301 Measurements of echocardiography in 1344 healthy children

Objective: To investigate the measurements of echocardiography in healthy children. Methods: One thousand three hundred and forty-four healthy children were divided into ten groups by age. Parameters including dimension of cardiac chambers and great vessels, left ventricular function and flow velocity through different valves were measured with HP/Sonos2500, HP/Sonos5500 or Philips/Sonos7500 ultrasound diagnostic systems. The data was analyzed by SPSS 11.0 software. Data were expressed as x±s. The Pearson correlation analysis was performed to determine the correlation between all measurements and age. Results: No correlations were observed between MVE, MVE/MVA, PAV, △D% and age. MVA, TVA, HR, FS, EF were inversely correlated with age. Other measurements including AOD, LAD, LVDd, LVDs, MPA, IVS, LVPWd, LVPWs, SV, CO, LVEDV, LVESV, TVE, TVE/TVA and AOV correlated positively with age. The measurements of cardiac and great vessels were significantly different between boys and girls above three-year-old children. The measurements of girls were slightly lower than those of boys. However, no differences were found in parameters of left ventricular function such as FS and EF between boys and girls. Conclusion: Measurements of echocardiography change in different age of children. But it should be noted that some parameters may not be affected by age.

304 Evaluation of left ventricular remodeling and function in dilated cardiomyopathy by echocardiography

Objective: To explore some indices for geometric evaluating left ventricular remodeling and their relationship with left ventricular function in dilated cardiomyopathy(DCM). Methods: A series of geometric indices such as sphericity index(SI), D2/L, (D1+D2+D3)/3L, D3-D2, D2-D1, (D2-D1)/D2 and 2D1/L were measured by echocardiography to describe left ventricular shape. Results: SI significantly decreased and D2/L, (D1+D2+D3)/3L, 2D1/L markedly increased in DCM compared with normal subject. D2-D1 also rised in DCM, but (D2-D1)/D2 did not show statistical difference in contrast to that of the control group. There was a significantly high frequency that D3-D2 was negative in DCM than in normal subject. Ejection fraction(EF) and stroke volume(SV) decreased markedly in DCM. Moreover, EF and SV were significantly positively related with SI, and negatively related with D2/L, (D1+D2+D3)/3L, 2D1/L. Conclusion: Left ventricular remodeling in DCM could be described more completely by several geometric indices than by only one index. Especially 2D1/L, which was a sensitive index to sphericity of apex, SI combined with 2D1/L as an improved system for geometric evaluation could provide more morphologic information about left ventricle.

307 MSCT evaluation of pre-operation of liver transplantation candidates

Objective: To study the value of three-phase ce-MSCT with 3D-MSCTA(P) for evaluation of pre-operation of orthotopic liver transplantation candidates. Methods: Sixty-three potential candidates for liver transplantation were evaluated with three-phase ce-MSCT scanning(intravenous contrast medium was administered at the rate of 3ml/s. Three-phase scanning acquisition at 22~25s, 60s and 120s respectively after initiation of contrast medium injection). Arterial phase and portal venous phase scan information was used for volume rendering CTA(P). All images were analyzed to define the parenchymal liver disease and the size of hepatocellular carcinoma, as well as the diameter of common hepatic artery and celiac axis. The hepatic arterial blood supply was categorized according to Michel’s classification. Portal vein thrombosis was defined. Hepatic volume was measured by CT as well as collateralization stage of the hepatic venous system and splenic artery aneurysms. Results: Masses were pathologically proven as hepatocellular carcinoma in 15 cases(small hepatocellular carcinoma in 3 cases), cholangiocellular carcinoma in 2 cases, cholecystic carcinoma in 1 case in all of the 63 patients. Michels IX anatomy in 2 cases, celiac axis stenosis in 3 cases, small-caliber hepatic arterial vessels in 2 cases, SA aneurysms in 2 cases. Portal vein thrombosis in 12 cases?穴long segment in 7 cases), the mean hepatic volume was 1114.41cm3. Conclusions: Three-phase ce-MSCT with CTA(P) can define parenchymal liver disease, show the anatomy of hepatic artery and hepatic vein as well as vascular diseases, hepatic volume can be measured. MSCT can provide comprehensive preoperative liver evaluation for transplantation candidates.

