediastinum or hilum of the same side in 10 cases, pleural involed in 4 cases, “pleural indentation sign” existed in 2 cases. Multiple planar reconstruction(MPR) could exactly display the mass spatial characteristic and relationship with normal lung field and pleura. Conclusion: Plain and enhanced CT(especially MSCT) with MPR were valuable methods to the diagnosis of LCLC.

419 The application of rest myocardial perfusion imaging in patients with cardiac autonomic neuropathy

Objective: Cardiac autonomic neuropathy(CAN) is one of the serious complications of diabetes, which might lead to myocardial ischemia and myocardial infarction. The aim of this study is to evaluate the clinical application of myocardial perfusion imaging(MPI) in the diagnosis of myocardial ischemia in patients with CAN. Methods: Forty-three patients with type 2 diabetes were divided into 4 groups: normal function group(N=8), early phase group(N=9), mid phase group(N=16) and late phase group(N=10). Rest-delay MPI using 201Tl and Holter were performed in all patients. Results: 55.8%(24/43) of patients were found by MPI to have myocardial ischemia, with a total of 67 reversible ischemic segments and 18 irreversible ischemic segments. The incidence of myocardial ischemia diagnosed by MPI was revealed to have a proportionate upward trend with the deterioration of CAN. Holter indicated a similar result with a comparatively low sensitivity(32.6%). Conclusions: MPI using 201Tl was proved by the study to be superior to Holter in the diagnosis of myocardial ischemia in patients with CAN. Early intervention will be accessible in CAN patients by using MPI.

423 The diagnostic and differential diagnostic values of CDFI for testicular torsion and testicular appendage torsion

Objective: To investigate the diagnosic and differential diagnosic values of color Doppler flow imaging(CDFI) for testicular torsion and testicular appendage torsion. Materials and Methods: A total of 44 cases of testicular torsion and 37 cases of testicular appendage torsion were analyzed and concluded by CDFI reviewed. Results: The specific sonographic signs of testicular torsion were that the blood flow was diminished or absent in the testicle, and there was a hyperechoic mass with no blood flow above the testicle. The specific sonographic signs of testicular appendage torsion were that unequal hyperechoic node with no blood flow was found between or beside testicle and head of epididymis as well as the blood flow was slight increased in the testicle and epididymis. Conclusion: CDFI has a great value in diagnosis of testicular torsion and testicular appendage torsion.

425 The ROC curve analysis of color Doppler ultrasound in differentiation of benign and malignant solid superficial soft tissue tumors

Objective: To investigate the value of color Doppler ultrasound in differentiation of benign and malignant solid superficial soft-tissue tumors using receiver operation characteristic curve(ROC curve). Method: Ninty-two solid superficial soft-tissue tumors were studied using combination of two-dimensional and color Doppler ultrasound and compared with the pathologic results. The tumors were evaluated semi-quantitatively to get the ROC curve and the Youden index according to the tumors’ size, margin, configuration, homogeneity and blood flow. Results: The color ultrasound demonstrated all the 92 tumors including 72 benign and 20 malignant ones. The area under the ROC curve was 0.994(P<0.001), and the Youden index was 10. Conclusion: The color ultrasound is accurate in the differentiation of benign and malignant superficial soft tissue tumors, and plays a directive role in treatment planning.

428 Synthetic methods research of 18F-FDG as phase-transfer calalyst by ionic liquid with one step methodology

Objective: To develop the new synthetic 18F-FDG method used ionic liquid(IL) as Phase transfer catalyst(PTC). Material and Method: Study the productivity of 18F-FDG with different volume (K2CO3, IL, acetonitrile). 1,3,4,6-tetra-O-acetyl-2-O-trifluromethanesulfonyl-D-mannopyanose(TATM) 25mg precursor and 1-Butyl-3-methylimidazolium trifluoromethanesulfonate(BMI) as PTC. Results: we got the highest 18F-FDG productivity(55%) and radiochemistry purity(93%) with 0.2ml K2CO3, 0.3ml BMI, 0.7ml acetonitrile and TATM 25mg. Conclusion: we can get the highest 18F-FDG productivity used BMI as PTC with TATM precursor.