311 Characteristics and appearances of rat C6 brain gliomas on MRI: correlationwith expression of VEGF, MMP-2 and MMP-9

Objective: To investigate the development regularity of rat C6 brain gliomas by MRI and to evaluate the relationship among the calculated MRI parameters of implanted rat C6 brain gliomas and to study correlation between the parameters and expression of VEGF, MMP-2 and MMP-9. Methods: C6 cells were inoculated into the right caudate nucleus of 30 Wistar rats by stereotactic procedure to established animal model. Conventional MRI including enhanced scanning on 1 week, 2 and 3 weeks after implantation were used to dynamically observed the tumor development. Immediately following MRI, some rats were killed for HE and immunohistochemical staining. Then tumor volume, increased enhancement and EI(edema index) were calculated on MRI and performed statistical analysis compared with immunohistochemical changes. Results: The characteristics of rat C6 brain gliomas on MRI were consistent with previous published reports. The tumor cell density and angiogenesis increased with time, and tumor volume, increased enhancement and EI increased simultaneously and correlated positively with each other. Expression of MMP-2 and MMP-9 were observed in 1-week gliomas except VEGF. From then, there were persistent expression of VEGF, MMP-2 and MMP-9. There were positive correlations between the parameters and expression of VEGF, MMP-2, MMP-9. Conclusion: MRI revealed many features of rat C6 brain gliomas progression. There were positive correlations among tumor volume, increased enhancement and EI calculated on MRI, and between the parameters and expression of VEGF, MMP-2 and MMP-9.

315 Comparative study of color power angioimaging and microvessel density in thyroid tumors

Objective: To evaluate the value of color power angioimaging(CPA) in thyroid tumors by studying the vascular pathology of the tumors. Methods: Thyroid tumors in sixty cases were examined preoperatively by CPA. Microvessel density(MVD) was calculated in surgical specimens after immunohistochemical staining using anti-factor CD34. Results: MVD in thyroid carcinomas(51.37±11.17) was higher than that in adenomas(21.32±7.95, P<0.05). MVD in CPA III of thyroid carcinomas(53.21±13.11) was higher than that in CPA II(36.11±7.92, P<0.05). The RI values of thyroid carcinomas and adenomas all were related positively to MVD(r=0.737, r=0.831, P<0.05). Conclusion: The degree of CPA, RI levels and MVD may provide important information for diagnosing and evaluating thyroid tumors.

318 Comparative analysis of PET/CT vs CT scan in the diagnosis of non-small cell lung cancer

Objective: To explore the diagnostic value of dual-modality positron emission tomographic-computed tomographic(PET/CT) imaging, as compared with that of CT alone in the diagnosis of NSCLC and metastasis. Materials and Methods: Fifty-six cases of lung cancer were proved by surgery, biopsy, pathology or follow-up. All cases were examined by PET/CT and CT. PET/CT images of all patients were analyzed and compared with CT scan. Results: In 56 cases, for lung primary lesion, the positive rate with PET/CT scan was 96.4%, and CT scan was 82.1%. And for the metastatic lesion, the positive rate was 100% and 80.5% respectively. It was statistically significant(P=0.008 and P<0.001). Conclusion: PET/CT is superior to CT scan in the detection of primary lung lesions and metastasis. Integrated PET/to imaging patients with lung cancer.

321 The CT features of epidemic hemorrhagic fever?穴a report of 38 cases?雪

Objective: To investigate the CT features of epidemic hemorrhagic fever(EHF) and its clinical application. Materials and Methods: Thirty-eight patients were confirmed by clinical practice and CT were performed in all cases. They are divided into 11 severe cases, 22 medium severe cases, 5 mild cases clinically. The CT imaging data were analysed and summarized. Results: There were 26 cases, whose kidneys on both sides were swelling obviously, they are all medium severe or severe patients. There were 12 cases in whom both kidneys were not obviously swelling, all 5 mild cases were in this group. In all the 38 patients, renal subcapsular hemorrhage were seen in 2 cases, perirenal fluid collection in 10 cases, seroperitoneum in 16 cases, hydrothorax in 16 cases, pneumonorrhagia 1 case, pulmonary edema in 2 cases, atelectasis 1 case. Conclusion: CT could accurately and promptly display the changes of relative viscera. CT could provide valuable information for the early diagnosis and evaluate the treatment of EHF. It might play an important role in the diagnosis of EHF.

324 The observation of culprit vessels in patients with acute myocardial infarction by coronary angioscopy

Objective: To provide the clinical pathological basis for AMI by means of observation culprit vessels in acute myocardial infarction(AMI) patients with or without ST segment elevation by coronary angioscopy. Methods: Twenty-eight cases of AMI with ST elevation(STMI) and 18 cases AMI without ST elevation(NSTMI) were included. The criminal vessels were investigated by coronary angioscopy when these patients were accepting coronary interventional examination and therapy. Results: ①Yellow plaque was detected in all the culprit vessels of AMI patients. ②There were a higher rate of plaque rupture detection in the group of NSTMI than that of STMI(88.89% vs 57.14% respectively, P=0.022). ③The red or mixed plaque was more common in STMI group than in NSTMI(92.85% vs 55.56% respectively, P=0.003). Meanwhile the occlusive thrombus was more common in STMI group(78.57% vs 11.11% respectively, P<0.0001) while the non-occlusive thrombus was more common in NSTMI group(44.44% vs 14.29% respectively, P=0.054). Conclusion: Rupture of unstable yellow plaque and its accompanied thrombosis was the pathological basis of AMI. Red or unstable plaque was predominant in STMI while non-occlusive plaque was mostly seen in NSTMI. Thus, to stabilize the yellow plaque before its rupture is the key point to prevent the onset of AMI.

327 Real contrast ultrasound combined with blood spectrum morphologyin determining the character of blood dynamics in breast tumor

Objective: Utilizing grey-harmonious contrast ultrasound combined with time-strength curve and compared with blood spectrum morphology to analysis blood dynamic character of breast tumor. Methods: Prophase research: pulse Doppler was added to 136 breast tumors to withdraw the blood spectrum. Another 16 breast tumors were checked with contrast ultrasound. Results: Blood spectrum morphology of malignant tumor and benign tumor was different. Blood spectrum morphology in breast cancers can be divided into 4 types. Morphology and parameter of time-strength curve of malignant and benign breast tumor were different. Conclusions: Blood spectrum morphology combined with real contrast ultrasound in determining vascular character of breast tumor can provide more valuable information in early diagnosing breast cancer.

331 Evaluated the value of diagnostic MRCP in patients with cholangiopancreatic disease by ROC

Objective: To evaluate the value of diagnostic ultrasound, CT and MRCP in patients with cholangiopancreatic disease by receiver operating characteristic curves(ROC), and introduce the principle of ROC simply. Methods: One hundred and five patients suspected with cholangiopancreatic disease were collected. All patients were studied with MR. Of them, 65 patients underwent US examination. CT were performed in another 59 patients, of them, ultrasound was performed in 31 patients. Pathological results, operating findings or clinical and laboratorial examination results were the reference standard. Images were interpreted blindly to the reference standard. True positive rate and false positive rate were calculated respectively. We analysed the data with ROC. According to location and cause of the obstruction, sensitivity, specificity and likelihood ratio of MRCP, US and CT were calculated respectively. Chi-square test and Fisher test were used as statistical methods. Results: ROC curve showed MRCP curve lie in the top left corner. Area under the ROC curve(AZ) of ultrasound, CT and MRCP were 0.981, 0.901 and 0.985 respectively. All AZ value of them was higher than 0.9. But there was no significant difference between MRCP and CT(Z=0.75, P>0.05). According to location and cause of the obstruction, MRCP, US and CT had different diagnostic ability. ①The sensitivity of MRCP in distinguishing the various pancreato-biliary tumors was significantly higher than that of US and CT(P<0.05). ②The sensitivity of MRCP in diagnosing pancreato-biliary stones was significantly higher than that of US and CT?穴χ2=10.058 and 5.046, P<0.05). The sensitivity of MRCP, US and CT in diagnosing choledocholithiasis were 92%, 39% and 62% respectively. There was significant difference between MRCP and US(χ2=17.412, P<0.001). Conclusion: ROC analysis is an objective index for comparing and evaluating difference in efficiency of two or more imaging diagnostic methods. MRCP has better discriminatory power than does US and CT for the work-up of obstructive jaundice disease. MRCP has better discriminatory power than does US and CT in diagnosing the malignant pancreato-biliary obstruction and pancreato-biliary stones.

335 CT in the differential diagnosis between vast multilocular cystic nephroma and carcinoma of kidney

Objective: To explore CT features of the vast multilocular cystic nephroma and carcinoma of kidney and their differential diagnosis. Methods: Three cases of vast multilocular cystic nephroma and 13 cases of carcinoma of kidney were retrospectively analyzed by comparing the imaging of CT findings with surgical operation. Results: All the vast multilocular cystic nephromas were entirely composed of locules and septa without solid nodules. Carcinoma of kidney mostly destroyed the renal parenchyma. Conclusion: Vast multilocular cystic nephroma is multilocular cysts and is arranged in order, the septa is smooth and with very clear border. Vast carcinoma of kidney do have clear border with no intact renal parenchyma, liquifiction, necrosis and cystic formation are the features of CT imaging. These are the main features which can be distinguished from the vast multilocular cystic nephroma.

338 The research of spiral CT in traumatic fracture of cattle’s non-calcified costal cartilage

Objective: To investigate the clinical value of spiral CT with MIP and 3D-SSD reconstruction in traumatic fracture of non-calcified costal cartilage of cattle. Methods: First, make the model of cattle of non-calcified costal cartilage. Ten fresh non-calcified costal cartilages with inter costal muscle were obtained from cattle killed within 3 hours. And the costal cartilages were examined by spiral CT, MIP and 3D-SSD reconstruction. Secondly, the costal cartilage in 33 clinical cases were studied. Results: The diagnostic sensibility of spiral CT in non-calcified costal cartilage of human individual is 93.54%, accurate rate is 93.93%, positive predictive value is 93.54%, false positive rate 3%, false negative rate is 3%. Conclusion: Spiral CT associated with MIP and 3D-SSD reconstruction have important clinical value in the diagnosis of traumatic fracture of non-calcified costal cartilage.

341 The research on MR imaging of cruciate ligament of knee joint injury

Objective: To supply reliable imaging data for clinical diagnosis and treatment, we combine the anatomy peculiarities of cruciate ligament of knee joint with the manifestation of MR imaging in order to find best protocol for MR imaging and observe the MRI manifestation of cruciate ligament. Methods: Thirty cases of adult knee joints with injury of cruciate ligament were scanned by MRI with spin echo of sagittal surface and inclined sagittal surface knee joint in extending position, slightly bending position and bending position. The manifestation of cruciate ligament from different position and different angle in sagittal view were observed on work station. Results: ①The display of injured anterior cruciate ligament(ACL): the complete display rate of ACL in vertical and inclined sagittal surface in extending position is 46.7% and 53.3% respectively. The complete display rate of ACL in inclined sagittal view of vertical and inclined sagittal view in slightly bending position is 80.0% and 80.0% respectively. Complete display of ACL in vertical and sagittal view of bending position is 90.0% and 90.0% respectively. ②The manifestation of MRI of cruciate ligament. It’s difficult to identify the slight high signal appeared in the partial cruciate ligament tears. In complete ligament tears it appears a break or defect in the ligament or a ball-like high signal at the attachment of the ligament. Conclusion: MRI in sagittal view of bending position was the best plane for showing ACL tear. It has a leading significance for the diagnosis and treatment in cruciate ligament injury.

344 The imaging diagnosis of bladder rhabdomyosarcoma in children

Objective: To study the cystography and CT features of bladder rhabdomyosarcoma in children for further understanding the disease. Materials and Methods: Clinical data and imaging findings in 16 cases with bladder rhabdomyosarcoma were retrospectively analyzed. All cases had intravenous urography or cystogram, and 6 cases had CT scan. Results: Botryoidal filling defects were found in 13 cases, massive phylloid filling defects in 3 cases and rigid bladder in 6 cases in intravenous urography or cystogram. Botryoidal shadow of low-muscular density and thick-walled bladder were showed on CT scan while after the administration of contrast media the tumors were mildly or moderately enhanced. Conclusions: There are some certain radiographic characteristics in pediatric bladder rhabdomyosarcoma, we can make a correct diagnosis combining with clinical features.
